    Prevention and management of adverse events related to regorafenib

    Regorafenib is an oral multikinase inhibitor that has shown antitumor activity in a range of solid tumors. Based on data from phase III clinical trials, regorafenib is indicated for the treatment of adult patients with metastatic colorectal cancer who have previously been treated with, or are not considered candidates for, other available therapies, and in patients with advanced gastrointestinal stromal tumors that cannot be surgically removed and no longer respond to other appropriate treatments. A panel of oncology nurses, research coordinators, and other medical oncology experts, experienced in the care of patients treated with regorafenib, met to discuss the best practice for the management of regorafenib-associated adverse events (AEs). The panel agreed that, in clinical trials and daily practice with regorafenib, AEs are common but mostly manageable. The most common and/or important AEs associated with regorafenib were considered to be hand-foot skin reaction, rash or desquamation, stomatitis, diarrhea, hypertension, liver abnormalities, and fatigue. This manuscript describes the experience and recommendations of the panel for managing these AEs in everyday clinical practice. Appropriate education, monitoring, and management are considered essential for reducing the incidence, duration, and severity of regorafenib-associated AEs. © 2013 The Author(s)

    Unraveling the SES-Health Connection

    While a debate rages on about competing reasons why SES may affect health, there is little recognition that the so-called reverse causation from health to economic status may be pretty fundamental as well. Even if the direction of causation is that SES mainly affects health, what dimensions of SES actually matter—the financial aspects such as income or wealth or non-financial dimensions like education? Finally, is there a life course component to the health gradient so that we may be mislead in trying to answer these questions by only looking at people of a certain age—say those past 50. This paper, which is divided into four sections, provides my answers to these questions. The first section examines the issue of reverse causation or whether a new health event has a significant impact on four dimensions of SES—out-of-pocket medical expenses, labor supply, household income, and household wealth. The next section switches the perspective by asking whether the so-called direct causation from SES to health really matters all that much. If the answer is yes and it will be, a sub-theme in this section concerns which dimensions of SES—income, wealth, or education—matter for individual health. Since the answer to that question turns out to be education, Section 3 deals with the very much more difficult issue of why education matters so much. The evidence in these first three sections relies on data for people above age 50. In the final section of the paper, I test the robustness of my answers to these basic questions of the meaning of the SES-health gradient using data that span the entire life-course.

    Le droit d'auteur – dont l'identité reste controversée – n'échappe pas à la discussion quant à la nature et au régime des contrats qui le mettent en œuvre. D'une terminologie légale ancrée dans une certaine tradition, mais hasardeuse, on infère l'originalité de l'ensemble des contrats de la matière. Or, la spécificité n'est sur ce point qu'apparente. De l'édition littéraire aux contrats de l'audiovisuel et aux œuvres diffusées sur les réseaux, l'analyse démontre que si l'auteur peut "céder" son œuvre – ce que dit la loi – il peut aussi la louer, c'est-à-dire en concéder la licence – ce qu'elle ne dit pas. Ce constat s'appuie sur une méthode de lecture renouvelée des contrats du droit d'auteur. L'attention portée par la loi impérative à certains contrats (édition, production audiovisuelle, etc.), a pu perturber l'étude de la licence, l'acte par lequel l'auteur se borne à autoriser l'exploitation de son œuvre pour un temps. Or, si la licence apparaît dans un premier temps en contrat spécial du droit d'auteur, elle sera également amenée à devenir la composante élémentaire d'un contrat complexe organisant une exploitation. Il importe donc de distinguer ces deux objets pour mieux en apprécier ensuite les interactions. Le droit d'auteur, droit "spécial", fait ainsi la preuve de son aptitude à accueillir - autant que de raison - les mécanismes du droit des contrats, droit "commun". Cette étude a pour ambition une meilleure compréhension de cette matière complexe des contrats du droit d'auteur, sans omettre le principal objectif de notre loi : la protection de l'auteur.In the field of the author's right (a concept with a controversial identity), the nature and regime of many contracts is a subject for debate. The purpose of this study is to analyze the management of Information Technology that is currently running at PT. POS (Persero) Indonesia lhokseumawe branch, to find out the Capability Level. The Post Office is an organization engaged in the delivery of goods. The organization has used information technology to support business activities. Money transfer is an application that is used to record every shipment of goods made by consumers starting from the data of the sender of the goods, the data of the goods, and the data of the recipient of the goods. However, it is undeniable that every risk that occurs can interfere with the performance of the money transfer application. Therefore, it is necessary to analyze the risk of the information technology used. In this study, researchers tried to implement the ISO 22301 risk management framework to see every possible risk contained in the money transfer application.     Enacting social accounting within a community enterprise : actualising hermeneutic conversation

    The research was carried out using a participatory action research approach to develop social accounts with Jesmond Swimming Pool (JSP). The original motivation to carry out this project was to see what social accounts would look like and whether it was possible to develop them in this organisation. The experience of doing social accounts is further examined using Gray, et al., (1997) to explore whether these were either ‘ideal’ or ‘good’ social accounts for the organisation. A communitarian philosophical framework is used in order to examine the conceptualisation of ‘good’ social accounts. The first part of the thesis explores the social and environmental accounting (SEA) and accountability literature, with the second part exploring the experience of reaching initial agreement to do social accounts (SAs). The agreement to do the accounts was then followed by two years of social accounts developed with JSP. This empirical data provides a detailed account from the perspectives of insiders and other sources as to the experience of doing social accounts. This experience is then opened up to interrogation from a wider view point. The third part of the thesis examines the experience of JSP using third sector communitarian philosophy and a voluntary accountability framework in order to demonstrate that JSP could provide an example of a ‘good’ or ‘ideal’ social account. This raises the issue of whether or not ‘good’ or ‘ideal’ social accounts for third sector organisations are only possible within a communitarian paradigm. If it is possible to establish what ‘good’ social accounts entail then it may be appropriate to extend this approach to other contexts, for example, the public sector or the corporate world
