Parents' psychosocial adjustment in families of children with spina bifida

Children with spina bifida live with a range of impairments depending on the severity and the location of the spinal defect. Medical treatment of the disorder is very complex and can involve decision-making processes around sensitive issues such as the termination of pregnancy, selection criteria for treatment, and euthanasia. There is however, insufficient scientific knowledge concerning the consequences of spina bifida available as a basis for these complicated decision processes. This is specifically true for the psychosocial consequences of spina bifida. Therefore, we studied the impact of spina bifida on parents’ mental health and on their social adjustment within the parent-child relationship, the partner relationship, and the family climate. We conducted systemic reviews of the existing literature and interviewed the parents of 78 patients with spina bifida who were born at or referred to the Radboud University Medical Centre in Nijmegen, The Netherlands. From the results we concluded that spina bifida causes increased levels of psychological strain in parents - particularly during the first years after birth - and high levels of parenting stress. The disorder nonetheless, on the long term spina bifida does not necessarily lead to psychopathology in parents, depending on the presence of protective factors, such as partner support, family support, social support, personality traits, and active coping styles as well as risk factors, such as the severity of the child’s functional impairments and family conflicts. Zijn jongeren in JeugdzorgPlus anders dan jongeren in de open residentiele jeugdzorg?
Doel van deze studie is om bij te dragen aan de systematische zorgevaluatie van JeugdzorgPlus door kenmerken van jongeren in JeugdzorgPlus en open residentiële jeugdzorg te vergelijken aan de hand van concepten uit het Risk-Need-Responsivity model (Andrews & Bonta, 2010) en het Meervoudig Risicomodel (Van der Ploeg, 1997). Bij een jeugdzorgaanbieder zijn 150 dossiers (38 jongeren in JeugdzorgPlus en 112 jongeren in de open jeugdzorg) blind gecodeerd. Middels een categorieënsysteem zijn statische en dynamische kenmerken gecodeerd. De onderzoeksgroepen verschilden niet qua gezinskenmerken. Jongeren in JeugdzorgPlus vertoonden wel meer spijbelgedrag en schooluitval voor opname. Ook bleken zij in hun vrije tijd vaker risicovol gedrag te vertonen, meer drugs te gebruiken en vatbaarder voor negatieve invloeden van leeftijdsgenoten. Daarentegen hadden jongeren in de open jeugdzorg vaker problemen in hun sociale netwerk. Wat betreft traumatische gebeurtenissen waren jongeren in JeugdzorgPlus vaker seksueel misbruikt, terwijl jongeren in de open jeugdzorg vaker verwaarloosd waren. Qua persoonlijke kenmerken hadden jongeren in JeugdzorgPlus vaker een negatief zelfbeeld, gebrekkig inzicht in de gevolgen van gedrag, een lage frustratietolerantie en vaker woede uitbarstingen. Jongeren in JeugdzorgPlus hadden meer antisociale en agressieve gedragsproblemen, terwijl jongeren in de open jeugdzorg meer angst en ook ADHD-achtige gedragsproblemen lieten zien. De hardnekkige en agressieve gedragsproblemen van jongeren in JeugdzorgPlus lijken meer aanleiding te geven tot beschermende maatregelen. Aanvullend onderzoek, eventueel met de monitor JeugdzorgPlus , is noodzakelijk om deze resultaten te bevestigen voor jongeren in andere Jeugdzorginstellingen

Stability of post-treatment functioning after residential treatment: The perceptions of parents and adolescents

126067.pdf (Publisher’s version ) (Open Access)Introduction: The present study examined parents' and adolescents' perceptions of adolescent functioning after being discharged from residential treatment as well as the stability of functioning over time.
Method: Data for the study were collected as a part of a continuous process of evaluating post-treatment functioning. Adolescents and their parents were interviewed at home at 3 months (T1), 6 months (12), and 18 months (T3) after discharge. Several outcomes were evaluated: living situation, contact with parents, social network, school/work, behavior problems, use of soft drugs, police contacts and well-being. The sample was divided into two subsamples: parents (n = 82) and adolescents (n = 75) participating in all three waves (sample 1) and parents (n = 288) and adolescents (n = 317) participating in at least one wave (sample 2).
Results: Cautioned by the inevitable high rates of attrition and the risk of selective bias in this type of study, there is preliminary evidence showing that adolescents generally show positive outcomes after residential care. Moreover, the outcomes were stable over time: adolescents who showed positive outcomes directly after discharge also showed positive outcomes at 12 and T3, and vice versa. Parents reported less positive outcomes than adolescents did.
Discussion: An important contribution of this study is that it provides longitudinal data supporting the notion that outcomes from residential treatment may be more sustainable over time than previously thought. Clinical and research implications are discussed
