Gliosarcoma with neuroaxis metastases.
Gliosarcomas are rare tumours of the central nervous system, with a well-known capacity for metastasis. When they metastasise, the dissemination occurs more frequently via the haematogenous route to extraneural sites. Metastasis-spread through the cerebrospinal fluid is extremely rare. We present the case of a 58-year-old man who underwent a gross total resection of a lesion in the left temporal lobe. The histological findings revealed a gliosarcoma and the patient received radiotherapy followed by chemotherapy. Seven months after surgery, while the patient remained neurologically intact, brain and spinal cord MRI revealed tumour recurrence and neuroaxis metastases through the traffic routes of the cerebrospinal fluid. The patient died 8 months after the diagnosis. A PubMed search regarding metastatic gliosarcoma up to June 2015 was also carried out. To the best of our knowledge, this is the first case report of gliosarcoma metastases to the brain and spinal cord leptomeninges
Vitamin D Insuffciency in a Hospital Population: A Photograph from the Laboratory Perspective
NTRODUCTION:
Although vitamin D deficiency is increasingly recognized around the world, there are few studies on the Portuguese reality. This study aims to analyse vitamin D levels in the assays performed in our hospital and their relationship with age, genre, requesting specialty and moment of sample collection.
MATERIAL AND METHODS:
Cross-sectional study of measurements of 25(HO)D performed in our Hospital between June 2012 and November 2014. Included variables: gender, age, requesting specialty, month of sample collection. Vitamin D status classified as: 'Deficiency' (≤ 20 ng/mL), 'Insufficiency' (21 - 29 ng/ml) and 'Sufficiency' (≥ 30 ng/mL).
RESULTS:
We included 5 439 assays; 55.0% from women; the median age was 64.0 years. Sixty per cent had 'Deficiency', 20.7% 'Insufficiency' and 18.9% 'Sufficiency'. We found a negative correlation between age and vitamin D level (p < 0.001). We didn't find differences in vitamin D levels between genres. Nine specialties requested 98% of the assays, namely Nephrology (56.2%). We found differences between specialties based on age and vitamin D level (p < 0.001). Vitamin D levels changed throughout the year, with higher levels in the summer, followed by autumn, spring and winter (p < 0.001). Despite this seasonal fluctuation, vitamin D sufficiency was only present in a minority of assays (27.8% in summer and 9.2% in winter).
DISCUSSION:
Vitamin D deficiency is prevalent in this population, affects individuals of all ages and is not offset by the seasonal variation of sunlight.
CONCLUSION:
Vitamin D deficiency is a real and prevalent problem in our population that needs further attention and action, given its clinical implications

The Bounds Of Education In The Human Brain Connectome
Inter-individual heterogeneity is evident in aging; education level is known to contribute for this heterogeneity. Using a cross-sectional study design and network inference applied to resting-state fMRI data, we show that aging was associated with decreased functional connectivity in a large cortical network. On the other hand, education level, as measured by years of formal education, produced an opposite effect on the long-term. These results demonstrate the increased brain efficiency in individuals with higher education level that may mitigate the impact of age on brain functional connectivity

Carcinoma of the cervix complicating a genital prolapse.
Although uterine prolapse and carcinoma of the uterine cervix are not rare events, their association is very uncommon. The treatment of cervical cancer has been protocolled, but the management of uterovaginal prolapse associated with carcinoma of the cervix is not standardised and therapy strategies vary considerably among authors. Our case reports a 74-year-old patient, admitted to the emergency department with an ulcerated prolapsed uterus. Biopsy of the cervical lesion confirmed a squamous-cell carcinoma. The patient underwent vaginal hysterectomy plus open bilateral iliopelvic lymphadenectomy complemented with radiotherapy with quimiosensibilisation. With this aggressive treatment approach, there was progression of the disease. The authors believe that this case typiaddition to the few published reports
Management of pregnancy after augmentation cystoplasty.
We report a case of successful pregnancy following augmentation ileocystoplasty in a patient with a neurogenic bladder dysfunction. A review of the literature reveals incidences of premature delivery and renal dysfunction. Careful urological monitoring of such patients should result in a successful pregnancy.
