    Cushing syndrome and the anesthesiologist, two case reports

    Cushing′s syndrome (CS) is associated with reduced life quality and increased mortality, mostly due to cardiovascular disease. The features of this syndrome are central obesity, moon facies, facial plethora, supraclavicular fat pads, buffalo hump, and purple striae. Other complications include hyperglycemia, hypertension, proximal muscle weakness, skin thinning, menstrual irregularities, amenorrhea and osteopenia. These make perioperative and anesthetic management difficult and present a challenge to the operating team, especially the anaesthesiologist. In this paper, we present two such cases of CS, which were treated with adrenalectomy. We aim to highlight the special care and precautions that need to be taken while administering anesthesia, and in the post operatory period. Anaesthesia induction in the two cases of CS was done prior to the adrenalectomy procedure and special pre and post operative care was taken. Continuous intra operative monitoring of vitals and checking for the stability of the haemodynamics was performed. With adequate care and using advanced anesthetic techniques, the patients showed uneventful post operative recovery. Though the anesthetic management of patients with CS is difficult, desired results can be achieved with continuous monitoring and special precautions

    U članku se iznosi kako je izgledala prvotna ranogotička kuća, koja je zauzimala prostor na današnjim česticama zgrade broj 111/1, 111/2 i 111/3 k. o. Hvar u predjelu Grada u Hvaru, prislonjena uz gradski zid u trenutku gradnje Novoga grada od 1278. g., te na temelju iznijetih elemenata spora iz 1524. g. između pjesnika Petra Hektorovića (1487-1572.) i Franje Paladinića, koje je objavio Joško Belamarić u svom predavanju na Danima Cvita Fiskovića u Splitu te u članku u časopisu Vijenac, glasilu Matice hrvatske, Zagreb, od 27. prosinca 2001., donosim svoja stajališta vezana uz predmetnu kuću i kuću pjesnika Petra Hektorovića podignutu na gradskom zidu između palače Paladinić na zapadu i palače Jakša na istoku.In this paper the author analyses the original look of an early Gothic house behind the town wall of Hvar, that belonged to Marin, son of Hektor, and to his son, the famous Croatian Renaissance poet Petar Hektorović (1487-1572).He also analyses the lawsuit between the poet Petar and his neighbour Franjo Paladinić in the early 1500' s, according to an article recently published by the art historian J .Belamarić, which is added to this paper in extenso. Comparing the documents with the architectural situation on the site, he concludes that the contention was about their neighbouring houses on the ramparts of Hvar

    Maternal and Neonatal Effects of Vasopressors Used for Treating Hypotension after Spinal Anesthesia for Caesarean Section: A Randomized Controlled Study

    AIM: The aim of the study was to examine whether ephedrine and phenylephrine were different in their efficacy for managing maternal hypotension and their effect of adverse maternal and neonatal outcome.METHODS: A double-blind randomized controlled study in healthy pregnant women ASA physical status 2, which underwent elective caesarian delivery under spinal anesthesia. Patients were randomized to receive an intravenous bolus of either phenylephrine (Ph group) or ephedrine (E group) immediately after the episode of hypotension after spinal anesthesia. Maternal and neonatal outcomes were recorded.RESULTS: Two hundred and two (202) pregnant women at term were entered in this study. There were no differences between group E and group Ph regarding the incidence of hypotension after vasopressor therapy, and the incidence of nausea and vomiting. There was no significant difference between groups in the first-minute and the 5th minute Apgar score, none of the neonates had the true fetal acidosis.CONCLUSIONS: Ephedrine and phenylephrine have the same efficacy in treating hypotension after spinal anesthesia for caesarean section. The use of Phenylephrine was associated with better fetal acid-base status, and there were no differences on Apgar score values and on the incidence of maternal bradycardia and hypotension

    Present day school readers in rapport with the dramatic works of a once-banned Albanian author

    The process of reading, rereading, scrutinizing, re-scrutinizing and appraising an author’s work is not fi nite; moreover it is very dynamic and full of suprises. No doubt that the appraisal and re-appraisal is too complex, including cultural, literary, gender and genre components. This becomes more obvious when referring to an author whose works had been locked up in library stacks during the whole 45 years of totalitarian rule in Albania, and whose appraisal process cannot help facing up to the critical thinking led by biased politicization. In this study the appraisal of the works of one of the most outstanding Albanian writers of the 30s of the twentieth century, Etëhem Haxhiademi, will be done by presenting them in rapport with the present-day school, pre-university and university level readers. This re-appraisal is conducted with a cold logic, devoid of the vindictive political principles which brought the distorted evaluation of the authors’ works, as it happened for many other au-thors as well, and is striving to place the writer into the system of the genuine literary values. Once the reader’s inquisitiveness has been satisfi ed, even after reading a less-known or, moreover, an oblivious or banned literary work, he/she begins reading and rereading it, looking closely into it, essaying to fi nd a proper place for this work in the general system of the national, inter-balkanic and international literary values.E. Haxhiademi’s literary work was known by an older generations of readers whose es-thetic satisfaction it brought forth was confi ned in their consciousness: it was unknown or partly known, not by direct reading but by several interpretations of others, by a middle generation of readers who displayed a certain artistic uncertainty and indifference. However, nowadays it is intently being studied although in excessively fragmentary ways and in many cases even unsuitable ones, by the younger generation of readers who should have a more motivated curiosity for divulging its real values.In the history of the Albanian literature, as well as in the other East European Countries, the authors’ biographies and especially their political aspects, were used as selective criteria to appraise authors and their works, instead of being pure cognitive and studying means. This study intends to reveal the present-day school readers’ affi rmative attitudes or even the non-affi rmative ones, regarding Haxhiademi’s literary works, in the process of re-dimen-sioning the author’s values and those of his literary works

    Pilot study on children's drawing in two sociocultural contexts (Argentina-Germany)

    El presente artículo tuvo como propósito analizar la evolución del dibujo infantil de acuerdo a las primeras etapas propuestas por Lowenfeld (1980): garabateo, preesquemática y esquemática, en niños residentes en Argentina y en Alemania. Se propusieron tres objetivos específicos: 1) indagar si, en la actualidad, las mencionadas etapas se suceden en las mismas edades que propuso dicho autor; 2) analizar las características del dibujo infantil en niños de 3 a 6 años residentes en Argentina y en Alemania y 3) comparar las producciones gráficas realizadas por niños residentes en ambos países, con el fin de indagar diferencias entre ambos contextos socioculturales. La muestra estuvo compuesta por un total de 136 niños de 3 a 6 años de edad, de los cuales 72 niños fueron argentinos y 64 alemanes. Se utilizó un enfoque cuantitativo, descriptivo-comparativo, con un diseño transversal. Los instrumentos utilizados fueron la técnica del dibujo libre, y una planilla de evaluación realizada ad hoc. Se observó que la secuencia en la que se presentaron las etapas del dibujo fue idéntica a las expuestas por Lowenfeld, sin embargo, fue notorio el avance en las edades en relación a dichas etapas. Al analizar las etapas del dibujo infantil, no se hallaron diferencias estadísticamente significativas entre ambos grupos de niños/as (Argentina-Alemania). Asimismo, las características del dibujo fueron similares en ambos grupos.The aim of this article was to analyze the evolution of children's drawings according to the first stages proposed by Lowenfeld: the scribbling stage, the pre-schematic stage and the schematic stage, in children living in Argentina and Germany. Three specific objectives were established: 1) to investigate whether, at present, the aforementioned stages occur at the same ages proposed by said author; 2) to analyze the characteristics of children's drawings in children aged 3 to 6 years old living in Argentina and Germany and 3) to compare the graphic productions made by children living in both countries, in order to investigate the differences between both sociocultural contexts. The sample included a total of 136 children between the ages of 3 to 6, 72 children of which were Argentinian and 64 were German. A quantitative, descriptive-comparative approach with a cross-sectional design was used. The instruments used were the free drawing technique and an assessment form made ad hoc. It was observed that the sequence in which the stages of drawing were presented was identical to that presented by Lowenfeld. However, the advance in the ages in relation to said stages was evident. When analyzing the stages of children's drawing, no statistically significant differences were found between both groups of children (Argentina-Germany).     Similes and Narrative Structure in the Iliad

It was submitted to the archives on a a CD.The Homeric simile functions on several levels in each of its appearances throughout the Iliad, creating emphasis and indicating the importance of a scene, character, or moment to the audience. Approaching the Iliad from an oral traditional perspective and accepting the idea of composition in performance, I take a close look at several sequences of similes in the poem, observing how the various sequences and patterns lend a degree of narrative structure to the poem and direct the audience's attention to a articulate thematic idea. I primarily focus on the similes repeatedly used to describes deaths on the battlefield and explain how the development of this pattern becomes familiar to an audience. I examine the absences of the anticipated death simile in the cases of both Patroklos and Hektor and interpret what implications this has for each of the characters and the larger narrative. In order to fully understand the context surrounding both of these scenes, I closely analyze the simile sequences in Books 16 and 22 that lead up to the deaths of Patroklos and Hektor, arguing that in exempting the simile at the moment of death, the narrator actually places a larger emphasis on the characters themselves, which becomes all the more powerful for an audience sensitive to the normal occurrence of the death simile.This paper was read and approved by Susan B.     The cardiac patient undergoing major urologic surgery is a complex case requiring a great attention by the anesthesiologist. Number of this group of patients having to go through this procedure is constantly increasing, due to prolonged life, increased agressiveness of surgery and increased anesthesia’s safety. The anesthesiologist usually has to deal with several problems of the patient, such as hypertension, chronic heart failure, coronary artery disease, rhythm disturbances, intraoperative hemodymanic changes, intraoperative bleeding, perioperative fluid imbalance, and metabolic disturbances. A cardiac patient undergoing major urologic surgery is a complex case requiring a great attention by the anesthesiologist. The scope of this review article is to present the most frequent issues encountered with this group of patients, and to synthetically discuss the respective strategies and maneuvers during perioperative period, which is the major challenge for the anesthesiologist
