Solución salina hipertónica para tratamiento del trauma craneoencefálico en niños: revisión sistemática de la literatura

Evaluar si el uso de la solución salina hipertónica en niños con trauma craneoencefálico moderado a severo disminuye la lesión secundaria.\ud

Materiales y métodos\ud
Revisión sistemática de la literatura donde se buscaron ensayos clínicos controlados y cohortes prospectivas. Se utilizó la escala de Jadad para la calificación de los estudios. 3 estudios cumplieron los criterios de inclusión.\ud
Resultados\ud
La solución salina hipertónica logró una disminución de la presión intracraneana (PIC) con aumento de la volemia y la presión de perfusión cerebral (PPC) más efectiva comparada con otros líquidos utilizados en reanimación como manitol y lactato ringer (p<0,01). Se encontró que estos pacientes tuvieron una menor estancia en UCIP y requirieron menos intervenciones para disminuir la PIC (p<0,04).\ud
Conclusiones \ud
La solución salina hipertónica como tratamiento de la hipertensión endocraneana secundaria a trauma craneoencefálico en niños ha mostrado beneficios importantes como aumento de la volemia y consecuentemente de la PPC, disminución de la respuesta inflamatoria, estancia en UCIP y la cantidad de intervenciones necesarias para disminuir la PIC. Se necesitan más ensayos clínicos aleatorizados controlados en niños que soporten su uso como primera línea en trauma craneoencefálico.\ud

Efectos de la asociación vasopresina-noradrenalina en pacientes pediátricos con choque séptico refractario
Objetivo: Evaluar el efecto sobre presión arterial media (PAM), estancia hospitalaria, mortalidad y perfusión tisular con el uso de vasopresores en niños con choque séptico refractario en la Unidad de Cuidado Intensivo pediátrico de la Fundación CardioInfantil. Materiales y Métodos: Estudio observacional de cohorte retrospectiva en niños de 1 mes a 18 años con choque séptico refractario atendidos de enero 2008 a diciembre 2013 tratados con noradrenalina sola ó vasopresina-noradrenalina. Se excluyeron pacientes con choque de otra etiología, cardiopatías congénitas, diabetes insípida y muerte encefálica. Se estimaron, como medidas de asociación riesgos relativos con sus respectivos intervalos de confianza al 95%.Resultados: Se evaluaron 129 pacientes, de los cuales 51% eran niños, edad promedio 52 meses con una mortalidad global del 34.1%. La PAM inicial en ambos grupos mostró incremento significativo que se mantuvo en las siguientes 3 horas (p<0,005). La mortalidad en el grupo PRISM III de bajo riesgo fue menor con la noradrenalina sola (p<0,005 RR3,7 IC95% 1,16-12,2) pero igual en ambos grupos en los de riesgo moderado según esta escala(p=0,42 RR 0,88 IC95% 0,60-1,30). No se encontraron diferencias estadísticamente significativas en estancia en UCIP (p=0,79) ni hospitalaria total (p=0,96) pero sí mejoría en función hepática (p=0,004)y renal (p<0,005) del grupo de vasopresina-noradrenalina Conclusiones: La asociación vasopresina-noradrenalina mejora la PAM cuando no se han alcanzado metas con un solo vasopresor en pacientes con choque séptico refractario. Se necesitan estudios prospectivos que corroboren estos hallazgos. \ud
Palabras claves:Choque séptico refractario, noradrenalina, vasopresina, mortalidad.Objective:To evaluate the effect on mean arterial pressure (MAP), hospital stay, mortality and tissue perfusion with the use of vasopressors in children with refractory septic shock in the pediatric intensive care unit of Cardioinfantil Foundation. Materials and Methods: A retrospective observational cohort study of children aged 1 month to 18 years with refractory septic shock served from January 2008 to December 2013 treated with norepinephrine alone or vasopressin-noradrenaline. Patients with shock due to other causes, congenital heart defects, diabetes insipidus and brain death were excluded. As a measure of association relative risks with confidence intervals at 95% were estimated. Results: 129 patients, of whom 51% are boys, average age 52 months with an overall mortality of 34.1% were evaluated. The baseline MAP in both groups showed significant increase and was maintained in the next 3 hours (p<0,005). The mortality in the mild risk group of PRISM III was lower with norepinephrine alone (p <0,005; RR3,7 95% CI 1,16 - 12,2) but equal in both groups at moderate risk according to this scale (p=0,42 RR 0.88 95% CI 0.60 - 1.30). Were found no statistically significant differences in the total stay in PICU (p = 0,79) or hospital (p=0,96) but improved liver (p = 0.004) and kidney function (p <0,005) in the group of vasopressin-noradrenaline. Conclusions: Association vasopressin-noredrenaline improves PAM when goals are not achieved with one vasopressor in patients with refractory septic shock. Prospective studies are needed to corroborate these findings.\ud

Here we present results of preliminary observations on the reproductive biology of Bactris bidentula Spruce of gallery forest of the savanna of Llanos de Moxos (Espíritu, Dept. Beni, Bolivia). Les observations réalisées ont permis d'obtenir des données sur les caractères végétatifs, la phénologie, l'anthése et les agents pollinisateurs possibles.Resumen

Evaluation of the renal angina index as a predictor of the development of acute kidney injury in critically ill children with sepsis
La lesión renal aguda (LRA) afecta con frecuencia a los pacientes críticamente enfermos. Se caracteriza por la rápida pérdida de la función excretoria renal y requiere un reconocimiento temprano (1). Puede llegar a ocurrir hasta en un 1.9% de los niños hospitalizados, mientras en pacientes con sepsis críticamente enfermos puede llegar a observarse hasta en un 40% de los casos según la severidad de la enfermedad (1,2). La presencia de LRA y choque séptico se asocia a 3-5 veces mayor riesgo de morir, mayor estancia hospitalaria y progresión a enfermedad renal crónica (3,4). La identificación de la LRA en niños con sepsis requiere un alto índice de sospecha y no existe una sola prueba que permita identificar todos los casos. Recientemente se ha aprobado por la FDA dos biomarcadores que pueden ser útiles para identificar de manera precoz los niños con lesión renal aguda. El inhibidor tisular de metaloproteinasas 2 (TIMP2) y la proteína de unión al factor de crecimiento similar a la insulina 7 (IGFBP7) pueden tener alta sensibilidad y especificidad, pero son costosos y no están disponibles en todos los centros (4,5,6). El índice de angina renal (IAR) propuesto por Goldstein et al (7,8) es una herramienta derivada y validada en la población pediátrica que ayuda a identificar el riesgo de desarrollo de lesión renal aguda en el día del ingreso y 3 días después de la admisión a unidad de cuidado intensivo pediátrico (UCIP). El IAR incorpora parámetros clínicos que para mejorar su rendimiento han sido agrupados en dos categorías: el riesgo (ingreso a UCIP, trasplante de células madre y necesidad de ventilación mecánica o soporte vasoactivo) e injuria (hace referencia a los cambios en el aclaramiento de la creatinina respecto al basal y el porcentaje de sobrecarga hídrica). Un IAR igual o superior a 8 puntos tiene adecuada capacidad para identificar a los pacientes con riesgo de desarrollar LRA a las 72 horas (AUC 0.77 IC95% 0.68-0.86). Este índice permitiría identificar los niños críticos de mayor riesgo y reducir el uso de biomarcadores costosos y no siempre disponibles. En vista de que la falta de identificación temprana, el acceso a los servicios de salud y la presencia de enfermedades crónicas no transmisibles en países de medianos y bajos ingresos son factores determinantes importantes del curso clínico en niños con sepsis, se consideró importante evaluar el IAR como predictor del desarrollo de lesión renal aguda en niños con sepsis en este contexto de recursos limitados.Especialista en Nefrología PediátricaEspecializaciónBackground: The renal angina index (RAI) provides a clinically feasible and applicable tool to identify critically ill children at risk of acute kidney injury (AKI).
Objective: To evaluate the performance of the RAI as a predictor of the development of AKI in children with sepsis hospitalized in the pediatric intensive care unit (PICU).
Methods: Retrospective cohort study in pediatric patients with sepsis hospitalized in the PICU from a university hospital between 2016 and 2020.
Results: We included 209 PICU patients with sepsis with a median of 23 months (IQR 7 - 60). We found that 41.1% (86/209) of the cases developed new acute kidney injury (AKI) on the third day of admission (KDIGO 1:27.8%, KDIGO 2:12.9% and KDIGO 3:3.3%). Overall mortality was 8.1% (17/209) higher in patients with AKI (7.7% vs. 0.5%, p<0.01). The renal angina index at day 3 predicted the development of AKI (AUC of 0.87) sensitivity of 94.2% and specificity of 100% (p<0.01) with a negative predictive value (NPV) greater than 95%. The IAR was associated with increased risk of mortality (aOR 2.6, 95% CI 2.0-3.2; p<0.01), need for renal supportive therapy (aOR 2.9; 95% CI 2.3-3.6 p<0.01) and stay longer than 10 days in PICU (aOR 1.54; 95% CI 1.1-2.1; p<0.01).
Conclusions
According to clinical considerations, IAR has predictive capacity for the development of AKI in pediatric patients with sepsis in intensive care. It should be used as part of the diagnostic and therapeutic approach scheme
Impacto de la antibioticoterapia empírica temprana en pacientes pediátricos con sepsis en la UCIP de FCI
Introducción: La sepsis es un importante problema de salud a nivel mundial con Incidencia en aumento, las primeras horas de tratamiento tienen un impacto dramático en la sobrevida. Las indicaciones de las guías actuales de manejo en antibioticoterapia temprana se extrapolaron de estudios en adultos. Objetivo: Determinar si existe una asociación entre el tiempo de inicio de antibioticoterapia temprana empírica y la mortalidad en pacientes con sepsis severa y choque séptico atendidos en UCIP de FCI. Métodos: Estudio analítico retrospectivo de casos y controles donde se incluyeron los pacientes de 1 mes a 18 años que cursaron con sepsis severa y/o choque séptico en quienes se conoció la hora del diagnostico por signos clínicos, la de la primera dosis del antibiótico y el desenlace final, fueron excluidos los pacientes que recibieron dosis terapéuticas de antibióticos previos, se comparo la mortalidad con el tiempo al inicio del antibiótico empírico, calculando los OR y controlando otras variables relacionadas con la mortalidad. Resultados: Se evaluaron 108 casos, la mortalidad global fue de 38,9%. El tiempo al antibiótico en el grupo de fallecidos fue de 9,06 horas y 6,1 en los sobrevivientes (p=0,003). Los que recibieron antibioticoterapia empírica tardía (>6h) tuvieron 3.8 veces mayor riesgo de morir (P:0,002). Conservaron asociaron con mayor riesgo de morir: La acidosis persistente OR:10.14(P:0.00001), Injuria renal aguda OR:7.86(P:0.003) y antibioticoterapia tardía OR10.1(P:0.005). Conclusiones: Existe una asociación importante entre el tiempo al inicio del antibiótico empírico y la mortalidad en pacientes con sepsis severa y choque séptico por lo que recomendamos iniciarla lo antes posible en estos pacientes.Introduction: Sepsis is an increasingly common worldwide health problem, in which the first few hours of treatment have a dramatic effect on the chances of survival. Results from studies on the early use of antibiotic therapy in adults were extrapolated to determine the effects of such use on children.\ud
Objectives: To determine if there is a connection between the early use of antibiotic therapy and the mortality rate amongst patients with severe sepsis and/or septic shock, who attended at UCIP of FCI.\ud
Methods: A retrospective analytical study was undertaken, of cases and controls of patients aged between 1 month and 18 years old who presented with severe sepsis and/or septic shock, where the time of initial diagnosis was known, as well as the time the first dose of antibiotic was administered, and the final outcome. Patients who had received previous doses of antibiotic therapy were excluded. The mortality rate was compared with the time the course of antibiotic therapy was first started, calculating the OR and controlling other variables associated with the mortality rate.\ud
Results: Of 108 cases evaluated the mortality rate was found to be 38.9%. In fatal cases the time between initial diagnostic and the first antibiotic being administered was 9,06 hours while in those who survived it was 6,1 hours. Empirical studies showed that those who received antibiotic therapy late (>6 hours) had a higher fatality rate by a factor of 3.8 (P:0,002). In addition to the late commencement of antibiotic therapy OR10.1(P:0.005) other factors which increase the risk of death include, persistent acidosis OR:10.14(P:0.00001); or acute renal injury OR:7.86(P:0.003).\ud
Conclusion: An important connection exists between the early commencement of antibiotic therapy and the mortality rate, in patients who present with severe sepsis or septic shock. It is recommended that a course of antibiotic therapy be started as soon as possible with these patients
Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases.
