    Lifestyle factors, hormonal contraception, and premenstrual symptoms: the United Kingdom Southampton Women's Survey

    Objective: to estimate the prevalence of premenstrual symptoms in women from the general population in Southampton, U.K., and examine their association with lifestyle factors and contraceptive use.Methods: this was a cross-sectional survey in the city of Southampton, U.K., of 974 women aged 20–34 years (53% of the 1841 women invited to participate). The survey consisted of interviews, questionnaires, and completion of a prospective 6-week menstrual symptom diary recording on a daily basis the presence and severity of 11 common premenstrual symptoms. Premenstrual symptoms were identified from the diaries by two clinicians who reviewed them independently using a predefined algorithm to assess the onset and decline of symptoms in relation to the start of menstruation.Results: of the women surveyed, 24% were considered to have premenstrual symptoms (95% confidence interval [CI] 21-27). Women were less likely to have symptoms if they had higher levels of educational attainment and suffered less from stress. No associations were found between premenstrual symptoms and diet, alcohol, or strenuous exercise nor after adjustment for other factors, with age, smoking, or body mass index (BMI). Use of any form of hormonal contraceptives was associated with a lower prevalence of premenstrual symptoms (prevalence ratio 0.66, 95% CI 0.52-0.84).Conclusions: premenstrual symptoms were common in this cohort. Use of hormonal contraceptive methods was associated with a lower prevalence of these symptoms

    The Imprisoned Traveler is a fascinating portrait of a unique book, its context, and its elusive author. Joseph Forsyth, traveling through an Italy plundered by Napoleon, was unjustly imprisoned in 1803 by the French as an enemy alien. Out of his arduous eleven-year “detention” came his only book, Remarks on Antiquities, Arts, and Letters during an Excursion in Italy (1813). Written as an (unsuccessful) appeal for release, praised by Forsyth’s contemporaries for its originality and fine taste, it is now recognized as a classic of Romantic period travel writing. Keith Crook, in this authoritative study, evokes the peculiar miseries that Forsyth endured in French prisons, reveals the significance of Forsyth’s encounters with scientists, poets, scholars, and ordinary Italians, and analyzes his judgments on Italian artworks.     Pain assessment in the elderly: a psychometric evaluation of self-report and behavioral methods.

    Limited research has been done that examines appropriate and reliable methods to assess for pain in the elderly population. For the cognitively impaired elderly, pain assessment is further complicated by their limited communication abilities. Reliable and clinically feasible methods are desperately needed to assess pain so that it can be managed appropriately. The purpose of this study was to examine the psychometric properties (Le., testretest and interrater reliability, criterion concurrent validity) of three verbal pain assessment tools (i.e., Faces scale, numerical rating scale, present pain intensity scale) and a behavioural pain assessment scale within the elderly population. This measurement study used a repeated measures design to examine the reliability and validity of these pain assessment tools across four groups of elderly participants: 1) cognitively intact, 2) mildly cognitively impaired, 3) moderately cognitively impaired, and 4) extremely cognitively impaired, using a nonrandom stratified sample of 130 elderly residents who live in long term care. The findings support the test-retest and interrater reliability of the behavioural pain assessment tool across all four groups of the elderly whereas the same measures of reliability for the verbal pain assessment tools decrease with increasing cognitive impairment. However, the majority of elderly with mild to moderate cognitive impairment were able to complete at least one of the verbal pain assessment tools. The Present Pain Intensity scale had the strongest criterion concurrent validity for the elderly with moderate cognitive impairment (r=0.64, p=O.OOl). Wilson's three year career in the Secret Intelligence Service ended when the country's spy chiefs decided his creativity got the better of his grasp of reality. His 'secret lives' extended to his private life. Four wives and four families; not necessarily one after the other. In this second Edition Tim Crook unravels more of the mysteries of this extraordinary story. The first edition of his biography is the foundation of the BBC drama series 'Mrs Wilson' produced by and starring his award-winning granddaughter Ruth Wilson. It is eight years since Tim first told the story of the life and times of the father of his friend Mike Shannon. In the spring of 1941 Mike was only 7 years old when he said goodbye to his father, Alexander Wilson, dressed as a lieutenant colonel in the Indian Army. As the steam train pulled away from the Yorkshire railway station platform that would be the last time he ever saw him. More than six decades later Tim Crook would help unlock the secrets of his father's life. The biography was published in October 2010 shortly before Mike passed away. Not only had he learned that his father had worked for MI6 during the Second World War, but that he had been one of the leading spy, crime and romance authors of the 1920s and 30s. At first he seemed to be a man with no beginning and no end. There was no record of his death in action in the North African desert, and there was no record of his birth in the identity his father had put on his son's birth certificate. Mike would have to come to terms with the fact that his father had faked his own death, had lived double, triple and quadruple lives. He would be revealed as a multiple bigamist, but at the same time also a celebrated and successful author. Details of crimes and imprisonment would be mixed with the discovery of relatives and a new family he had no idea existed. The second edition includes more revelations about Wilson's work in MI6, where his talent for invention is said to have done more harm than good, his role as a university chief in British India where he enjoyed great success despite getting the job with a fake CV, and more spy novels under a pseudonym that indicates that he must have loved the son he abandoned. Wilson created a chief of a fictional British Secret Service, Sir Leonard Wallace, who appears substantially based on the first real 'C' of MI6, Captain Mansfield Smith-Cumming. The story involves the Secret Intelligence Service, MI6, the Security Service, MI5, and two World Wars.
