    Soft skill elements in structured nursing clinical skill assessment on transfusion-related procedure: a clinical audit

    Introduction: The transfusion-related treatment is frequently used in clinical settings and has been proven to be life-saving therapy. Undoubtedly, the emphasis on students’ dexterity performance is placed in the current organized clinical skill assessments connected to transfusion-related procedures, and the assessment of soft skills utilized during the process is often absent. In the undergraduate nursing program, this study sought to investigate the soft skill components of objectively structured clinical skill assessment on transfusion-related procedure. Materials and Method: A clinical audit was carried out on the retrospective structured clinical skill assessment exam questions by using the clinical skill assessment index II (CSAI-II) instrument. A total of 7 transfusion-related procedures set of exam questions were audited. Results: Only the packed red blood cell (PRBC) procedure was found although six (6) categories of transfusion-related procedures were identified in this study. Regards to the soft skills elements, although there were 8 soft skills elements identified in this procedure: communication skills (CS), social skills and responsibilities (SSR), critical thinking skills (CTS), Problem-solving skills (PSS), teamwork skills (TS), professional and ethical decision-making skills (PEDMS), numeracy skills (NS), and interpersonal skill (IS), all the OSCE documents of PRBC procedure skill assessment were found to have CS 7 (100%) and TS 1 (14.3%). Conclusion: It was found that many things can be improved in the structured clinical skill assessment on transfusion-related procedures. Therefore, it is crucial to have a well-structured clinical skill assessment exam question to produce holistic graduate nurses who equip with clinical skills and soft skills for the current healthcare delivery system

    Hybrid CNN-ViT integration into Siamese networks for robust iris biometric verification

    Iris recognition has emerged as a critical biometric verification method, valued for its high accuracy and resistance to forgery. However, traditional convolutional neural network (CNN)-based models, despite their strength in extracting local iris features, struggle to capture global dependencies, which limits their generalization across different datasets. Additionally, conventional classification-based approaches struggle to accurately verify new individuals with limited training data. Thus, this study proposed a hybrid CNN-vision transformer (CNN-ViT) model within a Siamese network to enhance one-shot learning capability by combining CNN’s local feature extraction with vision transformers (ViT’s) global attention. To evaluate its performance, the hybrid model was compared with VGG16 and ResNet under the same training conditions for 20 epochs. VGG16 and ResNet rely on pre-trained models, whereas the hybrid CNN-ViT model is specifically designed to achieve this task with an increment to 98.9% training accuracy, surpassing the TinySiamese model's benchmark accuracy. It also attained a recall of 75%, demonstrating strong sensitivity in correctly identifying positive matches. The hybrid model maintained an excellent balance between learning and generalization by employing the binary cross entropy (BCE) loss function. These findings contribute to the development of efficient iris recognition systems, paving the way for advanced biometric applications in financial transactions, border control and mobile security

    In vitro study of probiotic Lactobacillus helveticus: antibacterial effects on Porphyromonas gingivalis

    Probiotics are gaining attention for their benefits as a supplement to improve oral health. This study aimed to evaluate the antibacterial effect of the probiotic Lactobacillus helveticus against Porphyromonas gingivalis, a significant pathogen in periodontal diseases. Antibacterial susceptibility was assessed using the well diffusion assay, with 0.12% chlorhexidine (CHX) served as the positive control. Biofilm biomass was evaluated using crystal violet staining. Cell viability in P. gingivalis treated with L. helveticus was determined using the LIVE/DEAD Baclight bacterial assay via fluorescence microscopy. Ultra-morphological alterations in these cells were further examined using Field Emission Scanning Electron Microscopy. The results indicated that L. helveticus significantly reduced the growth of P. gingivalis. The highest concentration of 109 cells/mL achieved the most substantial inhibition in the well diffusion assay, followed by concentrations of 108 cells/mL and 107 cells/mL, which demonstrated a clear dose-dependent response. Furthermore, biofilms of P. gingivalis treated with L. helveticus exhibited a notable biomass reduction of up to 85% at the highest concentrations. LIVE/DEAD staining confirmed a decreased in cell viability among the treated populations, while FESEM analysis revealed morphological disruptions in P. gingivalis cells treated with L. helveticus. These findings suggest that L. helveticus has a potent antibacterial effect against P. gingivalis, highlighting the need for further research to identify the optimal probiotic strategies that could enhance periodontal health

    Exploring the impact of the SBAR on nursing handover: a scoping review

    Effective communication during nursing handovers is vital for ensuring patient safety and continuity of care. The SBAR (Situation, Background, Assessment, Recommendation) communication tool offers a structured framework that enhances clarity, efficiency, and reliability during shift transitions. This scoping review assesses the impact of SBAR on nursing handovers, with a focus on its effectiveness in improving communication, reducing errors, and enhancing patient safety. The review methodology guided by the PRISMA-ScR checklist, a systematic literature search was conducted across Medline, CINAHL, EMBASE, and EBSCO databases for the period January 2021 to June 2024. Thematic analysis of the identified studies indicates that SBAR significantly improves communication quality and patient safety, reduces handover-related errors, and promotes teamwork among nursing staff. Three studies reported notable reductions in errors, while two studies documented increased satisfaction among nurses and patients. Additionally, SBAR was associated with educational benefits, including improved knowledge and confidence in the handover process. The structured nature of SBAR contributes to a decrease in sentinel events linked to miscommunication and enhances nurses' confidence and satisfaction with the handover process, thereby supporting teamwork and overall quality of care. Despite these positive outcomes, challenges remain in its implementation and integration across diverse healthcare settings. Further research is warranted to examine the long-term benefits of SBAR, compare its effectiveness with other communication tools, and optimize its application in varied clinical contexts. Addressing these areas will support SBAR's continued role in improving nurse performance, patient safety, and healthcare quality

    A fact not well understood by many is that success cannot be quantified. Success is relative and means different things to different people. To value success solely by one’s bank balance, it is not a success in the true sense. You are a winner and a champion when you succeed in reaching your mother’s womb, leaving behind billions of competitors who raced against you. You are a success story when you are the first spark of life that manages to emerge and become one with the growing fetus in your mother’s womb. For a pregnant mother, success comes at the end of the gestation period of her growing infant. In her eyes, success is experienced when she safely delivers the child after enduring many months of discomfort and a life-and-death struggle in the labour room. Success means different things to different people. To a business conglomerate, success is witnessing the business he started from scratch grows into a multimillion-dollar entity that reaches the pinnacle of achievement. For an author, success is seeing the publication of his creative ideas, which brings joy to his heart. For someone struggling with addiction—whether narcotics, alcohol, or other intoxicants— breaking free from those shackles means a rebirth into this world. For such an individual, parting ways with addiction is indeed a success that others may find hard to understand. For a dedicated teacher, success lies in bringing students out of ignorance into a world full of knowledge, wisdom, ethics, and enlightenment. For a diligent student, success is seen when he passes examinations with flying colours. One person’s success is never seen as success in the eyes of another, because success is relative. In the life of a farmer, success is felt when the season for harvesting crops arrives. The fact that his crops survived bad weather and pests in the field is indeed a moment to rejoice. To him, success comes as a reward for all the hard work and toil in the field. For a freedom fighter, success is felt when his country is freed from foreign occupation, subjugation, and interference. Successfully resisting those who come to steal land and possessions is a victory and success for the army defending the motherland. Since success cannot be quantified and is something felt deeply in the heart, only the individual has the right to claim that he has succeeded. Forget about the world and how others view him. Success doesn’t fall from the sky. It is earned through patience, perseverance, persistence, and prayer. For a Muslim, all worldly successes are temporary. True and everlasting success is gained in the hereafter. The ultimate success for a practicing Muslim is being granted entry into the garden of heaven (Jannah)

Halal bukan sekadar mematuhi sekatan diet kerana ia Adalah kaedah menyeluruh untuk berkelakuan secara etika, mematuhi prinsip agama dan menjaga integriti individu serta Masyarakat.     Optimisation of supercritical fluid extraction for fatty acids from Benincasa hispida seed oil: a response surface approach

    Introduction: The goal of this study was the optimisation of the process parameters for the extraction of Benincasa hispida seed extract using the supercritical carbon dioxide. Methods: Response surface methodology was carried out using the design expert software with the implementation of process parameters including the temperature (40°C – 70°C), pressure (100 bar – 400 bar) and supercritical carbon dioxide flow rate (2 g/min – 10 g/min) in this study due to their significant impact towards the oil yield and the polyunsaturated fatty acids. Results: The optimised parameter for this supercritical fluid model is 70°C, 247 bar and 7 g/min while 0.36 bar and 40 °C has been chosen from previous studies as the optimised parameter for Soxhlet extraction. The oil yield (33% from Soxhlet extract and 9.67% from supercritical fluid extract) obtained was quite similar with previous studies, however, the polyunsaturated fatty acids obtained throughout this optimisation were much higher indicating that this study provided better output of the polyunsaturated fatty acids obtained from the seed oil. Moreover, the polyunsaturated fatty acids contents were also compared between the extract obtained from the conventional Soxhlet extraction versus novel supercritical fluid extraction techniques.     Planned as a short view of the issue, the article has a limited focus on changing conditions in Asia-Pacific. As closely observed, in particular since the beginning of this century, the global eco-political dynamism has been restructured by the developments in this region, which contains two oceans, the Pacific and the Indian Oceans. It is beyond a power struggle between two rivals, The People’s Republic of China (PRC) and the United States of America (the U.S.) While we are observing some attempts to launch a round of negotiations between two parties, the conflictual situation still dominates the relationships, and its efficacies are inevitably felt by the whole nation-state in the region and beyond. This paper considers the recent past and current situation of the Asia-Pacific region. In addition, the repositioning of Türkiye’s foreign policy is dealt with concisely.     Base pressure control and temperature distribution along the duct at supersonic Mach number: a comprehensive CFD approach - part II

    The development of space shuttles and high-performance military aircraft has made the study of turbulent flow in separated regions a key area of research. Researchers are also interested in turbulent flow under transonic and supersonic conditions, particularly when it separates suddenly and expands after the cross-section of the larger duct increases rapidly. Two regions where the shear layer forms are the separated flow and the main flow. The split streamline reattaching to the duct causes significant drag and creates a recirculation zone where the pressure is lower than in the surrounding air. This study investigates the use of quarter-circular ribs to regulate base pressure as a passive control method, where the shear layer hits the straight part of the rib and the curved part is downstream. Computational simulations investigate how the rib radius and placement near a backward-facing step influence flow behavior, including recirculation and vortex formation. In the study, the inertia parameters considered were a Mach number of M = 1.7 and the nozzle pressure ratio (NPR) in the range of 3 to 11. The geometrical parameters considered were the area ratio A2/A1 = 4, the cross-sectional area of the duct-to-nozzle exit, and the length-to-diameter L/D ratio, ranging from 1 to 6. The quarter-circle radius considered was 1.5 mm, 2 mm, and 3 mm, and the ribs were located at L/D = 0.5, 1, 1.5, and 2. The goal is to identify the optimal rib geometry and location that maximizes aerodynamic efficiency. The results show the optimum rib radius and locations. Base Pressure Control and Temperature Distribution are 3 mm and L/D = 2. The rib radius of 1.5 mm does not show any definite pattern. In contrast, the 3 mm rib radius exhibits a progressive increase in the base at various locations within the duct. Hence, a 3 mm rib radius seems to be the best option if the application is to reduce the base drag to a considerable leve
