    Molecular elements of ion permeation and selectivity within calcium channels

    Voltage-dependent calcium channels are located in the plasma membrane and form a highly selective conduit by which Ca2+ ions enter all excitable cells and some nonexcitable cells. Extensive characterization studies have revealed the existence of one low (T) and five high-voltage-activated calcium channel types (L, N, P, Q, and R). The high voltage-activated calcium channels have been found to exist as heteromultimers, consisting of an alpha(1), beta, alpha(2)/delta, and gamma subunit. Molecular cloning has revealed the existence of 10 channel transcripts, and expression of these cloned calcium channel genes has shown that basic voltage-activated calcium channel function is strictly carried by the corresponding a, subunits. In turn, the auxiliary subunits serve to modulate calcium channel function by altering the voltage dependence of channel gating, kinetics, and current amplitude, thereby creating a likelihood for calcium channels with multiple properties. Although for calcium channels to be effective, Ca2+ ions must enter selectively through the pore of the alpha(1)-subunit, bypassing competition with other extracellular ions. The structural determinants of this highly selective Ca2+ filter reside within the four glutamic acid residues located at homologous positions within each of the four pore-forming segments. Together, these residues form a single or multiple Ca2+ affinity site(s) that entrap calcium ions, which are then electrostatically repulsed through the intracellular opening of the pore. This mechanism of high-selectivity calcium filtration, the spatial arrangement of pore glutamic acid residues; and the coordination chemistry of calcium binding are discussed in this review

    Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    Disputes in sport and modes of their solution and harmonization with legislation have imposed more problems than may have been expected at the first glance. Throughout history, as well as today, there have been various inconcilliable differences in opinions in the relation between sport and law. Only recently a completely new and young branch of law has started to develop: Sport Law. Sport law has a long and difficult way ahead of it on its way to further improvement, both in the sphere of legal theory and in the organization of its bodies and in the implementation of their decisions. In harmonizing decisions in its international sphere and in implementing the largely distraught laws, CAS is a pioneer in organizing sport law and finding solutions in this new and specific branch of law. This paper deals with the connection of for a long time irreconcilable concepts of sport and law through arbitration as a bridge bringing together the differences but also the needs of modern sports; the paper also brings a review of the institution that has emerged from this connection.sport, law, arbitration, CAS

    Introduzione Nelle Aziende Sanitarie Universitarie i medici in formazione specialistica (MFS) sono coinvolti attivamente nel percorso assistenziale del paziente. Per questo motivo la loro figura professionale e le loro specifiche competenze devono essere adeguatamente condivise dal team di lavoro. Un questionario è stato somministrato ai MFS e al personale medicoinfermieristico (HCW) dell’ASUIUD al fine di indagare la percezione sul ruolo dei MFS all’interno della struttura. Materiali e metodi Nel periodo compreso tra febbraio e marzo 2019 un questionario è stato distribuito, sia in formato cartaceo che online, a HCW e MFS dell’ASUIUD. Il questionario era composto da 27 domande (scala Likert: 1-totale disaccordo, 5-totale accordo) suddivise in 5 aree: rispetto della sicurezza e della privacy dei pazienti, qualità del servizio sanitario, gestione del rischio clinico, ruolo all’interno del team, soddisfazione riguardo alla pianificazione delle attività di reparto. Professione e genere sono state prese in considerazione come variabili indipendenti. L’analisi descrittiva, le mediane, i range interquartili [M(IQR)] e le regressioni logistiche sono state condotti tramite software EpiInfo e Stata. Risultati Sono stati raccolti 263 questionari (153/785 [19.5%] HCW, 110/358 [30.7%] MFS). L’analisi dei dati ha evidenziato una differenza significativa di percezione delle attività tra le due categorie considerate per quanto riguarda: impatto positivo dei MFS sulla qualità dell’assistenza: MFS = 5(5-5), HCW = 4(3-5), OR = 6.92 [95%CI, 3.89-12.29]; completezza delle informazioni fornite ai pazienti: MFS = 4(4-5), HCW = 4(3- 4), OR = 3.80 [95%CI, 2.30-6.28]; scetticismo del paziente nei confronti delle competenze dei MFS: MFS = 4(4-4), HCW = 3(2-4), OR = 5.37 [95%CI, 3.24-8.89]. Significativa è anche la differenza concernente il livello di integrazione dei MFS all’interno dell’équipe: MFS = 5(4-5), HCW = 5(4-5), OR = 0.40 [95%CI, 0.30-0.81]. Viceversa non vi è differenza di opinione tra MFS e HCW (p > 0.4) riguardo la difficoltà del paziente a distinguere gli MFS all’interno del reparto: M = 3(2-4). Non sono emerse associazioni significative in base al genere. Conclusioni Dalle analisi è emerso che il medico in formazione specialistica ritiene di aumentare la qualità dell’assistenza e di fornire informazioni esaustive al paziente, opinione parzialmente condivisa dagli HCW. Viceversa i MFS percepiscono sfiducia nei loro confronti da parte del paziente sebbene questa percezione non sia confermata dagli HCW. Allo stesso modo una percentuale rilevante di MFS non si sente parte integrante dell’équipe nonostante l’opinione di entrambi i gruppi sia complessivamente positiva. Infine, i due gruppi sono concordi nel sostenere che sussista difficoltà da parte del paziente nel distinguere il medico in formazione specialistica dalle altre figure lavorative del reparto
