A clinical evaluation of treatments that target cell cycle machinery in breast cancer
Introduction: The dysregulation of cell cycle control can lead to cancer development. In breast cancer, cyclin D, CDK 4,6 and the retinoblastoma protein play a central role in the control of cell proliferation, in crosstalk with the estrogen receptor and Her2 pathways. Although the mechanisms by which the CDK4/6 complex is involved in the control of cell growth in triple negative breast cancer (TNBC) are still unclear, some TNBCs might be sensitive to CDK4/6 inhibitors. Areas covered: The authors provide an overview of the treatments that target cell cycle machinery in breast cancer and provide their perspectives for the future. Expert opinion: CDK 4/6 inhibitors are active drugs in HR+ MBC, but some unresolved issues remain. We need to identify biomarkers of response. Moreover, we need to determine the optimal timing for the incorporation of CDK 4/6 inhibitors in the current treatment algorithm. In the Her2 positive subtype, the triple combination of anti Her2 therapies with CDK4/6 inhibitors and endocrine therapy seems to be a promising chemotherapy free approach. Efforts must still be made for the treatment of the TNBC subtype, even though new CDK 4/6 combinations are emerging as promising approaches to selected patients
Eye tracking and speech driven human-avatar emotion-based communication
Feelings, emotions, and empathy play an important role in many daily activities including verbal and non-verbal communications. Their automatic recognition and interpretation is important in a variety of applications requiring communication skills that are difficult to reproduce in computer-simulated environments, including those involving human-avatar interactions. Our recent work has begun investigating the development of intelligent avatars capable of detecting user (human) emotions to allow for realistic human-avatar interactions within medical-based virtual simulations and serious games. In this paper, we present a system that couples eye tracking and dialogue interpretation to allow for intelligent and realistic human-avatar communication. Although formal testing is required, preliminary results are promising, showing the potential of the system

Anemia has a high prevalence and incidence in patients with cancer and is associated with a range of symptoms, including fatigue, which affect the vast majority patients receiving chemotherapy exerting a considerable impact on patients' quality of life. The development of the erythropoiesis-stimulating agent epoetin represented a major step forward in the treatment of chemotherapy-related anemia, providing an effective and safe alternative to red blood cell transfusions. The subsequently introduced epoetin analogue, darbepoetin alpha, with a prolonged serum half-life, allowed for extended dosing intervals and less frequent administration. Recently published large prospective trials provided the oncology community with new important information on the use of currently available erythropoietic agents to improve anemia in patients suffering from cancer. However, it is also clear from recent reports that scrutiny on the safe use of these drugs is still required
Endocrine therapy in post-menopausal women with metastatic breast cancer: From literature and guidelines to clinical practice
Current international guidelines recommend endocrine therapy as the initial treatment of choice in hormone receptor positive advanced breast cancer. Endocrine therapy has been a mainstay of hormone responsive breast cancer treatment for more than a century. To date it is based on different approaches,such as blocking the estrogen receptor through selective receptor estrogen modulators, depleting extragonadal peripheral estrogen synthesis by aromatase inhibitors or inducing estrogen receptor degradation using selective down-regulators. Despite estrogen and/or progesterone receptor positive status, up to a quarter of patients could be either primarily resistant to hormone therapies or will develop hormone resistance during the course of their disease. Different mechanisms, either intrinsic or acquired, could be implicated in endocrine resistance. In the present work available endocrine therapies and their appropriate sequences have been reviewed, and the most promising strategies to overcome endocrine resistance have been highlighted
Efficacy and safety of eribulin in taxane-refractory patients in the ‘real world’
Aim: Recent clinical, randomized and observational studies showed that eribulin, an analogous of Halichondrin B, was beneficial and well-tolerated in heavily pretreated metastatic breast cancer patients. Here, we aim to evaluate the effectiveness and safety of eribulin in taxane-refractory metastatic breast cancer patients. Patients & methods: In this subanalysis of the ESEMPIO study database, we selected 91 subjects with well-defined taxane refractoriness and complete data available. Results: 41 patients (45.2%) showed clinical benefit; one complete response (2.2%) and 16 partial responses (17.6%) were observed. Median progression-free survival and median overall survival were 3.1 and 11.6 months, respectively. The most experienced adverse event was asthenia/fatigue (58%), followed by neutropenia (30%). The treatment-related toxicity led to eribulin-dose reduction in 19 patients and suspension in nine. Conclusion: This study shows that eribulin is effective and well tolerated also in taxane-refractory patients in clinical practice. </jats:p
