    Silent Changes in Sleep Quality Following Mandibular Setback Surgery in Patients with Skeletal Class III Malocclusion: A Prospective Study

    Mandibular setback surgery (MSS) for skeletal class III patients can result in a relative reduction of pharyngeal airway space (PAS). Consequently, there is a possibility of the decline of sleep quality after surgery. We investigated changes in sleep quality measured by overnight polysomnography (PSG) and the three-dimensional (3D) volumes of PAS following MSS with or without Le Fort I osteotomy (LF I) in class III patients (N = 53). Overnight PSG and cone beam computed tomography were conducted at preoperative stage (T0) and postoperative 3 months (T1). Measurements of PAS volumes were performed, and the subjective symptoms of sleep were evaluated by self-questionnaires. There were significant increases in respiratory disturbance index (RDI) and total respiratory effort-related arousal (RERA) index during T0-T1. The 3D volumes of PAS showed significant decreases in the oropharyngeal airway, hypopharyngeal airway, and total airway spaces. No significant changes were observed in subjective symptoms of sleep. MSS with or without LF I for class III patients could worsen sleep quality by increasing sleep parameters such as the RDI and RERA in PSG, and reduce volumes of PAS at postoperative 3 months. Although subjective symptoms may not show significant changes, objective sleep quality in PSG might decrease after MSS

    Effects of rhBMP-2 with various carriers on maxillofacial bone regeneration through computed tomography evaluation

    Background: rhBMP-2 is regarded as the most potent osteoinductive growth factor, and it has been used in the oral cavity with different carriers. The purpose of this study is to evaluate the bone-regenerative effect of rhBMP-2 delivered with different carrier systems through three-dimensional cone beam computed tomography analysis. Method: A total of 112 patients underwent oral surgery with rhBMP-2 application (Group 1, n = 53) or without rhBMP-2 application (Group 2, n = 59). Group 1 was divided into 3 groups according to carriers, rhBMP-2 with allograft (Group 1–1, n = 34), rhBMP-2 with xenograft (Group 1–2, n = 5), and rhBMP-2 with absorbable collagen sponge (Group 1–3, n = 14). Cone beam computed tomography scans were taken before surgery (T0) 6 months after surgery (T1). The volume of defects was measured through the three-dimensional image analysis tool. Results: The average bone regeneration rate of Group 1 was significantly greater than that of Group 2. Within Group 1, the group that used allograft as a carrier (Group 1–1) showed significantly higher bone regeneration rates than the group that used absorbable collagen sponge as a carrier (Group 1–3). Conclusion: The use of rhBMP-2 after oral surgery results in a superior bone regeneration rate compared to not using rhBMP-2, and its efficacy depends on the carriers it is used with. Allograft affects bone regeneration more than absorbable collagen sponge when it is carried with rhBMP-2. Therefore, the appropriate use of rhBMP-2 with suitable bone grafting materials is useful for promoting postoperative bone regeneration in oral surgery

    Regenerative effect of recombinant human bone morphogenetic protein-2/absorbable collagen sponge (rhBMP-2/ACS) after sequestrectomy of medication-related osteonecrosis of the jaw (MRONJ)

    OBJECTIVES: Beyond the original application approved by the U.S. Food and Drug Administration, recombinant human bone morphogenetic protein-2 (rhBMP-2) is used for medication-related osteonecrosis of the jaw (MRONJ) treatment because of its bone remodeling enhancement properties. The purpose of the study was to investigate the bone formation effect of rhBMP-2/absorbable collagen sponge (ACS) in patients with MRONJ. MATERIALS AND METHODS: In this retrospective cohort study, 26 female patients diagnosed with MRONJ and who underwent mandibular sequestrectomy at Ajou University Dental Hospital from 2010 to 2018 were included. The experimental group was composed of 18 patients who received rhBMP-2/ACS after sequestrectomy, while the control group was composed of 8 patients who did not receive rhBMP-2/ACS after sequestrectomy. A total dose of 0.5 mg of rhBMP-2 was used in the experimental group at a concentration of 0.5 mg/mL. Follow-up panoramic X-rays were taken immediately after the surgery and more than 6 months after the surgery. Using those X-rays, a radiographic index of bone defect area was calculated using the modified Ihan Hren method, which measures radiographic density of the normal bone and the defect site. RESULTS: This study suggests that rhBMP-2 contributes to new bone formation. The mean radiographic index immediately after surgery and more than 6 months after the surgery for the experimental group was 68.4% and 79.8%, respectively. The mean radiographic index immediately after surgery and more than 6 months after the surgery for the control group was 73.4% and 76.7%, respectively (Wilcoxon signed rank test, P>0.05). The mean radiographic index increased 11.4% in the experimental group and 3.27% in the control group (Mann-Whitney U-test, P <0.05). CONCLUSION: Based on the results, use of rhBMP-2/ACS on bone defect sites after sequestrectomy could be a successful strategy for treatment of MRONJ patients

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Radiographic changes of mandibular cortical bone in bisphosphonate drug holiday

    OBJECTIVES: There have been few studies to date on the residual effect of bisphosphonate. This study investigated the radiographic changes of mandibular cortical thickness upon bisphosphonate drug holiday. MATERIALS AND METHODS: This retrospective study includes 36 patients diagnosed with MRONJ (medication-related osteonecrosis of the jaw) at Ajou University Dental Hospital in 2010-2021. All patients stopped taking bisphosphonate under consultation with the prescribing physicians. Panoramic radiographs were taken at the start of discontinuation (T(0)), 12 months after (T(1)), and 18 months after (T(2)) discontinuation of bisphosphonate, respectively. Mental index and panoramic mandibular index were calculated using Ledgerton’s method. Paired t-tests were used to analyze differences over time. RESULTS: The difference in indices (mental index and panoramic mandibular index) between T(0) and T(1) was not statistically significant (paired t-test, P>0.05). However, the difference in these indices between T(1) and T(2) was statistically significant (paired t-test, P<0.05). CONCLUSION: The cortical thickness of the mandible decreased in the late stage (after 18 months) as observed by panoramic radiograph

    Jumun chosun(『朱文抄選』) is a textbook for royal lecture(經筵) which was compiled by Song Si-yeol(宋時烈) in 1683. At that time Soron(少論)s political offense to Noron (老論) and Song Si-yeol was intensified. Song Si-yeol thought that it was a serious political crisis and for overcoming this crisis it was positively necessary to strengthen education of king about the Learning of Zhuxi(朱子學). For this purpose he selected important writings of Zhuxi(朱熹) from Jujaseo jeolyo(『朱子書節要』) and Jumun Jakhai (『朱文酌海』) and compiled Jumun chosun. Thought the writings of Jumun chosun were written by Zhuxi, the one who selected and compiled them was Song Si-yeol. It means that the contents of Jumun chosun reflected Song Si-yeols view about the political situation of those days. The core arguments of Jumun chosun were two. First, the rectification of kings mind was the practical virtues of the Learning of Zhuxi. Second, the government operation lead by the party of exemplary person(君子黨) and the reinforcement of the theory of revenge on Qing dynasty(對淸復讎論) must be the fundamental principles of government administration. These principles corresponded with Song Si-yeols political thought and it means that Song Si-yeol wanted to imprint his political thought on kings mind through the learning of Jumun chosun

    Objectives: This study aimed to propose guidelines for the indications of non-extraction, extraction camouflage treatment, and orthognathic surgery by comparing the degree of soft tissue improvement effects. This comparison focused on changes in CKHA, a soft tissue indicator corresponding to the major hard tissue indicator, ANB. Materials and methods: Sixty-six patients, 25 males and 41 females, aged between 18 and 50 years and diagnosed with skeletal Class III malocclusion (ANB &lt; 0°), were enrolled in the study. Participants were categorized into three groups based on the treatment approach: G1, non-extraction (n = 20); G2, extraction (n = 20); and G3, orthognathic surgery (n = 26). To assess variations in treatment outcomes, measurements derived from lateral cephalometric radiographs obtained before and after treatment were analyzed and compared across the different treatment methods. Results: Significant differences were observed in the ANB between the G1 and G2/G3 groups. However, no significant differences were observed in CKHA levels after treatment. Furthermore, in G2, the CKHA approached the normal range at -1.8° (normal range: -2.0°), suggesting that soft tissue responses can normalize despite minimal changes in the skeletal structure. Conclusions: Although surgery resulted in the most significant skeletal changes, both camouflage groups demonstrated distinct advantages in the soft tissue paradigm. In G2, a noticeable posterior movement of the lower lip was observed along with a corresponding posterior shift in the soft tissue B point. Clinical relevance: This study provides guidelines for non-extraction, extraction and surgical treatment selection aimed at achieving soft tissue objectives
