    Learning outcomes of a psychiatric nursing skill course using scenario-based demonstration, "Nurse Management of Physically Restrained Patients"

    publisher青森The aims of this study were to use scenario-based demonstration in a training course on nurse management of physically restrained patients, and to assess the learning outcomes. The nursing of physically restrained patients requires skills involving both legal and safety aspects. To cover these aspects, we included narratives describing legal procedures and the background to nursing care in demonstration sessions. An open-format questionnaire survey was conducted among the participants. An exploratory analysis was then performed using the KJ method. The results showed three learning outcomes. First, the participants gained knowledge of the adverse physical effects and considered the psychological pain associated with physical restraint. Second, they gained a correct understanding of physical restraint measures and considered risks to nurses themselves. Third, they learned the importance of complying with the law when performing nursing duties. Thus, the three effects of attending this course were identified as follows: the course helped participants to (1) develop awareness of the protection of human rights, (2) recognize psychological stress mong nurses as a result of performing physical restraint, and (3) be aware of the need to learn accurate nursing skills. Taken together, the results suggest that this psychiatric nursing skill course was effective for helping nurses to understand the many ramifications, including the legal responsibilities, of using physical restraint in patient care. 本研究の目的は、身体拘束患者の看護の授業にシナリオに基づいたデモンストレーションを採用し、学生がどのような学びを得たのかを明らかにすることである。身体拘束患者の看護は、法的な側面と安全に援助する側面を併せもつ看護技術である。そこで、デモンストレーション中に法的手続きや看護援助の根拠などをナレーションとして挟み込んで教授した。研究方法は受講学生を対象に自由記述によるアンケート調査を実施した。そして、KJ法を用いて探索的に分析した。その結果、学生は以下の3つの学びを得ていた。(1)身体的な弊害がわかり精神的苦痛に思いを寄せる(2)身体拘束を正しく理解し看護師自身の危険と向き合う(3)法律遵守のもと看護の役割を全うする。以上の結果から、今回実施した授業から以下の3つの効果が示唆された。(1)人権擁護意識の獲得(2)身体拘束を行う看護師の精神的負担の認識(3)正確な技術習得の必要性の認識 これらのことから、今回実施した精神看護技術の授業は一定の効果があったことが示唆された

    Measurement of Distance between Spinous Processes of the Lumbar Spine : Assessment of Interrater Reliability , Concurrent Validity by Comparison between Palpation and Ultrasonograms

    publisher青森本研究の目的は腰椎の棘突起間距離を触察法と超音波画像法の2つの方法を用いて計測し、その再現性と妥当性を検討することである。触察法は棘突起を皮膚上から触察し、計測点を決定して棘突起間距離をキャリパーを用いて計測する方法である。 対象は健常男性36名、平均年齢20.1とした。 触察法では2人の計測者間の計測値の級内相関係数は0.92〜0.98であり、良好な再現性が示された。さらに、超音波画像法による計測値を加えた3群間には統計学的な有意差が認められず、触察法と超音波画像法の再現性と妥当性が示され、臨床的に有用な計測法であることが示唆された。 This study was intended to measure the distance between spinous processes of the lumbar spine by two methods and examine the reliability of the data. One method was to palpate the spinous processes from over the skin, determine a land mark, and then use a caliper to measure the distance between two adjacent lumbar vertebrae (i.e., distance between the spinous processes). The other method was ultrasonographic measurement. The subjects were 36 healthy men with a mean age of 20.1 years. The reliability of the two measurements was subject to test-retest using intraclass correlation coefficient (ICC) , which represent interrater reliability between 2 raters, was 0.92 -0.98.There were no significant differences in the measurement values between the palpation by 2 raters and the ultrasonographic measurement. The palpitation method were considered acceptable and provided reliable results by comparison with the ultrasonographic method

    An Application of an Objective Structured Clinical Examination for Learning Skills in Assessment of Physical Therapy : A Task Involving Measurements on the Range of Motion Test

    publisher青森OSCEは,臨床実習に出る前の医学生の臨床能力を客観的に評価できる優れた試験である。また,OSCEは臨床技能習得の有用な手法になり得ると考えられる。そこで,我々は臨床実習前の理学療法学科学生に独自のOSCEを試行して,有用性を検討した。 独自のOSCEでは,理学療法学科3年生10名が,課題として股関節のROM-Tを測定し, PT教員2名とSP1名に評価された。学生は基礎的な知識をほぼ理解していたが,角度計の操作,下肢の保持等の技術面で不十分さが認められた。ROM-T実施に関する学生の自己評価とSP・PT教員の評価は類似しており,学生はROM-T測定の応用能力が不十分であった。 OSCEは臨床に近い状態を設定でき,臨床実習前の学生にとって技術習得に非常に有用である。特に録画を見ながら実施した評価者から学生へのフィードバックが有効であった。OSCEは学生の学習の動機付けともなり,教員の教育方法・内容の改善にも結びつく貴重な機会であった。OSCEの今後の課題は,時間・労力の多大さ,SPの確保・教育等である。 The Objective Structured Clinical Examination (OSCE) is very useful for evaluating the clinical abilities of medical students before general clinical training. OSCE can also be a useful means for learning clinical skills. We devised and practiced an original OSCE for physical therapy students (PTSs) before general clinical training, and examined the usefulness of our original OSCE. In our original OSCE, ten third year PTSs measured for a task on the hip joints of a standardized patient (SP) in a range of motion test (ROM-T) and were evaluated by two examiners and the SP. PTSs mostly had basic knowledge of ROM-T, but they did not have enough skills in handling a goniometer and the lower limbs of a SP and so on; PTSs' self-evaluations of the performance of ROM-T were similar to the examiners' and SP' s evaluations; PTSs did not have enough applied abilities of measurements of ROM-T. We can set up similar situations to clinical scenes, so we think OSCE can be very useful to learn clinical skills for PTSs before general clinical training. In our OSCE, feedback from examiners and SP to PTSs while watching recorded video tapes was especially useful our original. OSCE can give a motivation of study for students and a valuable chance for improvements of education methods and contents for teachers. Problems with our OSCE are that we need a lot of time and manpower and it is difficult to train and keep SPs
