Given concern about the Aids pandemic in South Africa, this study used structural equation modelling and partial least squares to investigate whether the use of fear in social advertising increases the likelihood of adopting appropriate behaviour pertaining to HIV/Aids prevention. Fear, attitude towards the advertisements, severity, susceptibility, response efficacy and self-efficacy were examined for their effect on behavioural intent of young adults within specific market segments. Relationships were found among susceptibility, fear, attitude, and behavioural intent, and different relationship paths were identified for segments based on gender and culture/racial groupings.     A web-based survey of United Kingdom sedation practice in the intensive care unit

    PurposeThe purpose of this work was to obtain a detailed perspective of sedation practice. Sedation included sedative and opioid choice, presence of local guidelines, and use of scoring systems.MethodsA Web-based survey was designed. The aim was to gain sufficient detail of UK sedation while also being succinct enough to complete in 15 minutes. It was composed of relevant demographics, policy, sedative choice, and analgesia. The survey was piloted before launch. The investigators selected the intensive care unit (ICU) pharmacist as the respondent.ResultsOne hundred fifty-seven ICUs responded. Eighty-nine (59%) reported use of sedation guidelines, 78% undertook sedation holds, and 87% use sedation scores. Only 42% used a daily sedation target. Seventy (43%) assess for delirium; 27 of those use a validated tool.Propofol (89%) use was common, followed by midazolam (49%). Morphine (49%), fentanyl (34%), and alfentanil (34%) were the most frequently used opioids.ConclusionThis survey confirmed expected variation in UK sedation practice. Recognized strategies such as target sedation score and sedation policy are underused. A 43% uptake in delirium screening suggests that larger engagement is required to meet national standards

    Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    Implementing an optimisation model for turbine investment selection under fluctuating steam productions

    Engineering plants typically have a variety of interlinked production chains, where process flows are dependent on upstream or downstream events. Raw material feeds may fluctuate over time, resulting in fluctuating off-gas and steam productions, and therefore an inefficient energy resource usage. It is common practice to generate steam from burnable offgases, in boiler houses, where excess steam is allocated for power generation after plant usages. Over time fluctuating steam flow may result in turbine trips. A problem is that unused off-gases are flared where the energy potential goes to waste. This paper investigates turbine investment choices under fluctuating steam productions. A mathematical model is used that determines how steam should be distributed between turbines in a fluctuating environment, for optimal power generation. Investment options can be explored regarding which combination of turbines will result in higher power generation for the same combined power generation capabilities and how each turbine configuration will influence the total number of turbine trips due to steam shortages. A number of simulations are performed for various turbine configurations and it was found that an increase in the number of turbines for a fixed maximum total power generation capacity will deliver slightly higher power generation but significant more turbine trips.     Simulation of thermal plant optimization and hydraulic aspects of thermal distribution loops for large campuses

    Following an introduction, the author describes Texas A&M University and its utilities system. After that, the author presents how to construct simulation models for chilled water and heating hot water distribution systems. The simulation model was used in a $2.3 million Ross Street chilled water pipe replacement project at Texas A&M University. A second project conducted at the University of Texas at San Antonio was used as an example to demonstrate how to identify and design an optimal distribution system by using a simulation model. The author found that the minor losses of these closed loop thermal distribution systems are significantly higher than potable water distribution systems. In the second part of the report, the author presents the latest development of software called the Plant Optimization Program, which can simulate cogeneration plant operation, estimate its operation cost and provide optimized operation suggestions. The author also developed detailed simulation models for a gas turbine and heat recovery steam generator and identified significant potential savings. Finally, the author also used a steam turbine as an example to present a multi-regression method on constructing simulation models by using basic statistics and optimization algorithms. This report presents a survey of the author??s working experience at the Energy Systems Laboratory (ESL) at Texas A&M University during the period of January 2002 through March 2004. The purpose of the above work was to allow the author to become familiar with the practice of engineering. The result is that the author knows how to complete a project from start to finish and understands how both technical and nontechnical aspects of a project need to be considered in order to ensure a quality deliverable and bring a project to successful completion. This report concludes that the objectives of the internship were successfully accomplished and that the requirements for the degree of Degree of Engineering have been satisfied

    Utilising fluctuating off-gases more efficiently in a power co-generation environment through less flaring

    Common engineering practice is to generate steam from burnable off-gases, in boiler houses, where excess steam is allocated for power generation after plant requirements are met. If the quantities or chemical composition of raw material feeds are not constant, off-gases and therefore steam productions may fluctuate over time. Fluctuating steam flows may cause turbines to trip due to insufficient availability at times. All off-gases not utilised are flared to atmosphere, nullifying its energy potential.This paper investigates the effect for when the volume quantity of off-gas flaring is regulated and the improved potential that this could have on power co-generation, taking into account the maximum combined boiler houses' capacities. Although flaring must always take place for line pressure control; for the engineering works under consideration 46.9 % of all the off-gas productions were flared. A mathematical model is used that distributes steam between turbines for optimal power generation to assist with the investigation in determining the power generation capabilities of the available and potential steam flow. The patients’ knowledge of ART and HIV is thus crucial to ensuring good adherence, decreased risk for drug resistance and cost-effective treatment for these patients. Aim: To determine the knowledge of infected patients with regard to HIV and the ART they were receiving. Setting: The study was conducted at a comprehensive community health centre in a developing low socio-economic community near Cape Town, South Africa. Methods: A quantitative descriptive correlative research design was applied. A sample consisting of 200 (8.5%) respondents was selected from a population of 2349. A multiple-choice questionnaire, comprising 29 questions, including 14 critical knowledge testing questions, was used in individual interviews conducted by either the researcher or fieldworker who assessed the respondents’ knowledge regarding various key aspects of HIV and ART. Results: Misconceptions regarding HIV and ART were revealed and scores for the 14 critical knowledge testing questions in the questionnaire revealed that 0% of the respondents had good knowledge, 20% had average knowledge and 80% had poor knowledge. Conclusion: The respondents on ART in this particular community health centre had poor knowledge of HIV and ART. This may contribute to poor adherence rates, increased drug resistance, disease progression and increased costs for the government with regard to treating such patients.
