Management of Femoral Periprosthetic Fractures: An Institutional Experience at a District General Hospital.

Successful outcomes in arthroplasty surgery have significantly contributed to more hip replacement procedures being performed annually. This has also increased the incidence of femoral periprosthetic fractures, leading to more revision hip replacement procedures being performed. Methods This is a retrospective cohort study carried out at a district general hospital in the United Kingdom. Theatre records were reviewed from 2018 to 2022 to identify patients with a femoral periprosthetic fracture. Following the data collection, the patients were split into two groups. The first group analysed the patient outcomes through the type of surgical intervention they had, and the second group analysed the patients according to the timing of surgical intervention. Results There were 88 patients included in the study, out of which 49 had revision surgery and 39 had fixation for the femoral periprosthetic fracture. No statistically significant difference was observed in 30-day mortality and one-year mortality for the patients having revision or fixation surgery. Similarly, the results were not found to be significant in 30-day mortality and one-year mortality for the patients having surgery within 36 hours or after 36 hours of diagnosis. Conclusion Overall, the findings of this study are in keeping with the literature. Input from the specialist arthroplasty team is often required for the management of femoral periprosthetic fractures. There is no significant impact on mortality with a delay in surgical intervention for femoral periprosthetic fractures, unlike hip fractures. Medical optimisation and careful planning lead to better patient outcomes for this group of patients
Management of Ankle Fractures: Does the Timing of Surgical Intervention Have an Impact on Patient Outcomes?

It has been observed that these injuries can be present in both young and elderly patients. Surgical intervention with open reduction and internal fixation is often performed for unstable ankle fractures. Methods This study was performed at a district general hospital in the United Kingdom and is a retrospective cohort study. The electronic database was reviewed from January 2021 to December 2024 to identify and review the notes for the patients to be included in the study. After identifying the patients, they were divided into two groups. The patients who had surgical intervention in less than 10 days were allocated to the early surgery group, and the patients who had surgery at day 10 or after day 10 of the injury were allocated to the delayed surgery group. Results Overall, 96 patients were included in this study, where 29 (30.21%) had early surgical intervention and 67 (69.79%) had delayed surgical intervention. The results were not statistically significant for patient outcomes between the two groups. A correlation was observed between the age of the patients and the length of hospital stay. Conclusion At present, there is no definitive consensus with regards to the optimal timing for surgical intervention of ankle fractures. The results of this study highlight that there was no significant difference for post-operative complications between the two groups, which is consistent with the literature. A study with a larger sample size is required to analyse the long-term patient outcomes

Abstract
There has been a strong consensus among previous researchers that first language interferes with second language acquisition. A limited number of studies have been conducted to examine this interference in writing Chinese. Chinese writing exercises for primary school students of the Yi ethnic group in rural areas tend to encourage students to incorporate their native language habits into Chinese writing practices. This enables students to apply modern Chinese word order to the Yi language paradigm. Consequently, they are prone to grammatical errors, word order errors, and collocation errors in their compositions. This study focuses on elementary school students' writing abilities in order to alleviate their fears associated with Chinese writing. It also assists them in grasping the second language writing paradigm, and enjoying writing. In addition, by analyzing the composition errors of the students in the corpus, we propose some practical recommendations from three perspectives: educators, students, and institutions. These recommendations will enhance language and writing education within ethnic communities through theoretical and practical strategies.

Abstract
This paper studies the effect of Mandarin proficiency on poverty reduction and its mechanism. The China General Social Survey (CGSS) is taken as the data source. From the perspective of social integration, the poverty reduction effect of Putonghua and its mechanism are empirically studied from the perspective of social interaction, social fairness, and social trust. The findings showed that improving the ability to listen and speak Mandarin positively affects social interaction, social trust, and social fairness. It showed that Putonghua proficiency positively correlates with the suppression of economic poverty. After the occurrence of poverty, whether a sample of absolute poverty or relative poverty, the ability to listen and speak in Mandarin has a specific inhibitory effect on social interaction, social trust, and social fairness. After the occurrence of poverty, the frequency of social interaction, social trust, and social public since have been reduced to a certain extent. Specifically, the ability to express Mandarin has shown remarkable results in improving social interaction, and the ability to listen among ordinary people has shown remarkable results in enhancing social fairness. The results of this paper provide empirical evidence of poverty alleviation in China via Improving Mandarin Proficiency. This research is also of great significance for optimizing poverty alleviation paths through language in the post-poverty alleviation era.

The The Legitimized Use of Figurative Language: A Pragmatic Study of Multiculturalism and Political Intrigues of Shamsie’s “Burnt Shadows”: A Pragmatic Study of Multiculturalism and Political Intrigues
The study explores the legitimized use of the figure of speech in Burnt Shadows. The author uses language as a source of connection between the characters who are different in their eras, countries, and cultural backgrounds. The objective of the study is to explore the effects of language on multiculturalism and the impact of political unrest on the lives of common people during the second world war. She has used lingual devices to enhance the intensity of circumstances as well as for the accurate pasteurization of the sentimental and physical conditions of Hiroko and Sajjad. Specific figurative techniques like metaphor, simile, personification, irony, epigram, and symbolism have been explored in the text of Burnt Shadows. These techniques expose the hidden truths and aggressive nature of political conflict in the lives of the people of Japan, the Subcontinent, and European countries. Pragmatic studies have been applied to the text as a mirror to see the function and nature of wordplay in the novel. The results expose that the author has artistically applied specific devices in the formation of text in order to highlight the role of language in the development of cultural intimacy and maintaining social relationships. The figures of speech are also helpful to map the mind of readers according to the plot construction of the novel
Feto-maternal Outcome of Reverse Breech Extraction versus Dis-impaction of Fetal Head in Caesarean Section for Obstructed Labour
Objectives:
Obstructed labour is an obstetrical emergency with adverse feto-maternal consequences and caesarean delivery in such cases requires skillful handling of impacted fetal head. Objective of our study was to guide clinician about caesarean technique that facilitates the delivery with least complications for mother and baby.
Methods:
It was a randomized clinical trial with non-probability consecutive sampling conducted at –removed for blind review---from 1st july 2018 – 30th june 2020. Patients who underwent emergency cesarean section were randomized to undergo either push technique for delivery of impacted fetal head (Group A) or reverse breech extraction method (Group B) via lottery method. The data of 60 patients who fulfilled the inclusion criteria was analyzed using SPSS version 19. Maternal outcome measured were extension of uterine incision, blood transfusion, postpartum pyrexia, wound infection, postpartum hemorrhage and length of hospital stay. Fetal outcome measured were 5 minutes Apgar score, birth weight and NICU admission.
Results:
The results of our study showed statistically significant difference between extension of uterine incision(p-value=0.015), blood transfusion during surgery (p-value=0.021) and postpartum hemorrhage (p-value=0.020) in two groups with pull technique associated with less traumatic extension of uterine incision, less intraoperative transfusion and less PPH than push technique of fetal delivery. Length of hospital stay was also significantly less in reverse breech extraction group(p-value=0.001).More patients had postpartum pyrexia, wound infection, low 5-min Apgar score and NICU admissions in cephalic delivery group but results were not statistically significant.
Conclusion:
The results of our study recommend reverse breech extraction technique to be a safe alternative to conventional vaginal pushing of fetal head especially regarding maternal outcomes during caesarean section of patients with obstructed labour for fetal delivery.

Flexible control of pulse intensity and repetition rate for multiphoton photostimulation
In deep tissue imaging, pulsed near-infrared lasers commonly provide high peak powers needed for nonlinear absorption, but average power and linear absorption can be limiting factors for tissue damage through heat. We implemented intra-cavity dumping within a mode-locked Ti:Sapphire laser used for two-photon computer generated holography stimulation. This system enables photostimulation in which pulse energy, average power, and repetition rate can each be varied and harnessed as degrees of freedom. We demonstrate how this system provides a new dimension of temporal control in photostimulation experiments to alter the ratio of nonlinear to linear light-tissue interactions, namely by tuning the laser repetition rate between single-shot and ≈ 3 MHz. Repetition rates below 1 MHz, yielding pulse energies over 60 nJ, facilitated holographic projections with more regions of interest than the highest repetition rate of 3 MHz. Even lower repetition rates ( ≈ 10 kHz) diminished thermal load on the sample, as characterized by quantification of heat shock protein expression in zebrafish tissue.ImPhys/Microscopy Instrumentation & TechniquesChemE/Chemical Engineerin
Comparison of efficacy and safety of zinc gluconate versus placebo for treatment of Primary Dysmenorrhea
Background & Objectives:
Dysmenorrhea is a common gynecological complaint in women of reproductive age. Primary dysmenorrhea is defined as painful menstruation in the absence of any pelvic pathology and occurs in response to increased prostaglandin release from the ischemic endometrium. Conventional treatments include NSAIDs and oral contraceptive pills. Zinc is an essential micronutrient with anti-inflammatory properties. We conducted this study to evaluate the efficacy and safety of zinc supplementation in treatment of primary dysmenorrhea in our target population as it presents as very common gynaecological issue in young adolescents females.
Methods:
100 participants with primary dysmenorrhea were enrolled in study from outpatient department via non-probability consecutive sampling. They were randomized into two groups. The intervention group (Group A) received zinc gluconate 50mg once daily and the control group (Group B) received placebo drug once daily, for 5 days before and 2 days after onset of menstruation. After three and six months of treatment severity of primary dysmenorrhea was assessed and compared in both groups using visual analogue scale for pain, along with side effect profile.
Results: The treatment with Zinc gluconate reduced the mean pain score in females with primary dysmenorrhea after 3months with a further reduction after 6 months of treatment (P<0.001) as compared to placebo. Very few (4% after 3 months & 6% after 6 months) participants experienced the side effects of Zinc supplementation.
Conclusion: zinc gluconate 50mg daily for 5 days before and 2 days after onset of menstruation can effectively treat the primary dysmenorrhea. The drug has very few side effects at this dose and duration of use.

Comparison of Materials used in Facilitating External Cephalic Version
Background
External cephalic version (ECV) is a procedure to manipulate a fetus with breech presentation to cephalic presentation through maternal abdomen under ultrasound guidance. It is done to reduce number of assisted breech deliveries and caesarean sections for breech presentation . Talcum powder or aqueous gel is frequently used to facilitate ECV. We compared powder with gel in attaining successful version and level of practitioner satisfaction with powder versus gel use.
Methodology
This retrospective descriptive study was conducted by –removed for blind review---10th Oct 2015- 10th Oct 2019 . Total of 56 low risk gravid women underwent ECV after selection on the basis of inclusion and exclusion criteria. By using convenient consecutive sampling, powder or gel during ECV was allocated accordingly . ECV was performed by the single experienced practitioner in labour room with cardiotocograph and emergency caesarean facilities . Powder and gel use was compared in attaining successful version . ECV was declared successful on confirming head of fetus in lower uterine segment on ultrasound. Unsuccessful ECV’s were reattempted with crossover use to the other aid. Practitioner satisfaction was defined as, ease experienced by the practitioner in performing ECV. It was graded as high, medium and low on the base of ease or difficulty experienced by the practitioner while using powder or gel during ECV. The data was recorded & analysed using SPSS software. Descriptive statistics with Chi-square test is applied to find out the association between successful version & practitioner satisfaction level with powder versus gel use.
Results
In powder use group , 9 ECV’s were successful and 19 ECV’s were unsuccessful. In gel use group , 12 ECV’s were successful and 16 were unsuccessful .There were more successful ECV’s in gel group but the association was not significant (P value .408). Practitioner satisfaction was significantly high with gel than the powder use(Pvalue .<.001).
Conclusion
Powder or gel is not superior over the other as an aid for achieving successful ECV though gel use is more satisfying for the provider than the powder use in performing ECV. More studies are required regarding aids used in performing ECV for recommendations in clinical practice

Abstract A host of nutrients are needed by cattle to support functions associated with life, and to grow, reproduce, and nourish their offspring (i.e., produce milk). A vast amount of resources have been expended to quantify the amounts of specific nutrients needed to perform these function so that economically efficient diets can be formulated. Feeding diets that provide adequate, but not excessive, amounts of nutrients helps improve profitability of dairy operations while reducing the environmental impact of dairy farms. The trace minerals zinc, copper, manganese, and selenium are components of a wide variety of enzymes and proteins that support metabolism, growth, production, and reproduction. Trace mineral supplements are added to dairy cattle rations to prevent mineral deficiencies, and supplementation has traditionally been provided in the form of inorganic salts
