La cultura dell'error management: contributo al processo di gestione degli errori e ruolo della leadership
La letteratura sulla prevenzione degli errori sottolinea l’importanza di considerare anche la fase successiva al loro verificarsi (error management) come parte importante del processo di riduzione del rischio. Pochissimi studi tuttavia hanno verificato la relazione tra una cultura orientata all’error management (EMC) e il contenimento degli errori e delle loro conseguenze.
Il contributo presenta una ricerca esplorativa svolta a 280 infermieri dipendenti di 4 ospedali del Lazio. Si ipotizza l’EMC contribuisca a ridurre la frequenza degli errori assistenziali nei Reparti; si ipotizza inoltre che uno stile di leadership autentico da parte dei coordinatori sia un fattore capace di promuovere l’EMC e, attraverso essa, la riduzione degli errori.
Le analisi condotte attraverso una serie di regressioni gerarchiche lineari confermano le ipotesi, poiché hanno mostrato a) un effetto significativo negativo dell’EMC sugli errori (sia del tipo slips/lapses sia del tipo mistakes), e b) un effetto significativo positivo della leadership autentica sull’EMC, con mediazione totale dell’EMC nella relazione tra leadership ed errori.
Complessivamente, lo studio mostrano che l’EMC è un orientamento culturale importante che favorisce la riduzione degli errori, contribuendo al processo di risk management. Questo orientamento, inoltre, risulta modellato dall’adozione, da parte del coordinatore, di uno stile di leadership autentica che, attraverso la sua influenza sull’EMC, contribuisce anch’essa ad influenzare indirettamente gli errori. Complessivamente forniscono un supporto empirico importante e indicazioni per la gestione del rischio nel contesto sanitario infermieristico
What do we know about emotional labour in nursing? A narrative review
Nurses have to manage their emotions and the expression of emotion to perform best care, and their behaviours pass through emotional labour (EL). However, EL seems to be an under-appreciated aspect of caring work and there is no synthetic portrait of literature about EL in the nursing profession. This review was conducted to synthesise and to critically analyse the literature in the nursing field related to EL. Twenty-seven papers were included and analysed with a narrative approach, where two main themes were found: EL strategies and EL antecedents and consequences. Hence, EL is a multidimensional, complex concept and it represents a nursing competence to provide the best care. Moreover, nurses have a high awareness of EL as a professional competence, which is a fundamental element to balance engagement with an appropriate degree of detachment to accomplish tasks for best practice, and to provide high-quality patient care
The Nursing Organisational Context and the Surgical Site Infections: A Structural Equational Model
Aim: To assess the relationships existing among variables of the organisational context-staffing, stress-related demands, workloads-and the occurrence of Surgical Site Infections (SSIs). Design: An observational study was conducted. Methods: A convenience sample of nurses directly involved in patient care was recruited. Staffing levels, workload and stress were measured using valid and reliable instruments. The prevalence of SSIs was recorded over 30 days using the European Centre for Disease Prevention and Control (ECDC) classification. Data were analysed using structural equation modelling. Results: This study enrolled 133 nurses. Higher SSI rates were associated with inadequate nurse staffing during daily shifts, increased workloads, evolving patient care needs and elevated stress levels among nurses due to work demands. Discussion: While the prevention of SSIs requires multidisciplinary collaboration, findings from this study suggest that the nursing organisational context-especially staffing and workload-had a direct impact on nurses' stress levels and an indirect influence on the prevalence of SSIs. Conclusion: Nurse managers and head nurses should improve nurses' working conditions and promote specific interventions for their organisational well-being to influence SSI prevalence. Patient or public contribution: Despite patients and the public not being involved in the study's design and interpretation, the findings suggest new insights for healthcare organisations. Managing staffing levels, workloads and stress levels among nurses could improve patient safety and reduce the rates of SSIs. These outcomes advocate for strategic planning and organisational changes to boost the quality of care in surgical environments
Towards an integrated model of nursing competence: an overview of the literature reviews and concept analysis
Introduzione: La competenza è un elemento fondamentale quando si misura la capacità di infer- mieri o studenti di fornire assistenza infermieristica, ma non vi è consenso su ciò che realmente sia la competenza. Questo studio si propone di indagare i significati e i modelli di competenza infermieristica. Metodo: L’overview delle revisioni della letteratura e delle concept analysis è stata effettuata attraverso una ricerca su Pubmed, Cinahl e PsychINFO dal gennaio 2005 a settembre 2014, che includeva parole chiave come: Modello di competenza; Competenza professionale; Competenza infermieristica; Competenza. Risultati: Sono stati inclusi un totale di 14 articoli, provenienti dalla letteratura infermieristica sia in ambito formativo che clinico. È stato possibile individuare alcuni temi comuni negli arti- coli inclusi: la descrizione dei determinanti di competenza; la confusione intorno al concetto di competenza; la carenza nella valutazione delle competenze; la carenza nell’operazionalizzare il concetto di competenza. Conclusioni: I risultati forniscono una panoramica che, arricchita dalla letteratura proveniente dagli studi organizzativi, costituiscono la base concettuale di un modello integrato di competenza infermieristica. Ulteriore ricerca empirica è necessaria per verificare le assunzioni teoriche.INTRODUCTION: Competence is considered a fundamental element when measuring a nurse's or student's ability to provide nursing care, but there is no consensus on what competence really is. This paper aims to review the existing meanings and models of nursing competence.
METHOD: The overview of literature reviews and concept analysis was performed through a search on Pubmed, Cinahl and PsychINFO from January 2005 to September 2014. It included key words, such as: Competence Model; Professional Competence; Nursing Competence; Competency Model; Professional Competency; Nursing Competency.
RESULT: A total of 14 papers were found, coming from educational or clinical nursing field. It was possible to identify some common themes: description of competence determinants; confu- sion around the competence concept; lack in competence evaluation; lack when competence have to be operationalized.
CONCLUSION: The overview results, enriched by the literature coming out from the organiza- tional studies, build the conceptual basis of an integrated model of nursing competence. More empirical research is needed to test the theoretical assumptions
Healthcare-associated infections and nursing leadership: A systematic review
Healthcare-associated infections are strictly related to healthcare practices. A head nurse stimulates and motivates nurses, boosts nurses’ job performance and satisfaction, and can influence adverse event development.
To explore the relationship between healthcare-associated infections and head nurse leadership style.
A systematic review was conducted. The search was conducted from 1973 until March 2022 on PubMed, Cochrane Library, Scopus, CINAHL, Web of Science, Embase, and APA PsycInfo databases. The review followed the Joanna Briggs Institute Manual for Evidence Synthesis and the Preferred Reporting Items for Systematic Reviews and Meta-Analyses. A thematic synthesis and critical appraisal of the included studies have been conducted.
Eight articles were included in this review. Head nurses’ leadership, supported by the organization, can positively influence the job performance and job satisfaction of nurses by reducing infection rates associated with vascular access and urinary catheters.
Authentic and transformational nurse leadership styles can foster targeted interventions and improvements tailored to preventing and controlling healthcare-associated infections. Even if there is limited evidence, the results support that the occurrence of infections can be reduced by leadership strategies implemented by head nurses
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.
