Targeting hepatic immunometabolism in type 1 diabetes
Type 1 Diabetes (T1D) results from the loss of insulin-producing pancreatic β-cells, leading to insulin deficiency (ID). ID is characterized by profound metabolic derangements such as hyperglycemia, hypertriglyceridemia, and hyperketonemia, the latter of which can result in the life-threatening condition of diabetic ketoacidosis. Daily insulin administration is the cornerstone therapy for ID. However, insulin treatment is often sub-optimal due to its narrow therapeutic range. Insufficient insulin can lead to hyperglycemia, increasing the risk of long-term complications, while over-administration raises the danger of life-threatening hypoglycemia. Additionally, insulin's lipogenic effects can cause dyslipidemia. Critically, inadequate insulin levels also put T1D patients at risk of diabetic ketoacidosis. Hence, there is a need for new therapeutic strategies in the clinical management of T1D.
We have previously identified numerous anti-diabetic actions of the Ca2+-binding protein S100A9. Enhancing S100A9 is sufficient to improve hyperglycemia and double the lifespan of ID rodents. It also exerts potent lipid-improving effects, notably the normalization of diabetic ketogenesis via acting on Toll-Like Receptor 4 (TLR4) signaling.
These findings prompt further studies aimed at evaluating the therapeutic potential of S100A9-based treatment. The goal of my PhD thesis is to further explore S100A9’s cellular and molecular underpinnings to unveil novel insulin-independent mechanisms of metabolic regulation.
We recapitulated ID in a murine model by administering Diphtheria Toxin (DT) injections in a mouse model with selective expression of the DT receptor in pancreatic β-cells (RIP-DTR mice). Treatment with recombinant S100A9 improved adherence to glycaemic targets with only half as much insulin and without any life-threatening episodes of hypoglycemia. The glucose-lowering action of S100A9 is due to an enhancement of glucose uptake in the skeletal muscle via TLR4, without any changes in endogenous glucose production or insulin sensitivity. Moreover, we uncovered a potent anti-inflammatory action of S100A9 treatment in ID, partly but not entirely mediated via TLR4.
In addition, my thesis project aims to elucidate the effect of S100A9 on the cross-talk between hepatic parenchymal and non-parenchymal cells. Using a Cre-conditional reactivatable TLR4 Knock Out mouse model, we demonstrated the re-expression of TLR4 in the liver-resident Kupffer cells (KCs), is sufficient for S100A9 to rescue hypertriglyceridemia and hyperketonemia. Additionally, proteomic analysis revealed considerable S100A9-induced re-shaping of the KC proteome, affecting processes such as lipid and amino acid metabolism and revealing potential candidates mediating the intra-hepatic communication improving ID-derived dyslipidemia.
Overall, we underscore the significant potential of S100A9 as an adjuvant in insulin therapy, presenting a proof of concept for developing S100A9-based therapeutic approaches. Furthermore, our data reveal pleiotropic insulin-independent therapeutic effects of S100A9 in ID via TLR4-dependent mechanisms (in the skeletal muscle and in the liver) and offer considerable potential for the development of innovative treatments for T1D.</p
Usages politiques du religieux et pratiques parlementaires : le Parlement européen et la Turquie (1979-2009)
Résumé
Cette thèse de doctorat propose de réfléchir à la manière dont la Turquie a été perçue d'une part, et comment son islamité a impacté les décisions politiques prises par le Parlement européen à son propos d'autre part. Elle s’intéresse ainsi aux usages politiques du religieux qui sont faits par les députés européens au sein du dossier turc auprès de la Communauté économique européenne (CEE), puis de l’Union européenne (UE), entre 1979 et 2009. Le corpus de sources étudié est par conséquent principalement constitué des outils parlementaires à la disposition des eurodéputés. Ensemble, ces sources constituent autant de traces des pratiques et des stratégies des parlementaires, qui sous-tendent leur comportement vis-à-vis des relations CEE/UE-Turquie, ainsi que des attentes ou des réticences liées à aux représentations qu’ils se sont faits de la Turquie au fil du temps.</p
Distributional Dynamics and Business Cycles in the Post-War U.S. Economy (1949 - 2020). Towards an Empirical Understanding of Distribution-Growth Cycles
This study approaches the connection between distribution and business cycles by examining the profit shares' as well as the top 1% income and wealth shares’ long-term swings and patterns of movements over cycles in the post-war U.S. It is assessed that the primarily the profit share is subject to considerable fluctuations over business cycles, with the dominating pattern of the latter changing over time, along with the long-term trends of the distributional indicators that take the form of a ‘U-shape’ in the timeframe under consideration The majority of expansions from 1949 to 1970 were ‘wage-favouring’, while from 1982 to 2020, the majority of expansions were ‘profit-favouring’, with ‘wage-favouring’ phases preceding recessions. A widely based fundamental empirical analysis is used to operationalise and analyse concepts from ‘Classical’, ‘Keynes-Minskyan’, and ‘Kalecki-Steindlian’ theories as well as the interaction between functional income and purpose of expenditure in order to explain these phenomena. The findings suggest that, apart from the 1970s, under-investment was the dominant endogenous driver of output stagnation that both depends on and shapes distributional outcomes; however, not because of insufficient capital. Prior to 1970, persistent high investment was discouraged by the equitable institutional environment and declining profitability. After 1982, increasing inequality resulting from ‘profit-favouring’ expansion stages led to accumulating consumer debt and capitalists prioritising liability restructuring as well as speculation on asset price inflation over productive investments. Investment and aggregate demand decline as a result. The study underlines the complexity, significance, and dynamism of the relationship between distribution and economic activity, which is testament to the necessity of research trying to foster an integrative understanding of growth and distribution.</p
Le bon traitement au bon moment. Un nouveau programme pour les personnes en situation d’obésité
Dans le contexte d’une pandémie d’obésité, traiter cette maladie et faire reculer ses complications est un défi de santé majeur. La révolution des nouveaux traitements de l’obésité représente un puissant levier pour le relever à moindre risque. En cascade, la demande d’aide médicamenteuse surinvestie explose. L’impatience du public concerné à bénéficier de cette aide compromet la relation de soins et le projet de santé. Pour sortir de cette impasse, l’équipe interdisciplinaire de l’Unité d’éducation thérapeutique du patient des HUG a mis en place un nouveau programme sur les traitements de l’obésité. S’appuyant sur le partage entre pairs, une approche motivationnelle, une réflexion sur les compétences à développer pour initier le traitement médicamenteux et des apprentissages clés, il vise un engagement soutenu dans les changements comportementaux pour une perte de poids à long terme.Against the backdrop of the obesity pandemic, treating obesity and reducing its complications is a major health challenge. The revolution in the new treatments of obesity represents a powerful tool to meet this challenge with less risk. As a result, the demand for overinvested medications has exploded. The impatience of concerned people to receive such treatments jeopardizes the therapeutic relationship. To find a way out of this predicament, the interdisciplinary team of the Therapeutic Patient Education Unit at the University Hospitals of Geneva has implemented a new therapeutic program for obesity management. It is based on peer sharing, a motivational approach, reflection on the skills to develop to initiate medication, and key learnings, and aims for sustained commitment to behavior change to achieve long-term weight loss

Patients with serious illnesses wish to maintain their autonomy and decide the course of their end of life. The role of healthcare professionals is to assess the patient's understanding of their illness, help them become aware of the progression of their condition, and adapt these conversations according to the patient's emotional state, while providing regular spaces for discussion. Some patients continue to have expectations that may seem unrealistic despite a limited prognosis. The goal is to align treatments with the patient's ' objectives and values, while guiding them toward existential maturation, which represents the necessary process of accepting one's own mortality as a non-traumatic event. This process is built through repeated interactions with the patient, which is why it is crucial to begin these discussions early.Les patients atteints de maladies graves souhaitent conserver leur autonomie et pouvoir décider du déroulement de leur fin de vie. Le rôle des professionnels consiste à évaluer leur compréhension de leur maladie, les accompagner dans la prise de conscience de l’évolution de leur condition et d’adapter les échanges en fonction de leur état émotionnel. Certains patients continuent d’avoir des attentes pouvant sembler irréalistes malgré un pronostic limité. L’objectif est alors d’aligner les soins aux objectifs et valeurs du patient tout en l’amenant vers une maturation existentielle représentant le cheminement nécessaire à l’acceptation de sa propre finitude comme un événement non traumatique. Ce processus se construit lors d’interactions répétées entre le patient, d’où l’importance de débuter tôt ces discussions

Prise en charge des personnes en situation d'obésité
L'obésité est une maladie chronique, multifactorielle et récidivante. Cette pathologie résulte d'interactions complexes entre déterminants biologiques, psychologiques, sociaux, environnementaux et génétiques. La prise en charge en médecine de premier recours nécessite une approche globale, individualisée et interdisciplinaire, centrée sur la personne. L'éducation thérapeutique du patient, le dépistage des comorbidités e la lutte contre la stigmatisation doivent en constituer les piliers. Les traitements médicamenteux et la chirurgie peuvent être complémentaires dans le cadre d'un accompagnement structuré à long terme. le rôle du médecin traitant est déterminant pour restaurer une relation thérapeutique de confiance, soutenir la motivation aux changements comportementaux sur le long terme et coordonner les ressources du réseau.Obesity is a chronic, multifactorial, and relapsing disease. This condition is the result of a complex interplay of biological, psychological, social, environmental, and genetic factors. Management in primary care requires a comprehensive, individualized, and interdisciplinary approach centered on the lived experiences of people living with obesity. Therapeutic patient education, early screening for complications, and addressing weight stigma are essential pillars. Pharmacological and surgical options can be valuable, but only as part of a structured, long-term care plan. The primary care physician plays a key role in rebuilding trust, supporting motivation for long-term behavioral changes, and coordinating care within an interdisciplinary network

S’intéressant à deux marchés illégalisés à Genève, à savoir le deal de rue de cannabis et le nettoyage domestique au noir, cette thèse de doctorat en sociologie vise à décrire comment les acteurs·trices de l’offre et de la demande parviennent à coopérer afin de réaliser des échanges marchands malgré l’illégalisation de ces derniers. De nature qualitative, cette recherche se base sur des entretiens, des conversations, de l’observation et de l’analyse de documents dans la première étude de cas, et des entretiens et de l’analyse de documents dans la seconde.</p
