Home-Based Intervention Tool for Cardiac Telerehabilitation: Protocol for a Controlled Trial
Background: Among cardiovascular diseases, adult patients with congenital heart disease represent a population that has been continuously increasing, which is mainly due to improvement of the pathophysiological framing, including the development of surgical and reanimation techniques. However, approximately 20% of these patients will require surgery in adulthood and 40% of these cases will necessitate reintervention for residual defects or sequelae of childhood surgery. In this field, cardiac rehabilitation (CR) in the postsurgical phase has an important impact on the patient by improving psychophysical and clinical recovery in reducing fatigue and dyspnea to ultimately increase survival. In this context, compliance with the rehabilitation program is a key element for the therapeutic benefits of the program. The increase of mobile health care devices and software has greatly extended self-care capabilities across the spectrum of health care activities. Moreover, the possibility of telemonitoring the progress of this self-care provides elements of empowerment and awareness of one's state of health. As a branch of telehealth, CR can be optimized and facilitated using remote telemedicine devices. Objective: The principal goal of the Innovation in Postoperative Rehabilitation Training and Monitoring (IPOTERI) study is to design, realize, and test a composite and integrated system for postsurgical rehabilitation therapies at home specialized for cardiac surgery. The secondary aims are to implement the system in a "real-life" context of postcardiac surgical rehabilitation, and to create a data set and a data collection methodology to prototype data analytics algorithms and artificial intelligence techniques for customizing the rehabilitation pathway. Methods: The IPOTERI method consists of a telemonitoring platform that guarantees continuity of postoperative care, an intelligent home station based on an Android app for the patient with a user-friendly interface to record vital signals (electrocardiogram, blood pressure, oxygen saturation, and body weight) and access the planning of rehabilitation activities, and a decision support system that communicates with hospital medical records to transmit alerts and specific support information for the formulation and updating of the treatment and care plan. Results: The pilot test started in June 2023 (protocol number 20406/2021) including 50 patients who will be monitored for 12-14 weeks using the developed platform, as described in the Procedures subsection of the Methods section. Conclusions: The IPOTERI approach, based on the processing of data recorded during the monitoring of telemedicine devices used at home during the postsurgical rehabilitation of a cardiac patient, together with clinical data from the perioperative and postoperative periods could have positive effects on adherence to the rehabilitation program and clinical improvement as well as result in overall improvement of quality of life. A hybrid reduced-order model for segregated fluid-structure interaction solvers in an ALE approach at high Reynolds number
This study introduces a first step for constructing a hybrid reduced-order models (ROMs) for segregated fluid-structure interaction in an Arbitrary Lagrangian-Eulerian (ALE) approach at a high Reynolds number using the Finite Volume Method (FVM). The ROM is driven by proper orthogonal decomposition (POD) with hybrid techniques that combines the classical Galerkin projection and two data-driven methods (radial basis networks, and neural networks/ long short term memory). Results demonstrate the ROM's ability to accurately capture the physics of fluid-structure interaction phenomena. This approach is validated through a case study focusing on flow-induced vibration (FIV) of a pitch-plunge airfoil at a high Reynolds number (Re=107)
Don’t be afraid to breathe: an open social innovation case to face grand challenges
Open social innovation has emerged as a pivotal paradigm for
addressing grand societal challenges such as climate change,
inequality, and global health crises. In contrast to traditional open
innovation -which is predominantly firm-centric and marketoriented-
open social innovation underscores the importance of
inclusive, collaborative, and cross-sectoral efforts aimed at generating
solutions with meaningful societal impact. This study investigates
the distinctive characteristics and practices of open social
innovation by exploring an open social innovation process that
was successful during the COVID-19 pandemic. Drawing on
a comprehensive qualitative analysis of interviews, observations
and archival data with many stakeholders involved in the open
social innovation process, our findings highlight four key practices
of open social innovation: 1) decentralising external search; 2) flexing
existing resources, 3) co-creating value with communities, and 4)
combining intellectual property rights with open-source systems.
These practices differ significantly from traditional open innovation,
particularly in their focus on inclusivity, spontaneity, and shared
societal goals. The study contributes to the literature by demonstrating
how open social innovation transcends conventional, firmcentric
innovation frameworks. It highlights the capacity of open
social innovation to foster systemic change through voluntary,
trust-based, and purpose-driven multi-stakeholder engagement.
Furthermore, it provides new insights into how such practices can
address complex, grand challenges, offering both theoretical and
practical implications for innovation management

I progressi della medicina hanno rivoluzionato la gestione di molte condizioni, trasformando malattie un tempo fatali in patologie croniche. Le malattie croniche, caratterizzate da persistenza e ricorrenza ciclica, pongono una sfida multidimensionale che va oltre i sintomi fisici, influenzando gli aspetti psicologici, sociali e identitari della persona. Alterazioni nelle abitudini quotidiane, nelle relazioni e nella percezione di sé possono ridurre significativamente la qualità della vita di pazienti e famiglie. La ricerca ha evidenziato il ruolo chiave di fattori psicosociali, come supporto sociale, strategie di coping e virtù quali speranza, coraggio e saggezza pratica, nel favorire un adattamento positivo. Tuttavia, l’utilità dello humor in questo specifico contesto è ancora poco studiata, nonostante sia riconosciuta la sua importanza nella regolazione emotiva e nella ristrutturazione della percezione della malattia. Il presente lavoro si pone lo scopo di colmare questa lacuna, esplorando il valore specifico di questo tratto nell’esperienza della cronicità

PET and Cardiac Amyloidosis
: PET has recently demonstrated promising capabilities in the diagnosis and differentiation of various forms of CA. Tracers labeled with 18F, such as 18F-flutemetamol, 18F-florbetapir, and 18F-florbetaben, are being increasingly researched due to their extended half-life, eliminating the requirement for on-site cyclotrons. Unlike bone tracers, PET amyloid-binding tracers exhibit a higher affinity for light-chain fibrils, potentially enabling accurate differentiation between various types of CA. Communicate the environment and improve satisfaction and awareness in public stakeholders, the case study of a public environmental regional agency in Italy
Environmental regional agencies (ARPA) in Italy are public bodies responsible for monitoring, processing, and disseminating environmental data. In recent years, the significance of environmental and climate protection has markedly increased in public consciousness. A growing awareness of environmental impact and aspects is evident among stakeholders. These agencies play a pivotal role in disseminating environmental information, enhancing environmental awareness, and improving communication about environmental issues among citizens.
This study delves into the case of an ARPA, adopting a mixed-method approach that combines quantitative survey analysis and qualitative content analysis to assess stakeholder perceptions. The agency considers it imperative to consistently monitor the expectations, needs, and assessments expressed by its stakeholders regarding the agency's services. The results of such monitoring serve as a foundation for activity planning and the identification of organizational and management solutions, aiming to enhance the overall communication system.
The originality of this study lies in its integrated evaluation framework, which not only examines stakeholder satisfaction but also identifies potential gaps in environmental communication strategies. By offering an in-depth analysis of the interaction between public institutions and stakeholders, this research contributes to the broader discourse on public engagement in environmental governance, providing practical insights for improving institutional transparency and communication effectiveness
