„Real-life“-Ergebnisse aus der Therapie der chronischen Hepatitis C mit direkt antiviral wirksamen Medikamenten – eine Studie der Universitätsklinik Ulm und zwei spezialisierten Praxen in Ulm und Dornstadt
No full textDas Ziel der vorliegenden retrospektiven Auswertung war die Effektivität und Sicherheit der neuen direkt antiviral wirksamen Medikamente zur Therapie der chronischen Hepatitis C unter „real-life“-Bedingungen zu untersuchen. Hierfür haben wir die Daten von 344 mit den neuen direkt antiviral wirksamen Medikamente behandelten Patienten aus der Klinik für Innere Medizin I der Universitätsklinik Ulm und aus zwei Schwerpunktpraxen in Ulm und Dornstadt im Zeitraum zwischen Januar 2014 und August 2016 untersucht. 69,5 % der untersuchten Patienten hatte den Hepatitis C Genotyp 1, gefolgt von Genotyp 3 (20,6 %). Zwei Fünftel der untersuchten Patienten hatte bereits in der Vergangenheit eine erfolglose antivirale Therapie mit pegyliertem Interferon und Ribavirin. Die überwiegende Mehrheit der Patienten wurde für 12 Wochen mit den neuen DAA behandelt. Bei 32,6 % der Genotyp 1 Patienten mit geeigneten Kriterien zur Therapieverkürzung konnte tatsächlich eine auf 8 Wochen verkürzte Therapie durchgeführt werden. Die SVR12 betrug bei diesen Patienten 100 %. Insgesamt wurde eine Heilungsrate von 97,8 % erreicht, definiert durch den fehlenden Nachweis der Hepatitis C Virus Ribonukleinsäure im Serum 12 Wochen nach Therapieende (SVR12). Genotyp 1 Patienten erzielten eine SVR12 von 99,1 %, wohingegen bei Genotyp 3 Patienten nur 93,1 % eine SVR12 erreichten. Sieben Patienten aus unserem Kollektiv hatten einen erneuten Virusnachweis nach planmäßigem Abschluss der DAA-Therapie. Bei fünf von sieben dieser „Relapse“-Patienten konnten Resistenz-assoziierte Substitutionen (RAS) in verschiedenen Regionen des Virusgenoms identifiziert werden. 19,5 % der Patienten hatten bereits vor Therapiebeginn eine Leberzirrhose. Bei den meisten dieser Patienten wurde ebenfalls eine Ausheilung der chronischen Hepatitis C-Infektion erreicht (SVR12 = 93,8 %). Eine schwierig zu behandelnde Patientengruppe sind Patienten mit dem Genotyp 3 und Leberzirrhose. Hier betrug die Heilungsrate (SVR12) nur 80 % und war damit im Vergleich zum Gesamtkollektiv als auch im Vergleich zum Kollektiv der Patienten mit Leberzirrhose signifikant niedriger. Insgesamt kann man von einer sehr guten Verträglichkeit der direkt antiviral wirksamen Medikamente sprechen, lediglich im Falle einer 78-jährigen Frau musste die Proteaseinhibitor-haltige Therapie aufgrund eines generalisierten Exanthems abgebrochen werden. Auch wenn es unter bestimmten Subpopulationen noch Handlungsbedarf in der Hepatitis C Therapie gibt, bestätigen unsere Ergebnisse eine hoch effektive und sichere Behandlung der chronischen Hepatitis C unter „real-life“-Bedingungen und unterstreichen die rasanten Fortschritte und Erfolge durch die neuen direkt antiviral wirksamen Medikamente

No full textThis is the first of a pair of beautifully printed paperbound volumes I discovered among my books now in 2005. When and how did I get them? I can only guess. There is a $9 price tag on each from Kinokuniya with a San Jose telephone number. Were they perhaps a gift of Rafael Sakurai? As for the book itself, its dust-jacket has beautifully colored Ulm woodcuts of The Hawk and the Pigeons and of the man chopping down a tree (to get an axe-handle, I presume), with half of a beautiful peacock on the spine. Inside, the book moves from what we would call the back to the front with a succession of beautiful Ulm images, starting with FG on 11 and recurring every four to eight pages. The beginning T of C indicates that there are one-hundred-and-eighty-four fables on 221 pages. What a lovely pair of books!Language note: JapaneseLee Coope
Mikronährstoffstatus und kognitive Leistung bei Patienten der Gedächtnisambulanz Ulm
No full textDeficiency in micronutrients - vitamins and minerals - may add to impaired cognitive function and development of dementia in elderly. The objective of this study was to assess the micronutrient status and associated domain-specific cognitive performance of elderly out-patients of the memory clinic Ulm (Germany) with special focus on B-vitamins (B1, B6, folic acid, B12), antioxidant vitamins and provitamins (A, C, E, carotenes, lycopin), and redox-active minerals (Fe, Cu, Zn). Cognitive performance was assessed via a standardised test program.
Eighty-eight out-patients, aged 58 to 88, were evaluated. Major diagnoses were Alzheimer disease and mild cognitive impairment. B-vitamin blood concentrations were mostly in the respective reference ranges, but homocysteine was elevated above normal in almost half of the patients. The status of antioxidant vitamins and minerals was more variable. Despite adequate micronutrient status, some associations between cognitive performance and micronutrient status were detected. Low level of premorbid intelligence was associated with lower folic acid and vitamin B12 blood levels than normal premorbid intelligence level. Additionally, patients with poor performance in a test on visuo-construction and at least in some working memory tests were shown with lower folic acid levels than patients with normal test performance. Patients with lower levels of Fe status were shown with attention deficits. Clustering of patients according to their cognitive performance revealed two separate groups with differences in their folic acid and Fe status.
In conclusion, lower levels of folic acid- and Fe-status, even though within the reference range, were associated with deficits of performance in specific cognitive domains in out-patients of the memory clinic Ulm. Further prospective studies of longer duration including more patients should be initiated to verify these preliminary findings
