    Структурно-метаболічні зміни легень при експериментальному канцерогенезі та за умов корекції

    Дисертація присвячена з’ясуванню морфологічних та функціональних змін структурних компонентів легень за умов змодельованої диметилгідразин дигідрохлорид-індукованої (ДМГ-індукованою) аденокарциноми товстої кишки in situ. Вивчено коригуючий вплив наночастинок Au/Ag/Fe при індукованому канцерогенезі на компоненти легень білих щурів. Із застосуванням комплексу гістологічного, гістохімічного, імуногістохімічного, морфометричного, електронномікроскопічного, біохімічного та статистичного методів дослідження встановлено особливості змін структурних компонентів легень в нормі та за умов змодельованого канцерогенезу. Досліджено, що деструктивно-дегенеративні зміни судин, бронхів, респіраторного відділу легень відбуваються на тлі значних метаболічних порушень. Визначено, що ступінь морфологічних та метаболічних змін легень дослідних тварин залежить від впливу канцерогена та терміну спостереження. За умов змодельованого канцерогенезу товстої кишки in situ доведено коригуючий вплив наночастинок Au/Ag/Fe на структурні компоненти легень. Їх використання позитивно впливало на зниження проявів ендогенної інтоксикації, а відповідно сприяло відновленню морфологічного стану судин, бронхів, респіраторного відділу легень дослідних тварин

    Проведено аналіз метопрололу в субстанції та ГЛЗ згідно редакції ДФУ, ЄФ і USP. Проведено порівняльну оцінку пропранололу в АФІ та ГЛЗ на основі монографій провідних Фармакопей світу. Проведено порівняльну оцінку пропранололу гідрохлориду в АФІ та ГЛЗ на його основі (таблетки, комбіновані таблетки з гідрохлортіазидом, капсули, розчин для ін׳єкцій) згідно монографій провідних Фармакопей світу. Здійснено перевірку «зеленості» представлених методик аналізу провідних Фармакопей світу щодо досліджуваних АФІ і ЛЗ на їх основі інструментами AGREE та Еко-шкали. Встановлено, що всі запропоновані методики аналізу метопрололу та пропранололу в субстанціях лікарських речовин та в готових лікарських засобах відповідають принципам і нормам «зеленої» хімії і є безпечними у використанні

    Job burnout is defined as exhaustion syndrome, occurring as a consequence of constant emotional burden associated with work. The threat of job burnout is associated with the following organisational considerations: the specificity of the profession performed, the place and the conditions of work, as well as individual factors - personal, intra-psychic ones. The negative effects of job burnout concern both personal sphere (individual) and social one. Symptoms and signs that may indicate incoming job burnout refer to both physical and mental health, as well as professional one. The most common symptoms include: physical fatigue, apathy in relation to family members and friends, and growing dissatisfaction with the work, distancing from patients. Stress occurring in the workplace can be deepened by stressful events in ones personal life, such as death of a loved person, illness, divorce, family quarrels. The severity of stress is dependent on the degree of adaptation in the conditions of work and maturity of the individual characteristics affecting the reactivity to stress: personality traits, temperament, behavioural patterns. The objective of the paper. The main objective of the study was to analyse the problem of job burnout among nurses working in hospitals, taking into account socio-demographic factors and personality traits. Material and methods. The study included 160 nurses working in surgical, internal medicine and pediatric departments of hospitals. The study applied diagnostic survey method based on the technique of questionnaires. The following research tools were used in the study: AVEM test (Pattern of Behaviour and Experience Related to Work), NEO-FFI questionnaire (NEO Five-Factor Inventory). Results. Research conducted and statistical analysis showed that the most common causes of work-related stress among nurses related to financial factors (71,3%), shortage of staff (56,9%), the excess of duties (53,1%) and the overload of administrative tasks (51,9%). The most common and indicated ways of coping with stress at work was friends' help (63,8%). Studies have shown that 56,9% of nurses surveyed has the characteristics of job burnout or excessive professional workload. It has been shown that job burnout does not depend on factors such as age (p = 0,222), the overall length of professional experience (p = 0,167) or marital status (p = 0,304). Significant effect on the occurrence of job burnout included an internship in the current workplace (p = 0,015), education (p = 0,002), subjective assessment of stress (<0,001), position held (p <0,001) and workplace (p <0,001). 33,3% of nurses in the departments had some type of burnout, of which 21,6% were excessively overworked. Burnout was more common among nurses working in pediatric departments (38,0%) than in therapeutic departments (23,6%). Analysis of personality features of nurses showed high intensity of extraversion and conscientiousness and low openness severity to experience and neuroticism. It has been shown that the severity of personality traits is significantly dependent on age, education, subjective assessment of stress, as well as position held and workplace. Conclusions. The study revealed a high level of stress intensity among the surveyed nurses, with the most common stressors in the workplace being low pay for work performed, understaffing, excessive responsibilities and administrative tasks, and responsibility for another person. Two types of work-related behaviors prevailed among nurses: professional burnout and excessive overload. Socio-demographic factors such as age, marital status, and overall professional experience of the respondents did not affect the prevalence of work-related behaviors. The phenomenon of professional burnout is more common among nurses with secondary education, who experience high level of stress and have a long professional experience at their current workplace. The most frequent way for respondents to cope with stress was the support of friends, which is related to the lack of professional psychological help. Understanding the factors of stress in the workplace and the conditions for overcoming them will allow to make efforts to create a system of psychological support for nurse

    Palliative medicine, with its primary goal of alleviating patient suffering, plays an important role in the health system, since it is aimed at improving the quality of life in patients with severe, chronic or incurable illness. The present study is dedicated to exploring the specific aspects of the nursing process in palliative care, as well as the psychoemotional well-being of palliative care nurses and their role in improving the quality of life of palliative patients and their families. The aim of the study is a comprehensive investigation into the psychoemotional state of nurses working in palliative medicine, with an emphasis on detection of psychosomatic and psychological disorders, analysis of sources of stress in their professional activity, as well as developing and evaluating practical approaches to improving the quality of life in palliative patients and their families with involvement of nurses. The study has considered three key series: the first series was designed to identify psychosomatic and psychological disorders in nurses; the second series was designed to explore the sources of stress in their professional activities, and the third series was designed for evaluation of effectiveness of nursing interventions in improving the quality of life of patients and their families. In the course of the study, methods such as survey, questionnaires and qualitative analysis, as well as statistical methods of data processing were used. The results of the first series have shown that a significant part of nurses have signs and symptoms such as anxiety, depression, insomnia and psychosomatic disorders. The second series has identified the main sources of stress such as witnessing suffering of the patients, interactions with their families and organizational difficulties.   The third series has shown that nursing interventions substantially improve symptom control in the patients, ensure psychological and spiritual support, and help families learn how to care for their sick members. The data obtained underscore the importance of developing strategies to prevent burnout in nurses and refinement of nursing process in palliative care to improve the quality of care and support provided to patients and their families

    Patients admitted to Intensive Care Units (ICUs) face extraordinary physiological and psychological stressors that may significantly affect their short- and long-term well-being. While the primary focus in ICU care traditionally centers on physiological stabilization and life support, an increasing body of evidence suggests that psychological and emotional dimensions are equally crucial to patient recovery. The complex and often overwhelming ICU environment can precipitate a range of psychological reactions including anxiety, fear, confusion, hallucinations, and emotional detachment. These experiences, if left unaddressed, may contribute to long-lasting psychological morbidity, including post-traumatic stress disorder (PTSD), depression, and post-intensive care syndrome (PICS). This master’s thesis explores the critical role that nurses play in providing psychological and emotional support to ICU patients, with a particular focus on the ICU at Kindred Hospital South Florida, Hollywood. Nurses are uniquely positioned to recognize and respond to the psychosocial needs of critically ill patients due to their close proximity, continuous presence, and holistic approach to care. However, despite the acknowledged importance of emotional support, many ICU settings continue to lack formal protocols, adequate training, and institutional emphasis on the emotional care dimension. The primary aim of this study is to evaluate the effectiveness, scope, and limitations of nurse-led psychological and emotional support in the ICU. The study seeks to understand current practices, identify common challenges encountered by ICU nurses, and explore patients' perceptions of the emotional care received. Through both qualitative and quantitative methods, the study gathers evidence that informs best practices and policy recommendations for improving the psychosocial climate of critical care environments. The study was conducted at Kindred Hospital South Florida, Hollywood ICU, and involved both nursing staff and recently discharged ICU patients. Data were collected through structured interviews, self-reported questionnaires, and observational checklists. Nurses were asked to describe their approach to emotional support, perceived challenges, and institutional facilitators or obstacles. Patients, in turn, reflected on their ICU experience, their emotional state during hospitalization, and the perceived impact of nursing interventions on their psychological well-being. Findings from the study reveal that emotional distress is nearly universal among ICU patients, although its intensity and expression vary widely. Common stressors identified include fear of death, feelings of helplessness, loss of control, physical pain, communication difficulties, and sleep deprivation. Patients who received consistent and compassionate emotional support from nurses reported lower levels of fear and anxiety, stronger trust in the care team, and a more positive overall ICU experience. Nurses employed a range of emotional support strategies, including therapeutic communication, presence at the bedside, empathetic listening, touch, patient advocacy, and reassurance. However, nurses also reported substantial barriers to providing consistent emotional support, such as high workload, insufficient staffing, limited training in psychological care, and lack of formal institutional guidelines or protocols addressing psychosocial needs. Importantly, the study identified that nurse-patient interactions characterized by active listening, continuity of care, and respect for patient dignity had the strongest positive impact on emotional well-being. Conversely, patients who experienced abrupt care transitions, lack of communication, or impersonal interactions were more likely to report emotional detachment and persistent psychological symptoms post-discharge. The findings of this research underscore the urgent need to integrate structured emotional support into the standard ICU nursing practice. To this end, the study proposes a multifaceted nursing framework that includes: • Incorporating emotional assessment tools into routine ICU patient evaluations; • Providing training programs for nurses focused on psychological first aid, trauma-informed care, and therapeutic communication; • Embedding emotional care protocols into ICU guidelines and nursing workflows; • Strengthening nurse-patient continuity to foster trust and individualized care; • Establishing institutional recognition and support for emotional care as a core component of ICU nursing. The scientific novelty of this work lies in its practical, real-world exploration of emotional support practices in the ICU, a subject often underrepresented in clinical guidelines despite its significance. By grounding the study in firsthand observations and experiences from a functioning ICU in the United States, the research provides valuable insights into both the universal and context-specific dimensions of psychosocial care in critical settings. From a practical standpoint, the outcomes of this study offer actionable recommendations for healthcare institutions, nursing educators, and policy-makers seeking to improve the quality and humanity of intensive care services. The results also highlight the need for ongoing interdisciplinary collaboration between nurses, psychologists, physicians, and families to ensure holistic and compassionate care delivery. In conclusion, this thesis affirms that emotional and psychological support is not a secondary aspect of critical care but a central, therapeutic intervention essential for the well-being and recovery of ICU patients. Nurses have both the potential and responsibility to deliver this care, but to do so effectively, they require institutional support, appropriate training, and systemic integration of emotional care principles into ICU practice. Recognizing and addressing the emotional needs of patients can transform the ICU experience from one of fear and isolation to one of dignity, compassion, and healing

    У кваліфікаційній роботі здійснено комплексний аналіз стратегій управління персоналом у сфері охорони здоров’я, що охоплює теоретичні засади, українські практики, зарубіжний досвід та перспективи вдосконалення кадрової політики в медичних закладах. Проаналізовано основні моделі стратегічного управління персоналом і визначено, що вони мають різний ступінь застосування в українських умовах, проте їх адаптація є необхідною для підвищення ефективності кадрового менеджменту. Виявлено, що кадрові стратегії в Україні здебільшого мають фрагментарний характер, обмежені ресурсами та не завжди відповідають сучасним викликам. У розвинених країнах кадрові стратегії мають чітку структуру, базуються на довгостроковому плануванні, цифровій трансформації, розвитку лідерства та управлінських компетенцій. Робота поєднує управлінські, соціальні, економічні та технологічні аспекти кадрового менеджменту. Результати кваліфікаційного дослідження мають високу практичну значущість для керівників медичних закладів, HR-фахівців, управлінців у сфері охорони здоров’я та органів державного управління
