The coordination of public finance and the tax system is one of the subjects requested by ordinary-statute Regions advocating for differentiated regionalism. This paper analyzes how the autonomous Regions of Valle d’Aosta and Friuli-Venezia Giulia, as well as the autonomous Provinces of Trento and Bolzano, manage local public finance and local taxation based on the powers granted to them by their respective special statutes. Specifically, three topics are examined. The first concerns local property taxation, where the Provinces of Trento and Bolzano and the Region of Friuli-Venezia Giulia have each replaced the national IMU tax with their own local tax. The second topic is the tourist tax, where each autonomous Region and Province has introduced its own tax on tourist stays, addressing some of the drawbacks of the national tourist tax. Finally, the third topic is local public finance, where the autonomous Regions and Provinces allocate transfers for current and investment expenditures to their municipalities according to their own criteria and mechanisms
From Triportal to Uniportal Video-Thoracoscopic Lobectomy: The Single Surgeon Learning Curve by CUSUM Chart and Perioperative Outcomes
Background: Uniportal video-thoracoscopic lobectomy has improved postoperative out- comes in lung cancer patients. Thus, thoracic surgeons are increasingly required to learn this new approach. Methods: We evaluate the path of a single surgeon switch- ing from triportal video-thoracoscopic lobectomy to the uniportal, using the cumulative sum (CUSUM) analysis, in a single center to assess the learning curve, enrolling 107 unipor- tal video-thoracoscopic lobectomies consecutively performed. CUSUM analysis detected how many uniportal video-thoracoscopies occur to obtain changes in mean operation time, among all procedures consecutively performed. CUSUM analysis identified the cut-off at the 67th procedure; this value was used to divide all patients into two groups: group A (first 67 patients, early phase) and group B (40 patients, experienced phase). Then, we analyze the perioperative outcomes between the two groups. Results: Gender characteristics of the two groups were statistically similar. Median operative time de- creased significantly after the early phase [188 min (IQR: 151–236) vs. 170.5 (IQR: 134–202) (p-value = 0.02)], respectively. Similarly, during the second phase, the conversions rate de- creased: [10 (15%) (group A) vs. 1 (2%) (group B) (p-value = 0.04)], as did the postoperative complications [28 cases (42%) vs. 9 cases (22%) (p-value = 0.04)] and the length of stay [6 days (IQR 5–9.5) vs. 5 days (IQR 4–8) (p-value = 0.04)], giving evidence of skills acquired in the second phase. Conclusions: CUSUM analysis identified 67 uniportal lobectomies, after which operative time, conversion rate, and perioperative complications significantly decreased; the moving average analysis further supports a progressive reduction in opera- tive time. Despite prior multiportal video-thoracoscopic experience, switching to uniportal video-thoracoscopy requires a distinct learning process
Sequence-based prioritization of i-Motif candidates in the human genome
Introduction: i-Motifs (iMs) are cytosine-rich, four-stranded DNA structures with emerging roles in gene regulation and genome stability. Despite their biological relevance, genome-wide prediction of iM-forming sequences remains limited by low specificity and high false-positive rates, leading to considerable experimental burden. Method: To address this, we developed a refined computational approach that prioritizes high-confidence iM candidates using a Position-Specific Similarity Matrix (PSSM) derived from multiple sequence alignments. The human reference genome (hg38) was scanned using a custom regular expression targeting cytosine-rich motifs, followed by scoring each sequence with the PSSM. Statistical significance was assessed via permutation testing, one-sided t-tests, Benjamini-Hochberg correction, and Z-scores. Results: This pipeline identified 37,075 candidate sequences (15–46 nucleotides) with strong iM-forming potential. Validation against experimentally confirmed iMs and known G-quadruplexes (G4s) demonstrated significant differences in alignment scores and sequence similarity, confirming structural specificity. A random forest classifier trained on nucleotide features further supported the distinctiveness of the candidates, achieving a high classification performance. Conclusion: This work presents a scalable and statistically robust method to enrich for biologically relevant iM sequences, providing a valuable resource for future experimental validation and the rational design of ligands targeting iMs to modulate gene expression in contexts such as cancer
Changes in clinical presentation, management, and survival outcomes in patients affected by colorectal cancer following COVID-19 pandemic
Background: As an extended analysis of the COVID-DELAY study, we aimed to assess the impact of the COVID-19 pandemic on diagnosis, staging, and survival outcomes among patients with colorectal cancer (CRC) diagnosis performed from 2019 to 2022. Methods: All consecutive newly diagnosed CRC patients referred to 11 Italian Oncology Departments between March and December 2019, 2020, 2021, and 2022 were enrolled. Access rate, demographics, diagnostic-therapeutic temporal intervals, and first-line progression-free survival (PFS) and OS among metastatic patients were assessed. Results: Compared to 2019 (n = 690), an initial global reduction in new CRC cases in 2020 (n = 564, –18.3%) was observed, followed by a progressive increase in new CRC diagnoses in 2021 (n = 748, + 8.4%) and 2022 (n = 756, + 9.6%); a higher rate of TNM stage IV tumors was diagnosed in 2020 (35.4%) and 2021 (31.0%) compared to 2019 (29.6%), with normalization in 2022 (26.4%) (P < .001). Not clinically relevant differences between histological diagnosis and first oncological examination, cytohistological diagnosis and systemic treatment start, first oncological appointment and systemic treatment start, treatment start and first radiological assessment between 2020 and 2021-2022 years were found. After propensity score matching according to the year of diagnosis, median OS was significantly worse in 2020, 2021, and 2022 compared to 2019 (27.6 vs 24.8 vs not reached vs 38.9 months, respectively) (P < .001). Concordantly, the median PFS was significantly worse with each passing year: 13.0 vs 11.1 vs 9.2 vs 7.2 months in 2019, 2020, 2021, and 2022, respectively (P = .00027). Conclusions: A progressive normalization in the rate of new CRC diagnosis as well as TNM stages at diagnosis, in 2021 and 2022 compared to 2020 and 2019, was found. The increase in new CRC cases might have affected some diagnostic-therapeutic time intervals in 2021-2022 years compared to 2020. Significantly, compared to the pre-pandemic phase, pandemic years were independently associated with worse PFS and OS outcomes in patients affected by metastatic disease
