Parámetros predictores de colangiopancreatografía retrógrada endoscópica terapéutica. Hospital Nacional Arzobispo Loayza, Lima Perú
Determina si los parámetros convencionales clínicos, de laboratorio y de imágenes son predictores de Colangiopancreatografía retrograda endoscópica (CPRE) terapéutica. Se realizó un estudio observacional, prospectivo, transversal en el Servicio de Gastroenterología del Hospital Nacional Arzobispo Loayza en el que se incluyó 148 pacientes sometidos a CPRE. Estos fueron divididos en 2 grupos: CPRE terapéutica y CPRE diagnostica. Se evaluó los parámetros clínicos: diagnósticos de sospecha por las cuales se indicó la CPRE; de laboratorio: Bilirrubinas, fosfatasa alcalina, TGO, TGP, amilasa, GGTP; y estudio de imágenes: diámetro del colédoco por ecografía. Los datos fueron recolectados y registrados en una ficha de recolección de datos; y posteriormente tabulados utilizando el software estadístico SPSS versión 16. Aquellas variables en las que se encontró diferencia estadísticamente significativa entre los grupos de CPRE terapéutica y CPRE diagnóstica, en el análisis univariado, fueron posteriormente evaluadas en el modelo de regresión logística múltiple. Para diferenciar entre aquellos pacientes que fueron sometidos a CPRE terapéutica o CPRE diagnostica se desarrolló un modelo matemático usando el análisis de regresión logística múltiple. El 62,84% de la muestra correspondió al sexo femenino, registrándose una edad de 52,33 ± 17,89 años. La CPRE terapéutica representó un 81,76%. En el 90,08% de éstas se realizó papiloesfinterotomía endoscópica. La canastilla de Dormía y el Catéter Balón fueron los accesorios más usados: 38,84% y 36,36%; respectivamente. La indicación más frecuente de CPRE fue la Ictericia obstructiva en 62% de los casos, asociándose con una mayor probabilidad de CPRE terapéutica. Los valores de TGO para el grupo de CPRE terapéutica fueron 176,42 ± 157,08 U/L y de 107,63 ± 90,19 U/L para el grupo de CPRE diagnóstica. El valor – p fue <0,05, entre ambos grupos. Los valores de TGP mostraron una diferencia estadísticamente significativa, entre el grupo de CPRE terapéutica y diagnóstica: 232,77 ± 206,20 vs. 100,69 ± 94,82 U/L; p<0,01. No se encontró diferencia estadísticamente significativa entre el resto de variables estudiadas. De los diagnósticos post CPRE, la coledocolitiasis fue la patología más frecuente registrando un porcentaje de 43,33%. La presencia de Ictericia Obstructiva de forma aislada se asoció con una probabilidad de CPRE terapéutica del 34% y al combinarla con los parámetros de laboratorio la probabilidad aumenta al 78%. La presencia de los tres parámetros estudiados hace una probabilidad de CPRE terapéutica del 98%. Los parámetros convencionales clínicos, de laboratorio y de imágenes, cuando están presentes en conjunto, son buenos predictores de Colangiopancreatografía retrograda endoscópica terapéutica.Trabajo académic
Calidad de servicio a través del nivel de satisfacción del acompañante al usuario nuevo del consultorio externo del Departamento de Pediatría
La calidad percibida es definida como el juicio del consumidor relativo a la superioridad del servicio. Los usuarios de establecimientos de salud acuden con la expectativa de recibir una atención con calidad que solucione su problema de salud. La adherencia de los usuarios está asociada a su satisfacción por la atención recibida. Los usuarios nuevos a un servicio conocen poco o no tienen la experiencia previa, por ello deben ser convencidos de la calidad del servicio para mejorar su adherencia. OBJETIVO: Determinar la calidad de servicio medida desde la satisfacción del acompañante del usuarios nuevo a los consultorios externos del Departamento de pediatría del Hospital Nacional Arzobispo Loayza – 2006. METODOLOGIA: Investigación descriptivo transversal con cuestionario anónimo SEVQUAL adaptado para los servicios de salud. 265 parejas de acompañantes - usuarios nuevos que acudieron a los consultorios externos del 1º al 31 de enero 2006, fueron seleccionados sistemáticamente a la salida de la consulta externa de los Servicios de Neonatología y Pediatría, quienes aceptaron voluntariamente participar y cumplieron con los criterios de selección de la muestra. Dos encuestadores calificados, externos al Hospital, aplicaron la encuesta. RESULTADOS: El nivel de satisfacción global fue bajo (17.7%), la mayor satisfacción lo tuvo las dimensiones aspectos tangibles y confiabilidad. Las preguntas con mayor satisfacción son P8 - Confiabilidad: Dar recibos según las tarifas y precios establecidos con 54%, P3 - Aspectos Tangibles: Trabajadores aseados y correctamente uniformados con 44%, P2 - Aspectos Tangibles: Local agradable, limpio y ordenado con 36% y P5 - Confiabilidad: Cumplir con los servicios ofrecidos con 33% de satisfacción. Menor porcentaje de satisfacción global se encontró en P10 – Respuesta rápida: informar el horario de atención (17%), en segundo lugar P6 - Confiabilidad: cumplir con el horario establecido (19%) y en tercer lugar P16 - Seguridad: Apoyo entre compañeros para brindar la mejor atención posible (21%). El consultorio externo del servicio de Pediatría mostró mejor calidad de servicio (24%) que el servicio de neonatología (15%). Los usuarios priorizaron la dimensión respuesta rápida y seguridad. CONCLUSIONES: La percepción de calidad de servicio por los usuarios externos nuevos al Departamento de Pediatría es baja. La dimensión respuesta rápida y seguridad mostraron menor calidad de servicio y también fueron las más priorizadas, por lo que deben considerarse oportunidades de mejora
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
