Evaluation de la chimioprophylaxie antituberculeuse à Bamako et à Kati
Le risque de développer la tuberculose maladie est 20 à 37 fois plus élevé chez les personnes vivant avec le VIH. Le traitement préventif à l'isoniazide (TPI) fait partie des interventions de santé publique indispensables pour prévenir la tuberculose chez les personnes vivant avec le VIH. Notre travail avait pour but est d'évaluer le nombre d'intervention de cette recommandation, en faisant évaluer le niveau de connaissance des praticiens sur l'approche de la chimioprophylaxie a l isoniazide (CPI). Pour cela, nous avons conduit une étude de type transversale, longitudinale descriptive couvrant la période du 10 Mars 2018 au 10 octobre 2018 soit 7 mois, dans différents services dans le district de Bamako et la ville de Kati notamment les services de Médecine, de Maladies infectieuses, de Pneumo-phtisiologie au Centre Hospitalier Universitaire (CHU) du Point G, Médecine du CHU de Kati, Gastro-entérologie et Pédiatrie du CHU de Gabriel Toure, Médecine de l'Hôpital du Mali, Centre national de lutte et d'appui des maladies, et enfin les centres de référence de sante des commune I, commune II, commune III, commune IV, commune V et commune VI. Nous nous avons inclus 50 médecins qui au cours de leur expérience ont assuré la prise en charge les patients infectés par le VIH, avec une prédominance masculine, l'âge moyen était de 35,22 plus ou moins 4,386 ans avec des extrêmes de 27 et 48 ans et une prédominance des 30 à 39 ans. Dans notre étude, 96 p.100 des médecins reconnaissent avoir connaissance de l'existence de la CPI, 54 p.100 avoir connaissance de la note circulaire et 2 p.100 ont pu définir correctement la CPI, 88 p.100 des médecins ont déclaré que l'INH uniquement est la molécule appropriée pour le traitement préventif de la TB, 52 p.100 ont donné une réponse assez correcte en ce qui concerne la posologie de la CPI chez l'enfant et 64 p.100 ont donné une réponse insuffisante en ce qui concerne celle de l'adulte, 74 p.100 ont donné une réponse insuffisante en ce qui concerne les critères de mise en place de la CPI chez l'enfant et 70 p.100 ont donné une réponse insuffisante en ce qui concerne ceux de l'adulte, 50 p.100 des médecins pratiquent la CPI. La principale crainte à son utilisation mal contrôlée est la résistante aux antituberculeux. La chimioprophylaxie a l isoniazide recommandée depuis de nombreuses années et plus que jamais par l'OMS pour réduire le risque de passage de la tuberculose infection à la tuberculose maladie reste très peu utilisée dans le monde et au Mali. Il convient de renforcer la stratégie de communication et d'organiser des sessions de formation et/ou de recyclage pour améliorer la mise en oeuvre
Knowledge and attitudes towards labour pain relief amongst pregnant women attending antenatal clinic at the women and Newborn Hospital, University Teaching Hospitals, Lusaka
ThesisLabour pain is described as severe by most women and hence the use of both pharmacological and non-pharmacological methods for its relief. Low levels of awareness of labour pain relief have been noted in LMIC‟s compared to more developed countries. The present study sought to determine the knowledge and attitude towards labour pain relief amongst 385 parturients attending antenatal clinic at Women and Newborn Hospital, University Teaching Hospitals, Lusaka, Zambia from August to November 2017. This was a cross-sectional study and a semi-structured interview was conducted with data collected in form of questionnaires. Data was analysed using Stata SE13. The majority of the women were fairly well educated (87 percent secondary level education and higher) and multigravidas (77 percent). Only 35 percent of the women had knowledge of labour pain relief. The main factors influencing knowledge were education (P = < 0.001, CI 2.18 – 4.61) and employment (P = <0.001, CI 2.04 – 5.03). The main sources of information on labour pain relief were health workers and the internet. Most of the women (74 percent) expressed a desire to have labour pain relief at next delivery. None of the independent variables under study were found to influence attitude towards labour pain relief. There is a low level of knowledge on labour pain relief amongst pregnant women attending antenatal clinic at the Women and Newborn Hospital, however, once they know that such an option exists, most of them are willing to receive labour pain relief during delivery. Inclusion of such lessons as part of routine antenatal care as well as using the media i.e. television and online resources would greatly help in increasing women‟s knowledge on the matter. A case of Kenyatta national hospital.
Scaling-up quality referral processes strengthens the health system. Referral refers to any process in which healthcare providers at lower levels of the health system seek the assistance of providers who are better equipped or specially trained to guide them in managing or to take over responsibility for a particular episode of a clinical condition in a patient. Quality in health care falls under Service delivery, which is one of the six key health system strengthening building blocks. The main objective of the study was to find out the factors influencing the scaling up quality of referral processes at Kenyatta National Hospital and the specific objectives of the study were to; establish how the management of referrals can scale up the quality of referral processes at Kenyatta National Hospital, determine how the pre-referral communication processes scale-up the quality of referral processes, and establish how referral staff can scale up quality of referral process. The Research design was a descriptive cross-sectional and retrospective study, data was collected using a questionnaire and check list. A check list was used and 384 referrals entered, a total of 100 self-administered questionnaires were completed by staffs working at Kenyatta National Hospital and returned. Data analysis was done using SPSS version 21 and presented in tables, bar graphs and pie charts, reliability was done to establish regression co-efficient at (95%) confidence interval. The findings revealed that majority of the respondents 80(79.8%) indicated the referring institutions do not understand the referral policy, guidelines and Standard Operating Procedures, 12(13.5%) showing that they know and 6(6.7%) indicating that they don't know, majority of patients, 75(75%) came with referral letters which were incomplete, 13(13.7%) lab results and 12(12%) coming with X-ray results. The findings indicates that 51(56%) of the referred patients came with nurse, 40(44%) relative with none being escorted by doctor. According to the findings 63(67%) of the referring hospitals did not call prior to referring the patient, only 31(33%) called before transferring the patient to KNH. In conclusion, the findings showed that there was significance relationship between referral policy and staff knowledge on referrals at P< 0.05 with chi square test of 0.004. Scaling up the referral processes ensures quality of referral services. The study recommends that the management of Kenyatta National Hospital needs to ensure that dissemination, sensitazation and awareness of the referral policy, guidelines and SOPs is done for the staffs both in the referring and receiving hospitals, referral tools, forms and registers be distributed at all levels of the health system for standardised information in the referral letters, laboratory and radiological investigations. Aspect épidémio-cliniques, thérapeutiques et pronostique des complications métaboliques aigues du diabète au SAU du CHU Gabriel Touré du 1er Octobre 2018 au 31 Septembre 2019
Contexte : Du 1er Octobre 2018 au 31 Septembre 2019, nous avons mené une étude sur les aspects épidémio-cliniques thérapeutiques et pronostique des complications métaboliques aigües du diabète au Centre Hospitalier Universitaire Gabriel Touré de Bamako. Cette étude s’est basée sur l’analyse de la fiche d’enquête.
Objectifs : Il s’agissait d’étudier les aspects épidémiologiques, cliniques thérapeutiques et pronostiques des complications métaboliques aigües du diabète, à travers la détermination de leur fréquence, l’identification des facteurs de risque associés à leur survenue, la description des manifestations cliniques et la détermination des facteurs de mortalité.
Résultats : Au total, 33 patients ont été colligés. L’âge moyen des patients était de 52,5 ± 17ans. Le sex-ratio était de 0,33. La majorité des patients nous était amenée du domicile dans 57,6% des cas et l’altération de la conscience était le motif d’admission le plus fréquent dans 42,4% des cas. A l’admission, la clinique était majoritairement marquée par unepolypnée, une dyspnée de Kussmaul, une déshydratation, une tachycardie et une hyperthermie. L’infection aété le facteur de décompensation chez 63,6% des patients. Dans la majeure partie des cas nos patients avaient des chiffres glycémiques élevées. La cétoacidose a été la complication majeure avec 54,5% suivie du comahyperosmolaire avec 24,2% et du coma hypoglycémique avec 21,2%.
L’évolution a été défavorable chez 09 patients soit 27,3%.

La césarienne extra-péritonéale au centre de santé de référence de la commune III du district de Bamako : série de 50 cas
Il s’agissait d’une étude prospective effectuée dans le service de Gynécologie- Obstétrique du centre de santé de référence de la commune III du District de Bamako allant du 11 octobre 2017 au 06 septembre 2018(Soit 12 mois) et portait sur 50 cas de césarienne extra-péritonéale.
La fréquence de cette césarienne a été 25% sur un total de 621 césariennes et 2% sur un total de 2466 accouchements effectués durant la période d’étude.
L’âge moyen de notre population d’étude était 28 ans avec des âges extrêmes 14 et 42 ans et l’indication la plus fréquente a été utérus Multicicatriciel avec respectivement 30%.
L’objectif principal était d’évaluer le confort, l’autonomie et la durée d’hospitalisation post-opératoire. Cette satisfaction post-opératoire a été retrouvée chez 46 de nos patientes soit 92%.
Nous n’avons pas enregistré des cas : d’hémorragie, de suppuration, de décès maternel et néonatal
Therapeutic outcomes in aids-associated kaposi's sarcoma patients on antiretroviral therapy treated with chemotherapy at University Teaching Hospital and cancer diseases hospital in Lusaka, Zambia
ThesisThe incidence of HIV-associated Kaposi’s sarcoma remains high in Zambia in
the antiretroviral therapy era. The most efficacious treatment regimen for KS
has yet to be established. In both developed and developing countries,
treatment regimens have had limited efficacy. Late presentation in Africa
affects therapeutic outcomes. The aim of this study was to determine the
therapeutic outcomes of epidemic KS patients after completion of six cycles
of Adriamycin, Bleomycin, and Vincristine (ABV) chemotherapy. This was a
descriptive cross-sectional study. Study participants were drawn from a study
database of confirmed incident KS patients seen at the Skin Clinic of the
University Teaching Hospitals (UTH) during the period between August,
2015 and September, 2016. Of the 38 successfully recruited study
participants, a complete response was documented in 18 (47.4%) after 6
cycles of ABV whereas 20 (52.6%) experienced a partial response. KS
recurrence was observed in 8 (44.4%) of individuals that experienced an
initial complete response. At the time of the study, clinical assessment
revealed that KS lesions had completely regressed in 21 (55.3%) of all the
patients. ABV chemotherapy appears ineffective in long-term resolution of
epidemic KS patients on ART. Recurrence rates are high after chemotherapy
in patients that experience initially favorable responses to treatment. There is
a need to diagnose KS earlier, and to develop more efficacious treatment
options in order to reduce recurrence rates for epidemic KS.

A study of factors associated with mortality in patients managed for perforated peptic ulcers at University Teaching Hospital, Lusaka, Zambia
ThesisPerforated Peptic ulcers (PPU) is one of the common finding in patients undergoing emergency
laparotomy for peritonitis at UTH, Adult Hospital, department of surgery, representing about
26.45% of these cases in a calendar year. A high risk of mortality and morbidity is encountered in
patients treated with surgery for PPU. Patient stratification and appropriate management according
to patients risk of mortality, has been recommended as a way of trying to improve outcomes in
patients with PPU. The aim of the study was to investigate the factors associated with mortality in
patients that present with PPU managed surgically at UTH. This was a prospective cohort study,
conducted at UTH-Adult Hospital, Department of surgery targeting patients with PPU that had
undergone emergency laparotomy. They were enrolled using interview administered
questionnaire and followed up for 30-days post-operation. The primary outcome was 30 day postoperation
mortality. The study enrolled 38 participants of which 35 participants met the inclusion
criteria, 88.57% (n=31) male and 11.43% (n=4) female. The age ranged from 14 years to 84 years
with the mode age ranges 20 years – 30 years and 40 years – 50 years. Most patients presented
with two (2) or more complaints. Abdominal pain was common presenting complaint in all the
patients and other complaints included, nausea and vomiting, abdominal distension and
constipation. 82.86% (n=29) of the participants presented after 24 hours of onset of symptoms,
17.14% (n=6) were HIV positive and 42.86% (n=15) were in shock on presentation. More than
half of the participants were assessed as ASA score III or greater. On admission 40.00 % (n=14)
had elevated serum creatinine and 37.1 % (n=13) had low serum albumin. 91.43% (n=32) of PPU
were gastric perforations and 8.57 % (n=3) duodenal perforations and the mode and median range
of size of perforation was 5mm – 10mm. Length of hospital stay ranged from one(1) day to 13
days with a mean of 7.29 days. 30-day post-operation mortality was 34.29% and 40% of the
participants developed a post operation complication. Univariate binary regression analysis found
Age, ASA score, Serum creatinine, and Size of perforation to be significantly associated with
mortality (p-value <0.05, C.I. 95%). Univariate linear regression analysis found HIV positive
Status, and CD4 cell count level to be significantly associated with mortality(p-value <0.05; 95%
C.I.). Multivariate binary regression analysis found no significant association with mortality of the
above variables (p-value <0.05; C.I. 95%). PPU is a common finding among patients with
peritonitis undergoing emergency surgery at UTH and is associated with high mortality. Univariate
statistical analysis found Age, ASA score, Serum creatinine, Size of peptic ulcer perforation, HIV
Status and CD4 cell count level to be associated with mortality. However, no factor was found to
be significantly associated with mortality on multivariate regression analysis.

Motifs de consultation des enfants de 0 à 59 mois dans le centre de Santé de Référence de Nara de janvier à juin 2018
Notre étude a été prospective et s’est déroulée de janvier en juin 2018. Elle a porté sur 560 enfants de 0 à 59 mois vus en consultation. L’objectif principal était d’étudier les motifs de consultation des enfants de 0 à 59 mois dans le centre de santé de référence de Nara.
Objectifs spécifiques : Décrire les caractéristiques sociodémographiques des enfants de 0 à 59 mois vus en consultation durant la période d’étude, connaitre les motifs les plus fréquents pour lesquels les patients de 0 à 59 mois consultent dans le centre,
connaitre parmi les motifs de consultations les pathologies les plus fréquentes chez les enfants de 0 à 59 mois au centre de sante de référence de Nara, déterminer le nombre d’enfants de 0 à 59 mois hospitalisés parmi ceux qui ont été vus en consultation,
décrire l’itinéraire thérapeutique qui a conduit ces enfants de 0 à 59 mois au centre de santé de référence de Nara.
Il ressort de notre étude que :
- 56,61% des patients étaient de sexe masculin. Les enfants de 29 jours à 23 mois
ont été les plus nombreux avec 57,68% ;
- 58,39% de nos enfants provenaient de Nara ville ;- La fièvre a été le premier motif de consultation avec 38,39% ;
- Les infections respiratoires aiguës hautes ont été le premier diagnostic retenu
avec 30,18% ;
- 21,07% des patients ont été hospitalisés ;
- 60% de nos patients sont venus en consultation avant le 4 ème jour du début des
symptômes ;
- Le père a décidé la consultation dans 42,68% des cas et a pris en charge les frais du traitement dans 79,29% des cas
Accouchement chez les gestantes de 35 ans et plus à la maternité de l’HFD de Kayes.
Il s’agissait d’une étude descriptive et analytique à collecte prospective Cas-témoin qui s’est déroulée à la maternité de l’hôpital Fousseyni Daou de Kayes et a concerné la période du 1er janvier 2018 au 31 décembre 2018 soit 12 mois. Elles ont été incluses dans notre échantillon comme Cas toutes les gestantes de 35 ans et plus ayant accouché à la maternité ; et comme Témoins les gestantes de 20-30 ans ayant accouché à la maternité de l’hôpital Fousseyni Daou de kayes.
L’objectif principal est d’étudier le pronostic de l’accouchement chez les gestantes de 35 ans et plus à la maternité de l’HFD de Kayes.
Résultats : avec une fréquence de 6,3%, l’accouchement à l’âge de 35 ans et
plus était associé à un taux plus élevé de parturientes non scolarisées 85% (contre 66,5% chez les parturientes de 20 à 30 ans), à la grande multiparité 51,9% (contre 1,9%), d’anémie associé à la grossesse 10,8% (8,5%), à la césarienne 32,3% (contre 30%), l’HRP 9,2% (contre 6,2%), de présentation du siège 13,5% (contre 6,7%), de macrosomie 8,1% (contre 3%), de malformation 2,6% (contre 1,1%).
Conclusion : Les complications obstétricales augmentent avec l’âge materne
Prevalence, intensity and factors associated with soil transmitted helminths infection among children in Chililabombwe District, Copperbelt Province, Zambia: Cross sectional study
ThesisSoil transmitted helminths (STH) are among the Neglected Tropical Diseases (NTDs) of Poverty. They are a common type of parasitic infections in the world, caused by three main species commonly known as roundworms, whipworms and hookworms. This study investigated the prevalence, intensity and factors associated with soil transmitted helminths among children in Chililabombwe district of Zambia. The study was conducted between October and December, 2017.
A cross sectional design was used, consisting of 411 guardian – child pairs. A questionnaire and checklist was used to collect data on factors associated with helminth infection. Stool samples were collected and examined for presence of parasites using formal-ether concentration and Kato Katz techniques. Geometric mean was used to report intensity of infection. Bivariate and multivariate logistic regression was used to determine factors associated with worm infestations. An investigator led stepwise regression was used to identify factors associated with developing STH infection and level of significance was set at 0.05.
Prevalence of soil transmitted helminth infection was 14.4% and the most dominant parasite was round worms (14.1%). The overall intensity of infection was light (50000epg). Factors independently associated with soil transmitted helminth infection after adjusting for other variables were residence (AOR=0.26; 95% CI [0.09-0.73]), household income (AOR=2.49; 95% CI [1.01-6.12]) and overcrowding (AOR =1.33; 95% CI [1.09-1.62]).
This indicates that reinfection is common even after deworming. Hence policy makers should advocate for improved living conditions in urban areas coupled with reduced household overcrowding.
