    Efficacy and safety of parahisian accessory pathway ablation: randomized comparison between cryotherapy and radiofrequency energy sources

    Fundamentação: A ablação percutânea permanece como estratégia de escolha para tratamento definitivo de vias acessórias de condução atrioventricular, contudo a depender da localização a taxa de sucesso e de complicações podem variar. As vias acessórias para-hissianas (VAPH) encontram-se perto do sistema nativo de condução, em particular proximidade ao feixe de His, e por isso há risco aqui de bloqueio atrioventricular total (BAVT) durante ablação. Existem atualmente duas modalidades de energia que podem ser utilizadas neste cenário, crioterapia e radiofrequência, porém até o momento não há estudos comparativos prospectivos e randomizados entre as duas modalidades. Objetivo: Avaliar o perfil de segurança e eficácia da ablação percutânea de VAPH, comparando os resultados obtidos com uso das duas modalidades de fonte de energia existentes na literatura: crioterapia (CRIO) e radiofrequência (RF). Métodos: estudo piloto prospectivo intervencionista, randomizado 1:1, não-cego, unicêntrico. Foram incluídos pacientes portadores de VAPH confirmados por estudo eletrofisiológico (EEF) com indicação de ablação conforme diretrizes vigentes. Resultados: Entre outubro/2018 e fevereiro/2020 foram randomizados 30 pacientes, 90% do sexo masculino e com idade média de 25 anos. A taxa de sucesso inicial foi igual entre os grupos RF e CRIO (93% vs. 87%, p=0,54), bem como taxa de recorrência (14% vs. 30%, p=0,3) e trauma mecânico da via (6% vs. 20%, p=0,28). Houve 2 casos de BAVT transitório no grupo CRIO, nos quais a amplitude do sinal do feixe de His era significativamente maior do que naqueles em que não houve a complicação (0,8 + 0,07mV vs. 0,35 + 0,17mV, p=0,004). A taxa de recorrência foi semelhante entre os grupos, bem como sobrevida livre de eventos em um ano de seguimento (Log-Rank p=0,286 para múltiplos procedimentos), com média de 1,2 procedimentos por paciente. Não houve surgimento de BAVT ou distúrbio de condução persistente em ambos os grupos. Acréscimo de monitor de eventos ao exame clínico munido de eletrocardiograma de repouso não identificou casos adicionais de recorrência. Conclusão: respeitando-se as limitações de tamanho da amostra e de incidência de eventos, a ablação percutânea de vias acessórias para-hissianas foi realizada com segurança e taxas de sucesso aceitáveis. Não foram identificados casos de BAVT persistente em ambos os grupos. O presente estudo gera a hipótese de equivalência entre as modalidades de energia, desde que empregadas em centro de expertise, e abre portas para ensaios de maior número amostralBackground: Percutaneous ablation remains the strategy of choice for definitive treatment of AV accessory pathways; however, success rate and complications may vary according to cardiac site. Parahisian accessory pathways (PHAP) are positioned near the native conduction system, and for this reason they pose a greater risk of total atrioventricular block (TAVB) during the procedure. There are currently two types of energy sources that can be used in this scenario, cryotherapy (CRYO) and radiofrequency (RF), but so far there are no randomized prospective comparative studies described in the literature. Objective: To evaluate the safety and efficacy profile of percutaneous PHAP ablation, comparing cryotherapy and radiofrequency (RF). Methods: prospective, non-blinded, single-center interventional randomized 1:1 pilot study. Patients included had PHAP confirmed by electrophysiological study (EPS) referred for ablation according to current guidelines were included. Results: 30 patients were randomized (90% male) between October/2018 and February/2020 with a mean age of 25 years. The initial success rate was the same between the RF and CRIO groups (93% vs. 87%, p=0.54), as well risk of recurrence (14% vs. 30%, p=0.3) and mechanical trauma of the pathway (6% vs. 20%, p=0.28). There were 2 cases of transient AVB in the CRYO group, in which the His bundle signal amplitude was significantly greater than in those without AVB (0.8 + 0.07mV vs. 0.35 + 0.17mV, p=0.004). The recurrence rate was similar between groups, as well as event-free survival after one-year follow-up (Log-Rank p=0.286 for multiple procedures), with a mean of 1.2 procedures per patient. There was no emergence of TAVB or persistent conduction disturbance in either group. The addition of an event monitor to the clinical examination with resting electrocardiogram did not identify additional cases of recurrence. Conclusion: with due limitations of sample size and incidence of events, percutaneous ablation of parahisian accessory pathways may be performed safely and with acceptable success rates. No occurrence of persistent TAVB was found in both groups. The present study generates the hypothesis that radiofrequency and cryotherapy may be equivalent regarding PHAP percutaneous ablation, as long as they are used in a center of expertise, and points toward a need for larger randomized multicenter trial

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
