Evaluation of shoulder kinesthesia in patients with unilateral frozen shoulder
Background: Intact sensory-motor system provides kinesthesia for maintaining joint stability. Joint proprioception or joint kinesthetic sensation is often used alternately to describe a deep sensation of joint motion and joint position sense. Mechanoreceptors located in superficial layer of joint capsule, muscle spindle, golgi tendon organ complex and ligaments around joint provides joint kinesthetic sensation. Frozen shoulder or adhesive capsulitis is one of the common musculoskeletal disorders encountered in Indian population with a prevalence of almost 50% older patients with diabetes and 2-10% in non-diabetic patients. This study assessed joint kinesthetic sensation during shoulder movements in people with frozen shoulder and healthy controls which may be affected due to presence of disease process or pain. Authors hypothesized that pain and alterations in non-contractile tissues (mainly capsule) around shoulder joint due to frozen shoulder may interfere with the joint kinesthetic sensation.Methods: Case group consisted of 41 participants with frozen shoulder and 41 healthy participants without shoulder pathology. Joint kinesthesia was assessed in frozen shoulders and dominant shoulders using angle reproduction test by actively reproducing the target angle placed passively by the therapist during flexion, abduction, medial and lateral rotation movements. The angle reproduced was measured using universal goniometer by the same therapist and 3 readings were recorded.Results: Results indicate significant differences between the mean difference of angle reproduction test during shoulder movements in cases of frozen shoulder and healthy group (p value <0.05).Conclusions: Significant shoulder kinesthetic deficits were observed in patients with unilateral frozen shoulder. Therefore, clinicians should consider rehabilitation of kinesthesia in treatment of patients with frozen shoulder

Residual uncertainty estimation using instance-based learning with applications to hydrologic forecasting
A non-parametric method is applied to quantify residual uncertainty in hydrologic streamflow forecasting. This method acts as a post-processor on deterministic model forecasts and generates a residual uncertainty distribution. Based on instance-based learning, it uses a k nearest-neighbour search for similar historical hydrometeorological conditions to determine uncertainty intervals from a set of historical errors, i.e. discrepancies between past forecast and observation. The performance of this method is assessed using test cases of hydrologic forecasting in two UK rivers: the Severn and Brue. Forecasts in retrospect were made and their uncertainties were estimated using kNN resampling and two alternative uncertainty estimators: quantile regression (QR) and uncertainty estimation based on local errors and clustering (UNEEC). Results show that kNN uncertainty estimation produces accurate and narrow uncertainty intervals with good probability coverage. Analysis also shows that the performance of this technique depends on the choice of search space. Nevertheless, the accuracy and reliability of uncertainty intervals generated using kNN resampling are at least comparable to those produced by QR and UNEEC. It is concluded that kNN uncertainty estimation is an interesting alternative to other post-processors, like QR and UNEEC, for estimating forecast uncertainty. Apart from its concept being simple and well understood, an advantage of this method is that it is relatively easy to implement

Indian (Marathi) version of the Shoulder Pain and Disability Index (SPADI): Translation and validation in patients with adhesive capsulitis
Background: The Shoulder Pain and Disability Index (SPADI) is the most commonly used self-administered questionnaire which is a valid and reliable instrument to assess the proportion of pain and disability in shoulder disorders. There is no evidence of SPADI questionnaire being translated into regional Indian language (Marathi). Objective: This study aims to translate and culturally adapt and validate the Marathi version of the SPADI questionnaire. This was done as per the AAOS outcomes committee guidelines. Methods: Cross-cultural adaptation and psychometric testing of SPADI was done in the Outpatient Physiotherapy Department of Tertiary Care Hospital, Ahmednagar, India. Results: The internal consistency was assessed by calculating Cronbach alpha value for the pain score (0.908), disability score (0.959), and total SPADI (0.969) which were all high. The Test–retest reliability was assessed using the intraclass correlation coefficient (ICC) values for the pain score (0.993), disability score (0.997), and total SPADI (0.997) which showed excellent reliability. The criterion validity was assessed using Pearson correlation coefficient. In Males, weak to strong negative correlation was observed except for shoulder extension and in females, moderate negative correlation was observed between baseline shoulder range of motion and initial total SPADI scores and individual pain and disability except for shoulder internal rotation. The internal consistency of the Marathi SPADI (Cronbach’s alpha [Formula: see text]0.99) was higher than the original English version. The reliability of the total Marathi SPADI and its subscale (Intraclass correlation coefficient [Formula: see text]0.90) were found to be higher than that of the English SPADI and were consistent with the German, Brazilian, Slovene and Greek versions. Conclusion: The translated and culturally adapted Marathi version of the SPADI questionnaire is a reliable and valid tool for the assessment of pain and disability in Marathi population

Dharma and Polity Through the Character of Raju in R.K Naraayan’s “The Guide”
Dharma is a distinct quality that leads one to explore knowledge. In the novel ‘The Guide’, there are characters who reflect the concept of Dharma in a true sense of words. So, Dharma is a key concept. In the Western countries, the concept of Dharma is taken as a religion which appears to be a limited and incomplete as compared to the Indian concept of Dharma. Dharma indeed signifies patterns of behaviour considered to be accord of Rta that makes life in the universal possible. It includes duties, law, conduct, virtues and right way of life. R. K Narayana is a classical author in Indian fiction. Narayan is a true Indian both in spirits and thoughts. He has always been claimed as a novelist par excellence. Incidentally little has been written on how Narayan incorporates the profoundest Indian thoughts, philosophies and spiritualism in general and theory of Karma in particular in his novels. He is widely known for his lucid and natural writing style, often compared to William Faulkner. Most of his works show his deep interest in Hindu Religion and Myth. He does not modify or revise the myths through their symbolic representation. “To be a good writer anywhere, you must have roots - both in Religion and family. I have these things.”(Stephen Graubart) As a great writer, he translated and published shortened prose versions of the two great Indian epics, the Ramayana and the Mahabharata and a few Hindu mythical tales in Gods, Demons and others. His familiarity of Indian classical literature, philosophy, religion and ethics permeates his writing but a simple man that he was; he does not unnecessarily burden his readers with discourses on abstract philosophy and metaphysics. As an English writer, he depicted Dharma spirituality and ethics or the concept of religion in his novels. Through The Guide, we have come know how a fraudulent tourist guide, Raju playing the central role as a saint. Theoretically, morally, honorably, legally and ethically a tourist guide is not preferred to misguide the tourists. Raju changed his life from an ordinary guide to a sage and unfolds the concept of Dharma, finally realized his self and sacrificed his life for the sake of Dharma and Polity. The present paper deals with the role of dharma in ‘The Guide’ through Raju. The concept of dharma found in Indian philosophy is artistically incorporated in the ‘The Guide’. He himself mentioned that there is “a nucleus of absolute truth in all my novels.” He reveals this truth in different ways with the aim to regenerate the consciousness of malgudains. In one of his interview Narayan told that one should have root in the family and religion or Dharam. William wash praise Narayan for embodying this pure concept of dharma in his novels. The thoughtful concept of dharma with sincerity is deeply found in ‘The Guide’ R.K. Narayan has given a true common image of India through in ‘The Guide’ The central character, Raju, goes to the Mangala village on the Sarayu River and he stays in a temple on the sea shore. There he meets Velan. Raju narrates his past life as a successful tourist guide ‘Railway Raju’ and a rail road station food seller. Raju, the tourist guide is initially entrapped in the illusory world when the materialistic Charvaka philosophy guides and governs his life. Raju has been described as a spiritual martyr, one who finds himself in a insecure situation on account of the misunderstanding of a village idiot. Martyrdom is thus imposed upon him under certain unavoidable circumstances. But a close study of Raju’s actions, thoughts and behaviour significantly indicates a change in Raju. The sacrifice of Raju followed by the rains in Mangla village could very well be treated as a justification of Indian philosophy and its various paradigms. Once Raju, as a tourist, guide happened to meet Marco and Rosie. Marco concentrated more on other worldly affairs and gives less attention towards his gorgeous wife Rosie. Raju helped both Marco and Rosie to recognize her desire of becoming a dancer. Raju becomes very close to her. He was much impressed by Rosie. He ignored his friend Gaffur’s warning and even his mother’s advice. He turned a hard of hearing even to the voice of his own soul. The only truth in my existence was Rosie. All my psychological powers were now turned to keep her within my reach, and keep her cheery all the time, neither of which was at all easy. I would deliberately have kept at her side all the occasion, as a sort of scrounger. His over associate with a married lady becomes a serious burden to his tragedy. Raju helped Rosie in making her dreams come true. Rosie became a professional dancer. Raju forged her signature and mailed the document for which he was caught and sent to jail for two years. After coming out of the jail he has been accepted as a saint of Mangala. The villagers approached Raju to undertake a fast to please the rain God and this made him to confess his past life to Velan. Even after listening to the past life of Raju, Velan continued to acknowledge him as Swami; he took Raju’s confession as a mark of humility and godliness. He persuaded Raju for a fast and Raju finally agreed to fast ‘If by avoiding food I should help the trees bloom, and the grass grow, why not do it thoroughly?’ and that became his final decision. He slowly changed himself without his knowledge as a real sage. Sharan comments that: In the course of his ordeals, he changes himself thoroughly and accepts the challenges of reality. He plays his role of a holy man with a ring of sincerity and embraces death at the end of the novel. Thus, ‘Railway Raju’, the guide becomes a ‘spiritual guide’ and all his imperfection and impersonation turn into a real act of self-sacrifice. Raju has no other way but to undergo the fast though reluctantly he gained power to undertake the fast for real and through this power he sacrificed. After the twelve day Raju’s physical condition was very poor. He bows his head for prayer and said to Velan, ‘Velan it’s raining in the hills. I can feel it coming up under my feet, up legs –He sagged down. Thus the swami Raju’s was dedicated to the betterment for the poor people of the Mangala as a divine saint. It is true that R.K. Narayan has depicted that Indian belief and feelings can be articulated in foreign language without demanding to reproduce the native speakers of English. He gave his characters Indian thoughts and expressed it in his scenes and backgrounds. Indian culture and tradition is rich and it is not easy to summarize through few situations or characters. But Narayan made it exuberant and all his characters share Indianness. A close reading of the novel explores that Raju is not very bad person. Though he made affairs with Rosie, a married woman, his heart warns him of being in an Indian society and he is not at ease and peace. In his own words he establishes his fear: “My thoughts dwelt on her golden touch. A part of my mind went on saying. ‘No, no. It is not right. Marco is her husband, remember. It’s not to be thought of.’ Rosie, besides her modern –type relationship with Raju, regards Marco as her husband. Raju’s mother is portrayed as a woman born and bred up in an orthodox, conservative class of Indians

Study of sustainable development of architecture of Iceland
Idea zrównoważonego rozwoju – sustainable development - jest stosunkowo nowym
prądem myślowy. Termin ten został użyty po raz pierwszy w raporcie Światowego
Komitetu ds. Środowiska i Rozwoju „the Brundtland Commission”, chociaż ogólny
zarys zrównoważonego rozwoju nakreślony był już w 1972 roku w Sztokholmie
podczas Konferencji ONZ „Środowisko Człowieka”. [1] W architekturze pojawia się
od niedawna ale obecnie zaczyna mieć znaczący wpływ na kształtowanie myśli architektonicznej
współczesnych projektantów. Miasta coraz większy nacisk kładą na
architekturę ekologiczną. Przyczyną jest znaczna urbanizacja powierzchni i dynamiczny
rozwój miast. Aby oddać ziemi powierzchnię zabraną przez powstające budynki
coraz popularniejsze stało się stosowanie np. zielonych dachów. Zwiększa to
powierzchnię biologicznie czynną. Autor pragnie przedstawić rozumienie definicji
zrównoważonego rozwoju w kontekście architektonicznym. W niniejszym artykule
na podstawie wymienionych przykładów architektury zrównoważonego rozwoju
w Islandii, prezentuje sposoby myślenia oraz wykorzystywania naturalnych zasobów
jakimi dysponuje. Omówione są również rozwiązania projektowe stosowane w ekologicznej
wiosce Sólheimar w Islandii. Klimat na tej wulkanicznej wyspie jest niezwykle
surowy, dlatego też według autora stanowi ona doskonały przykład pełnego
wykorzystania wartości jakimi dysponuje. Niniejszy artykuł jest próbą przedstawienia
pozytywnej drogi do architektury zrównoważonego rozwoju. Przykłady zaprezentowane
w niniejszym tekście przez autora mają za zadanie zwrócenie uwagi na
doskonałe wykorzystanie połączenia współczesnych technologii z rozwiązaniami
stosowanymi już od wieków w Islandii.The idea of sustainable development is relatively well-dim current thinking. This term was first used
in the report-nity Interior and Administration Committee. Environment and Development, "the Brundtland
Commission", cho-pregnancy overview of sustainable development was already outlined in 1972 in Stockholm
at the UN Conference "Environment Man". [1] In the architecture-sculpture it appears recently but is
now beginning to have a significant impact on the shaping of architectural thought-Wani contemporary
designers. Cities increasing emphasis on ecological architecture. The reason is the vast urbanization after surface
and dynamic urban development. To return the land taken away from the surface formed by the
buildings became increasingly popular to use eg. Green roofs. This increases the area of biologically active.
The author wishes to understand the present-not the definition of sustainable development in the context of
architecture. In st of this article on the basis of these examples of architecture-sustainable development in
Iceland, it presents ways of thinking and the use of natural resources at its disposal. They are also discussed
design solutions used in ecological village Solheimar in Iceland. The climate on this volcanic island is extremely
severe, and therefore according to the author it is an excellent example of the full use of its disposal.
This article is an attempt to present a positive way to the architecture of sustainable development. Examples
presented herein by the author are intended blender!-Priced due to the excellent use of combination of
modern technology and the solutions used for centuries in Iceland

Avaliação da ação citotóxica de cardenolídeos em células tumorais
Tese (doutorado) - Universidade Federal de Santa Catarina, Centro de Ciências da Saúde, Programa de Pós-Graduação em Farmácia, Florianópolis, 2015.Os cardenolídeos, tais como digoxina e oubaína, são conhecidos por sua eficácia no tratamento da insuficiência cardíaca congestiva e como fármacos antiarrítmicos. Recentemente, foram detectadas novas atividades farmacológicas para esses "antigos" fármacos. Tendo em vista o crescente interesse na pesquisa e desenvolvimento desta classe de compostos como potenciais quimioterápicos, frente a diversas linhagens celulares tumorais, o presente trabalho de tese objetivou avaliar os efeitos citotóxicos em células tumorais de três cardenolídeos (glucoevatromonosídeo, digitoxigenina monodigitoxosídeo e convalotoxina), previamente selecionados pela sua potente ação citotóxica. Inicialmente, eles foram avaliados em linhagens celulares tumorais de diferentes origens e todos demonstraram uma potente ação, em concentrações nanomolares, em todas as linhagens testadas, especialmente nas células de tumor de pulmão (A549). Essa linhagem tumoral foi então selecionada para a continuação dos experimentos para detectar o tipo de morte celular causada por esses três cardenolídeos. O glucoevatromonosídeo (GEV), composto mais promissor, também foi investigado em células U937 (linfoma histiolítico). Todos os três causaram bloqueio da fase G2/M, enquanto que a convalotoxina (CON) aumentou o número de células em subG0 do ciclo celular. O GEV foi capaz de inibir a expressão de importantes proteínas relacionadas ao ciclo celular, como ciclina B1 e p53 em A549. Ainda, esse composto causou bloqueio em subG0 em células U937, demonstrando um efeito dependente do tipo celular. O efeito de morte celular causado pelo GEV também foi tipo celular dependente, já que foi observada morte celular pela ação de caspases nas células U937 e independente da ação das caspases em células A549. A digitoxigenina monodigitoxosídeo não apresentou efeito significativo de morte celular em células A549. A CON aumentou o número de núcleos picnóticos e células Anexina-V positivas, configurando morte celular apoptótica. Alem disso, os três compostos foram capazes de inibir a migração e invasão celulares, em células A549, bem como de reduzir a expressão das proteínas: MMP2, MMP9 e FAK (proteína de adesão focal), que são essenciais no processo de metástase. Além disso, o GEV foi capaz de inibir a expressão de importantes cinases, geralmente super expressas em células tumorais. O conjunto desses dados sugere que estes compostos, especialmente o GEV, podem ser considerados candidatos promissores para o desenvolvimento de uma forma farmacêutica a ser usada no tratamento do câncer de pulmão.Abstract : Cardenolídeos such as ouabain and digoxin are known for their efficacy in treating congestive heart failure as antiarrhythmic drugs. Recently, new pharmacological activities have been found (antiviral and antitumor) to these "old" drugs. Given the growing interest in research and development of this group of compounds, as potential chemotherapeutic agents, this work aimed to evaluate the cytotoxic effects on tumor cells of three cardenolides (glucoevatromonoside, digitoxigenin monodigitoxoside and convallatoxin) previously selected for its potent cytotoxic action. Initially, they were screened in tumor cell lines of different origins and all of them showed potent action at nanomolar concentrations in all cell lines tested, particularly in lung tumor cells (A549). This cell line was then selected for further investigation to detect the kind of cell death caused by these three cardenolides. Glucoevatromonoside (GEV) was the most promising compound, and also investigated in U937 cells (histiocytic lymphoma). All these compounds block G2/M phase, whereas the convallatoxin increased the number of cells in subG0 phase. GEV was able to inhibit the expression of important proteins related to cell cycle, such as Cyclin B1 and p53. Beyond that, this compound caused cell cycle blockage in subG0 phase in U937 cells, demonstrating dependent cell type effect. The effect of cell death caused by GEV was also cell type dependent. In U937 cells, GEV showed a caspase dependent cell death while it is independent of caspase in A549 cells. Digitoxigenin monodigitoxoside did not show a significant percentage of cell death in A549 cells. Convallatoxin increased the number of pyknotic nuclei and Annexin-V positive cells, setting apoptotic cell death in A549 cells. Furthermore, the three compounds are capable of inhibiting cell migration and invasion in A549 cells as well as to reduce the expression of some proteins as MMP2, MMP9 and FAK (focal adhesion protein), which are essential in the process of metastasis. Moreover, GEV was able to inhibit the expression of important kinases, usually over expressed in tumor cells.. Taken together, these data suggest that these compounds, especially GEV can be considered promising candidates for the development of a pharmaceutical form to be used in the treatment of lung cancer
