    The SF-36: a simple, effective measure of mobility disability for epidemiological studies

    BackgroundMobility disability is a major problem in older people. Numerous scales exist for the measurement of disability but often these do not permit comparisons between study groups. The physical functioning (PF) domain of the established and widely used Short Form-36 (SF-36) questionnaire asks about limitations on ten mobility activities.ObjectivesTo describe prevalence of mobility disability in an elderly population, investigate the validity of the SF-36 PF score as a measure of mobility disability, and to establish age and sex specific norms for the PF score.MethodsWe explored relationships between the SF-36 PF score and objectively measured physical performance variables among 349 men and 280 women, 59-72 years of age, who participated in the Hertfordshire Cohort Study (HCS). Normative data were derived from the Health Survey for England (HSE) 1996.Results32% of men and 46% of women had at least some limitation in PF scale items. Poor SF-36 PF scores (lowest fifth of the gender-specific distribution) were related to: lower grip strength; longer timed-up-and-go, 3m walk, and chair rises test times in men and women; and lower quadriceps peak torque in women but not men. HSE normative data showed that median PF scores declined with increasing age in men and women.ConclusionOur results are consistent with the SF-36 PF score being a valid measure of mobility disability in epidemiological studies. This approach might be a first step towards enabling simple comparisons of prevalence of mobility disability between different studies of older people. The SF-36 PF score could usefully complement existing detailed schemes for classification of disability and it now requires validation against them

    The Relationship between Self-Assessed Health and The Specialization of the Middle and Old Age Wai Tan Kung Participants from Chang-Hua County Taiwan

    [[abstract]]ABSTRACT Taiwan becomes old age and less new born baby society. Young population should pay more money to support elders, especially more medical expenses. If the new born baby rate cannot be increased recently, to improve health is the best way to decrease young generation?s loading. Wai Tan Kung is a kind of gentle and low intense exercise. It is good for middle-age and old-age participants? health. Short Form-36 (SF-36) is a form recognized by World Health Organization to access people?s health. To review the past Wai Tan Kung?s research, most studies focused on the physical impacts. There were few researches to use SF-36 as a tool to study Wai Tan Kung. The objectives of this research were to realize the relationships between the specialization of Wai Tan Kung participants and their self-assessed health by SF-36. Four hundred people were selected in this cross-sectional, descriptive study by using a structured questionnaire. Three hundred fifty-one valid data were used to analyze as the results, the valid corresponding rate was 87.75%. The surveys were conducted during the period of March to April 2008. The results of this study were the followings: 1. The relationship between gender and age of surveyed Wai Tan Kung participants and their self-assessed health (1) There were no significant difference between surveyed Wai Tan Kung participants? gender and their self-assessed health. (2) The value of vitality (VT) was significantly related to surveyed male Wai Tan Kung participant?s ages. (3) There were no significant differences between surveyed Wai Tan Kung participants? ages and their self-assessed health. 2. The relationship between the specialization of surveyed Wai Tan Kung participants and the self-assessed health. (1) Most surveyed Wai Tan Kung participants were assessed as the middle level of specialization, the primary level was 2nd high surveyed population, the highest level of specialization was 3rd high ones, new players were the fewest surveyed population. (2)The values of physical functioning(PF), role limitations due to physical health problems (RP), role limitations due to emotional problems (RE), social functioning (SF), bodily pain(BP), vitality (VT), mental health (MH), and general health (GH) were significantly positively related to surveyed one?s specializations. (3) The results of surveyed Wai Tan Kung participants were similar to females. (4) The values of role limitations due to physical health problems (RP), role limitations due to emotional problems (RE), vitality (VT), mental health (MH), and general health (GH) were significantly positively related to surveyed male one?s specializations. 3. The participant?s motivation, to make friends, was significantly related to surveyed Wai Tan Kung participant?s specializations. 4. The participant?s obstacles, not enough time and not affordable physical agility, were significantly related to surveyed Wai Tan Kung participant?s specializations. According to the results of this research, we can encourage people to play Wai Tan Kung to improve their health

    Production Of Bacteriocin Sf By Lactobacillus Gasseri Sf For Use In Dermal Bacterial Infection And Wound Healing

    Enam belas strain Lactobacillus dan Bifidobacterium telah disaring berdasarkan pertumbuhan di dalam susu skim. B. longum 8643, L. plantarum 8943, L. casei 1268, L. fermentum 8312, L. fermentum 8848, dan L. gasseri SF menunjukkan kemandirian yang lebih tinggi (P< 0.05), dan ekstrak extrasel daripada enam strain ini juga mengandungi bioaktif pada konsentrasi yang mampu meningkatkan kesihatan kulit. Di samping itu, protein mentah yang diperolehi daripada ekstrak extrasel juga menunjukkan aktiviti penghambatan yang lebih tinggi (P< 0.05) terhadap pertumbuhan patogen kulit, dan mungkin disebabkan oleh sebatian antimikrob protein. Unsur seperti bakteriosin yang dihasilkan oleh L. gasseri SF menunjukkan aktiviti penghambatan yang lebih tinggi terhadap Enterococcus faecalis FM 2138, dan memenuhi ciri-ciri bakteriosin kelas II, justeru dinamakan sebagai bakteriosin SF. Bakteriosin SF juga didapati stabil haba dengan jisim molekul ketara sebanyak 3.5 kDa. Bakteriosin SF pada kepekatan 10240 AU/mL juga mengurangkan cas negatif pada permukaan sel E. faecalis FM 2138 dengan signifikan (P <0.05), dan seterusnya menyebabkan depolarisasi membran dan pembentukan liang seni, seperti yang ditunjukkan dalam mikrograf elektron. Sixteen strains of Lactobacillus and Bifidobacterium were screened based on their growth in reconstituted skimmed milk. However, six strains (B. longum 8643, L. plantarum 8943, L. casei 1268, L. fermentum 8312, L. fermentum 8848, and L. gasseri SF) exhibited significantly higher viability (P< 0.05). Extracellular extracts of these strains contained bioactives at concentrations capable of promoting dermal health. Meanwhile, crude protein fractions fractionated from extracellular extracts of all six strains exhibited significantly higher antagonistic activity on skin pathogens, probably due to the production of putative bacteriocins. Putative bacteriocin produced from L. gasseri SF exhibited significantly higher (P< 0.05) antagonistic activity on Enterococcus faecalis FM 2138, and fitted the characteristics of class II bacteriocin and was thus renamed as bacteriocin SF. Bacteriocin SF was found to be heat-stable, with an apparent molecular mass of 3.5 kDa. Bacteriocin SF at a concentration of 10240 AU/mL significantly reduced (P< 0.05) the negative charge on the cellular surface of E. faecalis FM 2138, subsequently leading to membrane depolarization and pore formation, as visible in electron micrographs

    Systematic investigation of gastrointestinal diseases in China (SILC): validation of survey methodology

    Background: Symptom-based surveys suggest that the prevalence of gastrointestinal diseases is lower in China than in Western countries. The aim of this study was to validate a methodology for the epidemiological investigation of gastrointestinal symptoms and endoscopic findings in China. Methods: A randomized, stratified, multi-stage sampling methodology was used to select 18 000 adults aged 18-80 years from Shanghai, Beijing, Xi'an, Wuhan and Guangzhou. Participants from Shanghai were invited to provide blood samples and undergo upper gastrointestinal endoscopy. All participants completed Chinese versions of the Reflux Disease Questionnaire (RDQ) and the modified Rome II questionnaire; 20% were also invited to complete the 36-item Short Form Health Survey (SF-36) and Epworth Sleepiness Scale (ESS). The psychometric properties of the questionnaires were evaluated statistically. Results: The study was completed by 16 091 individuals (response rate: 89.4%), with 3219 (89.4% of those invited) completing the SF-36 and ESS. All 3153 participants in Shanghai provided blood samples and 1030 (32.7%) underwent endoscopy. Cronbach's alpha coefficients were 0.89, 0.89, 0.80 and 0.91, respectively, for the RDQ, modified Rome II questionnaire, ESS and SF-36, supporting internal consistency. Factor analysis supported construct validity of all questionnaire dimensions except SF-36 psychosocial dimensions.
