Better outcomes for hospitalized patients with TIA when in stroke units: An observational study, Author Response (Editorial)
Editors' Note: Drs. Phan and Srikanth point out that in “Better outcomes for hospitalized patients with TIA when in stroke units: An observational study,” the recurrence rate was much higher than that in an Australian rapid/outpatient-based TIA pathway. Inability to walk (which could indicate stroke instead of TIA), low usage of antiplatelet therapy, and time to antiplatelet therapy are possible explanations. In response, Cadilhac et al. comment that initiation of aspirin within several hours vs 24–48 hours has not been shown to clearly improve outcomes.No Full Tex
Stakeholder perceptions on the role of community development corporations and resident participation
Community Development Corporations (CDCs) were formed in the United States to address issues in urban communities and often work with municipal governments on these issues (Keating, 1997; Jennings, 2004). In some neighborhoods, CDCs seek to rebuild and revitalize communities that suffered from social unrest, while others react to the lack of government or inadequate programming addressing the economic and social maladies of urban communities. The work of CDCs requires the involvement of all community stakeholders – particularly city residents and public administrators. Does thedual relationship CDCs have with residents and local government suggest that they bring both closer together? This research studies this phenomenon by using Arnstein’s Ladder of Participation to examine the role CDCs play in cultivating citizen participation.Specifically, this study seeks to examine how community stakeholders perceive the roleCDCs play in fostering resident participation in local government. As a primary methodological tool for studying individual and group perspectives,Q-method was employed to empirically study community stakeholder perceptions ofCDCs. Key research findings suggest that: a) community stakeholders believe that CDCsare currently working to maintain the status quo of participation and advise that the Context of Conventional Participation (1995) is present, b) stakeholders overwhelmingly believe that CDCs should work to encourage participation that incorporates partnership, and c) there should be less distance between residents and decision making, thereby increasing opportunities for engagement. Through the exploration of the subjective, this study exposes the perspective ofresidents on citizen participation and identifies the need for a new direction of theory development that examines the creation of consensus and partnership building incommunity development organizations and initiatives. Furthermore, Conditions for Ideal Participation emerges from the data providing a practical guideline for strengthening participation in municipal government. Cloud service provider comparison for “TIA” insurance system
Mūsdienās vairākas organizācijas vēlas atteikties no savas infrastruktūras, pārceļot izmantotos risinājumus mākoņpakalpojumu sniedzēju platformās, lai nodrošinātu labāku ātrdarbību, mērogojamību, pieejamību vai, piemēram, vienkārši samazinātu sistēmu uzturēšanas izmaksas. Autors ir piedalījies projektā, kurā tika veikta TIA Technology izstrādātās apdrošināšanas sistēmas TIA tehniskā analīze paralēli tās izstrādei ar mērķi to migrēt uz mākoņpakalpojumu platformu Microsoft Azure vai Oracle Cloud, analīzes rezultātā attiecīgajai TIA implementācijai arī veicot visu izstrādes procesā iesaistīto komponenšu migrāciju uz Oracle Cloud. Šī darba gaitā autors apskatīja Oracle Cloud un Microsoft Azure mākoņskaitļošanas platformas un to piedāvātās iespējas TIA darbības nodrošināšanai, aprakstīja migrācijas procesu, izaicinājumus, motivāciju, kā arī šīs rīcības stratēģiju. Autors arī projekta izstrādes laikā apkopoja iegūtās zināšanas un pieredzi ar mērķi piedāvāt vadlīnijas līdzīgas problēmas risināšanai – TIA apdrošināšanas sistēmas migrācijai uz mākoņpakalpojumu platformu Oracle Cloud.Nowadays many organizations would like to discard their on-premise infrastructure by moving their solutions to cloud service provider platforms to provide better performance, scalability, accessibility or just for maintenance cost reduction. Author has been involved in a project, where the insurance system TIA developed by TIA Technology has been technically analyzed in parallel with its development and customization with the aim to migrate it to cloud service provider platforms Microsoft Azure or Oracle Cloud. After analysis this TIA implementation was migrated with all involved development process components to Oracle OCI. This paper describes author’s research regarding Oracle Cloud and Microsoft Azure cloud service provider platforms and their provided options to ensure operation of TIA, migration process, challenges, motivation, and action strategy. As a result, author summarized the obtained knowledge and experience with an intention to provide guidelines necessary to solve a similar problem – TIA insurance system migration to Oracle Cloud
The role of educative thought in the life and work of Antonio Gramsci
Many philosophers have propounded a vision of an improved society, what distinguishes Antonio Gramsci is his continuous effort to make it happen by understanding the process in order to put into practice. Gramsci's conviction about the importance of educative development came from both theory and experience. While there has been considerable examination of Gramsci's work in relation to the Prison Notebooks, this study will seek to address a lacuna in Gramsci scholarship. Using Gramsci's philological method, I analyse Gramsci's pre-prison activity; his pre-prison articles and letters, which, together with his letters from prison, formed part of his educative mission. This educative process was necessary, in order to construct a new party which would develop a collective will, collaboratively, with the masses.In this study therefore, I explore the contexts and formative experiences of the first part of his life together with the intellectual sources from which Gramsci developed his later theories, making central hitherto underemphasised connections between them which informed his writing and ideas. I intend to illustrate that Gramsci's underlying purpose in his writing, and political activity, was not only practical, on how to create a new socialist ruling class, but also educative in forming the mindset and values of his comrades. So that in addition to outlining his vision of a new order, he implicitly guided or explicitly explained the processes by which the necessary changes in social relations and moral climate could be made in order to achieve it. Each person had to engage with the values of the new order so that each could contribute to the construction of a new robust state. It was essential to build a hegemony at the most profound level, one which was dependent on collective understandings and a collective will
Pai-Raiol e Tia Balbina: negros e feiticeiros
Este ensaio analisa a imagem dos negros feiticeiros no Brasil pré-abolição com base nos personagens Pai-Raiol (As vítimas-algozes, Joaquim Manuel de Macedo, 1869) e tia Balbina (Motta Coqueiro ou a pena de morte, José do Patrocínio, 1877). O autor faz um resumo dos livros; analisa as características dos personagens e suas diferenças entre si; e analisa a visão dos autores em função de sua base católica e a imagem geral de negros e feiticeiros em seu contexto cultural. This essay analizes the image of coloured enchanters in pre-abolition Brazil based on characters Pai-Raiol (As vítimas-algozes, Joaquim Manuel de Macedo, 1869) and tia Balbina (José do Patrocínio, 1877). The author summarize the books; analizes the refered characters features and their differences from each other; and approaches the authors vision related to their catholic background and the general coloured and enchanters in their cultural context.

look on the authorship in the works of the Sítio do Picapau Amarelo of Monteiro Lobato. We start from conceptions
of authorship discussed by Bakhtin (1997), by Foucault (2001), Chartier (2012) and Eco (1986), in order to understand
how the author-role in the figure of Monteiro Lobato, as author of his work making this a revolution in children's and
youth literature in Brazil. We also seek to understand how, in the context of her narratives, the character Tia Nastácia
assumes the role-author and creates her two texts, which also revolutionize the direction of the stories: Emília and
Visconde de Sabugosa. In this perspective, we consider both the relation of the author and the one who constructs,
controls, inaugurates and becomes responsible for the discourse (FOUCAULT, 2001), as we consider the relation
author as a creative and productive principle of the hero. Based on the considerations on the concepts of text, utterance,
discourse and meaning, discussed by Bakhtin (2003, 2014), Foucault (1996, 2001), Gregolin (1995) and Orlandi
(2007), we intend to show, Lobato and his characters, as the role-author can be seen in the relationship between Tia
Nastácia, Emília and Visconde de Sabugosa, and how this relationship reconfigures the image of the character Tia
Nastácia in the work of Lobato. In the end, we also present a proposal of textual production with marks of authorship
for the first years of Elementary School II, from the book Memórias da Emília (1936).Esta pesquisa apresenta, a partir dos conceitos teóricos da Análise do Discurso de Linha Francesa (AD), um olhar
interpretativo sobre a autoria nas obras do Sítio do Picapau Amarelo de Monteiro Lobato. Partimos das concepções de
autoria discutidas por Bakhtin (1997), por Foucault (2001), Chartier (2012) e Eco (1986), a fim de compreendermos
como se dá a função-autor na figura de Monteiro Lobato, como autor de sua obra infantil, fazendo dessa uma revolução
na literatura infanto-juvenil no Brasil. Também buscamos compreender como, no cotexto de suas narrativas, a
personagem Tia Nastácia assume a função-autor e cria seus dois textos, os quais também revolucionam os rumos das
histórias: Emília e Visconde de Sabugosa. Nessa perspectiva, consideramos tanto a relação do autor, como aquele que
constrói, controla, inaugura e se torna responsável pelo discurso (FOUCAULT, 2001), como consideramos a relação
autor enquanto princípio criador e produtivo do herói (BAKHTIN, 1997). Apoiados nas considerações sobre os
conceitos de texto, enunciado, discurso e sentido, discutidos por Bakhtin (2003; 2014), Foucault (1996; 2001), Gregolin
(1995) e Orlandi (2007), pretendemos mostrar, a partir de enunciados da obra infantil de Lobato e de seus personagens,
como a função-autor pode ser vista na relação entre Tia Nastácia, Emília e Visconde de Sabugosa e como essa relação
reconfigura a imagem da personagem Tia Nastácia na obra de Lobato. Ao final, também apresentamos uma proposta
de produção textual com marcas de autoria para os primeiros anos do Ensino Fundamental II, a partir do livro Memórias
da Emília (1936)
Development of a Standardised stroke risk assessment for patients with MigraAinous symptoms Reviewed as suspected TIA (SMART): study protocol for a mixed methods study

BACKGROUND: Transient ischaemic attack (TIA) and migraine can generate identical symptoms but have very different short-term risks of stroke. Uncertainty about the diagnosis may lead to missed opportunities to prevent stroke if TIA is treated as migraine, or overtreatment if migraine is treated as TIA. This project aims to define the risk of stroke for people with migrainous symptoms reviewed as suspected TIA and develop a risk assessment tool that could promote standardisation of care. METHODS AND ANALYSIS: The project involves two interlinked studies:(1) Study A: prospective observational cohort study. SETTING: NHS TIA and stroke services. POPULATION: adults with migrainous symptoms undergoing review for suspected TIA by a TIA/stroke service and the initial specialist clinician symptom-based diagnosis is either possible migraine or possible TIA with migrainous symptoms. DATA COLLECTION: baseline clinical characteristics, investigations and treatments. Stroke, TIA and migraine events within 90 days. SAMPLE SIZE: 2709 participants.Main analyses: analysis of stroke risk, development of stroke risk prediction model, preparation of visual tools to represent the risk model.(2) Study B: qualitative co-design study. SETTING AND POPULATION: clinicians from NHS TIA and stroke services. DATA COLLECTION: focus groups/interviews exploring views about the potential role for a risk assessment tool, the most appropriate visualisation for the risk tool and barriers/facilitators for implementation. SAMPLE SIZE: approximately 16 clinicians. ANALYSES: framework approach using the Implementation Research Logic Model. ETHICS AND DISSEMINATION: This study has ethical, Health Research Authority and participating NHS Trust approvals. Dissemination of study results will include presentations at national and international conferences and events, publication in peer-reviewed journals, and plain English summaries for patient/public engagement activities. Line-start permanent-magnet motor: single-phase starting performance analysis
This paper presents a detailed quasi-steady-state approach to different torque components (average and pulsating) for a single-phase capacitor-run permanent-magnet (PM) motor. By employing average electromagnetic torque, and the expected envelope of the pulsating torque, an accurate prediction of starting torque components behavior is made. The quasi-steady-state analysis of the asynchronous performance of the single-phase capacitor-start capacitor-run PM motor is realized through a combination of symmetrical components and d-q axes theory. A Variable-Gain Low-Noise Transimpedance Amplifier for Miniature Ultrasound Probes
This article presents a low-noise transimpedance amplifier (TIA) designed for miniature ultrasound probes. It provides continuously variable gain to compensate for the time-dependent attenuation of the received echo signal. This time-gain compensation (TGC) compresses the echo-signal dynamic range (DR) while avoiding imaging artifacts associated with discrete gain steps. Embedding the TGC function in the TIA reduces the output DR, saving power compared to prior solutions that apply TGC after the low-noise amplifier. The TIA employs a capacitive ladder feedback network and a current-steering circuit to obtain a linear-in-dB gain range of 37 dB. A variable-gain loop amplifier based on current-reuse stages maintains constant bandwidth in a power-efficient manner. The TIA has been integrated in a 64-channel ultrasound transceiver application-specific integrated circuit (ASIC) in a 180-nm BCDMOS process and occupies a die area of 0.12 mm2. It achieves a gain error below ±1 dB and a 1.7 pA/ √ Hz noise floor and consumes 5.2 mW from a ±0.9 V supply. Ticagrelor and Aspirin or Aspirin Alone in Acute Ischemic Stroke or TIA.
Trials have evaluated the use of clopidogrel and aspirin to prevent stroke after an ischemic stroke or transient ischemic attack (TIA). In a previous trial, ticagrelor was not better than aspirin in preventing vascular events or death after stroke or TIA. The effect of the combination of ticagrelor and aspirin on prevention of stroke has not been well studied
