    Xanthogranulomatous Pyelonephritis (XPN) that was firstly described by Schlagenhaufer in 1916, is a rare chronic pyelonephritis. We describe an interesting case of XPN in which patient was treated only with antibiotics. An 80-year-old woman was hospitalized for fever and asthenia. No significant symptoms or signs were present at the admission. First instance ultrasound scan suggested the diagnosis of XPN, promptly confirmed by CT. Antibiotic therapy was successfully administered after urine analysis and culture

    Scopo del lavoro: Scopo principale del nostro studio è stato quello di analizzare la possibile relazione tra aumento della rigidità aortica e progressione della malattia renale cronica in un gruppo di pazienti ipertesi nefropatici, endpoint secondari riguardavano l’eventuale ruolo prognostico della pulse wave velocity (PWV) sugli eventi cardiovascolari (CV) e sulla mortalità totale. Materiali e metodi: sono stati arruolati 395 pazienti che sono stati sottoposti a esami bioumorali, monitoraggio ambulatoriale della pressione arteriosa e stima della stiffness aortica mediante dispositivo oscillometrico Arteriograph. Sono stati inoltre registrati i dati relativi all’insorgenza di eventi cardiovascolari, mortalità e progressione della malattia renale. Risultati: L’intera popolazione è stata suddivisa in due gruppi sulla base del valore soglia di PWV >10 m/s. I pazienti con PWV>10 m/s presentavano livelli significativamente più elevati di pressione di polso, uricemia ed età e un eGFR significativamente inferiore rispetto al gruppo con PWV<10 m/s. Alla regressione di Cox, la PWV predice un rapido declino della funzione renale, indipendentemente dall’eGFR, dall’età, dal tabagismo, dal sesso e dai valori pressori (HR 1,784, p=0,0082). Il cut-off che meglio discrimina i progressors dai non progressors, ottenuto mediante indice di Youden, si attesta a 9,99 m/s. Per quanto attiene alla mortalità, l’analisi di regressione ha mostrato un significativo aumento della probabilità di decesso precoce nei soggetti con PWV>10 m/s (best cut-off 12,3 m/s), tale associazione significativa permane dopo correzione per fattori confondenti. Riguardo l’insorgenza di eventi cardiovascolari, l’incidenza di questi ultimi è significativamente associata a un aumento della PWV, il cui best cut-off si attesta a 10,22 m/s. Conclusioni: Il nostro studio, il primo condotto utilizzando un dispositivo oscillometrico per la stima della PWV, conferma l’importanza dell’impiego della PWV nella stratificazione del rischio CV e renale dei pazienti ipetesi e nefropatic

    Brugada phenocopies (BrPs) are clinical entities that show an electrocardiogram (ECG) pattern similar to what is observed in Brugada syndrome (BrS). They are caused by different clinical conditions. We describe a case of BrP in a man that developed acute kidney failure secondary to rhabdomyolysis, after heroin addiction. His initial ECG showed Brugada type 1 pattern resolved after hemodialytic treatment. A provocative test with ajmaline, which resulted negative, was performed to confirm the diagnosis. BrPs can mimic a true BrS and a fast recognition of these clinical and electrocardiographic findings may avoid diagnostic mistakes thus preventing unnecessary or inaccurate treatme

Le modificazioni dello stile di vita costituiscono il trattamento non farmacologico di prima scelta della malattia ipertensiva, come indicato da tutte le Linee guida internazionali. Uno stile di vita sano può prevenire o ritardare l’insorgenza dello stato ipertensivo e può ridurre il rischio cardiovascolare del paziente iperteso, oltre ad offrire la possibilità di aumentare l’efficacia antipertensiva della terapia farmacologica

    Artificial Reality: Immersive but Factually Dishonest AR Experience

    As commonly known, technology is a double-edged sword, and augmented reality (AR) is no exception. This article raises concerns and promotes awareness of the use of AR in mass media, in particular in those industries, such as news reporting, that aspire to report facts. Our main message is that the standard workflow for creating AR content, albeit having no mala fide intent, might lead to artificial reality. This titular term was introduced by the first author during a talk organized by a technical committee of IEEE Consumer Technology Society (CTSoc), where we decided that we should urge more ethics and standard discussions on the issue through this article

    Introduction: Experimental evidence suggested that aldosterone can cause myocardial hypertrophy and fibrosis. However, previous studies on the association between plasma aldosterone concentration (PAC) and left ventricular (LV) mass (LVM) and geometry, in subjects without primary aldosteronism yielded conflicting results. Aim: To evaluate the relationships of PAC with LV mass and geometry in patients with essential hypertension (EH), and to assess the influence of gender on these relationships. Methods: We enrolled 478 subjects (men: 63%; mean age 44 ± 12 years) with untreated EH. The measurements included 24-h blood pressures, plasma renin activity (PRA) and PAC, obtained by radioimmunoassay and an echocardiogram. For its skewed distribution PAC was log transformed and expressed as median value and interquartile range. Results: PAC was significantly lower in subjects with normal LVM indexed for body surface area (BSA), as compared to those with concentric remodeling to the patients with eccentric LV hypertrophy (LVH), and to the subjects with concentric LVH (Figure 4). Significant correlations of Log (PAC) with LVM, either indexed for BSA (r = 0.20; p.0001), or for height^2.7 (r = 0.21; p.0001) and with relative wall thickness (RWT) (r = 0.18; p.0001) were found. These correlations were similar in men and in women and remained statistically significant in multiple regression analyses, even after adjustment for potential confounding factors (all p.01). Conclusions: Our results seem to suggest that in essential hypertensive patients circulating aldosterone levels are independently associated with concentric LV geometry, without gender related differences
