    Combined treatment with vemurafenib and cobimetinib in Langerhans cell histiocytosis and Erdheim-Chester disease overlap syndrome: A case Report

    Histiocytoses are clonal disorders diseases derived from the monocyte-macrophage lineage. The Erdheim–Chester Disease (ECD) and Langerhans Cell Histiocytosis (LCH) may occur in association with overlapping clinical, histopathological and molecular features, harboring somatic MAP2K1 mutations in more than 50% of patients. BRAF and MEK inhibitors have shown to be efficacious in ECD and LCH, including responses in patients with CNS involvement.This case report describes a 59-year-old woman who presented with vemurafenib-refractory ECD/LCH overlap syndrome treated with vemurafenib/cobimetinib dual therapy, with rapidly progressing neurological involvement after its initiation.Although targeted therapy plays a crucial role in the treatment of histiocytosis, only anecdotal clinical cases treated with dual therapy have been reported in ECD or LCH and collaborative trials are needed to improve outcomes.</p

    Evaluation of agrobiodiversity and its trophic interactions as an indicator of sustainability in productive systems

    Agriculture has intervened, modified, and simplified ecosystems to obtain some goods and services. Conventional o industrial agriculture emphasizes the use of external inputs and the homogeneity of the landscape; agroecology promotes biodiversity to maximize biological interactions and their ecosystem services. The objective was to evaluate agrobiodiversity and its trophic interactions as an indicator of sustainability by comparing: industrial productive systems, and agro-ecological and semi-natural sites in the southern Pampas region. Samples were taken of mammals, birds, arthropods, and vegetation; with different indices, the food chains and the state of the system were evaluated. The results show that agricultural intensification simplifies landscape structure with a loss of biodiversity and the absence of functional groups (herbivores and nectarivorous). In agroecological wheat, the highest density of links was found and in agroecological pastures the highest grouping coefficient, this reveals greater cohesion and integration among the components of the system and more mechanisms of self-regulation. The agroecological diversity Index showed no differences between the management, this could be related to the presence of a biological corridor in the conventional field that would be increasing biodiversity. The key to achieving sustainable agroecosystems is to procure biodiverse landscapes with patches and corridors of shrub and herbaceous species.</p

    The sweet potatoes new genotypes introduction in Ukraine

    Aim: Conduct research on the collection of new genotypes of sweet potatoes introduced under growing conditions in the Kharkiv district of Ukraine.Results and discussion: In the conditions in the Kharkiv district of Ukraine, an assessment was made of 13 genotypes of sweet potato a domestic and foreign selection. The genotypes were very different in length of the growing season, biometric parameters of plants, and productivity. The studied genotypes were divided into three ripeness groups. Samples that characterized the three groups - early ripening: V-1, V-6, А-7, В-4; medium ripening: Оr-3, D-2, М-8; late-ripening: B-9, P-11, B-10, J-12, Е-5, Оk-14. A wide range of shapes was obtained along the length of the stem. All genotypes that entered the mid-ripening group formed long, climbing stems. So, plants of the genotype Or-3 formed long stems similar to vines, 214 cm long. The genotypes of the early ripening group were characterized by lengths from 144 cm (V-6) up to 153 cm (А-7). According to the “Quantity of additional shoots” attribute, the variability range was 4–8 cm, and the coefficient of variation was 17%. Significant variation was observed along the length of the internodes (V = 30%). The variability range was 4.39–9.97 cm. According to the “Quantity of leaves” characteristic, the variability range was 40–77 pcs. And the coefficient of variation was 20%. The genotypes of the middle ripening group that had the largest “Quantity of leaves” were 77 pcs./plant (Оr-3) and 71 pcs./plant (D-2). The smallest was 40 pcs./plant (B-10), 42 pcs./plant (B-9), 44 pcs./plant (P-11). The root tubers in genotype V-6 had an oval shape, smooth peel, and pink color. The flesh was a creamy solid color; it was a sample of sweet potato. The shoots were long, climbing, leaves of a dark green color without anthocyanin color, kidney-shaped. The genotype of sweet potato D-2 of Ukrainian selection was a dessert variety. The tubers of the genotype were elliptical, orange with a smooth peel; the flesh was firm and bright orange. The shoots of the plants were very long, very climbing, and light green. The shoots of the leaves were kidney-shaped, light green. The results showed that table genotypes are of greater value for dietary nutrition. Dessert genotypes are conducive to a variety of diets for people with diabetes. A list of genotypes that can be used by people who follow a diet has been provided.Conclusion: In the conditions in the Kharkiv district of Ukraine, 13 introduced genotypes of sweet potato domestic and foreign breeding were evaluated. They already discovered that the genotypes were different in the growing season by biometric parameters of plants and yields. By using clonal selection in vitro culture, two completely new promising genotypes of domestic selection of sweet potatoes V-6 and D-2 were obtained. Tubers of genotype V-6 were oval, skin smooth, and pink. The average weight of one was 351 g. The raw flesh was cream-colored. Long shoots were woven, their length was 144 cm; the leaves were dark green, and kidney-shaped. Another example of a domestic selection of sweet potato D-2 was a dessert type. The root tubers were elliptical and had an orange color with smooth skin. The average weight of the tubers was 410 g. The raw flesh was a solid bright orange color. Shoots were extremely long -198 cm, very creeping, and had a light green color. The leaves were kidney-shaped and light green.</p

    Diabetes and COVID-19: Biological profile of type 2 diabetic patients with COVID-19 in Pointe-Noire, Congo

    Introduction: COVID-19 is new pneumonia caused by SARS-CoV-2 infection. Several factors of bad prognosis have been implicated including diabetes. Several poor prognostic factors have been associated with the severity of this disease, including diabetes. In Congo, no study to date has investigated the profile of diabetics hospitalized for COVID-19. The aim of this study was to evaluate the biological profile of Congolese diabetic patients admitted to the hospital for COVID-19 in Pointe-Noire.Materials and methods: This was a prospective study conducted among patients hospitalized for COVID-19 in Pointe-Noire between March and November 2020. Sociodemographic, clinical and biological data, duration of hospitalization, and viral load were investigated. A total of 84 COVID-19 patients have been admitted to Louise Michel Clinic and the Adolph Sice General Public Hospital. Two groups were formed: diabetic patients and non-diabetic patients based on the history of antidiabetic medication or fasting plasma blood glucose levels at admission. Results between the two groups were compared.&nbsp;Results: Out of 84 COVID-19 patients, 48 were diabetic (mean age: 48.50 ± 11.98 years) versus 36 non-diabetic (mean age: 45.56 ± 8.48 years). Significant increases in fasting blood glucose, D-dimers, white blood cells, low oxygen saturation (SaPO2), and higher mortality was observed were observed in COVID-19-positive diabetics when compared to non-diabetic patients(p &lt; 0.02). However, no significant differences were observed between the two groups in terms of clinical symptoms.&nbsp;An increased risk of death was associated with higher levels of D-dimers and HbA1c at admission in the diabetic group.Conclusion: An increase in D-dimer levels and high blood glucose levels at admission increased the risk of death in diabetic patients with COVID-19 in Pointe-Noire.</p

    Primary Cutaneous Anaplastic Large Cell Lymphoma (PC-ALCL) is a CD30+ lymphoproliferative disease of the skin characterized by single or focal nodules or plaques that ulcerate over time. Diagnosis of PC-ALCL relies heavily on clinicopathological correlations because of the potential morphological, clinical, and molecular overlap with other cutaneous CD30+ LPDs. Histopathologic features include diffuse nonepidermotropic infiltrates with an adherent layer of large undifferentiated CD30+ tumor cells.The Exact incidence of PC-ALCL is not known partially because of the difficulty differentiating from the variety of CD30+ Lymphoproliferative disorders. Only a few cases are reported even from developed countries &amp; reports from developing countries are lacking.We present a possible case of primary cutaneous Anaplastic Large cell Lymphoma in a 20 years old Female patient who presented with a 2 months history of lateral neck swelling.</p

    Meat, milk and eggs are nutrient-rich products that could effectively boost nutrient-poor diets, either as part of the normal diet or if access is increased through enrichment with vitamins and minerals, among others. Scientific evidence on the role of livestock products in improving nutrition is limited, especially in low-and middle-income countries, during the first 1,000 days of a child's life. Beyond food production, the livestock sector has additional positive and negative impacts on human health, the environment, societies and economies that must be understood and managed.</p

    Intrapulmonary percussive ventilation for children with bronchiolitis on non-Invasive Ventilation support

    Objective: Pediatric Intensive Care (PICU) admission of children with bronchiolitis as well as the use of Non-Invasive Ventilation (NIV) are increasing. The current treatment for bronchiolitis is supportive, and there are no specific studies addressing this group of severe bronchiolitis patients supported with NIV. Intrapulmonary Percussive Ventilation (IPV) is a lung recruitment physical therapy technique used in our PICU to augment lung aeration and improve gas exchange. We hypothesized that IPV treatment can be used to improve the clinical course of infants on NIV support suffering from bronchiolitis.&nbsp;Design: A prospective, open, randomized study.Setting: Single-center Pediatric ICUPatients: Children less than 2 years old admitted to our PICU between November 2016 and April 2018 with a diagnosis of bronchiolitis who were prescribed noninvasive positive pressure ventilation as their sole respiratory treatment modalityInterventions: Patients were randomly assigned to two intervention groups: IPV vs. control (standard treatment).Measurements and main results: Thirty-eight infants with bronchiolitis treated with NIV support were randomized into two groups. The probability of a superior outcome (less chance of invasive mechanical ventilation and fewer PICU days) was 62.7% (95% CI, 45%-77%, p = 0.18) in the IPV group compared to the control group. Among the IPV group, there were no failures that required intubation in comparison to three intubations (13.6%) among the control group (p = 0.24). For the IPV group, the PICU length of stay (LOS) was 4.13 ± 2.45 days, compared to 6.18 ± 4.72 for the inhalation group. This difference was not statistically significant.Conclusions: In this single-center study, the use of IPV had no adverse reactions. The study failed to show a statistically significant effect of IPV treatment on the course of hospitalization of patients with bronchiolitis on NIV support in the PICU.Trial registration: Clinical Trials.gov NCT03037801.</p

    Comparison of complication and success rates of perclose proglide device with surgical cut down in patients undergoing TAVI and TEVAR/EVAR Procedures

    Background: The Common Femoral Artery (CFA) is the most frequently used peripheral artery for Trans-catheter aortic valve implantation (TAVI) and Endovascular Aortic Repair (EVAR)/ Thoracic Endovascular Aortic Repair (TEVAR) procedures. CFA access hemostasis could be obtained by manual compression, surgical cut-down, or using arteriotomy closure devices, including ProGlide.Method: During a retrospective cohort study we compared ProGlide with surgical cut-down hemostasis in 225 patients who underwent TAVI or EVAR/TEVAR, including 290 access sites, during a 10 years period in terms of access site complications, procedure length and post-procedural hospitalization duration.Results: The success rate of hemostasis was 100% in the PP device group and 98.3% in the SCD group. The mean Procedure length was significantly shorter in ProGlide device hemostasis and the mean post-procedural hospitalization length had a non-significant difference between the two groups. Access site complications occurred in 21.1% of the ProGlide group and 26% of the SCD group.&nbsp;Conclusion: Perclose ProGlide device is safe and effective for access site closure in procedures that need large sheaths and it is non-inferior to standard surgical cut-down. Training, experience and careful application of the device have significant importance in ensuring successful hemostasis.</p
