    Endocrine alterations in HIV-infected patients

    Aims and objectives: To study the frequency of thyroid, adrenal and gonadal dysfunction in newly diagnosed HIV-infected patients and to correlate them at different levels of CD4 cell counts. Materials and Methods: Forty-three HIV-positive cases were included in the study group. Cases were divided into three groups on the basis of CD4 cell count. Serum free T3, free T4, TSH, Cortisol, FSH, LH, testosterone and estradiol were estimated by the radioimmunoassay method. Hormone levels between cases were compared and their correlation with CD4 count was analyzed. Results: Prevalence of gonadal dysfunction (88.3%) was the most common endocrine dysfunction followed by thyroid (60.4%) and adrenal dysfunction (27.9%). Secondary hypogonadism (68.4%) was more common than primary (31.6%). Low T3 syndrome, that is, isolated low free T3, was the most common (25.6%) thyroid dysfunction followed by secondary hypothyroidism (16.2%) and subclinical hypothyroidism (11.6%). Adrenal excess (16.3%) was more common than adrenal insufficiency (11.6%). The difference in hormonal dysfunction between male and female was statistically insignificant (P > 0.05). 27.9% of patients had multiple hormone deficiency. There was negligible or no correlation between CD4 count and serum hormone level. Conclusion: In our study, endocrine dysfunction was quite common among HIV-infected patients but there was no correlation between hormone levels and CD4 count. Endocrine dysfunctions and role of hormone replacement therapy in HIV-infected patient needs to be substantiated by large longitudinal study, so that it will help to reduce morbidity, improve quality of life

    Stacked-Value of Battery Storage: Effect of Battery Storage Penetration on Power Dispatch

    abstract: In this work, the stacked values of battery energy storage systems (BESSs) of various power and energy capacities are evaluated as they provide multiple services such as peak shaving, frequency regulation, and reserve support in an ‘Arizona-based test system’ - a simplified, representative model of Salt River Project’s (SRP) system developed using the resource stack information shared by SRP. This has been achieved by developing a mixed-integer linear programming (MILP) based optimization model that captures the operation of BESS in the Arizona-based test system. The model formulation does not include any BESS cost as the objective is to estimate the net savings in total system operation cost after a BESS is deployed in the system. The optimization model has been formulated in such a way that the savings due to the provision of a single service, either peak shaving or frequency regulation or spinning reserve support, by the BESS, can be determined independently. The model also allows calculation of combined savings due to all the services rendered by the BESS. The results of this research suggest that the savings obtained with a BESS providing multiple services are significantly higher than the same capacity BESS delivering a single service in isolation. It is also observed that the marginal contribution of BESS reduces with increasing BESS energy capacity, a result consistent with the law of diminishing returns. Further, small changes in the simulation environment, such as factoring in generator forced outage rates or projection of future solar penetration, can lead to changes as high as 10% in the calculated stacked value.Dissertation/ThesisMasters Thesis Electrical Engineering 202

    Abstract Atypical and unusual presentations of tuberculosis have become a major diagnostic problem for health professionals. We describe a 30-year-old male patient with a tuberculosis abscess in the teres minor muscle. The patient was initially misdiagnosed due to this unusual presentation and the definitive diagnosis was only made after the histological examination of the drainage material. The patient responded to the anti-tuberculosis therapy and there was no recurrence after 4 years. Özet Sıradışı ve nadir klinik tablolarla ortaya çıkan tüberküloz olgularının tanısı sağlık çalışanları açısından önemli bir sorun oluşturmaktadır. Çalışmamızda teres minör kası içinde tüberküloz apsesi ile başvuran 30 yaşındaki bir erkek hastayı değerlendirdik. İlk değerlendirmelerinde doğru tanı konamayan hastanın tüberküloz tanısı ancak drenaj materyalinin histolojik incelemesi sonucunda konulabildi. Hasta anti-tüberkülöz tedaviye yanıt verdi ve 4 yıllık takip sonrasında tekrarlama görülmedi
