ESC and EHRA lead a path towards integrated care for multimorbid atrial fibrillation patients: the Horizon 2020 EHRA-PATHS project

Atrial fibrillation, underlying heart disease and prognosis
Boezemfibrilleren is een veel voorkomende hartritmestoornis. Eén op de vier mensen boven de 40 jaar krijgt ermee te maken. Hierbij kunnen ernstige complicaties optreden, zoals hartfalen of een herseninfarct, maar ook de dood. De aandoening is geassocieerd met tal van onderliggende (hart)ziekten, zoals hoge bloeddruk, coronairlijden, hartfalen, suikerziekte of kleplijden.
Er zijn twee behandelstrategieën. Bij "rate control" wordt met medicijnen geprobeerd de hartfrequentie te verlagen en complicaties te voorkomen. Bij "rhythm control" wordt getracht het hartritme te herstellen en te behouden. Grote klinische studies hebben de afgelopen jaren aangetoond dat deze behandelstrategieën vergelijkbaar effectief zijn. Tot op heden was echter onduidelijk welke patiënt het meeste baat heeft bij welke behandeling.
Uit onderzoek van promovendus Michiel Rienstra blijkt dat niet het ritme maar de onderliggende hartziekte de prognose bepaalt. De keuze voor de behandeling moet dus, behalve op basis van de leeftijd en de klachten van de patiënt, gemaakt worden op basis van de onderliggende hartziekte. Uit het onderzoek blijkt bijvoorbeeld dat patiënten met hoge bloeddruk en vrouwen veel meer complicaties hebben bij een rhythm control behandeling. Bij deze patiëntengroepen heeft rate control behandeling waarschijnlijk de voorkeur.
Rate control in Atrial fibrillation
Patiënten met permanent boezemfibrilleren hoeven minder vaak ter controle naar het ziekenhuis dan vooralsnog wordt verondersteld. Een controlebehandeling met ‘matige frequentie’ volstaat. Ook hoeven de patiënten minder medicijnen te gebruiken en hebben ze minder aanvullend onderzoek nodig dan gedacht. Dat blijkt uit het promotieonderzoek van Hessel Groenveld.
Boezemfibrilleren is de meest voorkomende hartritmestoornis. Op dit moment hebben meer dan zes miljoen mensen in Europa last van boezemfibrilleren en naar verwachting gaat dit aantal de komende decennia verdubbelen, onder meer ten gevolge van de vergrijzing. Hessel Groeneveld vergeleek de effecten van strenge en gematigde controlestrategie bij permanent boezemfibrilleren op de levensverwachting en kwaliteit van leven.
Bij de gematigde behandelstrategie behaalden 97% van de patiënten de gewenste hartfrequentie, bij de strenge behandelstrategie ‘slechts’ 67% van de patiënten, zo stelt de promovendus vast. De behandelstrategieën blijken niet van elkaar te verschillen ten aanzien van kwaliteit van leven of levensverwachting. Groenveld stelt vast dat patiënten met permanent boezemfibrilleren volgens een gematigde frequentie controlestrategie behandeld kunnen worden. Er is minder medicatie nodig, minder aanvullend onderzoek en minder ziekenhuisbezoek. Dit maakt de behandeling eenvoudiger en goedkoper.
Atrial fibrillation is the most common type of arrhythmia. Many patients have permanent atrial fibrillation. One of the treatment aims for patients with permanent atrial fibrillation is a low heart rate, but this goal is not evidence based. We performed a comparative study on the prognosis of patients who were treated according to a strict, or a lenient rate control. Strict rate control was defined as a resting heart rate <80 beats/minute and <110 beats/minute during moderate exercise. Lenient rate control was defined as a resting heart rate <110 beats/minute. Prognosis was defined by cardiovascular death, hospitalization for heart failure, stroke, bleeding, life threatening arrhythmias, and pacemaker or defibrillator implantations. In the lenient rate control group 97% of patients was treated according to protocol, in the strict group 67%. There was no difference in prognosis between patients treated according to a lenient or strict rate control strategy. In addition, there was no difference in outcome between patients with lenient, successful strict, and failed strict rate control. There was no difference in quality of life between lenient and strict rate control at the end of the study. Heart rate did not influence quality of life. In conclusion, patients with permanent atrial fibrillation can be treated according to a lenient rate control strategy. This will result in less medication, and less frequent hospital visits. It simplifies the treatment of permanent atrial fibrillation for the patient and the physician.
Epidemiology and modifiable risk factors for atrial fibrillation
Published June 2023The global prevalence of atrial fibrillation (AF) has increased substantially over the past three decades and is currently approximately 60 million cases. Incident AF and its clinical consequences are largely the result of risk factors that can be modified by lifestyle changes. In this Review, we provide evidence that the lifetime risk of AF is modified not only by sex and race but also through the clinical risk factor and comorbidity burden of individual patients. We begin by summarizing the epidemiology of AF, focusing on non-modifiable and modifiable risk factors, as well as targets and strategies for the primary prevention of AF. Furthermore, we evaluate the role of modifiable risk factors in the secondary prevention of AF as well as the potential effects of risk factor interventions on the frequency and severity of subsequent AF episodes. We end the Review by proposing strategies that require evaluation as well as global policy changes that are needed for the prevention of incident AF and the management of recurrent episodes in patients already affected by AF.Adrian D. Boezemfibrilleren is de meest voorkomende hartritmestoornis. Meer dan zes miljoen Europeanen hebben last van deze aandoening en de komende decennia zal dit aantal waarschijnlijk verdubbelen, onder meer door de vergrijzing. De gevolgen zijn ernstig: een verhoogd risico op overlijden, herseninfarct, hartfalen en verminderde kwaliteit van leven.
Promovenda Marcelle Smit onderzocht de samenhang tussen “remodeling” in de boezems van het hart en boezemfibrilleren. Remodeling is het geheel aan veranderingen dat aan het boezemweefsel optreedt, door onder meer verhoogde bloeddruk, hartfalen, hartklepaandoeningen en suikerziekte. Remodeling treedt al op voordat de patiënt last krijgt van boezemfibrilleren, maar het heeft ook een zichzelf versterkend effect: zodra boezemfibrilleren optreedt, kan de schade aan de boezems verergeren. Zo ontstaat een vicieuze cirkel.
Uit het onderzoek van Smit blijkt onder meer dat patiënten die nog maar kort last hebben van boezemfibrilleren veelal ook ontstekingsverschijnselen hebben. Behandeling met ontstekingsremmers zou het hartritme kunnen herstellen, suggereert de promovenda. Bij patiënten met permanent boezemfibrilleren wordt niet meer geprobeerd het hartritme te herstellen, maar enkel nog de snelle hartslag verlaagd (“rate control”) met behulp van medicatie. Smit stelt vast dat in deze groep soepele rate control (hartslag < 110 slagen per minuut) niet tot meer nadelige remodeling leidt dan strenge rate control (hartslag < 80 slagen per minuut). Daarentegen treedt bij vrouwen juist meer nadelige remodeling op.
Marcelle Smit (Brunei, 1980) studeerde geneeskunde te Groningen. Ze verrichtte haar onderzoek aan de afdeling Cardiologie van het Universitair Medisch Centrum Groningen (UMCG). Het onderzoek werd mede gefinancierd door de Nederlandse Hartstichting, Biotronik, Medtronic en St. Jude Medical. Smit is inmiddels in opleiding tot cardioloog in het Martiniziekenhuis Groningen.

Advanced clinical approaches in complex cardiovascular disease : invasive options at the crossroad of ischemia, heart failure, and arrhythmias
In this thesis an overall assessment of a variety of advanced diagnostic and treatment approaches (PET, VEGF gene and laser treatment, NOGA, ablation therapy and PCI after immediate graft failure) in subjects with complex cardiovascular disease is made during daily clinical practice. It is argued that such an evaluation is facilitated by creating a knowledge platform based on the three major interrelated entities in cardiovascular disease: ischemia, heart failure and arrhythmia. The main driving force to elaborate on this topic is to provide more insight into the added-value of the aforementioned cardiovascular modalities or clinical approaches for main stream patients.
