Systemic sclerosis
Die Systemsklerose ist charakterisiert durch autoimmune Phänomene und eine progressive Fibrose. Klinische Charakteristika sind neben der Sklerodermie ein ausgeprägtes Raynaud-Phänomen und Abnormitäten der Nagelfalzkapillaren, ferner pulmonale, renale, kardiale und intestinale Veränderungen. In der Regel wird eine diffuse von einer limitierten Form unterschieden. Die diffuse Form führt zu einer Sklerose des gesamten Integumentes, ist assoziiert mit dem Auto-antikörper Scl-70 und zeichnet sich durch eine rasche Krankheitsprogression aus. Die limitierte Form betrifft die distalen Extremitäten und das Gesicht, ist assoziiert mit dem centromer Autoantikörper und führt schleichend, über mehrere Jahre zu viszeralen Problemen. Neue therapeutische Möglichkeiten können zu einer besseren Lebensqualität verhelfen und verpflichten zu einem systematischen Krankheitsmonitoring.Systemic sclerosis (scleroderma) is characterized by autoimmune phenomena and a progressive fibrosis. Clinical signs include Raynaud's phenomenon, pathognomonic nail fold capillary changes, pulmonal, renal, cardial and intestinal involvement. The two forms of systemic sclerosis, the diffuse and the limited show distinct disease phenomena and evolution. The diffuse form typically leads to a rapid sclerosis of the whole skin, it is associated with the auto-antibody Scl-70, it presents with an interstitial fibrosis of the lung, myositis and cardial fibrosis, a progressive loss of intestinal motility with maldigestion and it bears the risk of renal crisis. The limited form is characterized by sclerosis of the skin of the distal parts of the limbs and the face only, it is associated with the centromer auto-antibody and a much slower progression of visceral fibrosis. A typical late stage manifestation is pulmonary arterial hypertension. New drugs have improved quality of life. Their correct indication is based on a systematic monitoring of disease
Postgraduate training program
BACKGROUND: Faculties face the permanent challenge to design training programs with well-balanced educational outcomes, and to offer various organised and individual learning opportunities. AIM: To apply our original model to a postgraduate training program in rheumatology in general, and to various learning experiences in particular, in order to analyse the balance between different educational objectives. METHODS: Learning times of various educational activities were reported by the junior staff as targeted learners. The suitability of different learning experiences to achieve cognitive, affective and psychomotor learning objectives was estimated. Learning points with respect to efficacy were calculated by multiplication of the estimated learning times by the perceived appropriateness of the educational strategies. RESULTS: Out of 780 hours of professional learning per year (17.7 hours/week), 37.7% of the time was spent under individual supervision of senior staff, 24.4% in organised structured learning, 22.6% in self-studies, and 15.3% in organised patient-oriented learning. The balance between the different types of learning objectives was appropriate for the overall program, but not for each particular learning experience. Acquisition of factual knowledge and problem solving was readily aimed for during organised teaching sessions of different formats, and by personal targeted reading. Attitudes, skills and competencies, as well as behavioural and performance changes were mostly learned during caring for patients under interactive supervision by experts. CONCLUSION: We encourage other faculties to apply this approach to any other curriculum of undergraduate education, postgraduate training or continuous professional development in order to foster the development of well-balanced learning experiences
Treatment of rheumatoid arthritis during pregnancy: present and future.
INTRODUCTION
For the management of rheumatoid arthritis patients who plan to become pregnant, both disease activity and therapeutic regimens have to be taken into consideration. In the case of stable inactive disease, pregnancy can be planned and therapy can be adjusted with drugs compatible with pregnancy.
AREAS COVERED
Drugs to be discontinued before pregnancy are methotrexate, leflunomide, tocilizumab, rituximab, abatacept and tofacitinib. Pregnancy compatible disease modifying drugs are antimalarial drugs and sulfasalazine. TNF-inhibitors can be continued during the first half of pregnancy, yet if indicated during the third trimester TNF-inhibitors with a low rate of transplacental passage should be used. Glucocorticoids may be considered at the lowest effective dose throughout pregnancy. Non-selective COX-inhibitors can be continued until gestational week 32. Expert commentary: Together, a tailored treatment throughout pregnancy is possible with reasonable safety. Controlling disease activity during pregnancy is important for both, maternal and fetal health
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
