Cross‑sectional study of the prevalence of hyperextension of the first metatarsophalangeal joint and its relationship to onycholysis in women with hallux valgus

Hallux Valgus (HV) deformity is associated with misalignment in the sagittal plane that affects the first toe. However, the repercussions of the first toe hyperextension in HV have been scarcely considered. The purpose of this study was to provide evidence of the association between first-toe hyperextension and the risk of first toenail onycholysis in HV.Fisioterapi

Emotional Competences of Primary Education Teachers: A Need in School Post COVID-19

The COVID-19 pandemic has increased the number of students with mental health problems: depression, anxiety, stress. Faced with this reality, teachers and schools must be prepared to respond quickly and effectively. Therefore, the objective of this article is to analyze the emotional competences of primary school teachers in the city of Valencia based on the following sociodemographic variables: sex, age, professional experience, type of center and whether they have children. For this purpose, a quantitative methodological approach has been followed, through which the emotional competencies of primary education teachers are analyzed. These results allow us to establish teacher profiles according to sociodemographic variables and help to detect possible training deficiencies. A sample of 371 teachers of primary education in the city of Valencia has been analyzed. The Questionnaire on Teaching Competences of Primary Education Teachers, carried out under the Planned Action Model, has been used, and descriptive, univariate, bivariate and cluster analyses have been carried out. The mean, the standard deviation and the interquartile range (IQR) have been analyzed, as well as non-parametric tests such as the Wilcoxon, Kruskal–Wallis or Z test. The most significant results are that teachers have a greater ability to interpret emotions and to listen to students. On the contrary, it is teachers who most reject prejudice, discrimination and racism. Younger teachers are the ones who implement more inclusive learning environments. Finally, in general, all teachers are very respectful of students and claim to know how to manage classroom conflicts. The results obtained, in general terms, coincide with most of the research on teachers’ emotional competencies. Some aspects simply do not coincide with the literature. The teachers who participated in our research perceive themselves as having a greater capacity to observe and interpret students’ emotions, to generate learning situations that cater to diversity and to listen to their students. Other studies place these competencies at lower levels. Introduction: Current techniques of anterior vertebral growth modulation by vertebral body stapling or tethering provide an incomplete and unpredictable correction of the deformity, in addition to the disadvantages of the required thoracotomy. Recently, experimental evidence of growth modulation of vertebral epiphyseal plates using a posterior approach Objectives: This study aimed to report the corrective ability of a fusionless technique using a posterior approach without fusion in adolescent idiopathic scoliosis (AIS) patients. Methods: This was a retrospective analysis of prospectively collected data. A series of 36 patients with AIS (Risser 3 or less) underwent surgical correction of their deformities using posterior pedicle screws without fusion. Instrumentation was removed once the maturity stage advanced, with almost complete fusion of the iliac apophysis. Most cases had main thoracic Lenke-1 curves. Coronal and sagittal curve correction was assessed by conventional standing radiographs pre- and postoperatively, before instrumentation removal, immediately after removal, and at the 2-year follow-up after removal apex vertebra on the concave and convex sides of the main curve (MC). The range of motion (ROM) of the thoracolumbar spine and quality of life through the SRS-22 were Results: The mean preoperative coronal Cobb angle of the MC curves was 53.7° ± 7.5° (95% CI: 50.7-56.6°) and was corrected to 5.5° ± 7.5° (89.7%) immediately postoperatively. Before removal of the instrumentation, there was a loss of correction of 3.4° and another additional 2.8° just after removal. In the 2-year check-up after removal of the implants, the mean MC was 13.1°, which is a 75.4% correction of the initial curve. CI: 13.3–24.8) to 27.1° (95% CI: 20.1–26.1) 2 years after implant removal (29.9% in- was almost completely corrected. At the end of follow-up, the mean sagittal ROM of the T12-S1 segment was 51.2 ± 21.0°. The SRS-22 scores improved from 3.31 ± Conclusion: In conclusion, a fusionless posterior approach using common all-pedicle screws correctly constructed satisfactory scoliotic main curves and permitted instrumentation removal once bone maturity was reached. An acceptable ROM of the previously lower instrumented segments was observed, with an increased quality of life.MedicinaCiencias de la Salu

Illness and (hyper)masculinity in 'HIMM' comics from the USA

In this essay, I analyse HIMM comics from the USA, a specific textualisation of graphic medicine/pathography that deals with a variety of illness experiences by male cartoonists. It is my contention that, in the existing literature, the motif of masculinity in autobiographical health-related comics is an underdeveloped area of academic enquiry. As a result, my analysis focuses on how three North American men depict ill health in their work in relation to existing sociological understandings of male behaviour. The texts I discuss are John Porcellino's The Hospital Suite (2014), a story about his abdominal tumour; Matt Freedman's exploration of adenoid cystic carcinoma in Relatively Indolent but Relentless (2014); and Peter Dunlap-Shohl's My Degeneration (2015), which discusses the cartoonist's experience of Parkinson's disease. At the same time, I use the concept of hypermasculinity to explore the similar visual and verbal strategies through which these men respond to their physical and emotional suffering. It is my intention to illustrate how HIMM comics provide an important, non-medicalised lens through which clinical practitioners and lay readers alike can better see the subjectivised experience of male illness in the early 21st century. With a focus on the concept of bracketing, the representation of pain and vulnerability, men's loss of self-identity and hardiness, I explore how HIMM comics act as important counter-narratives to biomedical discourse by visualising the phenomenological aspects of men's ill health. In this way, the texts in my analytical corpus offer a valuable gender-oriented understanding of the connection between illnesses and (hyper)masculinity.Humanidade

Public and private entities launch communication campaigns in order to educate the population about this problem. The aim of this study is to analyze the evolution of the presence of road safety in the Spanish media and its influence on the reduction of road accidents. A content analysis of six major Spanish newspapers between 2000 and 2008 (an important period in the reduction of the accident rate in Spain) was carried out in which the presence of news related to traffic accidents and the intensity of them were quantified using a set of criteria designed for this study. Furthermore, the correlation between the accident rates and the presence and intensity of news in the same time frame was measured. There has been an evident increase in the news related to traffic accidents during the analyzed period, both in terms of the presence of such news in general and in terms of intensity. The correlation analysis also shows a strong relationship between the increase in news presence and intensity and a sharp fall in accident rates in Spain. Although correlation analysis is not enough to establish a causal relationship between the variables studied here, the findings suggest that informative campaigns had a positive effect on encouraging pro-social behavior, were successful in disseminating related information, and, therefore, contributed significantly to lowering accident rates, although that was not the only reason.Humanidade

PTCy vs CNI–based GVHD prophylaxis in HLA-matched transplants for Hodgkin lymphoma: a study of the LWP of the EBMT

Studies comparing the efficacy of posttransplant cyclophosphamide (PTCy) to conventional calcineurin inhibitor (CNI)–based graft-versus-host disease (GVHD) prophylaxis regimens in patients with Hodgkin lymphoma (HL) are scarce. This study aimed to compare the outcomes of patients with HL undergoing hematopoietic stem cell transplantation (HSCT) from HLA-matched donors who received GVHD prophylaxis with either PTCy- or conventional CNI-based regimens, using data reported in the European Society for Blood and Marrow Transplantation database between January 2015 and December 2022. Among the cohort, 270 recipients received conventional CNI-based prophylaxis and 176 received PTCy prophylaxis. Notably, PTCy prophylaxis was associated with delayed hematopoietic recovery but also with a lower risk of chronic (25% vs 43%; P < .001) and extensive chronic GVHD (13% vs 28%; P = .003) compared with the CNI-based cohort. The 2-year cumulative incidence of nonrelapse mortality and relapse was 11% vs 17% (P = .12) and 17% vs 30% (P = .007) for PTCy- and CNI-based, respectively. Moreover, the 2-year overall survival (OS), progression-free survival (PFS), and GVHD-free, relapse-free survival (GRFS) were all significantly better in the PTCy group compared with the CNI-based group: 85% vs 72% (P = .005), 72% vs 53% (P < .001), and 59% vs 31% (P < .001), respectively. In multivariable analysis, PTCy was associated with a lower risk of chronic and extensive chronic GVHD, reduced relapse, and better OS, PFS, and GRFS than the CNI-based platform. Our findings suggest that PTCy as GVHD prophylaxis offers more favorable outcomes than conventional CNIbased prophylaxis in adult patients with HL undergoing HSCT from HLA-matched donors.Medicin

All authors have read and agreed to the published version of the manuscript.Acetylcholine signaling is attenuated in early Alzheimer’s disease (AD) and other dementias. A significant reduction in the expression of nicotinic acetylcholine receptors (nAChRs) in the brain of AD patients has also been reported in several molecular biological and in situ labeling studies. The modulation of the functional deficit of the cholinergic system as a pharmacological target could therefore have a clinical benefit, which is not to be neglected. This systematic review was conducted to identify clinical trials, which evaluated the safety and efficacy of nicotinic acetylcholine receptor agonists using Clinicaltrial (CT) and EudraCT databases. Structured searches identified 39 trials, which used 15 different drugs designed to increase the function of the nAChRs. Most of the identified clinical trials were phase II trials, with some of them classified as ongoing for several years. The systematic screening of the literature led to the selection of 14 studies out of the 8261 bibliographic records retrieved. Six trials reported detailed data on adverse events associated with the intervention, while twelve trials reported data on efficacy measures, such as attention, behavior and cognition. Overall, smost of the physical side effects of cholinergic agonists were reported to be well tolerated. Some trials also reported improvements in attention. However, the efficacy of these drugs in other cognitive and behavioral outcomes remains highly controversial.Medicin
