This support is fuelled by a complex set of emotions linked to “being Afrikaans” in a culturally pluralistic society. In On Record, van der Merwe investigates the interplay between popular music and the unfolding of Afrikaans culture politics from the start of the twentieth century to the present. It includes a search for the earliest recorded Afrikaans songs and documents subsequent phases of music development that reflect the agency of ordinary individuals - artists and listeners - against a background of fundamental societal and political change. It regards both the music mainstream and the alternative, and reveals, among other things, historical cases of compliance and resistance regarding the master narrative of Afrikaner nationalist ideology, the attempts by cultural entrepreneurs to establish authority over popular Afrikaans culture, class tension, lasting racial exclusivity, protest and censorship, and the post-apartheid invocation of Afrikaner nostalgia and white victimhood. Comparison of infant-feeding practices in two health subdistricts with different baby-friendly status in Mpumalanga province

Design: This was a cross-sectional, descriptive, observational study with an analytical component. Eighteen fieldworkers assisted with the data collection, utilising two sets of interviewer-administered questionnaires – one on socio-demographic information and the other on infantfeeding practices.
Subjects: Mothers with infants from birth to six months old, attending postnatal care at public sector primary health care facilities in Emalahleni and Mbombela health subdistricts on the days of data collection were included. A total of 435 mother and infant pairs were included in the study.
Outcome measures: Five infant-feeding indicators were used, namely the early initiation of breastfeeding, exclusive breastfeeding, exclusive replacement feeding and mixed feeding rates, as well as the age at which complementary food was introduced.
Results: There was a significantly higher early initiation of breastfeeding (57% vs. 43%), exclusive breastfeeding rates (60% vs 48%), and a lower exclusive replacement feeding rate (18% vs. 33%) in Emalahleni subdistrict, where all the public sector maternity facilities are accredited as being baby friendly, compared to that in Mbombela subdistrict, where none of the public sector maternity facilities are baby friendly. The mixed feeding rate (19% vs. 15%) and the mean age of the introduction of complementary foods (50 days versus 35 days) did not differ significantly between the two subdistricts.
Conclusion: Implementation of the Baby-Friendly Hospital Initiative (BFHI) in a health subdistrict was associated with more optimal infant feeding practices in mothers with infants aged six months and younger. Managing dysfunctions and reinterventions in endoscopic ultrasound-guided choledochoduodenostomy with lumen apposing metal stents: Illustrated technical review (with videos)
Endoscopic ultrasound-guided choledochoduodenostomy (EUS-CDS) with lumen apposing metal stent is emerging both as a rescue strategy and a primary treatment for distal malignant biliary obstruction. The large-scale diffusion of the procedure and improved overall survival of patients with pancreatobiliary neoplasms is resulting in a growing population of long-term EUS-CDS lumen apposing metal stent carriers. Recent studies have reported a need for reintervention during follow-up as high as 55%, and the Leuven-Amsterdam-Milan Study Group classification has been developed, identifying five mechanisms of stent dysfunction and 11 possible rescue strategies aimed at restoring biliary drainage. This illustrated technical review aims to further dissect the recent classification through a comprehensive analysis of nine illustrative cases, offering insights into the pathophysiology underlying dysfunction and clinical reasoning behind rescue interventions, as well as technical considerations and practical tips and tricks. By exploring mechanisms of dysfunction, this review also assists clinicians in selecting the ideal candidates for EUS-CDS while identifying patients deemed high risk for dysfunction or clinical failure
Current landscape of therapeutic EUS: Changing paradigms in gastroenterology practice
Therapeutic EUS has witnessed exponential growth in the last decade, but it has been considered investigational until recently. An increasing body of good-quality evidence is now demonstrating clear advantages over established alternatives, adding therapeutic EUS to management algorithms of complex hepato-pancreato-biliary (HPB) and gastrointestinal (GI) conditions. In this review, the available evidence and clinical role of therapeutic EUS in established and evolving applications will be discussed. A Graphical Summary for each scenario will provide (1) technical steps, (2) anatomical sketch, (3) best-supporting evidence, and (4) role in changing current and future GI practice. Therapeutic EUS has accepted well-established applications such as drainage of symptomatic peripancreatic fluid collections, biliary drainage in failed endoscopic retrograde cholangiopancreatography, and treatment of acute cholecystitis in unfit-for-surgery patients. In addition, good-quality evidence on several emerging indications (e.g., treatment of gastric outlet obstruction, local ablation of pancreatic solid lesions, etc.) is promising. Specific emphasis will be given to how these technical innovations have changed management paradigms and algorithms and expanded the possibilities of gastroenterologists to provide therapeutic solutions to old and emerging clinical needs. Therapeutic EUS is cementing its role in everyday practice, radically changing the treatment of different HPB diseases and other conditions (e.g., GI obstruction). The development of dedicated accessories and increased training opportunities will expand the ability of gastroenterologists to deliver highly effective yet minimally invasive therapies, potentially translating into a better quality of life, especially for oncological and fragile patients
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
De rol van cyto-pathologie in de behandeling van patiënten met een pancreas tumor - speciale focus op neuro-endocrine tumoren
The pancreas being a deep-seated organ, the development of a minimally invasive technique such as endoscopic ultrasonography (EUS) with fine-needle aspiration (FNA) has been a revolution in assessing a symptomatic or incidentally discovered pancreatic lesion. The first challenge and aim of this dissertation is the optimization of the technical aspects not only at a pre-analytical level outside the pathology lab, but also the first steps inside the lab in a cost-efficient way with a low-non-diagnostic rate. In addition, the actual non-diagnostic rate in our institution using the described methodology will be assessed to compare it to the previous published one, discussing advantages and inconveniences of rapid on-site evaluation (ROSE) in our local setting.
Adenocarcinomas and neuro-endocrine tumours are the most frequent mass lesions encountered in the pancreas. Especially the first one has a disastrous outcome with a very short survival time when diagnosed in a late stage. Therefore, it is mandatory to get a precise diagnosis as soon as possible. EUS and EUS-FNA are therefore mandatory in the diagnostic algorithm for mass lesions of the pancreas. They are also very important for staging these tumors being able to sample suspicious lymph nodes and liver metastasis.
Neuroendocrine tumours of the pancreas (PNET), on the other hand, have a more indolent course, even if metastatic, depending on their differentiation grade. One of the elements used to grade a resected PNET and mandatory in the WHO and ENETS classification is the proliferation index measured with the Ki67 labelling index (Ki67 LI). Although, there are well-defined grade cut-offs for surgical specimens correlating with survival data, this is not the case for cytological specimens obtained by EUS-FNA. PNETs being less frequent, representing only 1 to 2 % of pancreatic neoplasms, it is difficult to study a large cohort with a long-follow-up to define a survival related grading system. The second aim of this dissertation is to assemble such a group and to confirm reproducibility and accuracy of the Ki67 count.
Furthermore, new developments in the field of EUS and EUS-FNA will be reviewed, such as different gauge needles to optimize the material obtained and newly described prognostic molecular markers such as ATRX/DAXX which might also, in addition to the Ki67 LI, help to determine the prognosis of a given tumour at diagnosis.status: Publishe
