    Anorexia, bulimia and search for identity : from the exploration of memories and future projections defining the self to the application of a therapeutic device by sensory reminiscences

    La capacité de voyager mentalement dans le temps constitue un élément fondamental du sentiment d’identité et de continuité de l’existence. Les souvenirs définissant le soi (SDS) et les projections futures définissant le soi (PFDS) font l’objet ainsi d’une voie privilégiée pour approfondir la compréhension du vécu subjectif des expériences personnelles et de la fragilité identitaire dont souffrent les personnes atteintes d’anorexie mentale (AM) et de boulimie nerveuse (BN). Par l’exploration des caractéristiques spécifiques des SDS et PFDS, l’objectif principal de ce travail de thèse est de proposer un dispositif expérimental basé sur la médiation par réminiscences sensorielles, dans une visée d’amélioration thérapeutique des troubles des conduites alimentaires (TCA). Nos résultats mettent en évidence que les femmes atteintes d’AM évoquent, en comparaison à des sujets sains, des événements définissant le soi généralisés, négatifs, avec une présence de contamination et de tension. Les patientes se souviennent davantage d’événements en lien avec des abus sexuels et se projettent dans l’avenir avec moins d’épisodes en lien avec les loisirs et les relations. Néanmoins, les événements en lien avec la maladie sont les plus intégrés par les patientes. Après avoir participé aux trois séances structurées du dispositif expérimental basé sur la réminiscence et la stimulation sensorielle, les patientes (n = 22) évoquent une majorité de réminiscences positives, en lien avec la nostalgie de l’enfance et perçues comme ayant une valeur importante dans la définition du soi. Les observations préliminaires de ce dispositif à vocation thérapeutique confortent l’intérêt de la stimulation polysensorielle comme support à la remémoration des souvenirs autobiographiques et la reconstruction du Self chez les patientes atteintes de TCA.The ability to travel mentally through time is a fundamental element of a sense of identity and continuity of existence. Self-defining memories (SDM) and self-defining future projections (SDFP) are thus the subject of a privileged path to deepen the understanding of the subjective experience of personal experiences and the identity fragility suffered from patients with anorexia nervosa (AN) and bulimia nervosa (BN). By exploring the specific characteristics of SDS and PFDS, the main objective of this thesis work is to propose an experimental device based on mediation by sensory reminiscences, with the aim of therapeutic improvement of eating disorders (ED). Our results showed that patients with AN, compared to healthy controls, generalized, reported negative self-defining events with a presence of contamination and tension. Patients remembered more events related to sexual abuse and project themselves into the future with fewer episodes related to leisure and relationships. Nevertheless, events related to the illness are the most integrated by patients. After participating in the three structured sessions of the experimental device based on reminiscence and sensory stimulation, patients (n = 22) reported a majority of positive reminiscences, related to childhood nostalgia and perceived as having an important value in defining the Self. The preliminary observations of this therapeutic device confirm the interest of polysensory stimulation as a support for the recollection of autobiographical memories and the reconstruction of the Self in patients with ED

    Le traumatisme psychique définit le choc émotionnel d'un individu à la suite à d'un évènement traumatique. Dans certains cas, la symptomatologie psychotraumatique s'installe et perdure entrainant l'apparition d'un Trouble de Stress Post Traumatique. La littérature décrit amplement les désordres mnésiques qui accompagnent la maladie et notamment l'altération de la mémoire autobiographique. Ce système de mémoire complexe renferme l'histoire de notre passé sur laquelle s'appuie notre individualité présente et nos aspirations futures. Cependant, l'origine du dysfonctionnement de la mémoire autobiographique reste confuse, cette altération pourrait être un facteur prémorbide, la conséquence de l'exposition traumatique ou celle de la maladie. Les recherches restent incertaines sur les relations existantes entre le dysfonctionnement de la mémoire autobiographique et le traumatisme psychique. Tout au long de ce travail, nous caractériserons le fonctionnement de la mémoire autobiographique dans le contexte d'un traumatisme psychique. Les deux premières études permettront d'analyser la mémoire autobiographique suite à une exposition traumatique unique et suite à des expositions répétées. La dernière étude nous éclairera sur le fonctionnement de la mémoire autobiographique chez des sujets en rémission partielle d'un trouble de stress post traumatique.Psychological trauma defines the emotional shock experienced by an individual following a traumatic event. In some cases, the psychotraumatic symptomatology develops and persists, leading to the onset of posttraumatic stress disorder. The literature has widely described the memory impairment associated with the disease, especially dysfunction of autobiographical memory. This complex memory system contains the story of our past, on which our present identity and future aspirations are based. However, it remains unclear whether this deficit is due to premorbid autobiographical memory dysfunction, to the trauma exposure itself, or to the subsequent development of posttraumatic stress disorder symptomatology. Research has not yet clarified the relationship between autobiographical memory dysfunction and psychological trauma. Throughout this work, we will define autobiographical memory function in the context of psychological trauma. The first two studies will analyze autobiographical memory function following a single traumatic exposure and following repeated exposures. The final study will shed light on how autobiographical memory functions among participants with posttraumatic stress disorder in partial remission

    Mémoire du passé et consolidation à très long terme dans l’épilepsie du lobe temporal

    L’objectif de ce travail de thèse est d’approfondir nos connaissances sur la mémoire du passé et la consolidation mnésique à très long terme dans l’épilepsie du lobe temporal (ELT) pharmacorésistante, avant et après traitement chirurgical. Notre première étude comprenant 70 patients a permis de caractériser la mémoire du passé dans l’ELT. Elle a mis en évidence un déficit de la mémoire des faits publics et du rappel d’épisodes autobiographiques. Seule la sémantique personnelle est préservée. De plus, l’ELT gauche a des conséquences plus délétères que l’ELT droit, non seulement sur la mémoire du passé, mais également sur les fonctions cognitives de manière générale. Par ailleurs, les variables liées à l’épilepsie ont un impact très limité sur les mesures de mémoire du passé. En revanche, ces dernières sont largement influencées par un nombre important de variables cognitives. Notre seconde étude s’inscrit dans la continuité de la première et s’intéresse aux éventuelles modifications des performances de mémoire du passé induites par une lobectomie temporale (LT). Quarante-cinq patients ont été examinés cinq mois après traitement chirurgical et 30 d’entre eux ont été revus un an après l’intervention. Deux principaux résultats se dégagent. D’une part, une stabilité des performances de mémoire autobiographique et de mémoire des faits publics, évaluées pour la période pré-opératoire, a été constatée lors de l’examen neuropsychologique post-opératoire à cinq mois. D’autre part, une normalisation de la mémoire autobiographique, pour le rappel d’épisodes personnels ayant eu lieu après l’intervention, en corrélation avec une amélioration des scores en rappel libre différé verbal, a été mise en évidence un an après LT, uniquement dans le groupe opéré à droite. Notre dernière étude vise à se prononcer quant à l’existence d’une vitesse d’oubli anormalement rapide chez les patients souffrant d’ELT, pour un matériel verbal et non verbal. Un oubli accéléré est clairement mis en évidence pour le matériel verbal au niveau d’un groupe de huit patients. Des analyses individuelles ont également révélé la présence d’un oubli accéléré pour le matériel non verbal. En revanche, nous n’avons pas réussi à prouver d’influence néfaste de l’accentuation des activités épileptiformes sur la consolidation à long terme. Enfin, des données préliminaires obtenues chez trois patients suggèrent une amélioration de la rétention à très long terme des informations mnésiques après LTD

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
