Behandling av pasienter med blodbårne infeksjoner ved Helgelandssykehuset HF før implementering av hurtigdiagnostikk (BCID2)
Bakgrunn og formål: Tidligere har reaktive blodkulturer ved Helgelandssykehuset HF måtte blitt sendt 300 km til mikrobiologisk laboratorium ved Nordlandssykehuset HF i Bodø for diagnostikk. I 2021 ble det innført et hurtigdiagnostisk verktøy (BCID2) ved Helgelandssykehuset HF som gir svar på bakterie- eller sopp-species og viktige resistensgener i positive blodkulturer innen 1 time. Formålet med denne oppgaven er å undersøke hvordan behandlingen av pasienter med blodbårne infeksjoner var før implementeringen av BCID2. Det vil bli sett nærmere på ulike parameter knyttet til bruk av blodkultur, samt antibiotikabruk i henhold til nasjonale faglige retningslinjer. Disse dataene vil senere bli sammenliknet med tilsvarende data etter implementering av BCID2 og inngå i doktorgradsprosjekter (NORBISH – studien) ved Nordlandssykehuset og Helgelandssykehuset for å se på ulike forhold ved innføring av ny diagnostikk.
Materiale og metode: Masteroppgaven tar utgangspunkt i NORBISH sitt forskningsprosjekt som er godkjent av REK og PVO. I materialet inngår data fra journalgjennomgang av 169 tilfeller av positive blodkulturer i perioden 01.01.2018 – 31.12.2019 fra 158 unike pasienter, etter gitte inklusjon- og eksklusjonskriterier. Journalgjennomgangen har blitt utført av medisinstudent Kenny Jensen og doktorgradsstudentene Hege Sjåvik og Kristoffer Endresen.
Resultater: Gjennomsnittlig alder ved innleggelse var 74.9 år, og pasientene var i gjennomsnitt innlagt 9.5 dager. Hyppigste mistenkte primære infeksjonsfokus var urinveier (31.4%), hyppigste forekommende species i blodkultur var Escherichia coli (31.7%). Vanligste agens ved antatt kontaminering var hvite stafylokokker. Gruppen som mottok antibiotika før blodkultur ble tatt, fikk dette i gjennomsnitt 24.40 t før prøvetaking. Gruppen som mottok antibiotika etter blodkultur, fikk dette i gjennomsnitt 8.95 t etter prøvetaking. Tid fra blodkultur ble samlet til speciesbestemmelse var i gjennomsnitt 68 t. Tid fra blodkultur ble tatt til ankomst Nordlandssykehuset i Bodø var 56.1 t. Gjennomsnittlig tid fra blodkultur ble tatt til første endring i antibiotika basert på species- eller resistensbestemmelse var 93.7 timer, og gjennomsnittlig tid fra empirisk behandling til første antibiotikaendring var 64.9 timer. Nasjonale faglige retningslinjer for bruk av antibiotika i sykehus ble fulgt i 72.7% av journalene. Av journalene som ikke fulgte retningslinjene, var det 70.5% som ikke begrunnet avviket.
Fortolkning: Funnene i denne studien må tolkes i lys av metodiske begrensninger. Studien belyser utfordringer knyttet til blodkulturer og behandling ved mindre sykehus uten lokale mikrobiologiske laboratorietjenester. Den avdekker både styrker og svakheter i etterlevelse av retningslinjer for bruk av antibiotika. Samtidig illustrerer den hvordan geografiske og praktiske forhold, som transporttid, kan påvirke hvor raskt målrettet behandling kan iverksettes. Resultatene gir også innsikt i potensialet for forbedret forskrivning av antibiotika gjennom bedre logistikk, diagnostikk og økt klinisk støtte. Det blir løftet frem problemstillinger som også kan være relevante for tilsvarende små sykehus.

Exploring the association between self-reported alcohol consumption and intake of micronutrients. An analysis based on data from a general population study: The seventh survey of the Tromsø Study (Tromsø7) in 2015-2016
Background:
Alcohol consumption may affect dietary patterns and nutrient intake, but the association between alcohol use and micronutrient could be investigated further. Understanding how alcohol consumption relates to the intake of micronutrients is important for public health, also in populations with moderate to high alcohol use.
Materials and methods:
This cross-sectional study used self-reported data from the seventh survey of the Tromsø Study (Tromsø7, 2015–2016). A total of 21,069 adults aged 40 years and older were included. Alcohol intake, measured in grams per day, was estimated from a validated food frequency questionnaire (FFQ). In addition, participants were categorized into low-, moderate-, or high/severe-risk groups based on their scores on the Alcohol Use Disorders Identification Test (AUDIT). Micronutrient intake was also derived from the FFQ and calculated using standardized food composition tables. Statistical analyses were conducted using IBM SPSS Statistics version 29.0. Descriptive statistics, Pearson’s correlation coefficients, and linear regression models (both unadjusted and adjusted for total energy intake) were used to examine associations between alcohol consumption and micronutrient intake also taking total energy intake into consideration.
Results:
Participants with higher self-reported alcohol intake also reported greater total energy intake and higher unadjusted intake of several micronutrients. However, after adjusting for total energy intake, alcohol consumption was negatively associated with key nutrients including folate, calcium, phosphorus, vitamin A, and zinc. Iron and magnesium were not significantly associated with alcohol intake in adjusted models. These findings suggest that alcohol may displace nutrient-dense foods when total energy intake is accounted for.
Conclusion:
Greater alcohol intake was linked to higher energy intake and slightly increased unadjusted micronutrient intake. After adjusting for total energy intake, alcohol showed inverse associations with several key micronutrients. This may have implications for dietary quality among frequent drinkers. Further studies are needed to explore potential health outcomes linked to these dietary patterns and whether similar associations are observed in other populations.Background:

Gjennom deres øyne: Et kognitiv-arkeologisk blikk på den slipte bergkunsten i Nordland
Denne avhandlingen utforsker hvordan teorier og perspektiver fra kognitiv arkeologi kan bidra til økt forståelse av det som ansees som Norges eldste bergkunst. I denne oppgaven fokuserer jeg på de slipte helleristninger i Nordland som dateres til eldre steinalder (9000–7000 fvt.), og vil undersøke hvordan menneskets biologi og samhandling mellom mennesker, dyr, og det materielle, påvirket fremstillingen av disse figurene.
Det argumenteres for at bergkunst er mer enn kunstneriske uttrykk, og kan også sees på som en materialisering av menneskenes forhold til dyrene og miljøet de omgav seg med. Det teoretiske utgangspunkter for å oppnå dette er aspekter fra biologisk psykologi, da med spesielt fokus på syn og hukommelse og «Material Engagement Theory». Jeg vil blant annet vise til hvordan visuelle stimuli er særlig knyttet til hvordan vi lærer og skaper minner. Teorien om vår biologi, og hvordan vi samhandler med omverdenene ligger som bakteppe for videre diskusjon av materialet.
Gjennom diskusjonen utforskes ulike perspektiver der dyrefigurene i materialet, som hovedsakelig er gjengitt i naturalistisk form og størrelse, kan ha trigget fysiologiske responser nært knyttet til de ekte dyrene, eller har fungert som «aid-memoires»- visuelle hjelpemidler for bevaring av kunnskap og historier. De kan også være knyttet til ideen om «kostbare-signaler» det det materielle man lager sammen kan styrker gruppesamhold, og hvordan vi også i større grad burde vurdere den aktive handlingen rundt det å skape bergkunsten. Disse teoriene og perspektivene bidrar også til forståelse rundt endringene vi ser i norsk bergkunst ved overgangen fra eldre til yngre steinalder, med økt befolkningstetthet og sosial kompleksitet, som igjen reflekteres i endringer i bergkunstens stil og innhold.
Jeg konkluderer med at den kognitive arkeologien kan gi verdifull innsikt i fortidige menneskers oppfattelse, forståelse, og gjengivelse av dyrene de delte en nær og sammenvevd tilværelse med. Jeg oppfordrer til å se på helleristningene som mer enn symbolske representasjoner, og at bergkunst er et produkt av det å være et agerende menneske som samhandler med hverandre, dyr, og omverdenen gjennom det materielle
Attempting to reduce the number of same-day elective-surgery cancelations using consumer-grade wearable devices and kernel density estimation
As people live longer and have increasingly sedentary and unhealthy lifestyles the pressure on the healthcare sector will be increasing in the future. To cope with this increased demand, technological solutions are being tested and implemented with the aim of offsetting some of the added workload. One of these new technological solutions is the use of consumer-grade wearable devices and machine learning to create sickness prediction models which can detect sickness, even ahead of symptom onset or in asymptomatic cases. Prior studies have shown that this combination can detect different sicknesses including COVID-19, Malaria, H1N1 and Rhinovirus. At the same time, studies have shown that several elective surgery cancellations are done on the day of surgery due to the patient being too sick to undergo the procedure, leaving the surgery room, equipment and staff underutilized. In this thesis I therefore implement a prototype alert system using a sickness prediction model based on kernel density estimation and data from a wearable device to predict sickness ahead of time. When testing the model on a publicly available dataset collected using wearable devices, the model was able to detect most sick users but struggled with a large rate of false positives, leading to an investigation of the causes and recommendations for future models
Associations of Alcohol with Body Composition, Physical Function and Mortality in Russia: The Know Your Heart Study
Alcohol consumption significantly affects health, but its relation to body composition, physical function, and mortality remain insufficiently understood. This thesis explores these associations in Russia, a country with high alcohol consumption and related health burdens.
Using data from the Know Your Heart study (Arkhangelsk, Russia, 2015–17), we analyzed records of 4,372 participants aged 35–69 from the general population and 272 in-patients treated for alcohol-related disorders (narcology patients). For the general population, alcohol consumption was categorized into non-drinking, non-problem drinking, hazardous drinking, and harmful drinking.
Compared to non-problem drinkers, hazardous drinkers had higher waist circumference, waist-to-hip ratio, and body fat percentage. Harmful drinkers and especially narcology patients had lower body mass index and other adiposity-related body composition parameters. Harmful drinking was associated with reduced physical performance, particularly in grip strength and chair rise tests. Narcology patients had a substantially higher 6-year mortality compared to non-problem drinkers, with hazard ratios of 3.2 for death from any cause, 3.3 for cardiovascular causes, 9.4 for external causes, and 7.8 for other causes. Compared to the general population, conventional risk factors were less predictive of mortality in narcology patients. Higher education increased cardiovascular mortality risk in this group, while the effects of male sex, smoking, and cardiovascular biomarkers were lower.Bruk av alkohol påvirker helsen i betydelig grad, men effekter på kroppssammensetning, fysisk funksjon og dødelighet er fortsatt utilstrekkelig forstått. Denne avhandlingen utforsker disse forholdene i Russland, et land med høyt alkoholforbruk og relaterte helsebelastninger. Ved å bruke data fra Know Your Heart-studien (Arkhangelsk, Russland, 2015–17), analyserte vi registreringer av 4372 deltakere i alderen 35–69 fra den generelle befolkningen og 272 in-patients behandlet for alkoholrelaterte lidelser (narkologiske pasienter). I den generelle befolkningen ble alkoholforbruk kategorisert i ikke-drikking, ikke-problemdrikking, hasard-drikking og skadelig drikking. Sammenlignet med ikke-problemdrikkere hadde hasard-drikkere høyere midjeomkrets, midje-til-hofte-forhold og kroppsfettprosent. Skadelig drikkende og spesielt narkologiske pasienter hadde lavere kroppsmasseindeks og andre fettrelaterte kroppssammensetningsparametere. Skadelig drikking var assosiert med redusert fysisk ytelse, spesielt i grepsstyrke og stolhevingstester. Narkologiske pasienter hadde en betydelig høyere 6-års dødelighet sammenlignet med ikke-problemdrikkere, med hasardrate 3,2 for død uansett årsak, 3,3 for kardiovaskulære årsaker, 9,4 for eksterne årsaker og 7,8 for andre årsaker. Sammenlignet med den generelle befolkningen, var konvensjonelle risikofaktorer mindre prediktive for dødelighet hos narkologiske pasienter. Høyere utdanning økte risikoen for kardiovaskulær dødelighet i denne gruppen, mens effekten av mannlig kjønn, røyking og kardiovaskulære biomarkører var lavere
Fra tillit til trussel - En kvalitativ studie av norske bankers håndtering av digitalt bedrageri i kontekst av sosial manipulasjon
Bedrageri gjennom bruk av sosial manipulering har økt i både omfang og profesjonalitet, og utgjør i dag en alvorlig samfunnsutfordring. Formålet med denne studien er å undersøke hvordan norske banker arbeider for å forebygge og bekjempe digitalt bedrageri av privatpersoner. Studien tar sikte på å bidra til økt forståelse og gi innsikt som kan brukes til praktisk anvendelse. Dette undersøkes gjennom problemstillingen:
Hvordan jobber norske banker for å bekjempe bedrageri basert på sosial manipulasjon?
For å besvare problemstilling ble det gjennomført 9 intervjuer med aktører fra bank, politiet og Økokrim. Studien tar utgangspunkt i et teoretisk rammeverk som inkluderer prinsipal–agent-teori og stakeholder-teori for å analysere hvordan banker forebygger digitalt bedrageri, mislighetstriangelet for å undersøke årsakene til bedrageri, og atferdsmodeller for å forstå hvordan kundene reagerer på sikkerhetstiltak og det å bli utsatt for svindel.
Banker og myndigheter tilpasser seg et trusselbilde i rask utvikling. Funn viser at effektiv svindelbekjempelse krever mer enn teknologiske barrierer. Det avhenger av kontinuerlig opplæring av kunder, godt samarbeid på tvers av aktører og bevissthet rundt balansen mellom sikkerhet og brukervennlighet. Videre viser funnene at kundene, selv med bedre erstatningskrav etter å ha blitt svindlet, fortsatt er forsiktige og ikke tar lettere på å unngå svindel.Fraud through the use of social engineering has increased in both scope and sophistication, and today represents a serious societal challenge. The purpose of this study is to examine how Norwegian banks work to prevent and combat digital fraud targeting individuals. The study aims to contribute to increased understanding and provide insights that can be applied in practice. This is investigated through the research question:
How do Norwegian banks work to combat fraud based on social engineering?
To answer the research question, 9 interviews were conducted with representatives from banks, the police, and Økokrim. The study is based on a theoretical framework that includes principal–agent theory and stakeholder theory to analyze how banks prevent digital fraud, the fraud triangle to investigate the causes of fraud, and behavioral models to understand how customers react to security measures and being exposed to fraud.
Banks and authorities are adapting to a rapidly evolving threat landscape. Findings show that effective fraud prevention requires more than technological barriers. It depends on continuous customer education, strong collaboration across stakeholders, and awareness of the balance between security and user-friendliness. Furthermore, the findings indicate that customers, even with improved compensation rights after being defrauded, remain cautious and do not take fraud prevention lightly
Hvorfor klarer ikke jeg? - En kvalitativ studie av sosial sårbarhet og emosjonelle utfordringer hos jenter med ADHD i ungdomsårene - i en skolekontekst
Sammendrag
Denne masteroppgaven undersøker hvilke sosiale og emosjonelle utfordringer jenter med ADHD står overfor i ungdomsårene, og hvordan disse kommer til uttrykk i skolehverdagen. Studien bygger på kvalitative dybdeintervjuer med tre unge kvinner diagnostisert med ADHD, og belyser deres erfaringer med å vokse opp med en usynlig og ofte misforstått vanske. Funnene viser at jentene i stor grad opplever å ikke bli sett eller forstått, og at symptomene deres ofte blir oversett eller tolket feil – noe som kan føre til sen diagnostisering, lav mestringsfølelse og psykisk uhelse.
Gjennom en tematisk analyse identifiseres sentrale mønstre knyttet til selvforståelse, sosial sårbarhet, skolefaglige utfordringer og opplevelser av tilrettelegging. Oppgaven drøfter funnene i lys av Vygotskijs sosiokulturelle læringsteori og selvbestemmelsesteorien, og viser hvordan manglende støtte i læringsmiljøet kan hindre utviklingen av indre motivasjon, tilhørighet og mestringstro. Studien synliggjør behovet for økt kunnskap om kjønnsforskjeller i ADHD, samt betydningen av relasjonell støtte og fleksibel pedagogikk i skolen.
På bakgrunn av funnene anbefales tidligere identifisering, større bevissthet blant lærere og et læringsmiljø som ivaretar jentenes behov for anerkjennelse, autonomi og sosial trygghet.Abstract
This master’s thesis explores the social and emotional challenges experienced by adolescent girls with ADHD, with particular attention to how these difficulties manifest within the school context. The study is based on qualitative in-depth interviews with three young women diagnosed with ADHD, offering insights into their experiences of growing up with an often misunderstood and invisible condition. The findings reveal that the participants frequently felt overlooked or misinterpreted, which contributed to delayed diagnoses, diminished self-esteem, and emotional distress.
Through a thematic analysis, key patterns are identified related to self-understanding, social vulnerability, academic struggles, and experiences of educational support. The discussion draws on Vygotsky’s sociocultural learning theory and Self-Determination Theory (SDT), highlighting how insufficient support in school environments may hinder the development of intrinsic motivation, belonging, and academic self-efficacy. The study emphasizes the importance of greater awareness of gender differences in ADHD and the role of relational support and flexible pedagogy in meeting girls’ educational needs.
Based on these findings, the thesis recommends earlier identification, enhanced teacher awareness, and the creation of supportive learning environments that foster recognition, autonomy, and social safety for girls with ADHD
Holdninger til snus blant medisinstudenter ved UiT, en oppfølgingsstudie fra 2. til 4. studieår
Bakgrunn – Snusbruk er blitt den vanligste formen for tobakksbruk i Norge, og utviklingen kan likne den kjente røykeepidemien i forrige århundre. Det finnes lite informasjon til leger om hvordan de skal forholde seg til den nye tobakkstrenden, grunnet magert kunnskapsgrunnlag. Hva leger anbefaler pasienter kan derfor i større grad være basert på deres holdninger til snusbruk. Vi ønsker med dette å undersøke hva medisinstudenter, som både unge voksne og fremtidige leger, tenker om snus.
Formål – Denne oppgavens hovedformål er å undersøke holdninger til snus, og om de har endret seg hos medisinkull 20 fra 2. til 4. studieår ved medisinstudiet på UiT. Vi ønsker også å undersøke hva som kan forklare en eventuell holdningsendring blant studentene.
Metode – For å undersøke formålet vårt har vi gjennomført en oppfølgingsundersøkelse med utgangspunkt i to tverrsnittsundersøkelser utført på samme populasjon med to og et halvt års mellomrom. Vi inkluderte også et til medisinkull ved siste undersøkelsestidspunkt for å se om en eventuell endring skyldtes lengre fartstid ved medisinstudiet. For å analysere dataene våre har vi brukt statistikkprogrammet SPSS.
Resultater – Det var kun et av spørsmålene som viste en signifikant holdningsendring, der 24 prosentpoeng flere av 2.årsstudentene ville anbefalt pasienten å slutte å snuse, sammenliknet med 4.årsstudentene (OR: 0,33, KI: 0,18 til 0,63, p-verdi <0,001). Av de andre spørsmålene var det derimot en svak tendens til at 4.årsstudentene var mer negative til snus, men disse endringene var ikke-signifikante. Vi fant ingen åpenbart mønster som kunne forklare at de observerte holdningsendringene skyldtes mer eller mindre medisinutdanning. Forekomsten av snusbruk hos medisinstudentene hadde en liten økning fra 8% til 11%, men var fortsatt lav sammenliknet med den øvrige studentmassens forekomst på 19%.
Konklusjon – Vi fant lite holdningsendring til snus blant medisinstudenter fra 2. til 4.studieår ved UiT. Funnet er basert på en liten oppfølgingsundersøkelse over kort tid, og det er dermed usikkert hvorvidt det har vært en holdningsendring til snus i løpet av studiet. For å undersøke formålet vårt bedre trengs større utvalg med lengre oppfølgingstid
Function of tertiary lymphoid structures in autoimmune disease – animal study model
Full text not availableSystemic lupus erythematosus (SLE) and lupus nephritis (LN) is an autoimmune disease characterized by autoantibody production, immune complex formation, inflammation, and infiltrating immune cells. In this thesis the role of T cells in the initiation of tertiary lymphoid structure formation during the development of SLE and LN was investigated in a spontaneous lupus mouse model and in an imiquimod (IMQ) induced model. By using gene expression analyses (qPCR) and immunohistochemistry (IHC) and multicolored IHC (OPAL) we demonstrated an upregulation of both proinflammatory (Il-6) and anti-inflammatory (Il-10) cytokines in both models. Other genes of interest (Tnfsf11 and Ctla4) were shown to be upregulated in the antibody positive group and in relation to activation of T cells and other immune cells. Infiltrating CD3 cells were shown to increase during the development of SLE and LN and most of them were in aggregates resembling TLS or located within large functional TLS. Using Opal IHC, we showed an increased infiltration of PSGL-1 positive CD4 T cells in the TLS. These cells could be detected early within the pelvic wall of young mice. In conclusion, the early detection of specialized T cells within the kidney of lupus prone mice indicates an important role of these cells in the initiation and propagation of TLS
Adopting Arctic Tourism: The case of Kirkenes Amid Geopolitical and Environmental Challenges
This thesis explores how the tourism industry in Kirkenes, a Norwegian border town in the Arctic, has adapted to contemporary geopolitical disruptions—most notably the consequences of the Russia-Ukraine war. Historically reliant on Russian visitors and situated near a politically sensitive border, Kirkenes faced substantial challenges following the collapse of cross-border mobility and increasing geopolitical tensions. Using a qualitative research design based on semi-structured interviews with tourism stakeholders and informed by academic literature across tourism resilience, destination branding, and curiosity-driven tourism, this study investigates how local actors have reimagined Kirkenes as a Finnish-Arctic destination. It analyses the impacts of reduced Russian visitation, the rise of "border curiosity tourism," and the strategic realignment with Finnish tourism networks. The research applies resilience theory to highlight how stakeholders leverage diversification, cross-border collaboration, and narrative reconstruction to maintain economic viability in a volatile regional context. The findings reveal that while tourism actors have embraced adaptive strategies and alternative branding, broader community resilience is constrained by the fragility of other non-tourism sectors. Ethical reflections are interwoven, particularly regarding the anonymity of informants and the implications of studying tourism connected to geopolitical conflict. Using a qualitative research design based on semi-structured interviews with tourism stakeholders and informed by academic literature across tourism resilience, destination branding, and curiosity-driven tourism, this study investigates how local actors have reimagined Kirkenes as a Finnish-Arctic destination. It analyses the impacts of reduced Russian visitation, the rise of "border curiosity tourism," and the strategic realignment with Finnish tourism networks. The research applies resilience theory to highlight how stakeholders leverage diversification, cross-border collaboration, and narrative reconstruction to maintain economic viability in a volatile regional context. The findings reveal that while tourism actors have embraced adaptive strategies and alternative branding, broader community resilience is constrained by the fragility of other non-tourism sectors. Ethical reflections are interwoven, particularly regarding the anonymity of informants and the implications of studying tourism connected to geopolitical conflict. Ultimately, the thesis contributes to scholarship on Arctic tourism by offering localised insights into how small peripheral destinations navigate uncertainty through innovation, repositioning, and strategic partnerships
