    Per cefalea s’intende un dolore a localizzazione prevalentemente, ma non esclusivamente, neurocranica, la cui estensione topografica non identifica il territorio di distribuzione di singoli tronchi nervosi. Si identificano due gruppi principali di cefalea: le cefalee primarie (o essenziali) in cui il dolore è al tempo stesso sintomo e malattia; le cefalee secondarie, nelle quali il dolore rappresenta il sintomo o uno dei sintomi di altre malattie (loco-regionali, a distanza, sistemiche). Numerosi sono stati i criteri tassonomici utilizzati per definire una classificazione ragionata delle cefalee primarie. Al fine di mettere ordine in materia di nosografia e fornire al tempo stesso i presupposti per l’identificazione dei vari sottogruppi di cefalee, l’IHS (International Headache Society) ha proposto una classificazione basata su precisi criteri diagnostici riferiti alle sole caratteristiche cliniche delle varie forme di cefalea. Benché percepite come patologie di minore importanza, in quanto non riducono l’aspettativa di vita, diversi studi hanno evidenziato che le cefalee rappresentano comune causa di assenza dal lavoro e di calo nella resa produttiva.     L’indagine sull’attività lavorativa rappresenta, per il medico, un aspetto importante, nel corso della raccolta anamnestica. E’ noto, infatti, che il tipo di lavoro e l’ambiente nel quale questo si svolge, possono avere una funzione determinante sullo sviluppo dei sintomi e sulle modalità di risposta ai trattamenti. Diversi fattori legati all’ambiente lavorativo sono in grado di determinare l’insorgenza di alcuna forme di cefalea, oltre che di aumentare la frequenza e/o l’intensità di forme preesistenti. (1-3). La cefalea è certamente uno dei sintomi più comunemente riscontrabili nella pratica medica (4-5); essa rappresenta un disturbo fastidioso ed invalidante per l’individuo che ne è affetto, oltre che un peso rilevante per la società (6-8). La cefalea, in particolare nella forma di emicrania, è più frequente fra i soggetti di età inferiore ai 55 anni (9), interessando quindi nella maggior parte dei casi individui in età lavorativa. Sebbene percepita come una patologia di minore importanza, in quanto non riduce l’aspettativa di vita, diversi studi hanno evidenziato che essa rappresenta una comune causa di assenza dal lavoro e di riduzione nella resa produttiva(10-12). Studiare il possibile nesso esistente tra cefalea e lavoro rappresenta motivo di crescente interesse per la ricerca scientifica. Con il presente lavoro, è nell’intento degli autori, richiamare l’attenzione su questo peculiare aspetto della storia clinica, delineando i punti principali da seguire nella conduzione dell’indagine. Tale approccio risulterebbe utile non solo al medico del lavoro, ma anche al medico di medicina generale, primo referente clinico per il paziente

Mondino Institute of Neurology” Foundation, University of Pavia, Pavia, ItalyAbstract: Almotriptan is an orally administered, highly selective serotonin 5-HT(1B/1D) receptor agonist that is effective in the acute treatment of moderate to severe migraine attacks. Since its introduction on to the market in 2001, several studies involving a large number of migraine patients have confirmed its efficacy and tolerability profile. Almotriptan, was found to be among the best-responding triptans in terms of pain relief and pain-free rate at 2 h. It has been reported that almotriptan has the best sustained pain-free (SPF) rate and the lowest adverse events (AEs) rate of all the triptans. When these clinical characteristics were combined to form the composite endpoint SPF and no AEs (SNAE), almotriptan emerged as the triptan with the best efficacy and tolerability profile. It also showed a good efficacy profile during the early treatment (within 1 h of onset) of migraine attacks characterized by moderate pain intensity. On the basis of these findings, almotriptan may be considered a therapeutic option for the acute treatment of migraine attacks.Keywords: almotriptan, triptans, migraine, treatmen

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
