    publisher青森市国立情報学研究所の「学術雑誌公開支援事業」により電子化されました。この研究の目的は、減塩教室の参加者と非参加者の相違を分析し、減塩教室参加者の特性を明らかにすることである。対象はA町の平成17年度住民基本健診結果に基づいた、尿中塩分のハイリスク者270名である。対象者全員に減塩教室の開催通知を行い、参加希望を募るとともに自記式質問紙を郵送し返送を求めた。主な質問項目は、属性、健康習慣及び食習慣、生活習慣改善及び減塩に関する必要性の認識、食塩摂取と生活習慣病との関連や減塩方法に関する知識、減塩実行に関する意欲と自信、健康に関する情報の入手先、近所づきあいの程度について、である。参加群がすべて女性であったため非参加群の女性と比較した。その結果有意な相違があった項目は、職業,減塩に関する必要性の認識,減塩実行の意欲,健康に関する情報の人手先の4項目であった。健康習慣および減塩に関する知識については有意差がなく近似していた。近所づきあいの程度には差がなく、両部とも比較的活発であった。これらの結果からA町減塩教室参加者の特性は、非参加者に比べ、時間に余裕があり、非参加者と同様の健康習慣及び食塩摂取に関する知識を有しながら、減塩改善の認識や意欲はやや高い集団であることが示された。また、健康情報を公的機関や健康教室等から積極的・能動的に得る傾向があり、近所づきあいの活発な集団であることが示された。これらの特性を有した参加者は、減塩教室での学びを家族や地域へ波及させるオピニオンリーダーと成り得、学びの受け手の行動に影響を及ぼす可能性が示唆された。 The purpose of this research is to analyze the difference between participants and non-participants in a salt restriction class, and to clarify the characteristics of the salt restriction class participants. Subjects are 270 high risk persons identified as having elevated urinary salinity in the 2005 fiscal year medical examination of the residents of A town. Notice of the opening of a salt restriction class was issued to all potential members, and a self recording questionnaire was mailed to the participants. From the 270 subjects, approximately 11.9% took park in the class, and approximately 61.8% of questionnaires were returned. The main questionnaire covered, subjects attributes, healthy habits and eating habits, improvement of the lifestyle and recognition of the necessity of salt restriction, the relationship between salt intake and lifestyle illness, a knowledge of salt restriction methods, volition and self-confidence in executing a reduced salt diet, places to obtain information and the degree of interaction with the neighborhood. The results were compared with the woman of the non-participation group. All of the participants were women. It was found that there was significant differences in 4 areas: occupation, recognition of the necessity of salt restriction, execution of salt restriction, and awareness of places to obtain information on health. There was no significance difference between the groups in their knowledge of healthy habits and salt restriction. There was no difference in the degree of interaction with the neighborhood, and both groups were also comparatively active. In comparison to non-participants, it was discovered that the participants of the salt restriction class in A town were a little higher in recognition and volition of reduced salt intake. Participants had the same knowledge of healthy habits and salt intake, but greater will to improve their lifestyle. In addition, it was shown that the participants tended to actively obtain information about health classes and public institutions, and they were active in society within their neighborhood. It is thought that the participants are capable of being opinion leaders who spread their learning from the salt restriction class with their family and region, and thus, the possibility of positively affecting society was indicated

    Management of rehabilitation department with convalescent rehabilitation wards

    publisher青森市国立情報学研究所の「学術雑誌公開支援事業」により電子化されました。Method: We conducted a mail-in questionnaire survey for managers (except physicians) of nationwide medical institutions (473 facilities) that have convalescent rehabilitation wards on the facility's overview, management, rehabilitation staff organization, convalescent rehabilitation wards, and challenges for the future. We received 307 responses (Response rate: 64%). Result: The hospitals that have convalescent rehabilitation wards are characterized by the increased employment of physical therapists, and some hospitals adopt irregular shift-work systems such as holiday work and staggered hours. The opinions on management include the necessity of improved medical performance, ability evaluation of staff, expansion of business, and management strategies. As for the personnel system, there were a lot of requests for placement of middle management as well as formation of nursing teams for inpatients, outpatients, and home guidance. Challenges for the future: Common challenges in convalescent rehabilitation wards include the necessity of training and development, methods for evaluation of services, information sharing, and collaboration with other types of business. Since occupations related to rehabilitation are greatly influenced by national policies of healthcare, medical care, and welfare, it is necessary to train and educate staff to improve their skills and eye to future care and welfare situations

    Research for Nursing Practice of Lymphoedema : Limbs-up

    publisher青森国立情報学研究所の「学術雑誌公開支援事業」により電子化されました。リンパ浮腫ケアの中では患肢の挙上が最も多く実践されている。そのケア効果検証の準備段階として、患者が行っている「患肢の挙上」の実態を把握することを目的として5名のリンパ浮腫患者に半構成的インタビューを行った。その結果、1.対象者の平均年齢は57.2歳、原疾患は乳がん1名、子宮がん4名、リンパ浮腫発症時期は、手術退院直後〜11年の幅があった。リンパ浮腫の病期は、I期2名、II期3名で、患肢部位は、子宮がん術後患者で左下腿のみ2名、左下肢2名、乳がん術後で左上肢1名であった。2.患肢挙上方法:挙上時間は平均7時間半で、高さは5〜13cm、挙上に用いる物品は、綿素材の薄い布団、テンピュール枕等で、患肢を乗せる面を広くする工夫をしていた。患肢挙上中に圧迫療法の併用はなかった。3.患肢挙上効果:手指や手背の浮腫軽減は軽度にある、あるいは不規則ながらあるという「効果あり」は2名。浮腫軽減はないが習慣で挙げている、挙げると気持ちがいいので行っているのは2名、残り1名は、挙上することで合併症を引き起こした。患肢挙上の目的は浮腫軽減だけではなく、また、リンパ浮腫の病期と効果の有無に関係はなかった。 Limbs-up is the most popular care for Lymphoedema patients. The aim of this study is to research the actual circumstances for limbs-up of patients. I interviewed to five patients. In results, Patient's age 57.2 (mean), Breast cancer ; 1 patient, Uterus cancer ; 4 patients. The term of Developing Lymphoedema were 11 years from right after operation. Stages of Lymphoedema were stage one ; 2 patients, stage two ; 3 patients. Part of swollen limbs were left shin ; 2 patients, left femur and shin ; 2 patients, and left arm ; 1 patient. The actual circumstances for limbs-up were 1. Lifting time was a half and 7 hour (mean), 2. Height was 5-13cm, 3. Goods for limbs-up were thin mattress with cotton, pillow with tempur. An idea was to keep wide on lifted bed, not partial. All of patients lifted their limbs without multilayer compression. Effect of limbs-up were mild or irregular ; 2 patients. 2 patients lifted their one despite none effect. because, custom for sleeping, relaxation. One patient drew urination disorder by low limbs lifting. Effect of limbs-up between stage of Lymphoedema was unrelated

    publisher青森市国立情報学研究所の「学術雑誌公開支援事業」により電子化されました。The infant mortality rate of Aomori prefecture had been the highest nationwide in recent years. We therefore started this research project about the present conditions and problems of health in the perinatal period in Aomori prefecture from 2002. From 2003, we conducted surveys on all pregnant women in Aomori. We surveyed these women 3 times in total during their first trimester, the second trimester, and after delivery. The purpose of this paper is to provide an analysis of the results of the first trimester. Comparing the awareness about the mothers' health before and after pregnancy, there was a rise in health awareness after becoming pregnant. Most of the pregnant women were satisfied with the information given by hospitals and clinics on the term of pregnancy and it seems that awareness about health issues was increased due to this information. Awareness regarding eating habits, exercise and daily life were altered a little in non-pregnant women but not as much with pregnant women. It seems therefore that we should focus more closely on the information contained within the health guidance

    publisher青森市国立情報学研究所の「学術雑誌公開支援事業」により電子化されました。ケニアでは暴力被害を受けた女性がHIV感染およびエイズ孤児の増加と同様に増えており、アウトカムが非常に乏しい状況である。暴力に対する対策の遅れの背景に、社会システムの悪化があげられる。HIV/AIDSに関しては、ケニア国の指針を定め管理し、様々な対策が練られている。具体的な保健政策は、(1)HIV感染予防、(2)包括的VCTサービスの強化、(3)エイズ患者治療体制整備と支援の推進、(4)保健医療システムの強化によるHIV/エイズ対応能力の向上、(5)HIV感染者・エイズ患者、家族、エイズ孤児とHIV/エイズに影響を受けている子どもへのケア、社会的サポートなどの支援などが展開されている。これらのドメスティックバイオレンスや性感染症の増加に対する、予防とケア提供システムおよび実現可能な連携システムの確立が必要である。そして、DV被害者や性暴力被害者に対して支援が効果的に提供されるためには、早期発見体制や発生監視等のサーベィランス体制を整備し、被害者のアドポカシーを促進することが重要であると示唆された。 Violence to women causes destructive outcomes, such as an icrease in HIV infected women and orphans due to the death of HIV infected parents. The violence is one of the main causes of the deterioration of the social service system. There are various programs fighting against HIV/AIDS, planned and conducted in line with the national health policy of Kenya. These are the prevention of HIV infection, increasing the capacity of Integrated Voluntary counseling and Testing (VCT) services, increasing treatment services particularly with Anti Retrovirus Treatment (ART), improvement of health systems to cope with emerging issues related to HIV/AIDS in the society, arrangement of social support specifically for social groups affected with HIV/AIDS issues including HIV carriers, AIDS patients, orphans and other vulnerable children (OVC). The emphasis is on establishment of workable linkage between the prevention of domestic violence and the care provision systems to victims including sexual transmitted infections (STIs) issues. Advocacy is necessary based upon the above viewpoints in strengthening the surveillance system on DV and sexual violence
