    Cervical spine sagittal alignment variations following posterior spinal fusion and instrumentation for adolescent idiopathic scoliosis

    The aim of this study is to quantify the changes in the sagittal alignment of the cervical spine in patients with adolescent idiopathic scoliosis following posterior spinal fusion. Patients eligible for study inclusion included those with a diagnosis of mainly thoracic adolescent idiopathic scoliosis treated by means of posterior multisegmented hook and screw instrumentation. Pre and post-operative anterior-posterior and lateral radiographs of the entire spine were reviewed to assess the changes of cervical sagittal alignment. Thirty-two patients (3 boys, 29 girls) met the inclusion criteria for the study. The average pre-operative cervical sagittal alignment (CSA) was 4.0° ± 12.3° (range -30° to 40°) of lordosis. Postoperatively, the average CSA was 1.7° ± 11.4° (range -24° to 30°). After surgery, it was less than 20° in 27 patients (84.4%) and between 20° and 40° in 5 patients (15.6%). The results of the present study suggest that even if rod precontouring is performed and postoperative thoracic sagittal alignment is restored, improved or remains unchanged after significant correction of the deformity on the frontal plane, the inherent rigidity of the cervical spine limits changes in the CSA as the cervical spine becomes rigid over time

    Évolution fonctionnelle et clinique d'individus ayant une amputation du membre inférieur

    L’utilisation d’une nouvelle prothèse à la suite d’une amputation d’un membre inférieur (MI) demande une ré acquisition de la capacité de marcher, ce qui peut mener à différentes stratégies de mouvement. Malgré les connaissances actuelles des stratégies de mouvement employées chez la population des individus ayant une amputation du MI (IA), aucune étude n’a évalué leur évolution lors d’un exercice de marche prolongée et à la suite d’un retour en milieu écologique. Pourtant, une diminution des performances à la marche pourrait être observée suivant l’apparition de la fatigue et/ou suivant une diminution du temps alloué aux activités physiques quotidiennes. C’est pourquoi l’objectif de cette étude est de quantifier les paramètres de marche des IA 1) lors d’un test de marche de six minutes (TM6) à la fin de leur réadaptation et 2) lors d’un TM6 six semaines suivant la fin de leur réadaptation. Nous posons l’hypothèse que l’ajout de centrales inertielles lors de l’évaluation de la marche des IA en milieu clinique permettra de caractériser les stratégies adoptées. Nous nous attendons à une dégradation de ces paramètres sur une période de six minutes de marche en continu ainsi que sur une période de six semaines suivant la fin des traitements de réadaptation. Pour ce faire, des centrales inertielles ont été utilisées, puisqu’elles permettent de quantifier les paramètres spatio-temporels et cinématiques de la marche dans différents environnements et populations, permettant l’acquisition de données « sur le terrain ». Au total, 17 IA on fait le TM6 à T1 et T2 avec des centrales inertielles posées sur chacun de leur pied pour enregistrer leurs paramètres de marche (c.-à-d. dégagement minimal des orteils (minTC), phase de chargement (LDr), phase de pied plat (FFr), largeur de pas en phase oscillante, longueur de foulée, vitesse et cadence). Une analyse ANOVA non paramétrique a comparé 1) l’évolution des paramètres de marche sur une période de six minutes et 2) l’évolution des paramètres de marche entre T1 et T2. Un test de Wilcoxon des rangs signés a été exécuté lorsqu’une interaction était détectée dans l’ANOVA (P < 0.05). Contrairement à l’hypothèse posée en début de projet, une amélioration des performances au membre prothétique (AL) entre T1 et T2 a été observée (largeur de pas en phase d’oscillation, vitesse et cadence augmentées, et la phase d’appui diminuée). Cependant, le minTC n’a pas évolué proportionnellement à la vitesse de marche. Finalement, les capacités physiques et à la marche se sont améliorées entre T1 et T2 dans la population des IA malgré une incapacité d’adapter leur minTC (c.-à-d.., d’augmenter le minTC proportionnellement à la vitesse de marche) à l’augmentation de leur vitesse de marche. Ce phénomène pourrait être expliqué par un patron de marche déficient utilisant la stratégie de « hip hiking » caractérisé par un large minTC et petite largeur de pas à T1 qui s’est amélioré à T2. Toutefois, un petit minTC est aussi relié à un plus grand risque de chute. Ainsi, l’évaluation des paramètres de marche des IA en milieu clinique de même qu’un programme de réadaptation à distance ciblant l’amélioration des paramètres de marche en lien avec la réduction du risque de chute est primordial pour assurer un retour sécuritaire en communauté.The use of a new prosthetic limb following a lower limb amputation demands a reacquisition of the ability to walk, leading to different gait strategies. Although those strategies have been identified in the lower limb amputee (LLA) population, no studies have assessed their evolution following a return in the ecological environment. Indeed, decreased gait performances could be observed following the onset of fatigue and/or a decline of the daily active time. Therefore, the aim of this study was to quantify the gait parameters of the LLA 1. Along a six-minute walk test (6MWT) following their rehabilitation and 2. Along a 6MWT six weeks following the end of the rehabilitation. It is hypothesized that the use of inertial sensors during the assessment of the IA’s gait parameters will allow the characterisation of their adopted gait strategies. We are expecting a deterioration of the gait parameters along a six-minute continuous walk and six weeks following the end of the rehabilitation treatments. To do so, inertial measurement units (IMU) have been used, as they are effective to quantify gait in different environments and populations, allowing data acquisition in “real-life”. Seventeen LLA performed a six-minute walk test (6MWT) at T1 and T2 with inertial sensors fixed on both feet to register their gait parameters (i.e. minimum toe clearance (minTC), loading ratio (LDr), flat foot ratio (FFr), swing width, step length, speed and cadence). A non-parametric ANOVA compared 1) the evolution of the gait parameters over a 6MWT and 2) the evolution of the gait parameters over T1 and T2. Post-hoc Wilcoxon signed-rank tests were performed if a main effect was detected (P < 0.05). A performance improvement on the amputated limb (AL) between T1 and T2 has been observed (swing width, speed and cadence increased, added to a decreased stance phase). However, the minTC did not evolve proportionally to the speed. Physical capacity and walking abilities have increased between T1 and T2 in LLA population despite the inability to adapt their minTC (i.e. to increase the minTC proportionally to the gait speed) to an increased speed. It could be explained by a poor walking pattern using a hip hiking strategy including a high minTC and a small step width at T1 that improved (lower minTC than at T1) at T2. However, a small minTC is related to higher risks of fall. Therefore, the evaluation of the LLA’s gait parameters in clinical settings added to a home rehabilitation programme focused on the reduction of the risks of fall is primordial to ensure a secure return in the community

    Pour maintenir notre équilibre debout, notre cerveau utilise des informations sensorielles. Lorsque ces signaux sont détériorés, le control postural est perturbé et peut être amélioré grâce à l'augmentation sensorielle (AS). Il n'est pas cependant établi si les rétroactions devraient être fournies à chaque déséquilibre. L'objectif était de comparer l'effet de différentes quantités d'AS de type vibrotactile sur le contrôle postural debout et d'évaluer si les effets persistent dans le temps, alors que la qualité des informations somatosensorielles et visuelles sera altérée. Méthode : 24 jeunes adultes en santé aléatoirement divisés en deux groupes (rétroaction 100 % et 33 %) étaient debout les yeux fermés sur un tapis de mousse. Ils recevaient d'abord des rétroactions synchronisées avec la direction des oscillations posturale puis aucune rétroaction. Ensuite, les rétroactions n'étaient plus congruentes avec la direction des oscillations posturales. Afin d'évaluer le contrôle postural, la valeur moyenne quadratique selon les axes médio-latéral et antéro-postérieur et 85 % de l'aire totale couverte en utilisant un ajustement en ellipse ont été calculées à partir de la cinématique du tronc (angle et vitesse angulaire) et des forces au sol (effort neuromusculaire). Résultats : L'AS vibrotactile congruente diminue l'angle du tronc et augmente la vitesse angulaire du tronc et les forces au sol de façon similaire pour les deux groupes. Cette réduction de l'angle du tronc perdure dans l'axe médio-latéral, à la suite de l'arrêt de l'AS, alors que la vitesse angulaire du tronc et les forces au sol diminuent. L'AS vibrotactile non congruente accentuent tous les paramètres suggérant une détérioration de l'équilibre. Conclusion : Les participants ont intégré les rétroactions vibrotactiles pour contrôler leurs oscillations posturales. Les deux quantités d'AS sont aussi efficaces pour améliorer le contrôle postural. L'AS vibrotactile fournie une fois sur trois pourrait être priorisée afin de limiter la charge cognitive.During upright standing, the brain uses sensory cues to estimate body sways amplitude and direction. These sensory information helps in controlling upright balance. For individuals with balance control impairment, sensory information is altered and vibrotactile feedback improves their balance. It is unclear, however, if feedback must be provided each time balance is compromised. The goal of this study was to compare two quantities of trunk vibrotactile feedback while visual and somatosensory information were altered and to evaluate if the results on upright balance control were maintained afterward. Method: Twenty-four healthy young adults randomly assigned to two groups (feedback 33% and 100%) stood upright on the foam surface with their eyes closed. They first received vibrotactile feedback provided according to body sway angle amplitude and direction and then without feedback. Thereafter, vibrotactile feedback was unrelated to body sway angle and direction. To assess balance control, we measured the ground reaction forces, body sway angle and angular velocity along the anteroposterior and mediolateral axes. We calculated the root mean square values and an ellipse covering 85% all these measures. Results: Balance control of both groups was similar. When vibrotactile feedback was provided, body sway angle amplitude decreased while body sway angular velocity and the ground reaction forces increased. Immediately following vibrotactile feedback, the reduction in body sway angle was maintained while the body sway velocity and the ground forces decreased. When vibrotactile feedback was unrelated to body sway angle, all balance control parameter increased. Conclusion: Participants processed vibrotactile feedback to control their balance. Vibrotactile feedback was effective in improving balance control despite the quantity of vibrotactile feedback provided. We argue that providing vibrotactile feedback one-third of the time that balance control is compromised could be the best choice as it enhances balance control, and it likely implies less cognitive load

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    Aide à l’écriture pour les enfants vivant avec des incoordinations de mouvements

    Plusieurs enfants vivant avec des difficultés de coordination et/ou la présence de spasticité musculaire peuvent avoir de la difficulté avec l’écriture manuscrite. Cette activité est cependant reconnue pour favoriser les apprentissages, notamment en soutenant la mémorisation, la reconnaissance de lettres, la lecture, et bien d’autres. L’objectif de ce mémoire est de concevoir une aide technique à l’écriture manuscrite capable d’aider les enfants vivant avec des difficultés motrices à écrire et à dessiner. Le mécanisme vise à stabiliser les mouvements incoordonnés et à maintenir le crayon fixe, tout en visant un concept le plus simple possible afin que le résultat demeure abordable et accessible. L’hypothèse est que l’utilisation de l’aide à l’écriture développée permettra d’améliorer la capacité des personnes vivant avec des limitations physiques aux membres supérieurs d’écrire et de dessiner de façon autonome. Ceci apportera des avantages majeurs pour l’utilisateur dans sa vie courante en lui permettant, entre autres, d’exprimer ses idées. Deux versions ont été développées : une première avec des amortisseurs mécaniques de grandeur fixe et une seconde avec la possibilité de modifier la grandeur de l’amortissement produit par les amortisseurs. Les deux versions utilisent un mécanisme qui consiste en un assemblage à deux degrés de liberté de barres. Elles sont assemblées de sorte que l’orientation de la poignée et du crayon sont constantes par rapport à la base en tout temps. Le choix d’amortisseurs limités dans la première version a mené au développement de la seconde version. Celle-ci utilise les mêmes amortisseurs, mais un mécanisme permet de faire varier le niveau d’amortissement en fonction des besoins de l’utilisateur pour couvrir une plus grande plage d’amortissement possible. Il existe deux versions de cet amortissement variable : une avec des amortisseurs angulaires, comme sur la première version du prototype et une avec des vérins linéaires.Many children living with movement disorders and/or muscular spasticity may have trouble with handwriting. This activity is known to be helpful during the learning phase, mainly by helping memorization, letter recognition, reading and more. The objective of the memoir is to develop a handwriting assistive device that can help children living with movement disorders to write and draw. The mechanism aims to stabilize movement disorders and to maintain the pen in a fixed orientation while having a simple design to have a final product affordable and accessible. The hypothesis is that the use of the developed mechanism will improve the capacity of people living with physical limitations in the upper limbs to write and draw autonomously. This will bring major advantages for the user in his everyday life as he/she will be able to, among others, express his/her ideas. Two versions have been developed: a first one with mechanical dampers of a fixed damping value and a second one with the possibility to vary the damping value. The two versions use a mechanism that consists in a bars assembly with two degrees of freedom. They are assembled so that the orientation of the handle and the pen are constant with respect to the base. With limited choice of mechanical dampers for the first version, the second version of the mechanism was developed. It uses the same dampers, but a four-bar mechanism allows the damping value to be change to match the user needs. There are two versions of the variable damping: one with angular dampers as for the first version of the prototype and one with linear cylinders

    Influence des caractéristiques anatomiques individuelles sur les altérations de marche : impact sur la cinétique articulaire du membre inférieur

    Introduction : Il est estimé que 10 à 15% des individus âgés de plus de 60 ans sont atteints d'arthrose de degrés variés, ce qui engendre des coûts élevés pour nos systèmes de santé. En particulier, l'arthrose fémoro-tibiale interne entraîne une diminution de l'activité physique et une plus grande sédentarité de la population atteinte à cause de la douleur ressentie durant les activités de la vie de tous les jours, comme la marche. Il a été montré que les personnes souffrant d'arthrose fémoro-tibiale ont souvent recours à des stratégies motrices compensatoires pour palier à la douleur. Ces stratégies motrices agissent en latéralisant les forces de contact articulaire, diminuant les contraintes entre le condyle fémoral et le plateau tibial interne. C'est ainsi que le ré-entraînement de la marche, une intervention prospective élaborée en recherche, qui vise à altérer le patron de marche des individus dans le but de diminuer les contraintes entre le condyle et le plateau tibiale a vu le jour. Toutefois, la relation entre les forces de contact articulaire et les modifications de marche n'a, à notre connaissance, pas été quantifiée. De plus, l'impact des modifications de marche sur les articulations de la cheville et de la hanche n'ont que peu été décrit dans la littérature scientifique. C'est pourquoi l'objectif général de la thèse est de décrire l'impact individuel d'une modification de la marche dans le but d'optimiser son implémentation dans la pratique clinique. L'hypothèse générale associée est qu'une modification de marche à un impact sur la cinématique et la cinétique de chaque articulation du membre inférieur. De plus, l'impact de ces modifications est modulée par l'indice de masse corporel et l'alignement du membre inférieur de chaque individu. Méthode : La collecte des données cinématiques et cinétiques a été effectuée à l'aide d'un système de capture du mouvement Vicon (9 caméras, Vicon motion system, Oxford, UK, 100Hz) et d'un tapis roulant instrumenté avec deux plateformes de force (Bertec, Columbus, OH, USA, 1000Hz). Les individus ont altéré leur marche à l'aide d'une rétroaction visuelle projetée sur un écran placé devant eux (Chapitre 4, N = 23 participants sains ; Chapitre 6, N = 29 participants sains). Les données de 120 individus souffrant d'arthrose fémoro-tibiale, ont été collectées la semaine précédant une arthroplastie totale unilatérale du genou. L'alignement du membre inférieur préopératoire a été évalué à l'aide d'une radiographie complète du membre inférieur et à l'aide d'un système de capture du mouvement (12 caméras, VICON Peak, Oxford, UK, 100Hz) dans une position debout standardisée. Résultats : Une modification de marche, simple ou combinée, visant à diminuer le moment d'adduction au genou impact aussi la cinétique à la cheville et/ou la hanche de manière significative (Chapitre 4). Un système de capture du mouvement n'est pas fiable pour estimer l'alignement du membre inférieur chez des individus avec un indice de masse corporel supérieur à 30kg/m² (Chapitre 5). L'interaction entre l'alignement du membre inférieur et les altérations de marche contribuent significativement à la variance observée aux moments de force externes chez une partie des individus (Chapitre 6). Conclusions : Une modification de marche implique aussi des variations significatives des moments de force externes au niveau de la cheville et de la hanche, et qui sont de plus modulées par les caractéristiques anatomiques individuelles. Les résultats soutiennent donc une approche personnalisée des modifications de la marche. En effet, d'un point de vue clinique, l'imagerie fournit des mesures objectives et directes des caractéristiques anatomiques individuelles, qui peuvent être utilisées en conjonction avec une analyse de mouvement spécifique au patient, pouvant fournir ainsi des informations importantes sur la façon dont l'anatomie et la biomécanique interagissent et peuvent être mises à profit pour améliorer et optimiser l'efficacité du ré-entraînement de la marche. En ce qui concerne la recherche, il sera important d'identifier les mécanismes qui expliquent la grande variabilité inter-sujet observée lors de l'adoption d'un nouveau patron de marche.Introduction : It is estimated that 10% to 15% of people over the age of 60 have osteoarthritis of varying degrees, which results in high costs to healthcare systems. In particular, femoro-tibial osteoarthritis leads to a decrease in physical activity and a greater sedentary lifestyle due to the pain experienced during activities of daily living, such as walking. It has been shown that people with femoro-tibial osteoarthritis often use compensatory motor strategies for pain relief. These compensatory motor strategies act by lateralizing the joint contact forces, reducing the constraints between the femoral condyle and the internal tibial plateau. Gait retraining, a prospective intervention developed in research, aims to alter the gait pattern of individuals in order to reduce the constraints between the condyle and the tibial plateau. However, the relationship between joint contact forces and gait alterations has not, to our knowledge, been quantified. In addition, the impact of gait alterations on the ankle and hip joints is not well documented. This is why the general objective of the thesis is to describe the individual impact of gait retraining in order to optimize its implementation in clinical practice. The associated general hypothesis is that a change in gait has an impact on the kinematics and kinetics of each joint of the lower limb. In addition, the impact of these alterations is modulated by the body mass index and lower limb alignment of each individual. Methods : Kinematic and kinetic data collection was carried out using a Vicon motion capture system (9 cameras, Vicon motion system, Oxford, UK, 100Hz) and an instrumented treadmill with two force platforms ( Bertec, Columbus, OH, USA, 1000Hz). Individuals altered their gait using visual feedback projected onto a screen placed in front of them (Chapter 4, N = 23 healthy participants ; Chapter 6, N = 29 healthy participants). Data from 120 individuals (Chapter 5) suffering from femoro-tibial osteoarthritis were collected the week preceding a unilateral total knee arthroplasty. Preoperative lower limb alignment was assessed using a total lower limb radiography and motion capture system (12 cameras, VICON Peak, Oxford, UK, 100Hz) in a standardized standing position. Results : A gait alteration, single or combined, aimed at reducing the knee adduction moment also significantly impacts ankle and/or hip kinetics (Chapter 4). A motion capture system is not reliable for lower limb alignment estimation in individuals with a body mass index greater than 30kg/m² (Chapter 5). The interaction between lower limb alignment and gait alterations significantly contributes to the variance observed in lower limb joints external moments in some individuals (Chapter 6). Conclusion : Impaired gait also involves significant variations in external moments at the ankle and hip, which are further modulated by individual anatomical characteristics. The results therefore support a personalized approach to gait retraining. Indeed, from a clinical point of view, imaging provides objective and direct measurements of individual anatomical features, which can be used in conjunction with patient-specific motion analysis, thus providing important information on how anatomy and biomechanics interact and can be used to improve and optimize the efficiency of gait retraining. As far as research is concerned, it will be important to identify the mechanisms that explain the great inter-subject variability observed during the adoption of a new gait pattern

    Analyse biomécanique du corps entier lors de tâches fonctionnelles chez des individus amputés trans-tibiaux

    Entre 2006 et 2011, 44 430 amputations du membre inférieur (MI) ont été réalisées au Canada, dont un tiers au niveau trans-tibial à la suite d'affections vasculaires (90%) et traumatiques (6%). Pour les individus amputés trans-tibiaux (iATT) un des enjeux de la réadaptation est la mise en place de prothèse. Cependant, l'utilisation d'une prothèse augmente la prévalence de troubles secondaires tels que l'arthrose, les lombalgies, et les ré-amputations. L'apparition de ces troubles secondaires est associée à une augmentation des contraintes articulaires et à une asymétrie lors des tâches fonctionnelles (marche, montée/descente d'escaliers, lever de chaise). L'évaluation de ces tâches fonctionnelles en clinique est importante, car elles sont en lien avec l'indépendance. Cependant ces évaluations se limitent actuellement à la capacité à réaliser ces tâches et non à quantifier les stratégies compensatoires. Une évaluation biomécanique (cinématique et cinétique tridimensionnelles du corps entier) permettrait de quantifier ces stratégies compensatoires afin de mieux comprendre les liens avec les troubles secondaires. L'objectif général de cette thèse était donc d'analyser les stratégies compensatoires tridimensionnelles du corps entier lors de l'exécution de tâches fonctionnelles auprès des individus amputés trans-tibiaux. Dans un premier temps, une revue de littérature, effectuée jusqu'à mai 2020, sur 17 articles a permis d'identifier les stratégies compensatoires lors du lever de chaise et de la montée d'escalier, ainsi que les limites des études actuelles. Parmi les limites, il ressort de cela que les iATT inclus dans les études sont majoritairement traumatiques et plus jeunes que les données épidémiologiques de la population générale. Cela pourrait être dû à des critères d'inclusion qui entraine un recrutement d'individus avec un niveau fonctionnel élevé. Ensuite, en ce qui concerne les variables cinématiques et cinétiques utilisées, il ressort que le tronc a peu été évalué lors de la montée d'escalier et que lors du lever de chaise l'analyse des membres inférieurs se limite à l'asymétrie dans le plan sagittal. À la suite de ces constatations, il a été décidé de valider une rampe instrumentée afin de pouvoir inclure des iATT avec un niveau fonctionnel moins élevé qui nécessitent l'aide d'une rampe pour la montée d'escalier. De plus, l'avantage d'une rampe instrumentée était de donner un retour direct sur son utilisation. Enfin, les tâches fonctionnelles de lever chaise et de montée d'escalier ont été analysées par la cinétique du tronc et des membres inférieurs dans les plans sagittal et frontal. Ces analyses du corps entier dans ces deux plans ont permis de mieux comprendre les stratégies compensatoires du tronc et des membres inférieurs et le lien avec les troubles secondaires reconnus dans la littérature, notamment grâce aux analyses dans le plan frontal. En effet, si certaines stratégies compensatoires peuvent varier entre les deux tâches, l'augmentation du moment d'adduction du genou, variable considérée comme prédictive de l'apparition de l'arthrose de genou, est quant à elle présente dans les deux.Between 2006 and 2011, 44,430 lower limb amputations were performed in Canada, a third of which at the trans-tibial level following vascular (90%) and traumatic (6%) conditions. For individuals with a trans-tibial amputation (iTTA), one of the challenges of rehabilitation is the fitting of a prosthesis. However, the use of a prosthesis increases the prevalence of secondary conditions such as osteoarthritis, low back pain, and re-amputation. The appearance of these secondary conditions is associated with an increase in joint stress and asymmetry during functional tasks (i.e., walking, stair ascent/descent, sit-to-stand). The evaluation of these functional tasks in clinical settings is important because they are related to independence. However, these evaluations are limited by the ability to perform these tasks and not to quantify the compensatory strategies. A biomechanical evaluation (three-dimensional kinetics and kinetics of the whole body) would allow us to quantify these compensatory strategies in order to better understand their links with secondary conditions. Therefore, the main objective of this thesis was to analyze the three-dimensional full-body compensatory strategies during functional tasks in iTTA. First, a literature review, up to May 2020, on 17 articles identified compensatory strategies during sit-to-stand and stair ascent, as well as the limitations of current studies. Among these limitations, it appears that the iATT included in the studies are predominantly traumatic and younger than the epidemiological data about the general population. This could be due to inclusion criteria which resulted in the recruitment of individuals with a highly functional level. Then, with regards to the kinematic and kinetic variables used, it appears that the trunk was rarely evaluated during stair ascent and that during sit-to-stand the analysis of the lower limbs is limited to the asymmetry in the sagittal plane. As a result of these observations, it was decided to validate an instrumented handrail to allow the inclusion of iATT with a lower functional level who require the aid of a handrail for stair ascent. In addition, the instrumented handrail gave direct measures of its use. Finally, the functional tasks of sit-to-stand and stair ascent were analyzed with respect to the kinetics of the trunk and lower limbs in the sagittal and frontal planes. These analyses of the whole body in these two planes made it possible to better understand the compensatory strategies of the trunk and the lower limbs and their link with the secondary conditions recognized in the literature, in particular due to the analyses in the frontal plane.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films.     Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
