Development and validation of simulator and teaching methodology in cricothyroidotomy
No full textA cricotireoidostomia é um procedimento pouco realizado, porém desafiador devido a pouca prática associada a necessidade de realizá-lo de modo rápido e seguro. O presente estudo teve por objetivo desenvolver um simulador de fácil replicação, e validar o simulador e a metodologia de ensino da simulação. Foi desenvolvido um simulador em impressora 3D baseado em partes anatômicas reais, associado aos mecanismos de simulador de cricotireoidostomia já existente, a fim de tornar a simulação mais realística. A simulação foi aplicada a dois grupos distintos, sendo um composto por cirurgiões torácicos (CT) e outro composto por médicos não torácicos (NT) do hospital Dr Carlos Alberto Studart Gomes, sendo o simulador ao final avaliado pelos participantes. Cada prática dos participantes foi gravada a avaliação da simulação nos quesitos de tempo, erros de avaliação e feedback de performance, sendo realizados por meio do OSATS (Objective Structure Assessment of Technical Skill). Os vídeos foram avaliados por dois especialistas e os resultados foram submetidos a análise estatística. RESULTADOS: A avaliação do tempo ao longo das etapas foi diferente entre os grupos CT e NT com significância estatística, com o grupo CT apresentando melhor desempenho. Ao avaliar a médias dos dois avaliadores percebeu-se uma melhora progressiva entre os 4 momentos de avaliação do grupo CT e do grupo NT com p <0,05. Em relação as médias dos erros dos dois avaliadores foi encontrado uma redução gradual dos erros com o decorrer dos procedimentos, com o grupo CT apresentando sempre uma média menor comparado ao grupo NT. A análise dos dados revelou que 100% dos participantes avaliaram o simulador como ótimo ou excelente nos quesitos aparência visual, tal quesito estabelece um realismo visual ao simulador, validando o face validity. Ao comparar a progressão dos dois grupos houve um aumento da diferença das médias expressiva entre as fases E1 e E2, com uma pequena redução da diferença entre as fases E2 e E3, e uma redução mais significativa da diferença entre as fases E3 e E4, com melhora das médias ao final dos procedimentos apresentando relevância estatística. A aproximação ao final entre os dois grupos permite supor que o simulador em conjunto com a metodologia de ensino tem a capacidade de aproximar as habilidades de grupos menos experientes com grupos mais experientes, sendo útil para o ensino da técnica.Cricothyroidotomy is a procedure that is rarely performed, but is challenging due to the lack of practice associated with the need to perform it quickly and safely. OBJECTIVE: The objective of this study was to develop an easily replicable simulator, and to validate the simulator and the simulation teaching methodology. A 3D printer simulator was developed based on real anatomical parts, associated with existing cricothyroidotomy simulator mechanisms, in order to make the simulation more realistic. The simulation was applied to two different groups, one made up of thoracic surgeons (CT) and the other made up of non-thoracic doctors (NT) from the Dr Carlos Alberto Studart Gomes hospital, with the simulator being evaluated at the end by the participants. Each practice of the participants was recorded and the simulation assessment was recorded in terms of time, assessment errors and performance feedback, carried out using OSATS (Objective Structure Assessment of Technical Skill). The videos were evaluated by two experts and the results were subjected to statistical analysis. The evaluation of time throughout the stages was different between the CT and NT groups with statistical significance, with the CT group showing better performance. When evaluating the means of the two evaluators, a progressive improvement was noticed between the 4 evaluation moments of the CT group and the NT group with p <0.05. Regarding the average errors of the two evaluators, a gradual reduction in errors was found over the course of the procedures, with the CT group always presenting a lower average compared to the NT group. Data analysis revealed that 100% of participants rated the simulator as excellent or excellent in terms of visual appearance, this aspect establishes visual realism to the simulator, validating face validity. When comparing the progression of the two groups, there was a significant increase in the difference in the means between phases E1 and E2, with a small reduction in the difference between phases E2 and E3, and a more significant reduction in the difference between phases E3 and E4, with improvement in averages at the end of the procedures showing statistical relevance. This approach to the end allows us to assume that the simulator together with the teaching methodology has the capacity to bring together the skills of less experienced groups with more experienced groups, being useful for teaching the technique
Going Beyond Counting First Authors in Author Co-citation Analysis

counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

Get PDF“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis

We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
