Phosphodiesterase Type 5 (PDE5) Inhibitors in Erectile Dysfunction: The Proper Drug for the Proper Patient
Introduction. Erectile dysfunction (ED) is a very common multidimensional disorder affecting men worldwide. Physical illness, reaction to life stresses, or an unhappy couple relationship influence clinical outcome. Phosphodiesterase type 5 (PDE5) inhibitors are recognized as efficacious and well tolerated, and are the first-line treatment for ED. Sildenafil, tadalafil, and vardenafil are the most widely used and studied PDE5 inhibitors. Data acquired during a routine diagnostic workup for ED should be taken into account when choosing the best PDE5 inhibitor for the individual patient, creating an individualized treatment plan, and going beyond "experience-based" subjective opinion and unfounded ideas and prejudice regarding currently available drugs. Aim. As the process of matching a given patient's profile to any selected PDE5 inhibitor often relies more on physician's personal convictions than on solid evidence, the aim of this review is to identify the main clinical, demographic, and relational factors influencing the choice of the PDE5 inhibitor to be used for the treatment of ED. Methods. A systematic literature search and current treatment guidelines were evaluated in a systematic manner. Main Outcome Measures. The main clinical, cultural, and demographical factors to be considered for the treatment of ED have been identified. Results. Main factors influencing the choice of the treatment for ED have been described. A short list of items that may help in choosing the right PDE5 inhibitor for the treatment of different patients in daily clinical practice has been prepared. Conclusions. The simple algorithms prepared should be a useful tool to be used in daily practice, which may help in choosing the right treatment for each subject affected by ED. retiredI completed my B.A. and M.A. at McMaster University, and obtained my doctorate at York University (Toronto) in 2003. My dissertation examined the lives of a group of Montrealers whose ideas and activities were central to the development of modernity and the welfare state in Canada in the post-World War II years.
I have experience teaching at a number of Canadian Universities, and have been a permanent Faculty member in the History Department at University of the Fraser Valley since 2005. I am a cultural historian, and my areas of specialization are Post-Confederation Canada as well as the History of Quebec. I define “cultural history” very broadly to include my research interests, which range from the study of food culture, to popular music and its significance as a cultural expression of society’s concerns and interests, to visual culture and its meaning in people’s lives.
Another of my interests is the study of teaching and learning, curriculum design, and the examination of various forms of content-delivery in the classroom and online. I’ve presented workshops on classroom skills, course design, and assessment design.
At the time of writing, my ongoing projects include the preparation of a conference paper on F. R. Scott and the challenge of socialism, a paper on St. Jean-Baptiste parades in Montreal, an article on the exemption tribunals in Montreal after Conscription was enacted during the Great War, and the revision of my doctoral thesis prior to submitting it for publication
incidence and risk factors for non-cardiac syncope in a cohort of patients addressing an accredited arterial Hypertension center: the innocent Study
BacKGrOUNd: Hypotensive susceptibility in hypertensive patients could facilitate orthostatic hypotension, syncope and fall. The aim of this study was to identify incidence, clinical form, complications and risk factors for non-cardiac syncope in a cohort of hypertensive patients. MeTHOdS: This is an observational, case-controlled, retrospective study carried out on 168 patients, evaluated at the Hypertension center of the University Hospital of Parma (italy). Based on the presence of episodes of syncope during the six months prior to enrolment, we identified cases and controls and then we compared them to personal data, comorbidities, current drug regimens, presence of orthostatic hypotension, office and ambulatory blood pressure monitoring (aBPM) blood pressure (BP) values. reSULTS: in patients with previous syncopal episodes (29.8% of total), we more frequently found female gender, comorbidities associated with autonomic dysfunction, diuretics and non-cV drugs potentially associated with hypotension in their current drug regimen, orthostatic hypotension and lower office and ABPM BP values. cONcLUSiONS: To identify hypertensive patients at higher risk for syncope and falls, physicians should focus on comorbidities and current drug regimens, systematically perform an active standing test to identify orthostatic hypotension, employ aBPM to compare BP values with the pre-established target and highlight systolic BP drops and abnormalities suggesting concomitant autonomic dysfunction. The modulation of antihypertensive therapy is an effective tool to counteract the risk of non-cardiac syncope, with possible trauma or other negative influences
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

Multidisciplinary approach to "accidental" falls in the elderly: a case report.
Falls in the elderly are commonly and often wrongly identified as “accidental”. We report
a case of an elderly woman admitted to first aid for a trauma due to an accidental fall.
Geriatric multidisciplinary evaluation revealed mild cognitive impairment associated with
depressive symptoms; both findings made the anamnesis uncertain. Syncope algorithm
was applied and “tachy-brady form of sick sinus syndrome” was diagnosed. Differential
diagnosis between “accidental” and “apparently accidental” falls in elderly patients is very
difficult but a multidisciplinary geriatric evaluation can clarify the correct diagnosis
