MitraClip Procedure in Advanced Heart Failure and Severe Mitral Regurgitation: Case Report and Literature Review
Mitral regurgitation (MR) is a common valvular disorder often seen in a severely dilated left ventricle (LV) and reduced LV function. In chronic heart failure (HF), severe functional MR increases preload, wall tension, LV workload, and worsening prognosis. The MitraClip device offers a percutaneous treatment option in HF, although its safety and efficacy in advanced and acute HF remain a gray zone. We present a successful case of the emergent MitraClip intervention in a patient with advanced HF and review the relevant literature

Analysis of the Development of Gender Stereotypes and Sexist Attitudes Within a Group of Italian High School Students and Teachers: A Grounded Theory Investigation
Gender stereotypes and sexist attitudes continue to persist in educational settings, with significant implications for students’ achievement and well-being. This study aimed to investigate the development of gender stereotypes and sexist attitudes among Italian high school students and trainee teachers. A series of focus groups were conducted with a group of Italian school students and trainee teachers to uncover the complex interplay of individ- ual, interpersonal, institutional, and societal factors that contribute to the formation and perpetuation of these biases. Analysis was conducted using a grounded theory approach. The findings reveal a nuanced and multifaceted understanding of the issue, highlighting the critical role of teacher attitudes, peer influence, and broader cultural norms in shaping students’ perceptions and behaviors. This study offers insights for educators, policymak- ers, and researchers seeking to address gender inequities in education and promote more inclusive and equitable learning environments
Impact of Chemotherapy Dose Intensity on Pathological Complete Response in Pembrolizumab-Treated Early Triple-Negative Breast Cancer: A Real-World Multicenter Analysis
Background: Pembrolizumab combined with neoadjuvant chemotherapy significantly improves pCR in early TNBC, but the effect of treatment intensity and baseline clinical factors has been insufficiently explored in real-world settings. Methods: We retrospectively included 169 consecutive patients with stage II–III TNBC treated across 11 Italian oncology centers (January 2022–January 2025) with the KEYNOTE-522 regimen. Clinical, pathological, and treatment data were collected, including relative dose intensity (RDI), dose modifications, and toxicities. The primary endpoint was pCR (ypT0/is ypN0). Results: The overall pCR rate was 65.7%, which is consistent with clinical trial data. Dose reductions occurred in 40% of patients and chemotherapy was discontinued in 18%. Patients maintaining RDI ≥85% achieved higher pCR (79.3% vs. 51.2%, p < 0.001). Similarly, patients without dose reductions (72.5% vs. 55.2%, p = 0.031) and those completing all cycles (73.1% vs. 41.0%, p < 0.001) had superior outcomes. Dose modifications occurred mainly during the taxane/carboplatin phase and were predominantly due to hematological toxicities (anemia 44%, neutropenia 30%, and thrombocytopenia 15%), neuropathy (18%), and gastrointestinal events (36%). Higher TILs correlated with increased pCR (70.6% vs. 60.7%, p = 0.049), while BRCA mutations showed a favorable trend. ECOG, BMI, pregnancy history, and comorbidities were not significantly associated with pCR. Conclusions: In this multicenter real-world cohort, maintaining chemotherapy dose intensity (RDI ≥ 85%) and completing all planned cycles were strongly associated with higher pCR rates, reinforcing the clinical importance of minimizing dose reductions and discontinuations during pembrolizumab-based neoadjuvant therapy for TNBC

Tale fenomeno, certamente riconducibile alla crisi dello strumento legislativo tradizionale, pone il Fisco nella condizione di esercitare un potere “para-normativo” rispetto al rapporto d’imposta, fornendo regole e/o interpretazioni che vincolano, nei fatti, il contribuente. Il nuovo comma 5-ter introdotto nel corpo dell’art. 6 del D.Lgs. 18 dicembre 1997, n. 472 ad opera del D.Lgs. 14 giugno 2024, n. 87 costituisce il più recente arresto di questa parabola e rende necessario riflettere sul ruolo attuale di questa “atipica” fonte del diritto, nonché sugli strumenti di tutela posti a disposizione del contribuente
Woody Biochar Differently Influences Plant Water Status and Growth of Five Quinoa (Chenopodium Quinoa Willd.) Varieties Under Water Stress
Only few studies have evaluated the responses of quinoa varieties to biochar amendment under water-limited conditions
during the early growth stages. We examined the interaction between soil treated with and without biochar and water
stress applied during the vegetative development on water relation and growth of quinoa varieties. Five varieties (Titicaca,
Quipu, Regalona, UAFQ7, and Q126) were grown in soil without and with 2% (w/w) woodchip biochar and subjected
to two successive water stress cycles, by withholding water until soil reached the permanent wilting point, started from
12-leaf stage. The morpho-physiological attributes, leaf nutrients and total bacterial count were assessed during the experiment.
Biochar significantly improved plant growth, water use efficiency (WUE), and yield-related traits across all varieties
under water-stressed conditions. Biochar under water-limited conditions enhanced plant water status, indicated by lower
pre-dawn water potential and increased relative water content compared to non-treated soil. The UAFQ7 showed superior
growth with biochar, producing more leaves and larger leaf areas (1517 vs. 1378 cm2/plant). Titicaca was more sensitive
to drought, with a considerable reduction in leaf area (1268 vs. 1386 cm2/plant), fresh and dry biomass (6.4 vs. 7.4 g)
compared to well-watered conditions. Quipu produced longer panicles, further enhanced by biochar (17 vs. 12 cm). A
significant increase of 17% in WUE was observed in biochar-treated plants compared to non-amended ones. Woodchip
biochar seems to mitigate the adverse effects of water stress in quinoa, but the responses vary among varieties
