REVERTING TOSOCIAL ORDER BY CONTAINMENT: JOHNNIE TO KEI-FUNG\u27S WHERE A GOOD MAN GOES The 1999 Hong Kong International Film Festival hailed the Hong Kong director Johnnie To Kei-Fung as a Director in Focus, featuring eleven of his films. The opening film of the Festival was his world premiere, Where a Good Man Goes, casting Lau Ching-wan and Ruby Wong, telling a story of the return of a prodigal son. The film, in short, evolves around a central theme "home". The author of this article suggests that this "home" is of the concept of a traditional home held by the Chinese, from ancient days to the present, and is also patriarchal. The film\u27s story also tells of the "grand narratives" governing the order of the Chinese society since the early ages, which incidentally coincide with the Western view, that no evil can escape judgment and if a prodigal son returns,..

Drug and heavy metal toxicity with special reference to calcium channel blocker overdose, inorganic mercury poisoning and blood lead level
Toxicity due to drug overdose and heavy metals are issues of public concern.
Of the heavy metals, mercury and lead are significant environmental hazards worldwide. Calcium channel blockers are now the most commonly used group of drugs for treating hypertension. They are therefore found in large quantities in many homes, creating a hazard because of the possibility of a large overdose.
Inorganic Mercury poisoning due to cosmetic products could induce pathological changes in kidney. Patients may present with proteinuria, minimal disease change or even membranous nephropathy upon admission. The efficacy of corticosteroid in these patients is in debate. The Poison Information and Clinical
Management System (PICMS) was used to identify patients with inorganic mercury poisoning with nephrotic symptoms due to cosmetic products in this study. Efficacy of concomitant treatment of corticosteroid in patients with inorganic mercury poisoning due to cosmetic cream in Hong Kong was compared among patients received different treatment regimen. There is significant difference in the duration of achieving proteinuria remission since admission.
Baseline urine protein levels were not correlated with the baseline blood or urine mercury levels.
Lead is a heavy metal without any biological role(s) in human. This toxic metal has been associated with reduced intelligence in children even at low level.
There is no safe blood lead level identified in the previous studies and the level of toxicity is significantly correlated with increase in blood lead level. The current blood lead reference level is 5μg/dL which is based on 97.5 percentile in the United States (US) National Health Nutrition and Examination Survey (NHANES). The NHANES database was used to study the latest trend in the blood lead level in the US population. The blood lead level in the United States has been continual decreasing trend. The mean blood lead level has been decreased from 1.65 (1.62-1.68) μg/dL in 1999-2000 to 0.84 (0.82-0.86) μg/dL in 2013-2014 significantly. The estimated proportion of children aged 1-5 years with elevated blood lead level was 0.5 [0.3-1.0]% in 2013-2014. The estimated new blood lead reference level was 3.48μg/dL.
Calcium channel blocker overdose is a potentially fatal poison in Hong Kong. Insulin and glucagon are common antidotes for treating calcium channel blocker overdose. The PICMS was also used to compare the mortality rate between insulin and glucagon in patients with calcium channel blocker overdose. There was no significant difference in mortality rate between insulin and glucagon. Compared to survived patients, there was significant difference in baseline systolic blood pressure, serum albumin level, serum alanine aminotransferase activity and PR interval in electrocardiogram in dead patients. Gout : its epidemiology and therapeutic effect of urate-lowering agent
Gout is chronic inflammatory arthritis characterised by monosodium urate crystal
deposition in peripheral joints. The global prevalence of gout is increasing except
Taiwan and the United States. However, there was no updated epidemiological data of
gout as well as the utilisation of urate-lowering agent in Hong Kong and the United
States (US). The risk factor of gout is essential. The association between high blood
lead level and gout has been shown in previous studies. However, the toxicity effect
of lead at low blood lead level was inconclusive. The association between gout and
depression was inconclusive. Whether the use of urate-lowering agent would increase
depression was not investigated. Therefore, this thesis aimed to study the
epidemiology of gout in Hong Kong and the US, risk factor analysis of urate control,3
toxicity effect of lead at low exposure level as well as the effect of urate control on
depression.
The Clinical Data Analysis and Report System was used to study the epidemiology of gout in Hong Kong in 2006-2017. Patients who used the service in a public hospital in Hong Kong in 2005 were included in this study. Patients who died before 2006 were
excluded. The incidence and prevalence of gout are increasing in Hong Kong.
However, the management of gout is suboptimal. Only one in four patients prescribed urate-lowering agents. Adult participants in the US National Health Nutrition and Examination Survey 2007-2016 were included to study the prevalence of gout, utilisation of urate-lowering agents, the association between blood lead level and gout as well as the association between urate control and depression. Gout was defined as self-reported gout. Urate-lowering agent prescription was defined using a pill bottle review. Serum urate level >6 mg/dL was used to define treatment failure. There was no significant increase in the prevalence of gout. However, the utilisation of urate-lowering agent was suboptimal. One in three patients with gout prescribed ULT. Stage 2-5 chronic kidney disease and thiazide diuretics prescription were the predictors of treatment failure. Blood lead level was associated with increased prevalence of gout. Gout was associated with an increased prevalence of depression. The use of uratelowering agent was not associated with increased depression.
Given that the incidence and prevalence of gout increased over the past eleven years in Hong Kong, the control for the risk factors, including lead exposure is important.
The suboptimal control of urate-lowering agent should be promoted for better management of gout. Patients should be screened for depression.published_or_final_versionMedicineDoctoralDoctor of Philosoph
Cancer Risk of Angiotensin II Receptor Blocker Valsartan: A Population-based Study
Nitrosamine contamination of generic valsartan was found in 2018. This study aimed to investigate whether long-term use of valsartan increases cancer risk. Patients prescribed valsartan or amlodipine (control group) from 1 January 1, 2003, to June 30, 2010, were identified using the Clinical Data Analysis and Reporting System of the Hong Kong Hospital Authority, a territory-wide database in Hong Kong. Patients previously diagnosed with cancer, prescribed both medications, taking the medication, or followed up for <1 year were excluded. Cancer incidence, adjusted for age, sex, and Charlson Comorbidity Index, was the primary outcome and was estimated using Poisson regression in R version 3.6.1. Among 5023 valsartan users and 3692 amlodipine users, 887 and 740 were diagnosed with cancers during median follow-up periods of 10.97 and 12.12 years, respectively. The adjusted incidence of cancer in valsartan and amlodipine users was 165.29 (95% confidence interval 154.76-175.53) and 180.12 (167.35-193.67) per 10,000 person-years, respectively. The cancer incidence rate ratio of valsartan relative to amlodipine was 0.94 (0.88-1.01). Adjusted incidence rate ratios of valsartan relative to amlodipine were significant for breast cancer (0.63, 0.46-0.86) only. Our findings do not suggest an increase in incidence of cancer with long-term valsartan use. The duration of follow-up of more than 10 years of the study provides the reassurance that an increase in cancer risk is unlikely. Further studies are needed to elucidate the long-term effect of valsartan use on the risk of specific types of cancer.</p
Kinetics of ortho-para conversion of solid hydrogen catalyzed by paramagnetic impurities
M.Phil.Based on the kinetic data obtained in our group, a model is developed in explaining catalytic ortho-para conversion of H₂ in solid H₂ doped with O₂ and NO respectively. 2 phonon emission mechanism is successfully applied to fit the experimental result of O₂ doped system. The unexpectedly higher conversion rate for NO doped system can be explained by using resonance ortho-para conversion mechanism. The more diffusive resonance conversion by NO gives larger effective radius than conversion with the 2 phonon emission by O₂, and thus gives a faster observed rate.對於組內對氫份子於固態氫的核自旋弛豫現象的反應動力學結果,我們建立一模型嘗試解釋摻雜氧份子或一氧化氮後氫份子的核自旋弛豫的速率及兩者間的差異。由氧份子催化的核自旋弛豫可以以雙聲子放射作為主要弛豫機理去解釋其速率。對比氧份子催化,一氧化氮的催化核自旋弛豫速率意外地快。我們提出一氧化氮的催化核自旋弛豫主要以諧振核自旋弛豫作為解釋。由於諧振核自旋弛豫的反應有效半徑比起雙聲子放射機理更大,而氫份子更容易遇上催化劑造成核自旋弛豫。Lai, Kin Fung."December 2018."Thesis M.Phil. Hong Kong government's financial assistance for maintenance scheme (FAMS) for privately-owned graded historic buildings : the case of the repair and maintenance works for Fung Chi Tsuen Tin Hau Temple, Yuen Long
The Conserve and Revitalize Hong Kong Heritage of Bureau department (BD) implement a Financial Assistance for Maintenance Scheme (FAMS) in 2007. The objective of this scheme is to provide financial assistance in the form of grants for the owner of privately-owned graded historic buildings in Hong Kong to process minor works by themselves from deterioration due to lack of maintenance.
This dissertation will evaluate the scheme to find out what works and what does not work as the FAMS has been in operation for seven years (from 2008 to 2015). Up to September 2015, there are fifty successful applications to grant under the financial assistance on privately-owned graded historic buildings. There are twelve applications being processed including nine applications obtained Stage one approval. (AAB, 2015).
This dissertation uses the author’s professional experience as a conservation contractor in Ding Hsung Construction Company, to identify the actual scope of work and actual costs involved in the repair and maintenance of a Chinese heritage building in Hong Kong, in order to evaluate the FAMS and make recommendations to make it work better for conservation.
This dissertation will focus on the FAMS as applied to the repair and maintenance of the Fung Chi Tsuen Tin Hau Temple, Yuen Long as a case-study. As the author has been involved in the actual repair and maintenance work for the case-study building using grant from the FAMS, she is in a good position to provide in-depth information and analysis for the research.published_or_final_versionConservationMasterMaster of Science in Conservatio
