    A case of rhupus treated with filgotinib: A promising therapeutic perspective

    Rhupus syndrome: Rhupus syndrome, characterized by overlapping clinical features of rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE), presents significant therapeutic challenges due to its complex pathogenesis and limited treatment options. This case report evaluates the efficacy of filgotinib, a selective JAK1 inhibitor, in a patient with refractory rhupus syndrome. Case presentation: A 42-year-old male, initially diagnosed with SLE in 2013, presented with persistent polyarthritis, cutaneous manifestations, and constitutional symptoms. Conventional treatments including hydroxychloroquine, methotrexate, and belimumab failed to achieve adequate disease control. Diagnosis and treatment: In October 2022, the diagnosis was revised to rhupus based on the evolution of clinical manifestations and laboratory findings, leading to the initiation of filgotinib, a JAK1 inhibitor. Disease activity was monitored over 24 months using standardized assessment tools. Results: Significant clinical improvement was observed within weeks of filgotinib initiation, enabling prednisone dose reduction and maintaining low disease activity throughout the 24-month follow-up period. Conclusion: This case highlights the therapeutic potential of filgotinib in managing rhupus syndrome but further research and larger clinical trials are necessary to establish filgotinib's therapeutic potential and safety profile in the management of SLE

    Consumers’ Behavior Toward Healthy Foods: A Critical Review

    Changing lifestyle patterns, characterized by a greater consumption of unhealthy and processed foods, has led to a fast increase in overweight and obesity. A better understanding of consumers' behavior toward healthy foods could help food producers and policymakers to determine a shift to healthier dietary models and, thus, to achieve 2030 global nutrition targets. Therefore, the aim of this critical review was threefold: (i) to identify the main factors and sub-factors that can affect consumers' behavior toward healthy foods; (ii) to discuss implications for food producers and policymakers; and (iii) to identify future research directions. Then, a total of 71 articles were identified and critically reviewed. A large variety of drivers have been found to affect healthy food choices among consumers, suggesting the need to consider all of them in implementing marketing strategies and food policies to efficiently meet consumers' expectations and encourage the consumption of healthy foods, respectively. Finally, this review pointed out the lack of a clear definition of healthy foods by the competent Authorities. Furthermore, it is necessary to conduct further investigations into the strength of the effects of the identified factors for healthy foods, to better understand how they contribute to consumers' decision-making

    This volume stems from a collective project aimed at exploring the historical forms of rationality—not as an ahistorical abstraction, but as a situated practice shaped by transformation, tension, and crisis. Originating with the conference Reason in History (Foggia, March 2023) and developed within two national research projects (PRIN), the volume goes beyond merely gathering essays on the concept of reason. It offers a genuine cartography of rationalities: philosophical, scientific, theological, symbolic. From the outset, the title reflects a double perspective: reason as a historical agent, and reason as itself historically constituted. The contributions engage with this dual tension through exemplary cases—from Islamic cosmology to Christian theology, from Jewish philosophy to scholastic logic, and from canonical models to less institutional forms such as alchemy and astrology. What emerges is a dense and layered landscape in which rationality appears not as a fixed foundation, but as a critical exercise and situated orientation. In a time marked by the delegitimization of rational discourse and the crisis of modern grand narratives, this volume offers a space for rethinking reason as a historical practice and as a still-possible instrument for understanding and transforming the present

    Bayesian Integration of Bronchoalveolar Lavage miRNAs and KL-6 in Progressive Pulmonary Fibrosis Diagnosis

    Background/Objectives: Progressive pulmonary fibrosis (PPF) represents one of the most severe and complex challenges in respiratory medicine, characterized by a rapid decline in lung function and often poor prognosis, making it a priority in research on interstitial lung diseases (ILDs). The aim of this study is to correlate classical clinical features and three genetic biomarkers with the diagnosis and prognosis of progressive pulmonary fibrosis in ILDs. Methods: This study involved 19 patients with progressive pulmonary fibrosis (PPF) and 20 patients with non-progressive pulmonary fibrosis (nPPF) from the S.C. of Respiratory System Diseases at the Policlinico of Foggia (Italy) between 2015 and 2022. All participants underwent pulmonary function tests (PFTs), a 6 min walk test (6MWT), and bronchoalveolar lavage (BAL) sampling, following the acquisition of written consent for these procedures. Bayesian analysis with generalized linear models has been applied for both diagnostic and prognostic classification. Results: The proposed Bayesian model enables the estimation of the contribution of each considered feature, and the quantification of the uncertainty that is consequential to the small size of the dataset. The analysis of miRNAs such as miR-21 and miR-92a, alongside the protein biomarker KL-6, was identified as a significant indicator for PPF diagnosis, enhancing both the sensitivity and specificity of predictions. Conclusions: The identification of specific genetic markers such as microRNAs and their integration with traditional clinical characteristics can significantly enhance the management of patients with the disease. This multidimensional approach, which integrates clinical data with omics data, could enable more precise identification and monitoring of the disease and potentially optimize future treatments through larger studies and extended follow-ups

    L'IA nel procedimento penale: tra riconoscimento facciale e finalità investigative

    Negli ultimi anni, le nuove tecnologie dotate di intelligenza artifciale (IA) hanno iniziato a permeare ogni aspetto della vita quotidiana, trasformando radicalmente il modo in cui ogni persona si relaziona con il mondo. A partire da queste semplici applicazioni, l’IA sta progressivamente potenziando il suo raggio d’azione, entrando a pieno titolo anche nelle dinamiche dell’investigazione penale. Ne costituisce un classico esempio l’impiego dell’IA a fni di riconoscimento facciale che consente di eseguire analisi sui tratti biometrici del volto per confrontarli con le caratteristiche inserite in un apposito database, nonché di conservarli all’interno della banca dati. Partendo da queste considerazioni preliminari, lo scritto analizza l’uso crescente dell’IA nel procedimento penale con particolare riferimento al riconoscimento facciale, ponendo l’attenzione sulle sue principali applicazioni investigative all’interno della legislazione nazionale ed eurounitaria.In recent years, new technologies equipped with artifcial intelligence (AI) have begun to permeate every aspect of daily life, radically transforming the way in which each person relates to the world. Starting with these simple applications, AI is gradually expanding its scope, becoming an integral part of criminal investigation. A classic example of this evolution is the use of AI for facial recognition, which allows biometric facial features to be analyzed and compared with characteristics stored in a specifc database, as well as stored within the database itself. Starting from these preliminary considerations, this paper analyzes the growing use of AI in criminal proceedings, with particular reference to facial recognition technologies, focusing on its main investigative applications within national and EU legislation
