    Bases moleculares da deficiência de G6PD e sua relação com o tratamento da hanseníase

    A hanseníase é uma doença infecciosa crônica, com alto poder incapacitante e endêmica no Brasil. O tratamento se baseia em um esquema de poliquimioterapia (PQT/WHO), que envolve basicamente três fármacos: dapsona (DDS), rifampicina e clofazimina. Embora a implementação desta tenha possibilitado a cura da hanseníase, a ocorrência de reações adversas (ADRs) associadas à intolerância aos fármacos tem se tornado frequente. Dados da literatura indicam que DDS é responsável por cerca de 70% das ADRs identificadas durante o tratamento da hanseníase. Dentre as ADRs induzidas por DDS a anemia hemolítica (AH) merece destaque, pois acarreta sérios riscos principalmente em pessoas com deficiência da enzima glicose-6-fosfato-desidrogenase (G6PD). A AH leva a hemólise prematura da hemácia e pode resultar em óbito do paciente. A G6PD é a enzima responsável por catalisar a primeira etapa do ciclo das pentoses, sendo esta crucial para a proteção das hemácias contra o processo oxidativo. Esta enzima é codificada pelo gene G6PD localizado no cromossomo X. Mutações pontuais (SNPs) nesse gene podem levar à deficiência na G6PD. Atualmente 180 variantes genéticas da G6PD já foram descritas. Contudo, somente 20 delas resultam na deficiência da atividade enzimática e estão associadas à AH. A variante Canton apresenta uma troca de um nucleotídeo G por um T na posição 1376, o que ocasiona uma baixa atividade enzimática (menos de 10%). A troca de um nucleotídeo A por G na posição 376 corresponde a variante A que não, que está relacionada à AH aguda. Já a variante Asahi, descoberta em meninos asiáticos, é resultado da troca de um nucleotídeo G por um A na posição 202. A troca do nucleotídeo C por T na posição 563 origina a variante Mediterrânea, a qual também confere menos de 10% da atividade enzimática residual associada à AH aguda No caso dessa última variante, a atividade enzimática da G6PD chega a ser nula já nas hemácias jovens. No presente estudo, foram avaliados 537 pacientes portadores de hanseníase, oriundos de quatro macrorregiões brasileiras e que haviam sido tratados através da PQT/WHO. Os indivíduos selecionados foram avaliados para sete SNPs no gene G6PD. De acordo com os resultados, na população estudada, o número de pacientes do gênero feminino foi significativamente maior que do gênero masculino (p=0,000001). Além disso, a frequência maior de pacientes multibacilares na população do gênero feminino também foi observada. A variante A foi a mais frequente, sendo observada em 14,8% da população, seguida da variante A-202A/376G observada em 4,8% e a variante Asahi em 3,9% da população estudada. 39% dos pacientes desenvolveram algum tipo de ADR associada à dapsona e destes 19% apresentaram AH. Dos pacientes que desenvolveram AH 31% apresentavam alguma mutação em G6PD sendo a variante A-202A/376G a mais frequente nesse grupo. A variante A apresentou uma distribuição homogênea em três macrorregiões, todavia, não foi observada na região sul. Estudos posteriores devem ser realizados para avaliar a influência de cada mutação na ocorrência da AH medicamentosa. Esses resultados sugerem que a deficiência de G6PD tem uma tendência a agravar as ADRs atribuídas a DDS e que a variante A-202A/376G é a mais severa nesse quesitoLeprosy is a chronic infectious disease with high incapacitating power and endemic in Brazil. Among the DDSinduced ADRs, hemolytic anemia (HA) deserves to be highlighted, as it makes serious risk, especially in people with deficiency of the enzyme glucose-6-phosphate dehydrogenase (G6PD). The HA leads to premature hemolysis and may result in death of the patient. G6PD is the enzyme responsible for catalyzing the first step of the pentoses cycle, being crucial for the protection of the red blood cells against the oxidative process. This enzyme is encoded by the G6PD gene located on X chromosome. Point mutations (SNPs) in this gene can lead G6PD deficiency. Nowadays, 180 genetic variants of G6PD have been described. However, only 20 of these Result in deficiency of enzymatic activity and are associated with HA. The Canton variant shows a nucleotide change from G to T at position 1376, which lead to a low of the enzymatic activity (less than 10%). The change of the nucleotide A by G at position 376 corresponds to variant A, that is not related to acute HA, but the Asahi variant, discovered in Asian boys results from the change of nucleotide G to A at position 202. The change of the nucleotide C to T at position 563 gives rise to the variant Mediterranean which also confers less than 10% of the residual enzyme activity associated with the acute HA In the case of this last variant, the enzymatic activity of G6PD gets to be null already in young red blood cells. In the present study, were evaluated 537 leprosy patients from four Brazilian macro regions and already treated with MDT / WHO. The selected individuals were evaluated for seven SNPs in G6PD gene. According the results, in the studied population, the number of female patients was significantly higher than the male patients (p = 0.000001). Besides that a higher frequency of multibacillary within women was also seen. The A variant showed to be the most frequent being observed in 14.8% of the population , followed by the A- 202A / 376G variant, being observed in 4.8% and the Asahi variant in 3,9% of the studied population. Thirty nine per cent of the patients presented any type of ADR associated with DDS from which, 19% presented HA. From patients who developed HA, 31% presented any mutation in G6PD being the variant A-202A / 376G the more frequent in this group. The variant A presented a homogeneous distribution among the three macro regions, however, not observed in the southern region. Further studies should be performed to assess the influence of each mutation on the occurrence of drug-induced HA. These results suggest that the G6PD deficiency has a tendency to get worse, the DDS-induced ADRs and that the variant A-202A / 376G is the most important regarding this issu

A pobreza, a falta de informação, a demora no diagnóstico de cepas resistentes e multi-resistentes e fatores de risco associados à infecção por Mycobacterium tuberculosis são os principais responsáveis pela manutenção de sua transmissão. Em nosso estudo, utilizamos duas abordagens moleculares, baseadas na reação de PCR, \201CSingle Strand Conformational Polymorphism-SSCP\201D e \201CReverse Line Blot Hybridization\201D-RLBH, para verificarmos genótipos mutantes e a freqüência de mutações no genoma de M. tuberculosis associadas à resistência a drogas, assim como avaliarmos a RLBH para a detecção rápida de resistência. Utilizando o SSCP, verificamos que a versão radioativa apresentou uma melhor resolução que a não radioativa. Na versão não-radioativa uma discordância foi observada (9,1%) ao comparar os resultados com a análise por sequenciamento. Na RLBH, uma membrana contendo sondas para detecção de resistência a: rifampicina, isoniazida, estreptomicina e etambutol foi avaliada em um estudo multi-cêntrico e 50% das cepas resistentes a rifampicina apresentavam mutações em rpoB nos códons 531-TTG (43%) e 526-GTC (7%) enquanto que 49% das cepas resistentes a etambutol apresentavam mutações no códon 306-GTG (34%) e ATA (14%) Uma segunda versão da RLBH, para detecção de resistência a rifampicina com sondas específicas para o gene rpoB foi avaliada. Analisamos 50 cepas de Mycobacterium Bovis, sensíveis e 157 cepas de M. tuberculosis sensíveis e resistentes a rifampicina. Todas as cepas de M. bovis apresentaram o genótipo de sensível e entre as cepas resistentes de M. tuberculosis, 93% apresentaram mutações na região de 157pb do gene rpoB. O Inno-LiPA (teste comercial), baseado em RLBH, para detecção de cepas de M. tuberculosis resistente a rifampicina, também foi analisado, apresentando uma sensibilidade de 97,6%.Concluímos que a hibridação reversa tem a sensibilidade adequada para detectar cepas de M. tuberculosis resistentes a rifampicina e que os mecanismos relacionados à resistência a estreptomicina e isoniazida necessitam de uma melhor compreensão. The Inno-LiPA (commercial test), based in the RLBH for the detection of strains resistant to rifampin, was also analyzed presenting a sensitivity of 97.6%.     Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    Tuberculose (TB) é a principal causa de morbidade e de mortalidade em todo o mundo, particularmente em países em desenvolvimento. Na cidade de Rio de Janeiro, 7.000 casos novos de TB são relatados anualmente, para uma incidência de 105 por 100.000 habitantes. Em três centros de saúde comunitários (CHCs) da área de planejamento 1 (AP-1.0) do município do Rio de Janeiro (área da cidade com taxa a mais elevada de TB), uma avaliação da influência do tratamento diretamente observado (DOT) sem ou com profilaxia (DOT-A) para contatos de risco foi realizada; os contatos do último grupo foram avaliados também ativamente para TB ativa ou latente. Para esta finalidade, quatro comunidades receberam o tratamento DOT enquanto outras quatro comunidades receberam DOT-A. Um estudo nessas comunidades foi realizado durante o período 2001 2004 e recebemos os isolados de 353 pacientes, distribuídos igualmente entre as vizinhanças com os dois grupos de tratamento (DOT e DOT-A) Os dados bacteriológicos, clínicos e epidemiológicos foram coletados e os genótipos de Mycobacterium tuberculosis definidos com MIRU-VNTR, \201Cspoligotyping\201D e RFLP-IS6110. Também, o uso da sonda do lado direito e esquerdo do sítio de restrição da PvuII na IS6110 foi avaliado. Todas as amostras foram genotipadas por ambos os métodos de PCR, enquanto uma grande fração das amostras de 2001 foi submetida a RFLP-IS6110. Os resultados dos agrupamentos entre os três métodos foram comparados e as taxas de \201Ccluster\201D foram, respectivamente, 20%, 29% e 72% para RFLP-IS6110, MIRU-VNTR e \201Cspoligotyping\201D. A combinação de MIRU-VNTR e \201Cspoligotyping\201D para a definição dos genótipos reduziu o número de \201Cclusters\201D para 7.8% e os dados iniciais obtidos que sugerem que os \201Cclusters\201D formados por RFLP-IS6110 foram respeitados por MIRU-VNTR. Entretanto, a tipagem por RFLP-IS6110 deve ser realizada em todos os isolados para se ter uma comparação mais sólida desta técnica com MIRU-VNTR e \201Cspoligotyping\201D. O maior \201Ccluster\201D obtido por MIRU-VNTR foi observado somente nas amostras de 2001 e podemos sugerir uma epidemia local temporária A determinação dos fatores de risco para TB foi avaliada pela associação da formação de \201Cclusters\201D dos isolados com transmissão recente da TB entre os pacientes. Abordagens adicionais da análise de dados devem, entretanto ser realizadas e estão em andamento.The Tuberculosis (TB) is a serious problem of world-wide public health, particularly in the developing countries. In the city of Rio de Janeiro, 7,000 new TB cases are reported annually, for an incidence de 105 per 100,000 inhabitants. In three Community Health Centers (CHCs) of Planning Area 1 (AP-1.0) of Rio de Janeiro, the section of the city with the highest case rate of TB, an evaluation of the influence of directly observed treatment (DOT) without or with (DOT-A) prophylaxis of risk contacts was performed; contacts of the latter group were also actively evaluated for active or latent TB. For this purpose, four communities received the DOT regimen while other four communities received DOT-A. A prospective community-based study was performed during the period 2001 to 2004 and we received strains of 353 patients, equally distributed among the neighborhoods with the two DOT schemes. Bacteriological, clinical and epidemiological data were collected and Mycobacterium tuberculosis genotypes defined through MIRU-VNTR (mycobacterial interspersed repetitive units - variable number tandem repeats) typing, spoligotyping and IS6110 restriction fragment length polymorphism (IS6110-RFLP). Also, the use of the left-hand side probe and a right-hand side probe of the PvuII site in the IS6110 was evaluated. All samples were typed by both PCR typing methods while a large fraction of the 2001 samples was submitted to IS6110-RFLP. Clustering results between the three methods were compared and cluster rates were, respectively, 20%, 29% and 72 % for IS6110-RFLP, MIRU-VNTR typing and spoligotyping. The combination of MIRU-VNTR and spoligotyping techniques for the definition of genotypes reduced the number of clusters to 7.8% and initial data were obtained suggesting that clusters formed by IS6110-RFLP are respected by MIRU-VNTR. However, IS6110-RFLP typing should be performed of all isolates to have a more solid comparison of this technique with MIRU-VNTR and spoligotyping. The biggest cluster obtained with MIRU-VNTR was only observed in 2001 and could suggest a local temporary outbreak. Risk factor determination for TB was evaluated through association of cluster formation of isolates with recent transmission of TB between patients. This was performed for the three typing methods and for the combined MIRU-VNTR and spoligotyping procedure. The only risk factor that was consistently related with recent transmission was being infected with a resistant or multi-drug resistant strain. No difference in clustering rates was observed between the two DOT procedures suggesting either the absence of association between clustering of genotypes and recent transmission of TB or the lack of influence of the additional procedures inherent to DOT-A in relation to DOT on TB transmission in the investigated population.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films.     Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods
