    Role of B-cell memory on CD38-targeting Human Leukocyte Antigen desensitization therapy in Highly sensitized kidney transplant candidates

The benefits of kidney transplantation include increased life expectancy, improved quality of life by eliminating the need for regular dialysis treatments and lower risk of complications, such as reduction of cardiovascular diseases, which are a major cause of mortality in ESRD patients. Overall, kidney transplantation not only offers a better quality of life but also reduces medical costs associated with long-term dialysis treatments. Unfortunately, only a small percentage of patients with ESKD receive a kidney transplant, being most barriers to access kidney transplantation patient-related, physician/providerrelated, and system-related. Still, beyond these healthcare system barriers, transplantation also faces some important limitations including the shortage of òrgans for all patients waitlisted and the lack of significant improvement of long-term graft survival rates, mainly due to activation of the alloimmune response leading to chronic (B-cell mediated) allograft rejection, calcineurin-inhibitors(CNI)-related nephrotoxicity, relapse of primary ESRD as well as the death of the patients with a functioning graft because of adverse events directly associated to chronic immunosuppression such as cancer and opportunistic infections and off-target effects related side effects inducing diabetes, hypertension, dyslipidemia that significantly increase the risk of fatal cardiovascular events. HYPOTHESIS: The hypothesis of this thesis is that considering the observed efficacy of anti-CD38 mAb to successfully deplete malignant cells in multiple myeloma patients, we hypothesize that anti-CD38 mAbs may target LLPCs and plasma cells also in highly sensitized transplant candidates, depleting one of the main production sources of anti-HLA antibodies leading to their elimination and increasing the probability of these patients to find an HLA compatible donor. Additionally, the assessment of the B-cell profile of each patient prior to therapy using high dimensional analysis with high-throughput technologies such as spectral flowcytometry, could identify specific biological features associated to a successful serological response to this therapy. Moreover, a thorough assessment of different cell sources of HLA-sp antibodies such as bone marrow-residing plasma cells, MBCs and serum antibodies in the context of patients undergoing desensitization with CD38-targeting therapy would provide deep insight on the immune mechanisms of allosensitization. OBJECTIVES The main objectives of this thesis are the following: •Assess the safety, pharmacokinetics and preliminary efficacy of the anti-CD38 mAb isatuximab in a phase 1/2 desensitization clinical trial (NCT04294459) in HS patients awaiting kidney transplantation. •Assess the biological effect of CD38-targeting therapy on peripheral blood and bone marrow B-cell subsets using phenotypical and functional immune assays. •Assessing the biological interplay of distinct HLA-sp immune memory compartments leading to serological sensitization •Investigate whether specific biological features of peripheral B-cell components prior to anti-CD38 therapy could discriminate between patients displaying successful serological responses from those showing low or no response to this therapy. •Validate these findings in an external, independent cohort of highly sensitized kidney transplant candidates receiving a different anti-CD38 mAb as desensitization therapy.[cat] INTRODUCCIÓ: El trasplantament renal és el millor tractament per a la malaltia renal terminal (ESRD), oferint resultats superiors en comparació amb qualsevol tipus de teràpia de diàlisi. Els beneficis del trasplantament renal inclouen l'augment de l'esperança de vida, la millora de la qualitat de vida eliminant la necessitat de tractaments regulars de diàlisi i un menor risc de complicacions, com la reducció de les malalties cardiovasculars, que són una de les principals causes de mortalitat en els pacients amb ESRD. En general, el trasplantament renal no només ofereix una millor qualitat de vida, sinó que també redueix els costos mèdics associats als tractaments de diàlisi a llarg termini. Malauradament, només un petit percentatge de pacients amb ERT reben un trasplantament de ronyó, sent la majoria de les barreres per accedir al trasplantament renal relacionades amb el pacient, el metge / proveïdor i el sistema. Tot i així, més enllà d'aquestes barreres del sistema sanitari, el trasplantament també s'enfronta a algunes limitacions importants, com ara l'escassetat d'òrgans per a tots els pacients en llista d'espera i la manca de millora significativa de les taxes de supervivència de l'empelt a llarg termini, principalment a causa de l'activació de la resposta al·loimmune que condueix al rebuig crònic (mediat per cèl·lules B), nefrotoxicitat relacionada amb els inhibidors de la calcineurina (CNI), recaiguda de l'ESRD primària, així com la mort dels pacients amb un empelt funcional a causa de Esdeveniments adversos directament associats a la immunosupressió crònica com càncer i infeccions oportunistes i efectes secundaris relacionats amb efectes fora de l'objectiu que indueixen diabetis, hipertensió i dislipèmia que augmenten significativament el risc d'esdeveniments cardiovasculars mortals. HIPÒTESI: La hipòtesi d'aquesta tesi és que, tenint en compte l'eficàcia observada dels mAb anti-CD38 per esgotar amb èxit les cèl·lules malignes en pacients amb mieloma múltiple, plantegem la hipòtesi que els mAbs anti-CD38 poden dirigir-se a LLPC i cèl·lules plasmàtiques també en candidats a trasplantament altament sensibilitzats, esgotant una de les principals fonts de producció d'anticossos anti-HLA conduint a la seva eliminació i augmentant la probabilitat d'aquests pacients de trobar un donant compatible amb HLA. A més, l'avaluació del perfil de cèl·lules B de cada pacient abans de la teràpia mitjançant anàlisis d'alta dimensió amb tecnologies d'alt rendiment com la citometria de flux espectral, podria identificar característiques biològiques específiques associades a una resposta serològica exitosa a aquesta teràpia. A més, una avaluació exhaustiva de diferents fonts cel·lulars d'anticossos HLA-sp com les cèl·lules plasmàtiques residents a la medul·la òssia, MBC i anticossos sèrics en el context de pacients sotmesos a desensibilització amb teràpia dirigida a CD38 proporcionaria una visió profunda dels mecanismes immunitaris de l'al·losensibilització. OBJECTIUS Els objectius principals d'aquesta tesi són els següents: •Avaluar la seguretat, la farmacocinètica i l'eficàcia preliminar de l'isatuximab anti-CD38 mAb en un assaig clínic de desensibilització de fase 1/2 (NCT04294459) en pacients amb HS en espera de trasplantament renal. •Avaluar l'efecte biològic de la teràpia dirigida a CD38 en subconjunts de cèl·lules B de sang perifèrica i medul·la òssia mitjançant assajos immunològics fenotípics i funcionals. •Avaluar la interacció biològica de diferents compartiments de memòria immunitària HLA-sp que condueixen a la sensibilització serològica •Investigar si les característiques biològiques específiques dels components perifèrics de les cèl·lules B abans de la teràpia anti-CD38 podrien discriminar entre els pacients que mostren respostes serològiques reeixides dels que mostren una resposta baixa o nul·la a aquesta teràpia. •Validar aquestes troballes en una cohort externa i independent de candidats a trasplantament renal altament sensibilitzats que reben un anti-CD38 mAb diferent com a teràpia de desensibilització

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
