Efficacia dell’azitromicina nel trattamento di un caso di papillomatosi confluente reticolata di Gougerot Carteaud
Un ragazzo di 16 anni giungeva alla nostra osservazione in luglio per la comparsa da circa 2 mesi di lesioni di colorito brunastro, finemente desquamanti, a distribuzione reticolare, localizzate al tronco ed in regione ascellare e antecubitale. L’esame dermoscopico mostrava una colorazione brunastra con bordi scarsamente definiti, ricoperti di squame bianche e un pattern definito a “sulci e gyri”. L’esame micologico diretto era risultato negativo mentre l’esame istologico mostrava una lieve paracheratosi, ipercheratosi compatta, iperpigmentazione dello strato basale, papillomatosi ed un infiltrato mononucleare perivascolare superficiale. Veniva pertanto posta diagnosi di papillomatosi confluente reticolata di Gougerot Carteaud. Questa rara malattia che interessa generalmente gli adolescenti è caratterizzata dalla comparsa sull’area sternale, parasternale, nucale e dorsale di papule piane e ipercheratosiche che confluiscono tra loro a costituire una sorta di reticolo eritematoso (nei soggetti con cute chiara) o brunastro (variante pigmentata dei soggetti con cute di colorito olivastro o scuro). Nel nostro caso è stata eseguita una terapia antibiotica sistemica con azitromicina 500 mg 1 compressa al giorno per 3 giorni a settimana per un totale di 3 settimane. Alla visita di controllo dopo 25 giorni si apprezzava una completa remissione del quadro clinico
Cutaneous myiasis in a traveller returning from Argentina
In returning travellers, dermatologic diseases are the third most common problems and myiasis accounts 7.3-11% of cases. We present a case of furuncular myiasis in a 52-years-old Caucasian man returning from a 1-month trip to Argentina. Clinical examination revealed an about 2 cm large erythematous, painful, poorly delimitated, tender nodule on the left cheek with a small pore on top through which a serous fluid drained. Moreover, patient described a movement sensation inside the nodule. Diagnosis of myiasis was made. The lesion was firstly squeezed and then incised in order to extract the larva, but these procedures resulted unsuccessful. Subsequently, it was indicated patient to apply an ointment on the lesion to determine the asphyxiation of the parasite and make the extraction easier. Pain did not make possible to remove the larva, so the patient, for psychological reasons, insisted on excision of the lesion. Therefore, we proceeded with local anaesthesia and surgical removal of the nodule containing the maggot. The larva was excised and identified as Dermatobia hominis due to the presence of rows of dark brown, caudally pointing, barblike spines. Since myiasis is uncommon in Europe, it is frequently misdiagnosed and its recognition is often delayed. Due to the continuous growth in international tourism, it is very important for Western dermatologists to became familiar with myiasis and consider this diagnosis in case of furuncle-like lesion, resistant to antibiotics in returning travellers. Moreover, travellers to endemic areas should be informed of preventive measures to reduce mosquito bites and transmission of the infestation
Allergia da contatto al laptop: dalla dermoscopia al patch test, passando per la microscopia laser confocale
Si presenta il caso di una paziente caucasica di 46 anni con storia personale di atopia muta che presentava da alcuni giorni dermatite eritematosa delle cosce. La paziente riferiva intenso prurito in sede lesionale, non associato a sensazione urente-dolorosa né febbre. Nulla di rilevante si evidenziava alla raccolta anamnestica e, inoltre, negava l’assunzione di famaci a domicilio. All’esame obiettivo si osservava la presenza di un eritema disomogeneo a margini sfumati localizzato bilateralmente alla superficie anteriore delle cosce, più esteso all’arto di destra, con aspetto infiltrativo-infiammatorio. Tali lesioni non avevano aspetto di fugacità, non si erano estese nel tempo e persistevano fisse da 6 giorni. Nessuna alterazione si rilevava a carico delle mucose o di altri distretti corporei. Alla dermoscopia si osservavano i criteri della dermatite eczematosa: vasi puntiformi in una distribuzione a chiazze in associazione con desquamazione giallastra.
Alla microscopia laser confocale si osservava spongiosi ed esocitosi a livello dell’epidermide, infiltrato infiammatorio, vescicole intraepidermiche ed edema a livello del derma papillare. Tale quadro deponeva per una dermatite allergica da contatto. Pertanto, veniva impostata una terapia con antistaminico orale alla sera per 15 giorni, clobetasolo propionato crema alla sera per 10 giorni ed una crema emolliente al mattino per 21 giorni. Alla visita di controllo dopo 21 giorni di terapia si osservava una completa remissione del quadro clinico senza esiti.
Per confermare il sospetto diagnostico iniziale la paziente veniva successivamente sottoposta ad esecuzione di patch test serie SIDAPA con riscontro di positività per nichel solfato +++, positività clinicamente rilevante in quanto la paziente era solita mantenere il laptop a contatto con la sede di dermatite
Imatinib-associated hyperpigmentation, a side effect that should be recognized
Introduction & Objectives: Imatinib mesylate is a tyrosine-kinase inhibitor used as the first-line treatment in chronic myeloid leukemia patients who are not candidate for allogeneic bone marrow transplantation. The most frequently reported drug-related side effects are edema, nausea, vomiting, muscle cramps, musculoskeletal pain, diarrhea and rash.
Imatinib treatment is often associated with hypopigmentation, but only a few cases of mucocutaneous hyperpigmentation are described in literature. We are reporting an additional case of mucocutaneous blue hyperpigmentation in a patient affected by chronic myeloid leukemia and treated with imatinib since 2003.
Material & Methods: A review of the available literature regarding the hyperpigmentation related to imatinib was performed and one additional case was analysed.
Results: In our case imatinib therapy was well tolerated for several years and it led to an excellent hematological and cytogenetic response. However, the patient gradually developed an intense blue hyperpigmentation that involved the oral mucosa and part of the skin. Other causes of hyperpigmentation were excluded.
Conclusions: Since 2001, when imatinib was approved from Food and Drug Administration for chronic myeloid leukemia, some cases of secondary hyperpigmentation were reported. This rare side effect should be recognized by the physicians. Moreover, the patient should be informed about this benign event before starting the therapy. Currently, no treatment is required for this condition and there is not indication to discontinue imatinib treatment. Further insight into the mechanisms of the pigmentary alterations caused by this drug is suggested for better treatment and prevention of these manifestations
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Cutaneous myiasis in a traveler returning from Argentina
Myiasis is a common travel-associated dermatosis. We describe a 52-year-old Italian man who acquired Dermatobia hominis when bitten by a mosquito during a visit to Argentina. He had a painful nodular lesion on the left cheek that had been present for about 3 weeks. The complete removal of the larva is the goal of medical treatment. Prescription of antibiotics to avoid secondary infections is not recommended. For psychological reasons and due to the failure of previous therapies, the lesion was excised. Travelers to endemic areas should be informed of preventive measures to reduce mosquito bites and transmission of the infestation
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
