Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Identificatie van nieuwe immunomodulerende doelwitten en agentia in B lymphocyten
Immunosuppressive drugs are used for both solid organ transplantation and the treatment of immune disorders such as autoimmune diseases (arthritis, lupus, multiple sclerosis, psoriasis
) or allergies. The majority of the marketed immunosuppressive drugs control the activity of the T cells but more and more pieces of evidence describe the B cells as another key-player in the pathogenesis of various immune disorders. Therefore B cells are interesting targets to develop new drugs. Identification of new crucial molecules in the B cell activation cascades will be performed in vitro by genetic knock-out assays with phenotypic read-out. This broad approach strategy avoids the biases induced by a focus on specific pathways and can highlight the central role of proteins not yet described to be involved in these processes. Deep investigations in the biological functions of these proteins will clarify their role in the activation of immune cells. Once a protein is validated as an interesting target for new B cell-specific immunosuppressive therapeutics, a target based assay will be developed to screen broad chemical libraries in order to identify new chemical entities as potential candidates for new immunosuppressive drugs.status: Publishe
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. Rol van T-cel-onafhankelijke B cellen en Natural Killer cellen in concordante xenogreffe rejectie en tolerantie
Xenoreactieve antistoffen spelen een belangrijke rol in xenogreffe rejec tie. Zogenaamde natuurlijke antistoffen, aangemaakt door B1 B cellen t egen het Gal xenoantigen zijn verantwoordelijk voor hyperacute rejectie in modellen van discordante xenotransplantatie tussen lagere species (di e Gal expressie vertonen) en niet-humane primaten (die geen Gal expressi e vertonen). Naast het Gal antigen zijn er nog andere xenoantigenen waar tegen antistoffen worden gevormd en deze komen tussen in acute en chroni sche xenogreffe rejectie, in zowel discordante als concordante situaties . De B cellen uit de milt zijn de belangrijkste bron van xenoreactieve a ntistoffen gericht tegen niet-Gal antigenen; zulke IgM xenoantistof resp ons kan verlopen zonder hulp van T cellen en de zogenaamde snel-geinduce erde IgM xenoantistofrespons is in concordante modellen verantwoordelijk voor acute xenogreffe rejectie. In dit werk behandelden we de rol van m arginale zone (MZ) B cellen (uit de milt) in de snel-geinduceerde T cel independente IgM xenoantistof respons. Daarnaast bestudeerden we ook de rol van NK cellen in snel-geinduceerde IgM xenoantistofvorming (kritisch betrokken in acute xenogreffe rejectie), alsook hun rol in het ontstaan van chronische xenogreffe rejectie. Vooreerst konden we aantonen dat MZB cellen een kritische rol spelen in snel-geinduceerde T-cel independente condordante IgM xenoantistofprodukt ie. Dit was duidelijk omdat T-cel deficiente muizen na splenectomie of n a lage dosis totale lichaamsbestraling (wat selectief MZ B cellen voor k orte tijd vernietigt), niet meer in staat bleken om een snelle IgM xenoa ntistofrespons teweeg te brengen. Het effect van lage dosis bestraling o p MZB cellen konden we documenteren zowel dmv. flowcytometrie als dmv. i mmunohistochemie. Onze data bevestigden een gelijkaardig effect van best raling op de IgM antistofrespons tegen TNP-Ficoll, een conventioneel typ e 2 T cel-independent antigen, gezien T cel-deficiente muizen na bestral ing een significante reductie vertoonden in de anti-TNP Ficoll IgM respo ns. Dmv. adoptieve transfer experimenten in SCID muizen konden we aanton en dat inderdaad MZ B cellen, en niet B1B cellen (tevoren aangetoond ver antwoordelijk te zijn voor de antistofrespons tegen Gal antigenen), vera ntwoordelijk zijn voor de in vivo IgM xenoantistofrespons. We besluiten daarom dat MZB cellen een kritische rol spelen in T cel-indepe ndente concordante xenogreffe rejectie. Niet alleen de MZ B cellen zelf maar ook een intacte anatomische structu ur van de marginale zone in de milt is een absolute vereiste voor een pr oduktieve IgM xenoantistofrespons. Dit bleek uit de waarneming dat IL7-/ - muizen, ondanks de aanwezigheid van voldoende cellen met het fenotype van MZ B cellen (CD21highCD23low), en zelfs na adoptieve transfer van bi jkomende IL7-/- miltcellen, deze muizen niet in staat bleken om een xeno greffe te rejecteren of een snel-geinduceerde IgM xenoantistofrespons te genereren. Inderdaad, adoptieve transfer van T cel-deficiente TCR-/- B cellen kon evenmin dit defect van IL7-/- muizen herstellen. De data sugg ereren daarom dat dit te maken heeft met een defect van de anatomische s tructuur van de marginale zone in IL7-/- muizen. Of dit te maken heeft m et een effect op non-B cellen in de marginale zone, zoals metallophilisc he macrofagen of dendritische cellen, of op bv. follicluaire dendritisch e cellen, is tot dusver nog onduidelijk. We besluiten dat MZ B cellen, m et een correcte lokalisatie in een intacte microanatomische marginale zo ne, noodzakelijk zijn voor een produktieve IgM xenoantistofrespons tegen concordante xenoantigenen. In een volgend deel van dit werk hebben we bestudeerd in welke mate NK c ellen bijdragen tot de snel geinduceerde IgM xenoantistofrespons, verant woordelijk voor acute xenogreffe rejectie in de T cel-deficiente setting . We stelden vast dat NK cellen een hulp signaal leveren aan MZ B cellen en hen in staat stelt IgM xenoantistoffen te produceren. Dit was duidel ijk uit verschillende waarnemingen. Ten eerste, in vivo NK depletie resu lteerde in een significante daling van de IgM xenoantistofresons. Ten tw eede, adoptieve transfer van NK 1.1+ cellen van C57BL/6 nude muizen naar BALB/c nude muizen corrigeerde het inherent defect van deze laatste mui zen om xenoantistoffen te produceren. Een gelijkaardig effec t zagen we voor de antistofrespons tegen TNP-Ficoll in BALB/c muizen, aa ngezien deze significant sterker was na adoptieve transfer van C57BL/6 N K1.1+ cellen. Wat betreft het onderliggend mechanisme, konden we aantone n dat 1° NKT cellen geen rol spelen, 2° NK cel-gemedieerde IFN-g product ie geen rol speelt, en 3° dat NK cel-gemedieerde lysis van xenogenetisch e doelwitcellen geen rol speelt. Daarnaast konden we evenmin verschillen aantonen in NK cel receptoren Ly49A, D, C/I in nude muizen die hetzij e en xenogreffe rejecteerden of die een xenogreffe permanent hadden aanvaa rd, hetgeen pleit tegen een specifieke rol van een of andere van d eze NK Ly49 receptoren. In een volgende stap onderzochten we of CD40-CD40L interactie een rol sp eelt in de MZ B cel xenoantistof respons. We konden aantonen dat in vivo blokkering van deze interactie door anti-CD40L antistoffen een sterke r eductie teweegbracht van de snel-geinduceerde IgM xenoantistofrespons na immunisering met hamster rode bloed cellen. Nochthans was de antistofti ter nog significant hoger dan die van muizen waarin MZ B cellen werden u itgeschakeld door lage dosis bestraling. We besluiten daarom dat CD40-CD 40L interactie slechts ten dele betrokken is in de IgM xenoantistofrespo ns. Of deze interactie plaatsgrijpt tussen MZ B cel en NK cel, wordt mom enteel verder bestudeerd. In het vierde deel van dit werk onderzochten we het effect van in vivo N K cel depletie in combinatie met lage dosis bestraling op acute en chron ische xenogreffeletsels. Twee verschillende NK cel-depleterende antistof fen werden getest: anti-NK1.1+ mAb en anti-asialoGM1 polyclonaal Ab. Gee n van beide toonde een effect op greffe-overleving, die in alle gevallen langer dan 3 maanden was. Echter hoewel de twee behandelingsregimes gel ijkaardige effecten toonden op depletie en herstel van NK cellen, NK cel subsets en NK cel functie, ontwikkelden de muizen behandeld met anti-NK 1.1+ mAb chronische vasculaire letsels, dit in tegenstelling tot diegene n behandeld met polyclonaal anti-asialoGM1 Ab. We stelden vast dat dit t ype vasculopathie niet geassocieerd was met verhoogde titers van circule rende IgM of IgG xenoantistoffen. Bovendien toonde immuunhistochemie gee n verband tussen vasculopathie en intensiteit van IgM of IgG vasculaire depositie. In tegenstelling, de greffes met vasculopathie toonden deposi tie van complement. We onderzochten vervolgens welke cytokines worden vr ijgezet als resultaat van de NK cel-depleterende behandeling. We stelden vast dat beide behandelingsregimes aanleiding gaven tot belangrijke vri jzetting van MCP-1, terwijl dit niet werd gedetecteerd in controledieren . Bovendien detecteerden we sterke vrijzetting van IL-6 na anti-asialoGM 1-Ab behandeling maar niet na anti-NK1.1+ mAb behandeling. De data sugge reren daarom dat vroege vrijzetting van IL-6, en vroege complement depos itie een rol spelen in de ontwikkeling van chronische vasculaire letsels . De mogelijke relatie tussen deze twee fenomenen, en de exacte relatie tot ontwikkeling van chronische letsels is tot dusver niet duidelijk.status: Publishe
Nieuwe benaderingen tot balans van Graft-versus-Leukemia and Graft-versus-Host immuun effecten van een allogene hematopoeitische stamcel greffe: studies in muizen
In dit proefschrift onderzochten we twee nieuwe strategieën van post-transplant immunotherapie die in muismodellen een antileukemisch effect kunnen induceren zonder het ontwikkelen van Graft-versus-Host ziekte (GvHZ): CTLA-4-blokkade en recipient leukocyten infusie. In beide benaderingen ligt de gastheer aan de basis van het antileukemisch effect, en bijgevolg kan deze vorm van immunotherapie geen risico dragen voor de ontwikkeling van GvHZ. Het doel was om de specifieke aspecten van deze 2 nieuwe benaderingen te onderzoeken in een model van MHC-matched allogene beenmergtransplantatie. Wij kozen voor een MHC-matched model aangezien 50 tot 85% van alle allogene hematopoietische stamceltransplantaties in de kliniek HLA-matched zijn. Het uiteindelijke doel van dit werk was om een bijdrage te leveren aan de verdere ontwikkeling van deze strategieën in de kliniek.Fase II/III klinische studies hebben aangetoond dat CTLA-4-blokkade, alleen of als combinatie-therapie, een immuungemedieerde vernietiging van solide tumoren kan bevorderen. In dezelfde lijn had vroeger onderzoek in het gastlabo aangetoond dat behandeling met het blokkerende anti-CTLA-4 mAb na allogene beenmergtransplantatie een antileukemisch effect kon teweeg brengen zonder GvHZ, maar het was geassocieerd met de ontwikkeling van een systemisch auto-immuun lymfoproliferatief syndroom. Kort na deze publicatie verscheen de eerste klinische studie waaruit bleek dat anti-CTLA-4 mAb therapie remissie kan induceren bij herval van hematologische maligniteiten na allogene hematopoietische stamceltransplantatie. Het ontwikkelen van autoimmuniteit is de voornaamste complicatie van deze CTLA-4-blokkerende therapie zowel in de kliniek als in muismodellen, maar de onderliggende pathogenese is nog onduidelijk. Het doel van het eerste deel van dit proefschrift was om de oorsprong en de pathogenese van het autoimmuunsyndroom, dat gezien wordt na anti-CTLA-4 mAb therapie, te onderzoeken in MHC-matched beenmerg chimeren.Ten eerste vonden we dat zowel bestraling als een beenmerggreffe van allogene oorsprong noodzakelijke voorwaarden zijn voor de ontwikkeling van autoimmuniteit na anti-CTLA-4 mAb therapie. Ten tweede onderzochten we of de transplantatieprocedure en/of anti-CTLA-4 mAb therapie een effect had op het clonale deletie proces in de thymus. We toonden aan dat het autoreactieve T cel repertoire reeds aanwezig was vóór aanvang van de behandeling, en dat de klonale deletie effectief verliep na de inductie van gemengd chimerisme. Ten derde onderzochten we het effect van CTLA-4-blokkerende therapie op de regulatoire T (Treg) cellen aangezien de Treg cellen self-reactieve T cellen onderdrukken en afhankelijk zijn van CTLA-4 voor hun functie. Dit is tevens een kritische vraag is in het onderzoeksdomein van de anti-CTLA-4 mAb therapie. We toonden aan dat anti-CTLA-4 mAb behandeling aanleiding geeft tot een expansie - zonder voorafgaande tijdelijke vermindering - van Treg cellen. Bovendien konden we geen defect in functie van Treg cellen, afkomstig uit autoimmune dieren, aantonen inhun in vitro onderdrukkende functie ten opzichte van pathogene autoreactieve effector T cellen, maar vertoonden ze eerder een geactiveerd cytokine profiel in vivo. Tot slot veronderstelden we dat in het post-transplantatie T cel milieu, memory T (Tmemory) cellen fungeren als het direct doelwit van CTLA-4-blokkerende therapie, en we vonden dat na allogene beenmergtransplantatie het T cel compartiment voornamelijk is samengesteld uit de gastheer Tmemory cellen, die toenemen tijdens de ontwikkeling van auto-immuniteit.We vermoeden dat de gecombineerde invloed van de blokkade van het CTLA-4 signaal in Tmemory cellen, de nabijheid van actieve hematopoiese, en de aanwezigheid van alloreactiviteit verantwoordelijk zijn voor de expansie van Tmemory cellen en het verlagen van hun drempel tot activatie in het post-transplantatie milieu. De autoimmuniteit is uitsluitend gastheer-gemedieerd, aangezien het post-transplant perifere T cel compartiment hoofdzakelijk is samengesteld uit gastheer Tmemory cellen. Tot slot veroorzaakt CTLA-4 blokkade niet zozeer een defect in Treg cellen, maar leidt het tot hun activatie en expansie, als een compensatoir fenomeen om autoimmune T cellen te controleren.Deze onderzoeksgegevens dragen bij tot het begrijpen van de effecten en neveneffecten van CTLA-4-blokkerende therapie bij zowel transplantatie als niet-transplantatie patiënten en op deze manier kunnen ze helpen bij het verbeteren van de efficiëntie en veiligheid van dit soort therapie.Recipient leukocyten infusie is effectief gebleken bij het induceren van antileukemische effecten in een muismodel van MHC-mismatched beenmergtransplantatie en een eerste klinische trial is lopende. De basis voor het ontwikkelen van een dergelijke strategie ligt in de klinische observatie dat een Host-versus-Graft gemedieerde lymphohematopoietische T cel allorespons, aanwezig tijdens de rejectie van een donorgreffe, gepaard kan gaan met een anti-tumor respons. De proof of principle van RLI werd gegeven in MHC-mismatched beenmerg chimeren waar RLI aanleiding gaf tot een significant gastheer-gemedieerd antileukemisch effect, maar waar dit gepaard ging met een volledig verlies van donor chimerisme. Strategieën die dit soort van gastheer-gemedieerde Host-versus-Graft antileukemische reactie uitlokken, hebben enerzijds het theoretische voordeel van het vermijden van GvHZ, maar anderzijds, hebben ze het gevaar van een volledige rejectie van de donorgreffe. We onderzochten de effecten van RLI in een model van MHC-matched beenmergtransplantatie omdat vorige onderzoekers in het gastlabo vonden dat MHC-matched chimeren anders reageren op een uitdaging met RLI dan MHC-mismatched chimeren: terwijl MHC-mismatched chimeren de beenmerggreffe rejecteren na RLI, doen MHC-matched chimeren dit niet. Deze bevinding wijst er bovendien op dat het verweer van MHC-matched beenmerg chimeren tegen een uitdaging met niet-tolerante gastheer lymfocyten minder sterk is dan die tegen de donor lymfocyten na DLI. Hier hebben we geprobeerd om meer in detail na te gaan hoe adoptieve celtherapie met RLI na MHC-matched beenmergtransplantatie een lymfohematopietische T cel alloreactiviteit beïnvloedt, ofdat zij ook een antileukemisch effect met zich meebrengt en zo ja welke mechanismen hierbij betrokken zijn.We toonden aan dat in de MHC-matched setting, RLI een lymfohematopietische Host-versus-Graft reactie uitlokt. Enerzijds herkennen RLI-afgeleide CD4+ T cellen antigen presenterende cellen (APCs) van donor origine aanwezig in de chimeer en anderzijds ontmoeten zich innestellende donor T cellen extra gastheer APCs afkomstig uit het RLI inoculum. Deze lymphohematopoietic alloreactiviteit resulteert in een late en gedeeltelijke daling van de donor chimerisme, dat toch voldoende is om te resulteren in een significant antileukemisch effect. De studie van de betrokken mechanismen toonde dat CD8+ T cellen kritisch bijdragen aan het RLI-effect en suggereerde een belangrijke betrokkenheid van asialoGM+CD8+ T cellen als alloreactieve T cellen in het tot stand brengen van een lymphohematopoietische en antileukemische activiteit. Bovendien hadden we aanwijzingen dat tijdens de innestelling van allogene stamcellen, NK cellen een anti-tumor cytotoxische capaciteit verwerven, wat een mate van bescherming tegen leukemie verleent aan klassieke allogene beenmerg chimeren. Hoewel dit het bewijs levert dat lymphohematopoietische alloreactiviteit kan leiden tot een in vivo 'lymphokine geactiveerd killer' activiteit, moet nog worden vastgesteld of dit type NK reactiviteit bijdraagt tot een RLI-geïnduceerde antileukemisch effect. Daarnaast is er de interactie tussen NK cellen en CD8+ T cellen die een rol kan spelen in deze effecten. Dit is het onderwerp van lopende onderzoeken. Deze studies kunnen helpen bij het voorspellen van de effecten van RLI wanneer dit zou toegepast worden op patiënten die een HLA-matched allogene hematopoïetische stamceltransplantatie hebben ondergaan.status: Publishe
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. De immuun anti-tumor effecten van allogene beenmergtransplantatie en adoptieve cel therapie met donor en gastheer leukocyten infusies in muizen met neuroblastoma
Allogeneic hematopoietic stem cell transplantation(HSCT) and posttransplant adoptive cell therapy with donor leukocyte infusions(DLI) are known to exert potent graft-versus-leukemia effects and convincing evidence is emerging that this approach can also elicit responses against solid tumors. Graft-versus-leukemia effects – specifically after DLI – largely rely on lymphohematopoietic graft-versus-host T cell alloreactivity which can result in graft-versus-host disease when it extends to epithelial tissues. Not only for hematological but also for solid tumors the association of anti-tumor effects with graft-versus-host disease represents the major obstacle to the successful application of allogeneic HSCT and adoptive DLI therapy as an immunotherapeutic strategy. Recent data in mice have shown that challenge with recipient-type rather than donor-type leukocytes (recipient leukocyte infusion,RLI) can equally provide antitumor effects in hematological malignancies. RLI provokes lymphohematopoietic host-versus-graft T cell reactivity with rejection of the graft and associated anti-leukemic responses and does not carry the risk of graft-versus-host disease. This strategy is currently being explored clinically in patients with hematological malignancies. In experimental mouse models, it was demonstrated that NK cells, activated in vivo in the course of RLI-induced T cell alloreactivity, contribute to the survival advantage produced by RLI. Allogeneic hematopoietic stem cell transplantation may also hold potential for the treatment of high-risk neuroblastoma, a common extracranial solid tumor in childhood that carries a bleak prognosis. Despite aggressive treatment including high-dose chemotherapy and lymphohematopoietic rescue with autologous hematopoietic stem cells, relapse is very frequent. This spurs research into post-transplant immunotherapeutic interventions to enhance antitumor immunity. Neuroblastoma cells are known to be sensitive to NK cell cytotoxicity and strategies targeting NK cells are therefore particularly of interest. Alternatively, case reports suggest that allogeneic HSCT in high-risk neuroblastoma patients may prolong survival and thus confers a graft-versus-neuroblastoma effect. This has been experimentally demonstrated in mice. A phase I/II trial is currently investigating the anti-tumoreffect of haplo-identical HSCT in children with neuroblastoma. The overall aim of this study was to investigate the potential combined effect of allogeneic HSCT and the novel RLI strategy on the growth of experimentally induced neuroblastoma tumors. Given the efficacy of RLI in enhancing graft-versus-leukemia effects in mice, the evidence that such effects involve not only T cells but also NK cells, and the knowledge that neuroblastoma cells are generally sensitive to NK cell cytotoxicity, we hypothesized that RLI may be an effective and safe alternative to DLI to enhance the immune-mediated graft-versus-neuroblastoma effect. Immune interventions after standard therapy with autologous HSCT for neuroblastoma can –to some extent– enhance endogenous anti-neuroblastoma immunity. In mice it has been shown that T cells devoid of anti-host alloreactivity may foster anti-neuroblastoma effects of allogeneic bone marrow transplantation(BMT) in mice. This supported our hypothesis that challenge with syngeneic mature effector cells may –in addition to causing alloreactive T cell responses– foster direct anti-tumor reactions. The specific aim of this work was to study the effects of RLI adoptive therapy on the local and metastatic growth of neuroblastoma tumors in allogeneic bone marrow chimeras. We studied RLI in comparison with DLI since this is the standard approach used in hematological malignancies ánd the approach currently studied in clinical trial for high-risk neuroblastoma. We used mouse models off local and systemic posttransplant neuroblastoma relapse in a setting of MHC-mismatched allogeneicBMT: we chose to inoculate mice after the transplant to study immune anti-tumor effects independently of direct effects of recent radiation therapy on the tumor. We used three different models. First, a focal model in which tumor cells are injected subcutaneously and give rise to a local and clinically measurable tumor (subcutaneous model). Second, a systemic model where tumor cells are inoculated intravenously and thus cause direct metastatic spread (intravenous model). Third, a more physiological and preclinical model of metastatic spread where tumor cells are inoculated under the kidney capsule and where the local tumor is removed in toto after 7 days by a nephrectomy (kidney model). For the systemic and kidney models, we generated a luciferase-expressing tumor cell line and developed a bioluminescence imaging protocol to non-invasively follow metastasis-free survival in vivo. In part I of this work we used the subcutaneous model to study the effects of allogeneic BMT and adoptive cell therapy on local neuroblastoma growth. We confirmed that allogeneic BMT in mice provides a baseline graft-versus-neuroblastoma effect. We next demonstrated that bothDLI ánd RLI, when given to mixed bone marrow chimeras, further slow the local growth of subcutaneous implanted neuroblastoma tumors. DLI provoked a rapid conversion to full donor chimerism but mice developed severe GvHD, whereas RLI produced complete graft-rejection but left mice healthy. The chimerism of intratumoral leukocytes evolved in parallel with peripheral blood chimerism and was -both after DLI and RLI- associated with increased intra-tumor CD8/CD4ratios, CD8+ T-cell IFN-g-expression and NK-cell Granzyme B-expression. The effect of RLI was weaker than that of DLI but co-transfer of naïve recipient-type NK cells could elicit a growth-limiting effect on neuroblastoma tumors in RLI chimeras. In part II we investigated the effects of posttransplant adoptive cell therapy on the systemic spread of neuroblastoma. The final prognosis for patients with solid tumors, and especially for neuroblastoma, is determined by the presence of distant therapy-non-responsive metastases. Novel forms of immunotherapy therefore only hold clinical potential if they are efficacious on both local and systemic spread of the tumor. Using the intravenous tumor model we documented that allogeneic BMT provides a significant overall survival benefit relative to syngeneic BMT. Moreover, using luciferase-expressing tumor cells and bioluminescence imaging we showed that both DLI and RLI produce a significant delay in metastatic spread. The bioluminescence imaging studies in the kidney model -although preliminary- were consistent with those of the intravenous model and support the notion that both DLI and RLI have the potential to limit metastatic spread of neuroblastoma. In part III we addressed the question whether syngeneic BMT by itself can provide an immune anti-neuroblastoma effect. The current standard treatment of high-risk neuroblastoma includes high-dose chemotherapy and autologous HSCT which serves merely as a lymphohematopoietic rescue. However, the potential intrinsic immune effects of this procedure have so far not been investigated. Our data are preliminary but show that, relative to naïve mice, syngeneic BMT facilitates tumorigenesis of subcutaneously inoculated tumor cells while on the other hand it delays the growth of established tumors. CD8+ T and NK cells synergistically regulated tumor growth. Direct evidence of CD8+ T cells could not be found. However, the role of NK cells is supported by the evidence of increased Fas-L expression and by the fact that adoptively transferred syngeneic NK cells could reinforce the reduction in tumor growth. In conclusion, the data support the potential value of DLI in mediating graft-versus-solid tumor effects. Importantly, they also deliver the first experimental evidence that RLI is able to target a solid tumor and that it thus may represent a safe alternative to DLI for this purpose. We document that lymphohematopoietic alloreactivity extends to the environment of the tumor itself, indicating that also for solid tumors it is closely related with the antitumor effect. Studies on the leukocytes contained within the focal neuroblastoma tumors provided evidence strongly suggesting that the antitumor effect of both DLI and RLI involves not only CD8+ T cells but also cytotoxic NK cells. Whereas in the DLI setting, the antitumor effect may rely predominantly on alloreactive CD8+ T and NK cells, we provided evidence that the T cell alloreactivity provoked by an RLI challenge suffices to provide syngeneic naïve NK cells with the potential to attack syngeneic neuroblastoma cells in vivo. We conclude therefore that RLI and DLI in allogeneic chimeras can provoke an in vivo lymphokine-activated killer phenomenon leading to cytotoxically active NK cells, even in the syngeneic setting. The alloreactive T cell response produced by RLI and DLI may also facilitate the activation and expansion of tumor-specific donor or host CD8+T cells. Consistent with prior studies in leukemia models the RLI effect was weaker than the DLI effect but this could be compensated by the additional infusion of syngeneic NK cells, a procedure that does not compromise the safety profile of RLI. Lastly, although to some extent preliminary, the data indicate that the effects of both DLI and RLI are not limited to the local growth of atissue-localized tumor but can also limit the metastatic spread of a solid tumor. This is a critical requirement for a novel form of immunotherapy to beclinically relevant. The data may be particularly helpful to understand the value of allogeneic HSCT and adoptivecell therapy for neuroblastoma. We demonstrate that this approach may provide asynergistic T cell and NK cell interaction resulting in an effective antitumorresponse against neuroblastoma, even in the setting where RLI is used. This is particularly interesting since neuroblastoma is wellknown to be sensitive to NK cell cytotoxicity. With this respect, the data also suggest that NK cells equally contribute to the baseline antitumor effect of allogeneic HSCT inneuroblastoma. Last, our preliminary observations in the syngeneic BMT model show that the effects of conditioning and autologous lymphohematopoietic reconstitution on neuroblastoma growth are complex and may include paradoxical effects on tumorigenesis and tumor progression.status: Publishe
Immuunmodulerende werking van 1,25-dihydroxyvitamine D3 en analogen: focus op T lymfocyten
The bioactive form of vitamin D3, 1,25-dihydroxyvitamin D3 [1,25(OH)2D3], and its synthetic analogs are known as immunomodulators, which are able to prevent the development of type 1 diabetes (T1D) in non-obese diabetic (NOD) mice. Up to now, the disease-modifying actions of these compounds were believed to rely on their ability to modulate dendritic cell function, thereby indirectly shifting T cell responses from an immunogenic towards a tolerogenic profile. Importantly, receptors for vitamin D (VDR) are also expressed by T lymphocytes. However, when we initiated this work, effects involving direct T cell modulation were controversial and poorly characterized. Therefore, we focused in this work on the potential role of T cells as direct targets of 1,25(OH)2D3 and potent analogs. The data obtained in Chapter 4 clearly unveil human T cells as direct targets for 1,25(OH)2D3 in the immune system. T cell activation, triggered by distinct activation stimuli, strongly elevated the expression of genes related to vitamin D metabolism, including VDR and the vitamin D-activating enzyme 1-α-hydroxylase. In accordance, pre-activated T cells that feature maximal VDR expression levels exhibited an enhanced sensitivity to the actions of active vitamin D as compared to resting T cells, and this effect was even further amplified by chronic exposure to the ligand. Thus, with the appreciation that both timing and frequency of 1,25(OH)2D3-treatment represent critical factors for efficient activation of VDR signaling pathways, we developed an in vitro protocol that can be used as screening system to identify new immunomodulatory actions of 1,25(OH)2D3 or synthetic agonists on T cells. In Chapter 5, we then used this optimized in vitro protocol to study the direct impact of TX527, a structural vitamin D analog, on human T cell responses. A microarray analysis demonstrated that TX527 affected T cell responses in an integrated manner, revealing a preferential regulation of genes involved in growth and proliferation, cell death, cellular development, cellular movement and cell-to-cell signaling and interaction. Additional analysis confirmed these findings at a phenotypical and functional level, showing that TX527 decreased T cell activation and proliferation, as well as pro-inflammatory cytokine expression. At the same time, this vitamin D analog triggered the induction of a functional CD4+CD25highCD127low regulatory T cell (Treg) population and imprinted T cells with a unique migratory signature which would target them to sites of inflammation. Together, these findings markedly improve our understanding of the role of vitamin D analogs as direct regulators of T cell responses and further complement our current knowledge on the mechanisms by which these compounds can modulate the immune system in vivo. In the second part of this thesis (Chapter 6), we exploited the immunomodulatory effects of TX527 in vivo in the model of syngeneic islet transplantation in diabetic NOD mice. As previously shown, vitamin D analogs can prevent autoimmune diabetes development in NOD mice, but co-administration of other immunomodulating agents such as cyclosporine A (CsA) is needed to intervene in established diabetes. Here, we chose to combine TX527 also with anti-CD3 monoclonal antibodies (mAb) because to date, anti-CD3 mAbs represent one of the most promising immune therapies to revert recent-onset T1D, but their clinical use encounters important side effects. Given the ability of vitamin D analogs to cooperate with other immunomodulators, we here investigated whether TX527, either alone or combined with CsA, could enhance the therapeutic efficacy of anti-CD3 mAbs to prevent diabetes recurrence upon syngeneic islet transplantation in diabetic NOD mice. Remarkably, combining the three agents at low, subtherapeutical doses strongly prevented immune-mediated islet destruction and subsequent recurrence of diabetes, by far exceeding the therapeutic efficacy of each of the mono- and duo-therapies. Despite the clear improvement of the clinical outcome by the combination therapy, the observed alterations in T cell numbers, Treg frequencies and expression of inflammatory homing receptors were predominantly mediated by anti-CD3 at the early time point of analysis. Here, additional analysis at later time points will further clarify the exact contributions of TX527 and CsA in this experimental setting.In conclusion, this work provides novel insights into the immunomodulatory mechanisms of active vitamin D and structural analogs, clearly identifying T cells as prominent and direct targets for these compounds. In addition, we propose that VDR agonists should be considered as dose-reducing agents in combination therapies for autoimmune diseases, in which low doses of other immunomodulatory compounds such as anti-CD3 mAb and CsA are inefficacious whereas high doses can be efficacious but cause side-effects.status: Publishe
