Etiological diagnosis of endophthalmitis by direct analysis of vitreous humor in blood culture bottle by MALDI-TOF mass spectrometry
Introdução: As endoftalmites infecciosas apresentam prognóstico visual reservado, sendo essencial o diagnóstico rápido assegurando tratamento imediato. O diagnóstico etiológico precoce pode ser importante para adequação do antibiótico e definir a melhor conduta. A cultura de amostra de humor vítreo para isolamento e identificação do agente etiológico apresenta como principal desvantagem o tempo necessário de alguns dias para um resultado definitivo. Desta forma, a busca por técnicas que proporcionem a identificação rápida e precisa se faz necessária. Objetivos: avaliar a análise direta do humor vítreo, inoculado em frasco de hemocultura infantil, de pacientes com endoftalmite infecciosa utilizando a espectrometria de massas (EM) por ionização e dessorção a laser assistida por matriz por tempo de voo (matrix-assisted laser desorption/ionization, MALDI-TOF); comparar os resultados obtidos com o método convencional de cultura; analisar as características clínicas de acordo com os agentes etiológicos detectados; analisar os fatores que podem influenciar a positividade na identificação do agente etiológico. Métodos: estudo prospectivo, observacional com análise de amostras de humor vítreo, não diluído e diluído, de 96 pacientes com suspeita de endoftalmite infecciosa (critério de inclusão) diagnosticados no período entre outubro de 2015 e junho de 2017. O material foi inoculado em frasco de hemocultura e analisado pela cultura convencional e pela análise direta do humor vítreo pela EM MALDI-TOF. O tempo de identificação (turnaround time, TAT) pelos dois métodos foi comparado (teste de Wilcoxon pareado). Prognóstico visual após 3 meses do diagnóstico foi avaliado conforme o agente identificado. As variáveis avaliadas quanto à amostra foram: uso de antibiótico intravítreo prévio, amostra diluída ou não, obtida por biópsia vitrea ou por vitrectomia via pars plana e obtida do 1º ou 2º procedimento. Resultados: Dos 96 pacientes avaliados, foram excluídos dois casos por contaminação da amostra e sete casos por não preencherem os critérios de inclusão. Dentre os 87 pacientes incluídos, a cultura foi positiva em 60,9% (53 pacientes), sendo isoladas bactérias Gram-positivas em 46 casos (86,7%) e bactérias Gram-negativas em seis casos (11,3%); dois casos com cultura polimicrobiana. Não foi identificada nenhuma bactéria anaeróbia. Em três casos foram identificados Candida albicans. A mediana do TAT do agente etiológico foi 50,6 horas (variação entre 18,5 e 187,50 horas) e de 15 horas (variação entre 3,1 e 94,0 horas) pela cultura convencional e com EM MALDI-TOF, respectivamente (p < 0,001). A concordância da análise direta do frasco de hemocultura infantil com EM MALDI-TOF em relação à cultura convencional foi 81,1%, sendo 80,4% para bactérias Gram-positivas e 100% para bactérias Gram-negativas. Não houve significância na análise dos fatores que podem interferir nos resultados: uso de antibiótico ou não (positividade de 42,8% vs 64,3%, p=0,131); amostra de humor vítreo diluído ou não (TAT de 18,05 horas vs 17,03 horas, p=0,126); biópsia vítrea em relação à vitrectomia (positividade 93,1% vs 100%, p=0,531). Os agentes mais prevalentes foram Staphylococcus epidermidis (n=15; 28,3%), Streptococcus pneumoniae (n=9; 17%) e Staphylococcus aureus (n=6; 11,3%). Dentre os casos por Staphylococcus epidermidis, 60% e 86,7% apresentaram, no 3º mês pós-tratamento, acuidade visual melhor ou igual que 20/60 e 20/200, respectivamente. Nos demais casos, apenas 12,1% tiveram acuidade visual melhor ou igual a 20/200. Observou-se resistência a ciprofloxacino (93% a 100%), moxifloxacino (93% a 100%) e oxacilina (50% a 79%) pelos Staphylococcus epidermidis e Staphylococcus haemolyticus. Conclusão: O presente estudo aplicou a análise direta pela EM MALDI-TOF em amostras de humor vítreo inoculadas em frasco de hemocultura e demonstrou uma redução mediana de 67,6% no tempo para identificação do agente etiológico em relação ao método de cultura convencional. Este método mostrou-se viável na rotina de um laboratório de microbiologia. Observou-se que os casos de endoftalmite por Staphylococcus epidermidis apresentaram melhor prognóstico visual em relação aos outros agentes. Possíveis fatores que possam interferir na positividade das análises não foram significativosIntroduction: Infectious endophthalmitis presents a limited visual prognosis; rapid diagnosis is essential to assure prompt treatment. The early etiological diagnosis may be important to guide antibiotic therapy and to adjust treatment. The main disadvantage of using culture of vitreous humor for isolation and identification of the etiological agent is the required time of a few days for definitive results. Thus, the search for techniques that provide fast and accurate identification becomes necessary. Objectives: To evaluate direct analysis of vitreous humor in pediatric blood culture bottle of patients with infectious endophthalmitis using mass spectrometry (MS) with matrix-assisted laser desorption / ionization (MALDI-TOF); to compare the results obtained with the conventional culture method; to analyze the clinical characteristics according to the detected etiological agents; to analyze the factors that may influence positive identification of the etiologic agent. Methods: A prospective, observational study with undiluted and diluted vitreous humor samples of 96 patients with suspected infectious endophthalmitis (inclusion criterion), diagnosed from October 2015 to June 2017. The material was inoculated in a blood culture bottle and analyzed by conventional culture and by direct analysis of the vitreous humor by MALDI-TOF MS. The identification time (turnaround time, TAT) of the two methods was compared (Wilcoxon paired test). Visual prognosis three months after the diagnosis was assessed according to the identified agent. The variables evaluated in the sample were: the use of previous intravitreal antibiotics, diluted or not, obtained by vitreous biopsy or pars plana vitrectomy and obtained from the 1st or 2nd procedure. Results: Of the 96 patients evaluated, two cases were excluded due to contamination of the sample, while seven cases did not meet the inclusion criteria. Among the 87 patients that were included, the culture was positive in 60.9% (53 patients); Gram-positive bacteria were isolated in 46 cases (86.7%) and Gram-negative bacteria in six cases (11.3%); in two cases there was polymicrobial culture. No anaerobic bacteria were identified. In three cases Candida albicans were identified. The median TAT of the etiological agent was 50.6 hours (ranging from 18.5 to 187.50 hours), and 15 hours (ranging from 3.1 to 94.0 hours) in the conventional culture and with MALDI-TOF MS, respectively (p < 0.001). The agreement of results from direct analysis with MALDI-TOF MS in relation to the conventional culture was 81.1%, being 80.4% for Gram-positive bacteria and 100% for Gram-negative bacteria. There was no significance in the analysis of the factors that may interfere with the results: the use of antibiotic or not (positivity of 42.8% vs 64.3%, p = 0.131); diluted or non-diluted vitreous humor sample (TAT of 18.05 hours vs 17.03 hours, p = 0.126); vitreous biopsy in relation to vitrectomy (positivity 93.1% vs 100%, p = 0.531). The most prevalent agents were Staphylococcus epidermidis (n = 15; 28.3%), Streptococcus pneumoniae (n = 9; 17%) and Staphylococcus aureus (n = 6; 11.3%). Among the cases due to Staphylococcus epidermidis, 60% and 86.7% showed visual acuity better than or equal to 20/60 and 20/200, respectively, in the 3rd month after treatment. In other cases, only 12.1% had visual acuity better than or equal to 20/200. Resistance to ciprofloxacin (93 to 100%), moxifloxacin (93 to 100%) and oxacillin (50 to 79%) by Staphylococcus epidermidis and Staphylococcus haemolyticus was observed. Conclusion: The present study applied direct analysis by MALDI-TOF MS in vitreous humor samples inoculated in a blood culture bottle, and it showed a median reduction of 67.6% in time to identify the etiological agent in relation to the conventional culture method. This method proved to be feasible in the routine of a microbiology laboratory. It was observed that cases of endophthalmitis caused by Staphylococcus epidermidis had a better visual prognosis than other ones individually. Impact of visual function, inflammation and treatment in the quality of life of patients with non-acute Vogt-Koyanagi-Harada disease
OBJETIVO: Avaliar a qualidade de vida (QdV) e seus determinantes relacionados à função visual, inflamação e tratamento em pacientes com DVKH na fase não aguda. MÉTODOS: Estudo de coorte prospectivo, no qual foram incluídos 22 pacientes com diagnóstico da DVKH na fase não aguda atendidos na Clínica Oftalmológica do Hospital das Clínicas HCFMUSP, Faculdade de Medicina, Universidade de São Paulo. Todos os pacientes foram acompanhados sistematicamente desde a fase aguda, com avaliações clínicas, de imagem e funcional, em intervalos e tratamento padronizados. Como critério de inclusão, todos os pacientes tinham um seguimento >12 meses desde a fase aguda (DVKH na fase não aguda). Para avaliação da QdV relacionada à saúde geral e visual foram utilizados os questionários de QdV validados em português 36-Item Short Form Survey (SF-36) e National Eye Institute Visual Function Questionnaire 25-item (NEI VFQ-25), respectivamente. Os questionários de QdV, a sensibilidade ao contraste (SC), o campo visual (CV) e os eletrorretinogramas campo total (ERGct) e multifocal (ERGmf) foram avaliados em dois momentos com intervalo de 24 meses: entre junho e dezembro de 2017 (M1) e entre junho e dezembro de 2019 (M2). Como grupo controle, foram recrutados 22 indivíduos saudáveis pareados por sexo e idade ao grupo de estudo. RESULTADOS: Os 22 pacientes (20 mulheres; 90,9%) apresentaram idade média 35,8 ± 12,4 anos e duração média da doença 42,5± 22,2 meses no M1. Mais pacientes apresentaram fundoscopia moderada/grave no M2 vs M1 (p=0,046). Não houve diferença estatística entre os sinais de inflamação clínica e subclínica entre os dois momentos. Seis pacientes (27,3%) utilizaram monoterapia com corticoide sistêmico; em 16 pacientes (72,7%) foi associada imunossupressão sistêmica (IS). O imunossupressor mais utilizado foi a azatioprina (15 pacientes; 68,2%), seguida de micofenolato (oito pacientes; 22,7%). O número de IS foi maior no M2 (p=0,046) e houve mais infecção, como complicação no M2 (p=0,016). A acuidade visual (AV) era 20/20 no pior olho em 14 (63,6%) e 16 (72,7%) pacientes nos M1 e M2, respectivamente. Em relação aos indivíduos controles, os resultados em pacientes foram piores em três domínios (aspecto físico, estado geral da saúde e componente físico) do questionário SF-36 e em seis domínios (saúde geral, dor ocular, limitações nas funções, dependência, saúde mental e escore total) do questionário NEI VFQ-25; destes domínios, quatro permaneceram com resultados piores que os controles no M2. A SC monocular, os parâmetros do CV, do ERGmf e a fase fotópica do ERGct foram piores nos pacientes em ambos os momentos. Observou-se que, no questionário SF-36, todos os domínios, exceto saúde geral, foram impactados negativamente por fatores relacionados ao tratamento com corticoide e/ou IS tanto no M1 como no M2; fatores relacionados à presença de inflamação (células na câmara anterior) impactaram negativamente na capacidade funcional e nos aspectos sociais no M2; fatores relacionados à pior função visual (episódios de piora da AV, mean deviation no CV, gravidade do fundo de olho) impactaram negativamente na capacidade funcional, saúde geral e dor. Em relação ao questionário NEI VFQ-25, todos os domínios, exceto dirigir e saúde mental, foram impactados negativamente pelo tratamento e suas complicações no M1 e/ou no M2; inflamação (reativação da uveíte anterior no 1º ano de doença ou entre M1 e M2) impactou negativamente nos escores saúde geral, visão de perto, dirigir, dor ocular e no escore total, especialmente, no M2; pior função visual (episódios de piora da AV no 1º ano de doença, mean deviation e sensibilidade foveal no CV, fundoscopia e despigmentação difusa) impactou negativamente na maioria dos domínios deste questionário. Melhor SC monocular apresentou impacto positivo nos escores visão periférica no M1 e visão de perto no M2 e melhor SC binocular impactou positivamente no escore funcionamento social no M2. Ao se avaliar os resultados dos questionários SF-36 e NEI VFQ-25 no M2 vs M1, houve tendência à estabilidade/melhora dos resultados. Nos escores que apresentaram melhora entre os dois momentos, observou-se que esta se relacionou à ausência de tratamento sistêmico imunossupressor (estado geral da saúde, SF-36) e a sinais de melhor controle inflamatório, melhor função visual e menor necessidade de tratamento local e/ou sistêmico (dor ocular, dependência e saúde mental, NEI VFQ-25). CONCLUSÕES: Demonstrou-se que fatores relacionados à inflamação clínica e ao tratamento impactaram negativamente na QdV de pacientes com DVKH na fase não aguda. Dessa forma, este estudo ressalta a importância de se atingir um ponto de equilíbrio entre o benefício do controle inflamatório a longo prazo e o impacto negativo do tratamento sistêmicoOBJECTIVE: To evaluate the quality of life (QoL) and its determinants in relation to visual function, inflammation and treatment of patients with Vogt-Koyanagi-Harada disease (VKHD) in the non-acute phase. METHODS: This was a prospective cohort study that included 22 patients diagnosed with VKHD in the non-acute phase attended at the Ophthalmology Clinic of Hospital das Clinicas HCFMUSP, Faculdade de Medicina, Universidade de São Paulo. All patients were systematically followed up from the acute phase, with clinical, imaging and functional assessments at standardized intervals and treatment. As an inclusion criterion, all patients were required to have had > 12 months of follow-up since the acute phase (non-acute VKHD). Questionnaires validated in Portuguese, including the 36-Item Short Form Survey (SF-36) and the National Eye Institute Visual Function Questionnaire 25-item (NEI VFQ-25), were respectively used to assess general and visual health QoL. The QoL questionnaires, contrast sensitivity (CS), visual field (VF) and full-field (ffERG) and multifocal (mfERG) electroretinograms were evaluated twice with an interval of 24 months: between June and December 2017 (M1) and between June and December 2019 (M2). As a control group, 22 healthy individuals matched by sex and age to the study group were recruited. RESULTS: The 22 patients (20 women; 90.9%) had a mean age of 35.8 ± 12.4 years and mean disease duration of 42.5 ± 22.2 months at M1. More patients had fundoscopy moderate/severe in M2 vs M1 (p=0.046). There were no statistical differences between clinical and subclinical inflammation between the two moments. Six patients (27.3%) used systemic corticosteroid monotherapy; in 16 patients (72.7%) it was associated with systemic immunosuppression (IS). The most widely used immunosuppressant was azathioprine (15 patients; 68.2%), followed by mycophenolate (8 patients; 22.7%). The number of IS was higher in M2 (p=0.046) and there was more infection as a complication in M2 (p=0.016). Visual acuity (VA) was 20/20 in the worst eye in 14 (63.6%) and 16 (72.7%) patients at M1 and M2, respectively. Relative to the control subjects, patient outcomes were worse in three domains (physical aspects, general health status, and the physical component) of the SF-36 questionnaire and in six domains (general health, eye pain, functional limitations, dependence, mental health and total score) of the NEI VFQ-25 questionnaire. Of these domains, four remained with worse results than the controls in M2. Monocular CS, VF, mfERG parameters, and the ffERG photopic phase were worse in patients at both time points than the controls. It was observed that, in the SF-36 questionnaire, all domains except general health were negatively impacted by factors related to treatment with corticosteroids and/or IS in both in M1 and M2. Factors related to the presence of inflammation (cells in the anterior chamber) had a negative impact on functional capacity and social aspects in M2; factors related to impaired visual function (episodes of worsening VA, mean deviation in VF, eye fundus severity) had a negative impact on functional capacity, general health and pain. Regarding the NEI VFQ-25 questionnaire, all domains except driving and mental health were negatively impacted by the treatment and its complications in M1 and/or M2; inflammation (reactivation of anterior uveitis in the 1st year of disease or between M1 and M2) had a negative impact on general health scores, near vision, driving, eye pain and on the total score, especially in M2; worse visual function (episodes of worsening VA in the 1st year of the disease, mean deviation and foveal sensitivity in the VF, fundoscopy and diffuse depigmentation) had a negative impact on most domains of this questionnaire. Better monocular CS had a positive impact on peripheral vision scores at M1 and on near vision scores at M2, while better binocular CS had a positive impact on social functioning score at M2. When evaluating the results of the SF-36 and NEI VFQ-25 questionnaires at M2 vs. M1, there was a trend towards stability/improvement in the results. In the scores that showed improvement between the two moments, it was observed that this was related to the absence of immunosuppressive systemic treatment (general health status, SF-36) and to signs of better inflammatory control, better visual function, less need for local and/or systemic treatment (eye pain, dependency and mental health, NEI VFQ-25). CONCLUSIONS: Factors related to clinical inflammation and treatment were shown to have a negative impact on the QoL of patients with VKHD in the non-acute phase. Multimodal and electrophysiological analysis of patients with acute Vogt-Koyanagi-Harada disease treated prospectively with corticosteroid and early immunosuppression during a 24-month follow-up
OBJETIVO: Avaliar a evolução da inflamação ocular mediante análise multimodal e da função visual em pacientes com doença de Vogt-Koyanagi-Harada (DVKH), acompanhados desde a fase aguda e tratados com imunossupressão precoce associada à corticoterapia em altas doses, em um período de 24 meses de seguimento. MÉTODOS: Estudo prospectivo e longitudinal, incluindo pacientes com diagnóstico da DVKH desde a fase aguda, tratados com pulsoterapia de metilprednisolona seguida de prednisona 1mg/kg/dia com redução lenta, associada precocemente (em até 2 meses) à azatioprina. As avaliações sistemáticas consistiram em: exame clínico oftalmológico, retinografia, angiografias com fluoresceína sódica (AGF) e indocianina verde (ICGA), tomografia de coerência óptica (OCT) e com profundidade de imagem aprimorada (OCT EDI) e eletrorretinograma de campo total (ERGct). RESULTADOS: Foram incluídos 15 pacientes (14 mulheres), com idade mediana de 34 anos e tempo mediano de início do tratamento de 21 dias. No primeiro mês, 76,6% dos olhos apresentaram acuidade visual >= 20/40; houve redução da espessura de coroide (EC) subfoveal em 100% dos olhos e melhora do escore de dark dots (DD) em 80% dos olhos (comparando-se o mês 12 com o mês 1). Após os 6 meses de início da doença, foram observados sinais de inflamação clínica: 12 olhos (40%) com persistência/recorrência da celularidade de câmara anterior e cinco olhos (16,7%) com edema macular. Quanto aos sinais sugestivos de inflamação do segmento posterior, foram observados: flutuação no escore dos DD em 24 olhos (80%); aumento da EC em 13 olhos (43,3%); extravasamento perivascular em 17 olhos (56,7%) e dobras de coroide em seis olhos (20%). Os olhos foram agrupados de acordo com a presença ou não de redução >= 30% de qualquer um dos parâmetros da fase escotópica do ERGct entre os 12 e 24 meses de seguimento. Desse modo, 11 olhos (36,7%) foram classificados no grupo de piora e 19 olhos (63,3%) no grupo estável. Aos 24 meses, 23 olhos (76,7%) apresentavam ERGct subnormal em pelo menos um parâmetro. O grupo de piora apresentou uma maior média do escore de DD no mês 24 (p=0,047) e mais flutuações no escore de DD (90,9% dos olhos) (p=0,032). O grupo estável apresentou 52,6% dos olhos com flutuação da EC (p=0,002). A dose de prednisona = 20/40 em 90% dos olhos e controlando a inflamação clínica em 60% dos olhos. Entretanto, não foi capaz de suprimir os sinais subclínicos, sendo que 36,7% dos olhos evoluíram com piora dos parâmetros escotópicos do ERGct. Estes resultados sugerem a necessidade de um tratamento individualizado em uma parcela pequena de pacientes com evolução mais grave.OBJECTIVE: To evaluate ocular inflammation behavior through multimodal analysis and visual function of patients with Vogt-Koyanagi-Harada disease (VKHD) followed from the acute phase and treated with early immunosuppression (IMT) associated with high-dose corticosteroid therapy for a minimum period of 24 months. METHODS: A prospective and longitudinal study, in which patients diagnosed with VKHD from the acute phase were included. These subjects received initial treatment with methylprednisolone pulse therapy followed by prednisone 1 mg/kg/day with slowly regressive doses associated with early immunosuppression (within two months) with azathioprine. Systematic evaluations consisted of: ophthalmological clinical examination; retinography; fluorescein (FA) and indocyanine green (ICGA) angiographies; optical coherence tomography (OCT) also with enhanced depth imaging (EDI OCT) and full-field electroretinogram (ffERG). RESULTS: 15 patients (14 female) were included, with a median age of 34 years and median time of treatment onset of 21 days. In the first month, 76.6% of the eyes had best corrected visual acuity >= 20/40; subfoveal choroidal thickness (CT) reduced in 100% of the eyes, and dark dots (DD) score improved in 80% of the eyes (comparing month 12 with month 1). Six months after the onset of the disease, there were: 12 eyes (40%) with persistence or recurrence of anterior chamber cells and 5 eyes (16.7%) with macular edema. Regarding signs suggestive of posterior segment inflammation, the following were observed: DD score fluctuation in 24 eyes (80%); increase of CT in 13 eyes (43.3%); perivascular leakage in 17 eyes (56.7%) and choroidal folds in 6 eyes (20%). VKHD patients\' eyes were divided into stable and worsening groups, in the presence of any ffERG scotopic phase parameters reduction >= 30%, when 12- and 24-month exams were compared. Eleven eyes (36.7%) were classified into worsening group and 19 eyes (63.3%) into stable group. In the 24th month, 23 eyes (76.7%) presented subnormal ffERG in at least one parameter. The worsening group presented a higher mean DD score at month 24 (p=0.047) and more DD score fluctuations (90.9% of eyes) (p=0.032). 52.6% of the eyes of the stable group evolved with a fluctuation of CT (p=0.002). The dose of prednisone = 20/40 in 90% of the eyes and controlling clinical inflammation in 60% of the eyes. However, this therapy was not able to suppress subclinical inflammation and 36.7% of the eyes evolved with worsening of scotopic ffERG parameters. Optical coherence tomography angiography and microperimetry in retinal vasculitis associated with Behçets syndrome
Achados na angiotomografia de coerência óptica e na microperimetria têm potencial para servir como biomarcadores de diagnóstico e prognóstico em indivíduos com vasculite de retina associada à síndrome de Behçet (SB). Este estudo teve como objetivo analisar as alterações estruturais e funcionais da mácula em indivíduos com uveíte associada à SB, comparando-os com dois grupos controle. Dos 151 participantes elegíveis que atenderam aos critérios do International Criteria for Behçet Disease (2014), foram incluídos 14 indivíduos (23 olhos) com uveíte associada à SB, sem atividade por pelo menos 3 meses, com idade média de 40,6 ± 11,7 anos. Como controles, foram pareados por sexo e idade 13 indivíduos com SB sem uveíte (26 olhos) e 13 voluntários saudáveis (26 olhos). Assim, 75 olhos de 40 indivíduos foram analisados. A vasculite de retina foi avaliada por meio de angiografia fluoresceínica em todos os indivíduos com SB. Tomogramas da retina e angiogramas dos plexos vascular superficial, capilar intermediário e capilar profundo foram obtidos utilizando o tomógrafo de coerência óptica SPECTRALIS® (Heidelberg Engineering). Espessura total da retina, espessura das camadas de fibras nervosas e de células ganglionares, área e circularidade da zona avascular foveal, densidade vascular parafoveal e a presença de achados qualitativos (disrupção da arcada perifoveal, áreas de não perfusão e anormalidades microvasculares) foram analisadas. A sensibilidade macular central e parafoveal foi testada em 17 pontos com o microperímetro MP-3 (Nidek). Os dados estruturais e funcionais foram comparados com os grupos controle, assim como entre subgrupos definidos por dados demográficos (sexo e idade) e achados qualitativos. Entre os participantes com uveíte associada à SB, 18 olhos (78%) apresentaram melhor acuidade visual corrigida 20/25. Em comparação com os grupos controle, foram observadas as seguintes diferenças significativas (p < 0,05) por setor: afinamento da camada de células ganglionares nasal (22,4%) e inferior (14,9%); afinamento da espessura total da retina superior (6%), nasal (13,2%) e inferior (7,5%); e redução da densidade vascular superior (14,9%), nasal (23,6%), temporal (12,6%) e inferior (15,8%) no plexo capilar profundo. A microperimetria revelou uma redução difusa de 23,6% na sensibilidade macular. Elevações na camada plexiforme externa nos tomogramas e alterações qualitativas nos angiogramas foram observadas exclusivamente nos participantes com vasculite associada à SB e corresponderam a áreas de menor sensibilidade na microperimetria em 30,4% dos olhos. A densidade vascular no setor superior do plexo vascular superficial correlacionou-se diretamente com a sensibilidade macular central. Além disso, observou-se uma associação direta entre a densidade vascular e a espessura da retina e uma associação inversa entre a área da zona avascular foveal e a espessura da retina. A análise de subgrupos revelou que sexo masculino, idade menor que 45 anos e achados qualitativos nos plexos vasculares da retina estão associados a alterações estruturais retinianas mais proeminentes em olhos com uveíte associada à SB. Todas as alterações se mantiveram estáveis ao longo de 12 meses de seguimento. Nossos resultados sugerem que os achados quantitativos e qualitativos na angiotomografia de coerência óptica, em indivíduos com uveíte associada à SB, podem ser úteis para avaliar o comprometimento da microvasculatura retiniana, indicando indiretamente alterações na função macularFindings from optical coherence tomography angiography and microperimetry have the potential to serve as diagnostic and prognostic biomarkers in individuals with retinal vasculitis associated with Behçets syndrome (BS). This study aimed to analyze the structural and functional alterations of the macula in individuals with BS-associated uveitis, comparing them with two control groups. Of the 151 eligible participants who met the International Criteria for Behçet Disease (2014), 14 individuals (23 eyes) with BS-associated uveitis, without activity for at least 3 months, with a mean age of 40.6 ± 11.7 years, were included. As controls, 13 individuals with BS without uveitis (26 eyes) and 13 healthy volunteers (26 eyes) were matched by sex and age. Thus, 75 eyes from 40 individuals were analyzed. Retinal vasculitis was assessed by fluorescein angiography in all individuals with BS. Retinal tomograms and angiograms of the superficial vascular plexus, intermediate capillary plexus, and deep capillary plexus were obtained using the SPECTRALIS® optical coherence tomography (Heidelberg Engineering). Full retinal thickness, thickness of the nerve fiber and ganglion cell layers, area and circularity of the foveal avascular zone, parafoveal vascular density, and the presence of qualitative findings (disruption of the perifoveal arcade, non-perfusion areas, and microvascular abnormalities) were analyzed. Central and parafoveal macular sensitivity was tested at 17 points using the MP-3 microperimeter (Nidek). Structural and functional data were compared with the control groups, as well as among subgroups defined by demographic data (sex and age) and qualitative findings. Among participants with BS-associated uveitis, 18 eyes (78%) had a best-corrected visual acuity 20/25. Compared to the control groups, the following significant differences (p < 0.05) were observed by sector: thinning of the nasal (22.4%) and inferior (14.9%) ganglion cell layers; thinning of the superior (6%), nasal (13.2%), and inferior (7.5%) full retinal thickness; and reduction of the superior (14.9%), nasal (23.6%), temporal (12.6%), and inferior (15.8%) vascular density in the deep capillary plexus. Microperimetry revealed a diffuse 23.6% reduction in macular sensitivity. Elevations in the outer plexiform layer on tomograms and qualitative alterations in angiograms were observed exclusively in participants with BSassociated vasculitis and corresponded to areas of reduced sensitivity on microperimetry in 30.4% of eyes. Vascular density in the superior sector of the superficial vascular plexus was directly correlated with central macular sensitivity. Furthermore, a direct association was observed between vascular density and retinal thickness, and an inverse association was found between the area of the foveal avascular zone and retinal thickness. Subgroup analysis revealed that male sex, age under 45 years, and qualitative findings in the retinal vascular plexuses were associated with more prominent retinal structural changes in eyes with BS associated uveitis. All alterations remained stable over 12 months of follow-up. Our results suggest that quantitative and qualitative findings on optical coherence tomography angiography in individuals with BS-associated uveitis may be useful in assessing retinal microvascular impairment, indirectly indicating macular function alteration
Study of optical coherence tomography findings in Vogt-Koyanagi-Harada disease and their associations as prognostic biomarkers
Os avanços da tomografia de coerência óptica (OCT) têm permitido o estudo morfológico da retina em detalhes, assim como seu uso ampliado como marcador de doença, de prognóstico e de resposta ao tratamento. Na doença de Vogt-Koyanagi- Harada (DVKH) sabe-se que o tratamento precoce e adequado proporciona um bom prognóstico visual, no entanto, parte desses pacientes apresentam desfechos funcionais e estruturais desfavoráveis. Assim, a busca de biomarcadores de prognóstico poderia individualizar esses pacientes, permitindo um tratamento diferenciado. OBJETIVO: Avaliar as alterações morfológicas no exame de OCT de domínio espectral, desde a fase aguda e durante os primeiros 12 meses, de pacientes com DVKH e correlacioná-las com parâmetros de atividade inflamatória, tratamento, função visual e avaliação multimodal. MÉTODOS: Estudo retrospectivo baseado em levantamento de dados e exames de imagem de 33 pacientes (66 olhos) com DVKH, atendidos na Clínica Oftalmológica do Hospital das Clínicas HCFMUSP, Faculdade de Medicina, Universidade de São Paulo, desde junho de 2011 até março de 2021. Os critérios de inclusão foram: pacientes com o diagnóstico de DVKH em seguimento desde a fase aguda da doença, no mínimo de 12 meses, sob protocolos de tratamento e seguimento pré-definidos. As imagens obtidas na OCT (Spectralis HRA-OCT system, Heidelberg Engineering, Heildeberg, Alemanha) no início do tratamento, nos 1º, 3º, 6º, 9º e 12º meses foram lidas, de forma independente, por três especialistas em retina. As características analisadas foram: descolamento seroso (DRS), descolamento bacilar (DB), cistos intrarretinianos (CI), material sub-retiniano indefinido, integridade da zona elipsoide (ZE) e da zona de interdigitação (ZI), ondulações do epitélio pigmentado da retina (EPR), comprimento da linha do EPR, área e densidade óptica do fluido do descolamento retiniano (DR). Foram realizadas a análise estatística descritiva, equações de estimação generalizadas (EEG), função de ligação identidade com matriz de correlações permutáveis entre os olhos e regressão logística. RESULTADOS: A idade mediana dos pacientes foi de 33 anos (15 a 67 anos), sendo 29 do sexo feminino (88%). O tempo mediano de duração da doença foi de 85 meses (45 a 139 meses). A mediana da acuidade visual (AV) inicial foi 1,8 logMAR (0 a 2,3 logMAR) e 0 logMAR (0 a 0,7 logMAR) em M12. Aos 12 meses, sunset glow fundus (SGF) foi visto em 58% dos olhos (38/66), fibrose em 30% dos olhos (20/66) e o eletrorretinograma campo total (ERGct) subnormal em 76% dos olhos (47/62). O DRS estava presente em 96,4% (54/56) dos olhos em M0, em 42,4% (28/66) em M1 e 9,1% (6/66) em M3. O DB estava presente em 48,2% (27/56) dos olhos em M0 e em 6,1% (4/66) em M1. O período mediano de resolução do DRS e do DB foi dentro de um mês. Os CI foram vistos em todos os momentos. Nenhum olho apresentou a ZE e a ZI totalmente intactas em M0. Em M12, a ZE e a ZI estavam intactas em 48,4% (31/64) e 7,8% (5/64) dos olhos, respectivamente. As ondulações do EPR foram classificadas como moderada ou grave em 60% (32/53) dos olhos em M0 e em 13% (9/66) em M1. A persistência do DRS em M3 apresentou associação com fibrose aos 12 meses (p=0,030), membrana neovascular (p=0,04) e extravasamento perivascular (p<0,001). Foram observadas associações significativas entre o DB em M0 com SGF (p=0,008), fibrose (p<0,001) e maior área de DRS em M0 (p=0,036). Na análise de regressão logística, a presença do DB em M0 aumentou o risco em 12 vezes para evolução com fibrose e 14 vezes para SGF no primeiro ano da doença. A presença de CI em M0 foi associada com maior tempo de início de tratamento (44 vs 19 dias; p=0,012). A alteração da integridade da ZE em M3 apresentou associação com autofluorescência grave em M1 (p=0,047) e ERGct subnormal (p=0,036) em M12. O padrão denteado na camada plexiforme externa/ camada de fibras de Henle (CPE/HFL) foi visto em 81% (54/66) dos olhos e associado com melhor AV em M1 (0,175 vs 0,35 logMAR; p=0,006) e em M12 (0 vs 0,14 logMAR; p=0,004). CONCLUSÃO: Os achados tomográficos na fase aguda da DVKH apresentaram associações significativas com parâmetros de atividade inflamatória, com desfechos estruturais e funcionais. A presença do DB no diagnóstico destacou-se como um biomarcador precoce de pior prognóstico, evidenciado pelas alterações de fundo aos 12 meses. Por outro lado, o padrão denteado da CPE/HFL, achado descrito pela primeira vez em DVKH, associou-se ao melhor prognóstico visualAdvances in optical coherence tomography (OCT) have allowed the morphological study of the retina in detail as well as its expanded use as a marker of disease, prognosis, and response to treatment. In Vogt-Koyanagi-Harada disease (VKHD), it is known that early and adequate treatment provides a good visual prognosis; however, some of these patients have unfavorable functional and structural outcomes. Thus, the search for prognostic biomarkers could individualize these patients, allowing a differentiated treatment. OBJECTIVE: To evaluate the morphological alterations in the spectral domain OCT, from the acute phase and during the first 12 months, in patients with DVKH and to correlate them with parameters of inflammatory activity, treatment, visual function, and multimodal evaluation. METHODS: Retrospective study based on data collection and imaging studies of 33 patients (66 eyes) with VKHD treated at the Clínica Oftalmológica do Hospital das Clínicas HCFMUSP, School of Medicine, University of Sao Paulo, from June 2011 to March 2021. The inclusion criteria were: patients diagnosed with VKHD under follow-up since the acute phase of the disease, for at least 12 months, under predefined treatment and follow-up protocols. The images obtained in the OCT (Spectralis HRA-OCT system, Heidelberg Engineering, Heildeberg, Germany) at the beginning of the treatment in the 1st, 3rd, 6th, 9th, and 12th months were independently read by three retina specialists. The characteristics analyzed were serous retinal detachment (SRD), bacillary detachment (BALAD), intraretinal cysts (IC), undefined subretinal material, integrity of the ellipsoid zone (EZ) and the interdigitation zone (IZ), undulations of the retinal pigmented epithelium (RPE), RPE line length, sub-retinal fluid (SRF) area, and optical density of the SRF. Descriptive statistical analysis, generalized estimating equations (EEG), identity linkage function with an interchangeable correlation matrix between eyes, and logistic regression were performed. RESULTS: The median age of patients was 33 years (15 to 67 years), 29 of whom were female (88%). The median disease duration was 85 months (45 to 139 months). Median visual acuity (VA) at baseline was 1.8 logMAR (0 to 2.3 logMAR) and 0 logMAR (0 to 0.7 logMAR) at M12. At 12 months, sunset glow fundus (SGF) was seen in 58% of eyes (38/66), fibrosis in 30% of eyes (20/66), and subnormal fullfield electroretinogram (ffERG) in 76% of eyes (47/62). SRD was present in 96.4% (54/56) of eyes at M0, in 42.4% (28/66) at M1, and in 9.1% (6/66) at M3. BALAD was present in 48.2% (27/56) of the eyes in M0 and in 6.1% (4/66) in M1. The median SRD and BALAD resolution time was within one month. ICs were seen at all times. None of the eyes had the EZ and IZ fully intact at M0. At M12, the EZ and IZ were intact in 48.4% (31/64) and in 7.8% (5/64) of eyes, respectively. RPE undulations were graded as \"moderate\" or \"severe\" in 60% (32/53) of eyes at M0 and in 13% (9/66) at M1. The persistence of SRD in M3 was associated with fibrosis at 12 months (p=0.030), choroidal neovascular membrane (p=0.04), and perivascular leakage (p<0.001). Significant associations were observed between BALAD in M0 and SGF (p=0.008), fibrosis (p<0.001), and a larger SRF area in M0 (p=0.036). In the logistic regression analysis, the presence of BALAD in M0 increased the risk by 12 times for evolution with fibrosis and 14 times for SGF in the first year of disease. The presence of IC in M0 was associated with a longer interval to start treatment (44 vs. 19 days; p=0.012). The damage to the integrity of the EZ in M3 was associated with severe autofluorescence in M1 (p=0.047) and subnormal ffERG (p=0.036) in M12. The \"dentate\" pattern in the outer plexiform layer/Henle\'s fiber layer (OPL/HFL) was seen in 81% (54/66) of eyes and was associated with better VA in M1 (0.175 vs. 0.35 logMAR; p=0.006) and at M12 (0 vs. 0.14 logMAR; p=0.004). CONCLUSION: Tomographic findings in the acute phase of VKHD showed significant associations with parameters of inflammatory activity and with structural and functional outcomes. The presence of BALAD at diagnosis stood out as an early biomarker of a worse prognosis, as evidenced by fundus changes at 12 months. Integrated analysis of clinical, structural and functional parameters in the acute and non-acute phases of Vogt-Koyanagi-Harada disease: a prospective study
OBJETIVO: Descrever prospectivamente o curso da doença de Vogt-Koyanagi-Harada (DVKH) com integração de parâmetros de atividades clínicos, estruturais e funcionais. MÉTODOS: Foram incluídos pacientes com diagnóstico da DVKH na fase aguda (parte I) e não aguda (tempo de doença maior que 12 meses; parte II). Os pacientes na fase aguda receberam tratamento inicial padronizado com pulsoterapia de metilprednisolona seguido de corticoterapia oral em doses lentamente regressivas, pelo período de 15 meses. As avaliações consistiram em exame clínico, retinografia, angiografias com fluoresceína (AGF) e indocianina verde (AIV) e tomografia de coerência óptica (TCO). Foram realizadas nos seguintes momentos: parte I, no diagnóstico e meses 1, 2, 4, 6, 9 e 12; parte II, na inclusão e a cada três meses. Eletrorretinograma campo total (ERGct) e eletrorretinograma multifocal (ERGmf) foram realizados na parte I, no 1.o mês e a cada seis meses e, na parte II, na inclusão e com 12 meses. A leitura dos exames, na parte I, foi efetuada por duas leitoras, não mascaradas; na parte II, foi realizada por três leitores mascarados e treinados, sendo considerada a leitura concordante entre, pelo menos, dois examinadores. As angiografias e TCO foram realizadas no aparelho Spectralis® (HRA+OCT, Heidelberg Engineering). Tratamento adicional com corticoterapia em doses imunossupressoras ou intensificação da imunossupressão sistêmica foi indicado nos casos com recidivas clínicas, na presença de sinais de atividade à AGF ou duas pioras consecutivas >= 30% no ERGct. Os sinais de atividade detectados na AGF, AIV e TCO foram denominados sinais subclínicos. RESULTADOS: Na parte I, foram incluídos nove pacientes (7F/2M) com idade mediana de 33 anos e intervalo mediano entre início dos sintomas e tratamento de 13 dias. Na apresentação inicial, sinais clínicos característicos da doença (coroidite difusa com hiperemia do disco óptico, descolamento seroso de retina e uveíte anterior acompanhados de sinais extraoculares) melhoraram dentro dos primeiros 30 dias em todos os casos. Os principais sinais subclínicos variaram no tempo de melhora ou desaparecimento: espessura de coroide (EC) subfoveal diminuiu para o valor mediano de 347u m aos 30 dias; dark dots diminuíram ao longo do seguimento, porém ainda estavam presentes aos 12 meses. Piora da inflamação foi observada em 17 de 18 olhos no tempo mediano de sete meses quando a redução do corticoide oral atingiu a dose média de 0,3mg/kg/d. Os sinais subclínicos mais frequentemente observados foram dark dots, fuzzy vessels e aumento da EC. Em 10 destes 17 olhos a piora foi acompanhada de queda da função pelo ERG. Três padrões de evolução puderam ser caracterizados: sem recidivas clínicas ou subclínicas (padrão A, 1 olho), com recidivas subclínicas somente (padrão B, 11 olhos) e com recidivas clínicas (padrão C, 6 olhos). Identificou-se que a EC aos 30 dias após início do tratamento >= 506u m teve sensibilidade e especificidade > 80% na detecção dos casos com recidivas clínicas (padrão C). A função pelo ERGct e ERGmf permaneceu alterada em relação ao grupo controle com 24 meses, apesar da melhora progressiva observada desde o início do tratamento. Na análise longitudinal dos pacientes, a função entre 12 e 24 meses permaneceu estável no grupo de doentes que recebeu tratamento adicional (8 olhos), enquanto no grupo que não o recebeu (4 olhos) houve deterioração da função ( 30% on ERG. RESULTS: Nine patients (7F/2M) were included in part I; median age was 33 years old and median time elapsed from onset of symptoms to treatment was 13 days. At disease presentation, classic signs (choroiditis, anterior uveitis, serous retinal detachment, optic disc hyperemia and extraocular manifestations) were observed; they improved in 30 days after treatment. Subclinical signs improved in variable periods of time: subfoveal choroidal thickness (CT) decreased to a median value of 347 ?m, 30 days after the beginning of treatment, dark dots diminished during the follow-up but they were still observed at 12 months. Relapse (worsening of inflammation) was noticed in 17 of 18 eyes at a median follow up time of seven months, when tapering schedule corticosteroid dosage reached the mean dose of 0.3mg/kg/d of prednisone. Dark dots, fuzzy vessels and choroid thickening were the most frequent subclinical signs. Relapses in 10 of 17 eyes were concomitant with worsening on ERG. Three patterns of evolution could be delineated: no signs of inflammation (pattern A, 1 eye), only subclinical signs of inflammation (pattern B, 11 eyes) and clinical signs of inflammation (pattern C, 6 eyes). CT>=506 ?m 30 days after the beginning of treatment was more than 80% sensitive and specific to detect more severe cases (pattern C). ERG parameters at 24 months were subnormal as compared to the control group, despite improvement during follow-up. Further long-term results after 24 month demonstrated stabilization of ERG parameters in patients that had received additional treatment, whereas there was worsening in those patients who had not received additional treatment (p= 506 ?m had a greater than 80% sensitivity and specificity to detect cases with pattern C evolution. Considering subclinical signs, CT increase reliably detected recurrence, whereas angiographic signs required cautious interpretation. Sequential analysis was more conclusive than an isolated exam. Anterior chamber cells seemed to be the \"tip of the iceberg\" of a more diffuse inflammation. ERG analysis was subnormal even after 24 months of follow up since disease onset; additional treatment could prevent functional worsening in patients with subclinical signs of inflammation. Worse retinal function in patients with clinical recurrences (pattern C) in part II and subclinical recurrences (pattern B) in parts I and II suggest that ideal treatment of recurrences should be further pursue
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
