Steven-Johnson syndrome is a cytotoxic immune reaction to drugs and infections. This is a case report of Tribulus terrestris induced Steven-Johnson syndrome in geriatric women.     In emergencies, we encounter many local and allergic reactions associated with bee stings, but the venom of a bee sting can also cause neurovascular complications very rarely. We here discuss a case of a 53-year-old woman who was stung by a group of bees her on face and back and presented with swelling and itching of the face and 1 episode of Generalised Tonic Clonic Seizures type seizures followed by right upper limb and lower limb weakness 10 hours later. MRI brain revealed multiple acute bilateral frontal, parietal-occipital lobes, and basal ganglia infarcts

    Assessment of Quality of Life Through Patient Counselling and Medication Adherence among Tubercular Patients in Tertiary Care Hospitals

    Mycobacterium tuberculosis causes TB, and medication adherence is defined as "the extent to which a person follows the agreed-upon advice of a medical practitioner".  Quality of life (QOL) is a patient\u27s assessment of their daily life, while health-related QOL evaluates the impact of a disease or disorder on their health. The study aimed to evaluate patients\u27 QOL while taking anti-tubercular drugs, identify factors contributing to poor medication adherence, and determine the effectiveness of counseling based on age, literate status, medication duration, and comorbidities. A prospective observational study was carried out in tuberculosis department at different healthcare centers. 240 TB patients were studied. Using a pre-designed proforma and a Microsoft Excel worksheet, data was collected and analyzed using Statistical Package for the Social Sciences (SPSS). Patient counselling is a major factor in tuberculosis patients\u27 adherence to anti-tubercular treatment. 178 (82%) tuberculosis patients adhered to counselling, whereas 40 (18%) did not. In the age group of >70 (32.5%) and 70-80 (23%), non-adherence is higher. Males 23 (57%) adhere more than females 17 (43%).The characteristics of study population revealed that male patients (41-60) with hypertension were more likely to be non-adherent. Maximum 30% of non-adherence was due to patient feeling unneeded and 25% to forgetfulness. Age, gender, education, and case type were identified. Future researches are needed to confirm the reasons.     &nbsp

    Prescription Patterns in Anti-Psychotic Medication and Drug Related Problems in Schizophrenia Patients

    ABSTRACT Objectives: The main objective of the study was prescription patterns of antipsychotics and drug related problems in schizophrenia patients. Methodology: A prospective observational study was conducted in psychiatry (OPD) of tertiary care hospital. The data was collected from OPD for a period of 6 months in 100 cases by using data collection forms Socio-demographic’s, Positive and Negative symptom assessment scale and Glasgow antipsychotic side effect scale used for detection of drug related problems. Statistics was done to the data by using SPSS software. Result: A total of 100 psychiatric patients, female (58%) patients predominated over males patients (42%). Majority of the schizophrenic patients were under age group of 30-40years (43%), positive symptoms (86%) and Paranoid type (75%). Higher number of patients was treated with combination therapy than mono therapy. Among Atypical antipsychotics Risperidone (34%) and Olanzapine (30%) and Typical-antipsychotics Haloperidol (14%) was the most used for patients. The use of Anticholinergic drug like Trihexphenidyl accounts to most of the prescriptions. Mild side effects (81%) were observed like are weight gain, sedation, restlessness, muscle jerky, slower body movements. Some moderate interactions with combination of Olanzapine+Risperidone (increased sedation), Risperidone+Haloperidol (increased QTc interval), and Olanzapine-Quetipine (increases anti dopamergic effects). Conclusion: Our study demonstrates schizophrenia is mostly seen in middle age females. Combination therapy was preferred and Atypical anti-psychotics are used relatively more than Typical’s

    The Risk of Cancer Correlated with Nitrosamine Exposure

    The nitrosamines are a class of carcinogenic compounds that have garnered significant attention due to their association with an increased risk of various cancer types. More than thirty nitrosamines are on California\u27s Proposition 65\u27s list of substances that can cause cancer. Commonly used medications for the treatment of heartburn, hypertension, and type 2 diabetes have recently been contaminated with nitrosamine, which has led to multiple recalls by the US Food and Drug Administration (FDA). Among chemicals and within a particular species of laboratory animal, these nitrosamines share target tumor sites for their common genotoxic and tumorigenic properties. Sometimes the drug valsartan is used to estimate the additional cancer risks associated with NDMA (N-nitrosodimethylamine) and NDEA (N-nitrosodiethylamine) contamination. These estimates are based on nitrosamine levels reported by the US FDA, cancer potencies developed by California\u27s Proposition 65 program and the US Environmental Protection Agency (EPA), and specific exposure scenarios. These estimates indicate that long-term drug use may raise the risk of cancer due to nitrosamine contamination, which is a major public health concern.
