A case of gas gangrene in a vesico-vaginal fistula
Get PDFA case of gas gangrene is presented. The patient was a 78-year-old woman who was admitted with the diagnosis of vesico-vaginal fistula. Cystography revealed a vesico-intestinal fistula and leakage of contrast medium into the prevesical space, in addition to the vesico-vaginal fistula. Right ureterostomy was performed. Two weeks postoperatively, she complained of severe pain and swelling of her right thigh. The swelling grew rapidly and general condition became worse. A diagnosis of gas gangrene was made 8 days after her first complaint by demonstrating subcutaneous and intramuscular gas formation in X-ray. She died the next day. By the bacteriological examination, this case was non-clostridial gas gangrene

counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Short- and long-term outcome after urethroplasty in the patients with hypospadias treated by Hodgson III method
Get PDFHodgson III法一期的尿道形成術29症例を臨床的に検討した.初回成功率は62.1%で, おもな短期合併症は, 瘻孔10例, 狭窄3例で, 瘻孔のうち5例は長期においても残存した.長期合併症において, 新たに勃起障害1例が認められたが, 術式に起因する合併症ではないBetween 1977 and 1997, 29 patients with hypospadias were surgically treated by Hodgson type III urethroplasty at Tsukuba University Hospital. The duration of surgery ranged from 150 to 535 minutes with an average of 269.3 minutes and the urethral catheter was removed between 4 and 14 days (average 8.7 day) after surgery. The postoperative hospital stay ranged from 10 to 29 days with an average of 18.4 days. The overall success rate of initial surgery was observed in 18 of the 29 patients (62.1%). As early postoperative complications, urethral fistula and stricture were seen in 10 (34%) and 3 (10%) patients, respectively; of these four fistulas persisted. As late complications revealed by questionnaire, erectile disorder was observed in one case. To obtain a higher success rate on initial treatment, skillful surgical technique and selection of appropriate cases are required

Get PDF“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
A case of malignant paraganglioma within the renal sinus
Get PDFA case of malignant paraganglioma within the renal sinus is presented. A 51-year-old woman underwent left transabdominal nephrectomy for renal tumor. Pathologically, the tumor was malignant nonchromaffin paraganglioma. This tumor seemed to originate from parasympathetic paraganglions around the left renal artery. Paraganglioma is rare in the literature. Especially, this may be the first report of such a lesion in the renal sinus
Appropriate Similarity Measures for Author Cocitation Analysis
Get PDFWe provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. No full text30歳男.第2次性徴の遅延, ED, 嗅覚低下を自覚し, 性腺機能低下症との類似を知り, 外陰部の増大および勃起機能の改善を希望した.頭部MRIにて両側嗅裂, 下垂体の形態に異常は認められず.内分泌学的検査およびホルモン負荷試験よりKallmann症候群と診断した.hCG5000単位およびhMG150単位を週2回の治療を開始し, 直後より勃起力の改善を自覚し, 12ヵ月後の時点で血中テストステロン, 遊離テストステロンは正常化し, LH, FSHは若干その増加を示し, PRLは正常範囲を推移した.陰茎長, 精巣容積も増加し, 陰毛も発達し精液検査でも改善されIIEF5は23点で, AMS総スコアは39点から20点となったWe report a case of Kallmann syndrome in which the aging male's symptoms rating scale (AMS) was useful for assessment of subjective symptoms. A 30-year-old male was admitted to Tsukuba University Hospital with a complaint of delayed puberty in May 2003. He presented with hypogonadism, gynecomastia and anosmia. The plasma levels of luteinizing hormone (LH), follicle stimulating hormone (FSH) and testosterone were very low. However, the LH-releasing hormone test and human chorionic gonadotropin (hCG) loading test identified normal function of pituitary and Leydig cell. After 12 months of treatment with hCG and human menopausal gonadotropin (hMG), the amount of pubic hair and the volume of testes had increased as well as the level of serum testosterone. The total score of AMS after treatment has been improved from 39 to 20 as compared with that before treatment

use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
