    Current evidence suggests that critically ill patients tolerate anaemia well and that blood transfusions may increase the risk of adverse outcomes. Dr Sakr and colleagues present a contradictory analysis of a surgical ICU cohort, finding an association between blood transfusions and lower hospital mortality after adjustment for a range of potential confounders. Analyses of this kind are interesting and provocative, but are limited by residual confounding and bias by indication. The data emphasise the need for additional high quality trials of transfusion practice in critical care

    Discovery of bent functions using the Fast Walsh Transform

    Linear cryptanalysis attacks are a threat against cryptosystems. These attacks can be defended against by using combiner functions composed of highly nonlinear Boolean functions. Bent functions, which have the highest possible nonlinearity, are uncommon. As the number of variables in a Boolean function increases, bent functions become extremely rare. A method of computing the nonlinearity of Boolean functions using the Fast Walsh Transform (FWT) is presented. The SRC-6 reconfigurable computer allows testing of functions at a much faster rate than a PC. With a clock frequency of 100 MHz, throughput of the SRC-6 is 100,000,000 functions per second. An implementation of the FWT used to compute the nonlinearity of Boolean functions with up to five variables is presented. Since there are 22n Boolean functions of n variables, computation of the nonlinearity of every Boolean function with six or more variables takes thousands of years to complete. This makes discovery of bent functions difficult for large n. An algorithm is presented that uses information in the FWT of a function to produce similar functions with increasingly higher nonlinearity.     Hormonal modulation of breast cancer gene expression: implications for intrinsic subtyping in premenopausal women

    Clinics are increasingly adopting gene-expression profiling to diagnose breast cancer subtype, providing an intrinsic, molecular portrait of the tumor. For example, the PAM50- based Prosigna test quantifies expression of 50 key genes to classify breast cancer subtype, and this method of classification has been demonstrated to be superior over traditional immunohistochemical methods that detect proteins, to predict risk of disease recurrence. However, these tests were largely developed and validated using breast cancer samples from postmenopausal women. Thus, the accuracy of such tests has not been explored in the context of the hormonal fluctuations in estrogen and progesterone that occur during the menstrual cycle in premenopausal women. Concordance between traditional methods of subtyping and the new tests in premenopausal women is likely to depend on the stage of the menstrual cycle at which the tissue sample is taken and the relative effect of hormones on expression of genes versus proteins. The lack of knowledge around the effect of fluctuating estrogen and progesterone on gene expression in breast cancer patients raises serious concerns for intrinsic subtyping in premenopausal women, which comprise about 25% of breast cancer diagnoses. Further research on the impact of the menstrual cycle on intrinsic breast cancer profiling is required if premenopausal women are to benefit from the new technology of intrinsic subtyping.Sarah M.
